1 종설 비뇨기과영역에서의보완대체요법 Complementary and Alternative Medicine in Urology 대한비뇨기과학회지제 49 권제 3 호 2008 Changhee Yoo, Choung-Soo Kim From the Department of Urology, Asan Medical Center, University of Ulsan College of Medicine, Seoul, Korea An increasing use of complementary and alternative medicine (CAM) is apparent both in Asian and in Western countries for the management of numerous diseases to improve symptoms and to improve general health and well being. Many patients with benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS), erectile dysfunction (ED), urinary tract infection (UTI) are using CAM. Complementary and alternative medicines for urologic malignancies such as renal cell carcinoma, bladder cancer, and prostate cancer are also one of the important issues in the management of patients. Dietary supplements and nutraceuticals are commonly used modality for the patients with BPH/LUTS or ED. Vegetables and cruciferous fruits showed significant preventive effect on urologic malignancies in many studies. Phytotherapy for BPH including beta-sitosterols, saw palmetto, pygeum, and nettles etc, is under vigorous research for their therapeutic effect. No solid evidence showing better effective treatment modality for ED than placebo was found yet in CAM area. Recently, a potent NO donor, L-arginine is under research with promising results. Cranberry juice has been used for the patients with recurrent UTI but safety was a limit for its use. Various antioxidants have been studied for prevention of urological malignancies. Lycopene, isoflavone, vitamin A, C, and E, and polyphenols showed the possibility of decreasing cancer incidence in many studies but their effect is still controversial. Vitamin D is known to be associated with cancer progression and the effect of combination treatment with other chemotherapeutic agents for advanced cancer is under trial. CAM is used by a number of patients with urological diseases and urologists need to have both accurate knowledge and apprehension of CAM. The possible effects