However, adherence rates have not been as high as expected. Therefore, we tried to compare the eating, smoking, and drinking habits of cancer survivors to those of non-cancer controls. Methods: We collected data from the Korea National Health and Nutrition Examination Survey IV-V. We included participants who completed a 24-hour dietary recall, a food frequency questionnaire, and an interview. A total 14,832 individuals were included in this study. Among those, 434 were cancer survivors. We compared the relationship between healthy habits (balanced diet, intake of fruit or vegetables more than 5 times a day, fat intake less than 25%, sodium intake less than 2,000 mg, non-current drinker, and non-current smoker) in cancer survivors with that in the noncancer controls. Results: The adjusted odds ratios (AOR) of cancer survivors related to a healthy diet were statistically non-significant compared to non-cancer controls after adjustment for confounding factors. The AORs of cancer survivors related to being a non-current drinker and non-current smoker were 1.42 ( ) and 0.94 ( ), respectively. Conclusion: In this study, there were less statistically significant non-current drinkers in the cancer survivors than in the non-cancer controls. However, current smoking and healthy eating patterns were not significantly different between cancer survivors and non-cancer controls. More education and an information campaign would be needed to improve cancer survivors adherence to healthy lifestyles. Unhealthy Eating Habits among Cancer Survivors 그러나, 암경험자에서흔히권고되는건강한생활습관에대한 순응여부를본여러논문들에서암경험자의 1/5 만이과일과채소 를적정하게섭취하였고, 암경험자의 90% 는흡연하였으며, 90% 는 2, 8-10) 금주에관련된권고안을따르지않았다는논문들이있다. 우리나라의경우에도국민건강영양조사를이용한여러연구들 에서상대적으로흡연율과음주율이낮다는보고가있었으나, 절대 적인흡연 / 음주율은권고치보다는높았다 ) 현재까지우리나라에 서암경험자의흡연, 음주, 만성질환, 암검진에대한연구는지속적 으로있어왔으나건강한식이습관에대한연구는거의없었다. 따라 서, 우리나라의암경험자를대상으로하여건강한식이습관의여부 및흡연, 음주현황에대한연구를해보고자한다. 따라서우리는이 연구를통하여국민건강영양조사를이용하여우리나라암경험자 의건강한식이습관을확인해볼예정이다. 방법 본연구는국민건강영양조사제 4 기 ( 년 ), 제 5 기 ( 년 ) 의건강설문조사결과를이용하였다. 국민건강영양조사는 한국질병관리본부에서상시적으로실시되고있으며, 건강면접조 사, 검진조사, 영양조사로구성되어있다. 이연구에서는국민건강영 양조사 4 기, 5 기에해당하는 48,811 명중나이가 40 세미만인사람을 제외하고 (23,824명 ) 40세이상인사람들을대상으로하였고, 이중 24시간식이회상과식품섭취빈도조사지 (food frequency questionnaire, FFQ) 에데이터가없는사람 (6,882명 ) 을제외하였다. 이중암치료중이라고대답한사람 (268명 ), 체중과키에대한정보가없는사람 (932명 ), 흡연, 술, 운동, 교육에대해대답하지않은사람및모른다고대답한사람 (2,073명 ) 을제외하였다. 따라서, 총 14,832명을대상으로하여연구를진행하였다 (Figure 1). 암경험자를구분하기위하여 의사로부터암을진단받은적이있습니까 라는질문에 예 라고대답한사람을 암환자 로정의하였다. 이중현재암치료중인사람을제외한사람을 암경험자 로정의하였다. 11) 또한, 암경험자를진단 5년이내, 5년이후로구분하여하위분석에이용하였다. 현재나이에서암진단나이를빼 5년미만일경우 5년미만의암경험자, 5년이상일경우 5년이상의암경험자 로분류하였다. 또한전체대상자중암으로진단받지않은사람을 비암경험자 로정의하였다. 건강한식습관은여러권고안에서각각의식습관을정의하였다. 