and side effects should be defined and offered to the urologic patients by urologists. (Korean J Urol 2008;49: ) Key Words: Complementary and alternative medicine, Benign prostatic hyperplasia, Urinary tract infections, Erectile dysfunction, Urological malignancy 울산대학교의과대학서울아산병원비뇨기과학교실 유창희ㆍ김청수 교신저자 : 김청수울산대학교의과대학서울아산병원비뇨기과서울시송파구풍납 2 동 TEL: FAX: cskim@amc. seoul.kr 보완대체요법의정의와역사보완대체요법은일반적으로의과대학의주된교육과정에서제외된내용으로서병원에서도통상적으로제공되거나사용되지않는가벼운질병이나만성질병을대상으로하는치료법으로정의한다. 1 보완대체요법은동서양을막론하고고대사회이래로질병의치료에이용되었던수많은경험적치료에기반을두고있다. 국내에서도전통적으로 한의학이이범주에포함되어있어서수많은보완대체요법이치료에이용되고있는실정이다. 2 비뇨기계질환에서는요로결석에대하여통증완화, 결석배출, 예방을위하여중세시대를거치면서다양하게발전하여왔다. 1 또한아랍권에서도 8-11세기에걸쳐다양한보완대체요법이사용되었고, 3 15세기에이집트에서는전립선비대증에약초를사용하였다는기록도전해진다. 4 한국을비롯한아시아권에서는전통적으로한의학을중심으로하는치료들이질병치료의주된수단이었으며 1세기전서양의학이도입되어 193

2 194 대한비뇨기과학회지 : 제 49 권제 3 호 2008 현대의학의주류로자리잡았지만최근까지도전통적인치료에대한개념들은사회에깊이뿌리를내리고있다. 또한전통의학에대한시장수요도꾸준히증가하여 2004년기준으로시장규모가한국은연간 17조원, 중국은연간 6조 7천억위안에달하였으며매년증가하는추세에있다. 5 동아시아지역에서양의학이도입된후전통의학과다양한경험적인민간요법들은정규적인의과대학교육과정에서제외되어있으며따로한의학을전공하는제도로분리되어있다. 6 최근에는한국, 중국, 일본등을중심으로전통의학에현대의학의개념들을접목시켜과학적체계를갖추려는노력이진행되고있다. 7 미국과서구유럽에서도다양한질환에대한치료, 증상의완화, 건강증진및 well-being을목적으로보완대체요법에대한관심도가증가하면서널리이용되고있다. 미국에서는전체인구의 1/3 이상이보완대체요법을받은경험이있고연간 5천만달러이상의비용이보완대체요법에쓰이고있으며, 1차의료기관보다척추지압교정요법, 침, 생약등의보완대체요법을찾는사람들이더많다고보고되고있다. 8 보완대체요법에는주로식이및영양요법 ( 생약, 비타민, 미네랄, 아미노산등 ) 이이용되지만요가와같은스트레스해소법, 침, 아로마요법, 운동요법등도포함된다 년미국국립보건원 (National Institute of Health; NIH) 은산하에국립보완대체의학센터 (National Center for Complementary and Alternative Medicine; NCCAM) 를설립하고다음과같이보완대체요법을 5가지영역으로나누었다 동종요법, 전통동양의학, 인도의아유르베다장수법등의대체요법 2. 명상, 기도, 정신수양등의정신-신체중재요법 3. 비타민, 미네랄, 아미노산, 생약, 특정식이요법등의생물학기반의치료 4. 척추교정, 마사지등의신체기반치료 5. 기공, 치료적접촉을통한에너지치료, 일본의영기요법 (Reiki), 바이오필드등의에너지치료 3번영역이비뇨기과에서가장많이사용되는 CAM 요법이다. 국내에서는한의학에이와같은분야가일부포함되어있으나아직보완대체의학전반에대한체계적인분류가되어있지않다. 보완대체요법에대한관심의필요성한국과중국, 일본등동아시아권국가들에서는전술한바와같이이미전통의학을중심으로하는보완대체요법이다양한증상과질병의치료에널리이용되고있다. 미국을비롯한서구에도과거수십년사이에다양한보완대체요 법이소개되었으며환자들의관심도는물론시장규모도계속성장해왔다. 환자들은기존의학체계가제공해주지못하는좀더실생활에가깝고쉽게접할수있는의료서비스를원하며, 이러한부분은보완대체요법이채워가고있다. 일부치료법들은그효과에대한과학적검증을시도하고있으며정확한기전을설명하기는어렵지만효과적인결과들을보이는것들도있다. 이러한치료법이과학적인연구를통하여안전성과효능이입증되기어렵다는견해도있지만넓은안목을가지고이러한보완대체의학의영역을기존의학에서수용해야한다는의견도많다. 비뇨기과영역에서도보완대체요법은환자들에게대중화, 보편화되고있다. 의사가직접적으로질문하기전에환자들이밝히지는않지만하부요로증상및전립선비대증으로비뇨기과를찾는환자들의 90% 이상에서보완대체요법을경험하고있다. 10 따라서비뇨기과의사들도보완대체의학에관심을가지고장단점과위험성등에대하여환자들에게설명해줄수있어야한다. 보완대체요법의문제점환자들의관심과수요에도불구하고보완대체요법은아직많은문제점들을가지고있다. 우선대부분의보완대체요법에대한과학적연구가부족하고, 현재까지보고된연구결과들도소규모의단기간연구가대부분이며결과에대한이견들이많다. 약동학, 약역학적정보도부족하며, 성분과용량에대한표준화도되어있지않다. 일부의성분은기존치료에사용하는약물과상호작용을일으킬수도있으며심지어독성작용을일으킬수도있다. 또한, 대부분의의사들은이러한이유로질병및증상의치료에있어서보완대체요법에대하여관심을가지지않거나처방하기를꺼리고있다. 보완대체요법의안전성에대하여사회적으로큰문제가제기되었던일례로 2002년미국식품의약국 (US Food and Drug Administration; FDA) 에서는진행성및호르몬불응성전립선암환자들에면역증강제로서알려진 PC-SPES와 SPES 관련식이및생약요법을심혈관계안전성문제로중단하도록한경우가있다. 11 비뇨기계질환에서의보완대체요법의이용실태비뇨기질환에서사용되는보완대체요법약물은 Table 1 과같다. 국내에서최근다기관연구로서비뇨기과외래에내원한비뇨기종양환자 711명을대상으로설문조사를실시한결과 39.2% 에서대체보완요법을받은경험이있다고대답하였으며 80.7% 는저지방식, 생식, 금식등식사요법을

3 유창희ㆍ김청수 : 비뇨기과영역에서의보완대체요법 195 주로하였고, 그외 6.8% 는생약, 6.3% 는뜸, 부항, 3.4% 는침요법을선호하는것으로조사되었다. 주로악성종양환자들이 44.3% 로관심도가높았는데, 특히면역치료나방사선치료를받는사람들이대체보완요법을선호하였고, 선호도는교육정도에비례하였다. 장기별악성종양에서는방광암, 전립선암, 신암에서각각 40.7%, 45.0%, 45.2% 의환자가대체보완요법에관심을가지고있었다. 2 발기부전환자들을대상으로한다른조사에서는 38.2% 의환자들이보완대체요법을이용하고있었으며, 48% 의환자들이관심을가지고있었고 14% 의환자가의사에게서보완대체요법에대하여설명을들었다고대답하였다. 발기부전환자들에서는 77% 가식이요법을사용하고있었으며이중 62% 는콩, 인삼, 버섯등의가공된건강식품을, 15% 는토마토, 콩, 인삼등의가공되지않은건강식품을사용하고있는것으로조사되었다. 생약을복용한경우는 45% 였으며, 그외에도 25% 가침요법을 17% 는뜸및부항요법, 11% 는마사지요법을사용한것으로조사되었다. 12 미국에서조사된자료에의하면교육정도가높고, 수입이많은젊은사람들에서보완대체요법에더관심을가지는것으로보고되었다. 8 하부요로증상 / 전립선비대증, 악성종양의치료와예방, 전립선염, 요로감염, 발기부전, 요로결석, 불임등많은비뇨기질환에서도다양한보완대체요법이이용되고있는것으로조사되었다. 10 Table 1. Complementary medicine in urology BPH Prostatitis Prostate cancer UTI OAB ED Infertility 전립선비대증에서의보완대체요법 1. 전립선비대증과식이 섬유질이많은과일이나채소, 항산화물질이풍부한식품, phytoestrogen이포함된콩으로만들어진된장, 두부등의음식에함유된 isoflavone이많은음식, carotenoid 등은하부요로증상, 전립선비대증, 전립선염및전립선암등의유병률을낮춘다는많은보고들이있다. 반면에붉은고기류, 우유를비롯한낙농제품은전립선비대증의증가와관련이있다. 아시아인에서는저지방식이및식물성에스트 Saw palmetto, pygeum Quercetin, rye pollen Vitamin E, selenium, PC-SPES Cranberry juice, acidophyllius Ginger Ginseng, yohimbine, L-arginine L-carnitine BPH: benign prostatic hyperplasia, UTI: urinary tract infection, OAB: overactive bladder, ED: erectile dysfunction 로겐이풍부한고식이섬유로인하여서구사회의인구보다전립선암을비롯한전립선질환유병률이낮은것으로생각된다. 이를뒷받침하는과학적배경으로서아시아인에서는서구인에비하여식이를통하여전립선내에서세포고사율이높아지고, DNA를변화시키는 free-radical이낮아지며, insulin-like growth factor-1 (IGF-1) 의생산이줄어들고 angiogenesis factors, 5-알파환원효소, aromatase, estrogen 수용체활성도가줄어들어세포의성장이억제된다는보고들이있다. 9 Bravi 등 13 은곡류와빵이혈당을높여고인슐린혈증, IGF의증가로인하여전립선의조직성장이활성화되어전립선비대증이증가하였다고하였으나이점에있어서는아직논란이있다. 2. 전립선비대증에서의생약치료 유럽에서는식물추출물이전립선비대증의일차치료제로서흔히처방되고있으며미국에서는건강보조식품으로서대부분처방없이구입이가능하다. 이중전립선비대증에서가장많이사용되는생약제제는 beta-sitosterol, pygeum (Pygeum africanum) 나무추출물, 톱야자 (saw palmetto), 쐐기풀류 (nettles) 등이다 (Table 2). 10 이러한식물추출물이함유하고있는성분들은 phytosterols (beta-sitosterol, campestrol, stigmasterol, delta-5-sterol, delta-7sterols), phytoestrogens (lignins, flavonoids, isoflavonoids (genistein, daidzein)), fatty acids (lauric and myristic acid), lectins, plant oils, polysaccharides, lupenone, lupeol, terpenoids 등이다. 그리고식물추출물들이가지고있는기전들은항안드로겐효과, 안드로겐및, 에스트로겐수용체억제, 5 알파환원효소억제, 전립선요도저항감소, 알파교감신경억제효과, 항염증효과, 항부종효과, prostaglandin 대사억제, phospholipase A2와 5- lipoxygenase의억제, arachidonic acid 분비억제, 전립선세포 Table 2. 4 196 대한비뇨기과학회지 : 제 49 권제 3 호 2008 대사및성장억제, prolactin의전립선성장효과억제, 방광근의보호및강화, aromatase 억제, cholesterol 대사변화, sex-hormone binding globulin 감소, free radical 청소및세포막안정화, apoptosis 유도등이다. 10 Beta-sitosterol은가장풍부한식이 phytosterol로서채소류와콩류에많이함유되어있다. 이성분은 pygeum, 톱야자등과같은전립선에특이적인생약에서중요한성분으로생각되고있다. 이성분은실험실적으로주로전립선암세포주또는 xenograft 모델로연구되었으며전립선비대증에관련된증상에대한효능및안전성과관련된많은임상연구가진행되었다. 전립선비대증환자 177명을대상으로 beta-sitosterol 130mg으로 7개월이상시행한무작위이중맹검위약대조시험에서 betasitosterol은위약군에비하여유의하게국제전립선증상점수를감소시켰다. 15 그러나, 이성분을전립선비대증에서사용한여러임상시험들을체계적으로분석한결과는아직효과에대하여논란이많았다. 16 Pygeum은중남부아프리카에서흔히발견되는나무로서껍질부분이하부요로증상을완화시키는것으로알려지면서전립선비대증의생약제제로서사용되기시작하였다. 이껍질의성분은항산화제, beta-sitosterol, ferrulic ester 등으로밝혀졌다. 쥐의전립선기질세포의증식에대한 pygeum (Tadenan R ) 의영향을알아보기위한전-임상시험에서 pygeum은 epidermal growth factor (EGF), IGF-1, basic fibroblast growth factor (bfgf) 를각각 4.5, 7.7, 12.6μg/ml의 IC 50 값에서억제하여쥐의섬유아세포의증식을억제할수있었다 개월간 100mg의 pygeum을 209명의전립선비대증환자들에게투여한임상연구에서국제전립선증상점수 (International Prostate Symptom Scores; IPSS) 를 16점에서 9점까지 46% 를감소시켰다는보고가있다. 18 또한 85명의전립선비대증환자들을대상으로한시험에서는치료 2개월후에도유의한 IPSS의감소를나타냈다는결과를보고하였다. 19 최근 pygeum의작용기전이크게두가지로정리되었다. 첫째는 protein kinase C 수준에서 b-fgf, EGF 및남성호르몬매개전립선증식을억제하는기전이며, 둘째는 myosin isoform을표현하고 synaptic denervation을줄이며, mitochondria의기능을향상시킴으로써방광기능을증진시키는것이다. 20 그러나 pygeum의전립선비대증관련증상에대한효과에대한임상연구결과들을분석한연구에서는관련연구들이대부분소규모면서방법적으로상이한문제점들을가지고있다고기술하였다. Pygeum이전립선비대증치료에도움을줄수있을것으로추정하고있으나위약대조시험, 장기간의추적관찰에의한결과등좀더잘디자인된연구가필요하다. 톱야자 (Serona repens) 는 American dwarf palm tree의표준화된지방추출물로서 Permixon R 이라는상품명으로판매되 고있으며비뇨기계에특이성을가진생약중가장잘알려진약재중하나이다. 이식물열매는 beta-sitosterol, 지방산, 항산화제등을함유하고있다. 초기의전임상실험에서는이러한성분들이알파환원효소억제제의효능과비슷한결과를얻었다. 21 톱야자는전립선비대증에대하여비교적많은임상시험이실시되었다. 톱야자추출물의전립선에대한항염증효과를평가한다기관연구에서치료군이대조군에비하여전립선조직에서림프구가적었으며 (p=0.097), 치료 3개월후국제전립선증상점수가유의하게낮아졌다 (p<0.006). 혈청의 tumor necrosis factor-α(tnf-α), interleukin-1β(il-1β) 역시치료군에서유의하게낮았다. 22 또한 12개월간매일 320mg의톱야자를투여한군과매일 tamsulosin 0.4mg 투여한군을비교한연구에서평균 IPSS 감소치가톱야자치료군에서높았다 (p=0.051). 같은연구에서방광자극증상역시 tamsulosin 치료군에비하여톱야자치료군에서유의하게개선되었으며 (p=0.049) 이러한효과는 12개월후에도유지되는결과를얻었다. 23 톱야자치료가성기능에미치는영향을 tamsulosin 및 finasteride와비교한연구에서전체적으로성기능의변화가유의한차이를보이지는않았으나 tamsulosin과 finasteride가사정장애를포함하여성기능을감소시킨반면, 톱야자는성기능을약간호전시켰다. 24 그러나톱야자의전립선비대증에대한효능에대한연구들을분석한여러종설들에서는긍정적, 부정적결론이양립하고있으며아직이견이있다. 최근에는 225명의환자들을대상으로 1년이상치료하며위약군과비교한무작위임상시험에서효과가없는것으로보고된후전립선비대증의치료에대한이용이줄고있는추세이다. 25 쐐기풀류 (Urtica dioica) 의뿌리부분이비뇨기계에영향을주는것으로생각되며전립선비대증관련증상을완화시킬수있는지에대한연구들이있었다. 작용기전은약한이뇨효과와항염증효과가있는것으로알려져있다. 543명의전립선비대증환자들을대상으로하여 finasteride 단독치료군과쐐기풀과병합치료한군을비교한연구에서치료 24주후에전립선크기와 IPSS점수에는유의한차이가없었다. 26 다른연구에서는톱야자와쐐기풀을병합하여 24주간치료하여대조군에비하여 IPSS 점수가유의하게감소되었다. 27 전립선비대증에대한쐐기풀의효과는아직확실치는않으며아직많은연구가필요하다. 그외에도 Roystonea regia는전립선비대증쥐모델에서전립선무게를톱야자에비하여 75% 감소시켰고, 28 영지버섯 (Reishi mushroom: Ganoderma lucidum) 은역시쥐모델에서전립선의성장을방해하는강한알파환원효소억제제효과를보였다. 29 반면에다른종류의버섯인상황버섯 (Phellinus linteus) 는전립선을증식시키는효과를보여 30 전립선비대증환자는주의하여섭취하여

5 유창희ㆍ김청수 : 비뇨기과영역에서의보완대체요법 197 야할것으로생각한다. 발기부전의학에서약물치료를할경우에위약효과는잘알려져있으며특히발기부전에서는위약효과가뚜렷이나타나는경향이있다. 발기부전치료제에대한임상시험에서위약효과는 25-41% 에달한다. 발기부전에서보완대체요법의효과는제5형 phosphodiesterase (PDE-5) 억제제에대한임상시험에서의위약군에비해높지않다. 침요법, androstenedione, dehydroepiandrosterone (DHEA), 아연등의식이보충요법과여러생약제제들은소규모연구에서위약에비해유의한효과를보이지못하였다. 10 은행나무추출물 (Ginkgo biloba) 은주로치매환자에서뇌혈류를개선시키는효과가있는것으로알려져있는데, 선택적세로토닌수용체차단제사용으로인한발기부전을개선시키는효과가있을것으로추정되었으나입증되지는않았다. 더구나다른많은약물과의교차반응, 간질을일으키거나출혈시간을연장시키는등부작용으로사용에제한이많다. 31 서아프리카 yohim 나무껍질의추출물인 yohimbine은 alpha-2 adrenergic antagonist로서 PDE-5 억제제가쓰이기전까지발기부전에많이사용되었다. 7개의무작위대조군시험을분석한메타분석에서 yohimbine은특히비기질성발기부전에효과가있었다 mg씩하루 3회, 그리고 10.8mg씩하루 4회사용한두개의대규모임상시험에서각각 34%, 42% 의환자에서발기능을개선시켰다. 33 최근 6년간이약제에대한임상시험은이루어지지않았으나 L-arginine과병합했을경우효과가있었다는보고가있다. 34 심계항진, 고혈압, 불안, 동맥수축등부작용이많아 1997년미국식품의약청에서는 yohimbine을불안전약제로분류하였다. 한국홍삼 (Panax ginseng) 은다른나라의인삼제품에비하여발기부전에대한효과에대하여가장많은연구가이루어졌다. 동물모델에서홍삼의 saponin glycoside 성분이일산화질소합성효소를유도하여음경혈관확장을유도하는것으로알려졌으며, 35 음경해면체의일산화질소방출, 세포내의칼슘이온감소를유발하는반면에역설적으로자유라디칼이온뿐만아니라일산화질소자체도제거하는기능을가지고있다. Trazodone 및위약과비교한임상시험에서다른약제를투여한환자군에서는 30% 에서발기능개선효과가있었으나홍삼을투여한군에서는 60% 에서발기능개선효과를보였다. 36 8주간투여한소규모의교차시험에서홍삼은국제발기능점수및음경혈류를 60% 에서향상시켰다. 37 아미노산인 L-arginine은신경및혈관내피세포일산화질소합성효소에대한일산화질소공여자로서 3.0g/day의고용량을투 여한통제시험에서는 31% 의발기능개선을보였으나 1.5g/day를투여한경우에는위약에비하여효능에차이가없었다. 38 일산화질소의농도를증가시키기위하여 L-arginine은일산화질소합성자극물질인소나무껍질에서추출한 pycnogenol과병합투여하여 92% 의환자에서발기능개선효과를보였다. 39 요로감염 Cranberry juice의 proanthocyanidin은세균이방광점막에붙는것을방지하여요로감염을줄인다고알려져있다. 153 명의고령여성환자들을대상으로한무작위, 이중맹검위약-대조군시험에서 cranberry juice가농뇨를동반한세균뇨의빈도를줄이는것으로보고되었다. 40 4편의잘디자인된무작위대조군시험을종합하였을때특히재발성요로감염을호소하는성인여성에서 cranberry juice는 12개월이상증상을동반한요로감염빈도를위약 / 대조군에비하여유의하게낮추었다. 41 위장장애, 오심, 설사등소화기계부작용, 요로결석등의부작용이있으며특히 coumadine을복용하는환자에서출혈을증가시킨다. 비뇨기계악성종양가장흔한비뇨기종양인신암, 방광암, 전립선암에대한보완대체요법에대하여살펴보고자하며본논문에서는약제에의한화학적항암예방효과에관한연구들은제외하였다. 1. 신암주로야채, 과일섭취등식이요법과비타민등이신암의예방에효과가있는것으로알려져있다. 야채와과일섭취와신암발생에대한대부분의증례-대조군연구는대부분역상관관계를보이는결과를보고하고있다. 주로청록색야채와양배추나브로콜리등의평지과야채 (cruciferous vegetables), 오렌지등이신암의발생을예방하는것으로조사되었다. 채소에풍부한 flavonoids 중에서 flavone과 flavonol 또는채소의섬유질이신암의발생을낮춘다는보고도있었다. 42 커피와차는신암과의연관성에대하여이견이많지만최근 774,952명을대상으로 13개의전향적연구를분석한 meta-analysis에서커피와차는신암발생의위험도를낮추는효과가있다고보고하였다. 43 고기, 햄버거, 소시지등가공된육류의섭취가많은서양식식이습관이신암발생과연관이있다는보고도많이있으나이에대하여는아직이견이많다. 44 또한총에너지섭취량이많을수록신암

6 198 대한비뇨기과학회지 : 제 49 권제 3 호 2008 위험도가증가하며탄수화물, 지방또는단백질섭취가위험도를높인다는보도들도있다. 비타민은비타민 A, C, D, E 등이신암과관계가있을것으로생각되고있으나임상시험이이루어진것은비타민 C, E 등으로이들은특히비흡연자에서신암의예방에효과가좋았다. 비타민 D는신암발생보다는진행과연관이있는것으로생각되고있으며종양의성장및혈관생성을억제한다는보고가있다. Carotinoid에는 alpha-, beta-carotene 및 cryptoxanthine, lutein 등이있으며이들모두신암과연관이있는것으로보고되고있다. 그러나최근 carotinoid 및비타민섭취는 VHL 유전자변이와관련이없다는보고도있다. 알코올및와인섭취는주로여성에서신암의발생을낮추는효과가있는것으로알려져있으나이역시이견이많다. 2. 방광암수분섭취가방광암의발생을낮출것이라는의견도있으나방광암의발생및재발에대하여는이견이많다. 45 수분에포함된비소, 질산염등은방광암의위험인자이며 46 많은소변량은방광을팽창시켜기저세포가발암물질과장시간접촉될기회가많아방광암발생이증가할것이라는가설도있다. 45 녹차의 polyphenol 역시 in vivo 실험에서방광암을억제하는효과를보였으나임상시험에서는일관된결과를보이지못하고있다. 47 지방및콜레스테롤의섭취가방광암의발생과연관이있을것으로생각되고있다. 반면에신암과마찬가지로채소와과일의섭취는방광암의발생을낮추는효과가있다고보고되고있다. 특히브로콜리와양배추가유의한효과를보인다는보고도있었다. 48 방광암과비타민에관한연구도많았다. 비타민 A 유사체인 retinoid 는항산화작용과함께면역증강작용이있다. 방광암과의관련성에대하여도많은실험실적연구와임상연구가있었지만효과는불분명하다. 비타민 B6 (pyridoxine) 과그대사산물은여러아미노산대사의보효소로서특히발암성질이있는 tryptophan 대사산물의제거에관련하여항암작용이있을것으로추정되고있다. 49 그러나, pyridoxine 20mg/day 를 291명의방광암환자들에게투여하여위약군과비교한임상시험에서재발방지에이득이없었다. 50 비타민 C는항산화작용과환원작용이있어자유라디칼을제거하고방광암발암물질인 N-nitroso compound 형성을억제하여방광암억제작용이있을것으로생각되며식이비타민 C 섭취보다는비타민 C 보충제가이러한효과가있는것으로생각되고있다. 51 비타민 E 역시항산화제로서 nitrosamine 형성을억제한다. 비타민 E 보충제는 10년이상장기복용한경우방광암의위험성및방광암으로인한사망률을낮출수있는것으로보고되었다. 52 셀레늄은항산화작용, 면역증강 기능이있어서여러암에서효과가있을것으로생각되고있다. 방광암환자에서는혈청셀레늄치가낮게측정되었고흡연을계속하는경우이러한셀레늄치가낮아방광암위험성이증가할수있다는가설이제시되었다. 53 콩단백인 isoflavone과 phytoestrogen 역시 in vivo 실험에서 nitrosoamine에의해유발된방광점막의이형성및방광암을감소시키는효과가있었으나 54 임상시험에서는효과가불분명하였다. 55 마늘 (Allium sativum) 은발암물질의제거, 암세포의성장억제, 면역증강등의작용이있으며, 많은 in vivo 실험에서방광암억제에대한효과가유망한것으로제시되고있으나아직임상연구는없다. 3. 전립선암전립선암은보완대체요법분야에서가장많은관심을모으고있는비뇨기계악성종양이다. 1,000명이상의새로이진단된전립선암환자를대상으로한캐나다연구에서 39% 의환자들이기능성건강보조식품 (dietary supplements and nutraceuticals; DS/N) 을사용한것으로조사되었다. 56 2,500 명을대상으로한미국의연구에서도약 1/3의환자들이보완대체요법을사용하고있는것으로조사되었다. 57 형제에게서전립선암의가족력이있었던성인남성에대한조사에서는 55% 가보완대체요법을이용하였고이들이가장많이사용한약제는톱야자추출물과비타민 E였다. 그러나, 최근의화학적예방에대한연구는보완대체요법분야보다는 Prostate Cancer Prevention Trial (PCPT), Reduction by Dutasteride of Prostate Cancer Event (REDUCE) 등알파환원효소억제제의효과에대한대규모연구에관심이집중되고있다. Lycopene, 비타민 A, 비타민 E, 셀레늄, 폴리페놀 (polyphenols), 석류즙 (pomegranate juice), 콩단백질 (isoflavone) 등은항산화효과를가지고있어서전립선암에대한예방효과를기대하여이들에대한많은연구들이진행되었다. 토마토의성분인 lycopene은전립선조직에상대적으로고농도로존재하며혈중에고농도로존재하는경우전립선암발생위험도를 25-80% 까지낮출수있다는많은보고들이있었다. 그러나최근 30,000명을대상으로한다기관관찰연구에서는 lycopene이전립선암을예방하지못하였다고보고하였다. 58 비타민 A는 beta-carotene이낮은남성에서전립선암의유병률이낮다는보고가있었으나반대의결과도보고되어아직전립선암과의관련성은확실치않다. 비타민 E는전술하였듯이강한항산화효과를가지고있어서많은연구에서그혈중농도가낮은경우전립선암의발생률이높아짐을보고하였다. 30,000명의남성흡연자를대상으로한 Alpha-Tocopherol Beta-Carotene (ATBC) 항암임상시험에서비타민 E 보충제는전립선암의발생률을 40% 까

7 유창희ㆍ김청수 : 비뇨기과영역에서의보완대체요법 199 지낮추는것으로보고되었다. 59 비흡연자를대상으로한몇몇소규모임상시험들을제외한, 다른대규모연구에서도같은결과를얻을수있었다. 최근 gamma-tocopherol은특히전립선암예방에효과가있는것으로보고되었다. 60 셀레늄및아미노산 selenomethionine과같은셀레늄함유성분들은세포고사를유도하고자유라디칼에의한세포손상을방지함으로써항암효과를가진다. 61 셀레늄은남성호르몬과여성호르몬조절에도관여하는것으로알려져있다. 62 상이한결과들도있으나대체로낮은혈중농도에서전립선암의발병률이높은것으로알려져있으며이러한남성들에게셀레늄보충요법을시행함으로써유의하게전립선암발병률을낮춘보고도있었다. 최근에는비타민 E와셀레늄을병합하여전립선암에대한예방효과를조사하는임상시험 (Selenium and Vitamine E Cancer Prevention Trial; SELECT) 이 2013년종료를목표로진행중이다. 63 녹차는강한항산화작용을가지는 polyphenol 계열인 catechins을다량함유하고있다. 녹차의전립선암에대한예방효과에는이견이많으며 Mayo Clinic에서전립선암환자들을대상으로녹차를섭취하게한연구에서는약 2% 에서 PSA 반응이있었으며구토, 불면, 피로감, 설사복통등의부작용이약 69% 에서발현되었다. 64 석류즙은 elagic acid, gallotannins, anthocyanins 등의 polyphenol을함유하고있으며실험실적으로전립선암세포주에대한항암효과를보였다. 65 최근전립선암환자에게방사선치료후석류즙을하루 8온스 (227g) 를마시게하여 PSA doubling time을 15개월에서 54개월로늘렸다는보고도있었다. 66 콩단백질인 isoflavone은성호르몬합성과작용에영향을준다고알려져있다. 많은연구에서콩의소비와전립선암의역상관관계가보고되고있다. 전립선암환자에서는테스토스테론과 PSA를떨어뜨리는효과가있다는보고도있다. 67 아연은높은혈중농도와낮은전립선암발병률관계에대한보고가있었으나최근하루 100mg 이상의아연을복용한경우전립선암의발병률이 2배증가하였다는보고도있어 68 이견이많다. 적포도주는전립선암의위험성을약 24% 정도낮추는것으로보고되고있다. 또한적포도주를 1주일에 1잔을더마신다면약 6% 정도전립선암의위험성이낮아지며 1주에 4잔이상을마신다면전립선암의빈도가절반가지감소한다고한다. 69 그러나과음으로위, 간질환발생의위험성이있고그외에도해로운점들이있어이에대한더많은연구가필요할것이다. 결론보완대체요법은최근의료시장에서그관심도및시장 규모가계속증가하는추세이며일부는과학적검증을통하여기존의료체계에서이용되고있다. 이러한현상은비뇨기질환의영역에서도예외는아니며비뇨기과의사들은보완대체요법에대하여좀더관심을가져야할것이다. 전립선비대증, 발기부전, 요로감염의치료및예방, 신암, 방광암, 전립선암등비뇨기계악성종양의예방에있어서식이요법, 비타민, 항산화성분이풍부한채소, 과일류가사용되고있다. 이들질환에대한보완대체요법의효과에대하여는아직체계적인과학적검증과정이좀더필요한실정이며, 이과정에서항암기전및새로운치료제의발견도기대해볼수있을것이다. 보완대체요법에대한효과나치료법이불확실한상황이지만비뇨기과의사는적절한지식을가지고비뇨기과영역에서환자들이사용하는보완대체요법에대한지침을하고부작용에대한지도를할수있는지식과경험을갖추어야할것으로생각한다.