하루과일과채소의섭취횟수를하루 5회미만섭취여부, 14) 하루 25% 이상지방섭취여부, 15) 하루 2 g 이상으로염분섭취여부, 15) 탄수화물 55% 70%, 지방 15% 25%, 단백질 7% 20% 섭취만족여부 15) 에따라건강한식습관을정의하였다. 따라서, 하루에과일이나채소를 5회이상섭취, 25% 미만의지방섭취, 2,000 mg 미만의나트륨섭취, Participants in both the Korean National Health Interview and Examination Survey of KNHANES IV ( ) & V( ) (n=48,811) Age <40 (n=23,824) Aged more than 40 years (n=24,987) Missing information on 24 hour dietary recall data (n=2,483) Missing information on FFQ (n=4,399) n=18,105 Study population (n=14,832) Who responds to be on the treatment of cancer (n=268) Missing information on body mass index (n=932) Missing information on smoking, alcohol, exercise, education (n=1,976) Who answered don t know to smoking, alcohol, exercise, and education questionnaire (n=97) Figure 1. 3 박진곤외. 암경험자의식습관 균형잡힌식단을만족하는경우, 현재음주자가아닌경우를건강한식습관으로정의하였다. 이를위하여지방섭취, 나트륨섭취, 단백질섭취는 24시간회상식이를이용하여정의하였고, 과일과채소섭취횟수는 FFQ 를이용하였고, 음주와흡연에대한정보는건강면접조사를통하여확인하였다. 현재비음주자는현재술을마시지않는다고대답한사람으로정의하였고, 한달에 1회이상술을마신다고대답한사람을현재음주자로정의하였다. 현재비흡연자는현재흡연하지않고있다고대답한사람으로정의하였고, 현재흡연자는현재흡연하고있다고대답한사람으로정의하였다. 그외의독립변수역시국민건강영양조사 4기, 5기에서정보를수집하였다. 나이군 (40 49세, 50 59세, 65세이상 ), 교육수준 ( 초등학교이하, 중학교 / 고등학교, 대학교이상 ), 비만군 ( 체질량지수 23 kg/m 2 미만, kg/m 2, 25 kg/m 2 이상 ) 으로구분하였다. 체질량지수는몸무게 (kg)/ 키 (m) 2 으로계산하였다. 또한, 운동에대한설문을통하여중등도강도의운동은격렬한운동을하루에최소 20분씩 3일이상한경우, 중등도이상의운동을 5일이상 30분이상한경우로정의하였다. 16) 환자의기본적인특성은암경험자와비암경험자로나누어절대적인수 (%) 로나타내었다. 교차비와 95% 신뢰구간을계산하여암경험자가비암경험자에비하여건강한식습관을얼마나지키는지계산하였다. 추가적인혼란변수를보정하기위하여점진적인모델링 (gradual modeling) 을사용하였고, 단변량교차비를구한후모델 1에서는나이군 (49 49세, 50 59세, 65세이상 ) 과성별을보정하였고, 모델 2에서는나이, 성별군, 총칼로리섭취, 비만군 ( 체질량지수 23 kg/ m 2 미만, kg/m 2, 25 kg/m 2 이상 ) 을보정하였으며, 모델 3에서는모델 2에운동, 흡연, 음주, 교육수준을추가하여보정하였다. 모델 3 에서암경험자의음주여부와의상관관계를보기위하여보정할때에는음주를제외한나머지를보정하였고, 흡연여부와의상관관계를보기위하여보정할때에는흡연을제외한나머지를보정하였다. 또한, 이연구에서암진단후 5년미만과 5년이상을나누어분석하여, 암진단받은시점에따른변화에대해서확인하고자하였다. 이연구에서는 STATA software 13.0 (Stata Co., College Station, TX, USA) 을사용하였고, P값 <0.05를통계적으로유의하다고보았으며 P 값 <0.1을한계유의성이있다고정의하였다. 결과 1. 일반적인참여자의특성연구에포함된총 14,832명중 434명이암경험자였다. 이중위암 이 89 명, 간암이 6 명, 대장암이 36 명, 유방암은 71 명, 자궁경부암이 94 명, 폐암이 6 명, 갑상선암이 106 명, 나머지는기타암이었다. Table 1 에암경험자와비암경험자들의기본적인특성에대하여 기술하였다. 암경험자는남성이 104 명으로 23.96% 였고, 비암경험자 의경우남성이 5,077 명으로 35.26% 였다. 나이는암경험자에서는 세가 70 명 (16.13%), 세가 162 명 (37.33%), 65 세이상이 202 명 (46.54%) 이었고, 비암경험자에서는 세가 4,177 명 (29.01%), 세가 5,575 명 (38.72%), 65 세이상이 4,646 명 (32.27%) 이었다. 교육 정도는암경험자에서초등학교이하가 240 명 (55.30%), 중 / 고등학교 가 241 명 (32.49%), 대학교이상이 53 명 (12.21%) 이었다. 현재음주자, 현 재흡연자, 비운동자는암경험자군에서 307 명 (70.74%), 107 명 (24.65%), 236 명 (54.38%) 이었고, 비암경험자군에서 11,374 명 (79.00%), 4,314 명 (29.92%), 7,634 명 (53.02%) 이었다. Unhealthy Eating Habits among Cancer Survivors 2. 건강한식습관여부 Table 2에서는비암경험자에비해암경험자들이건강한식습관을얼마나잘지키는지를로지스틱회귀분석을통해비암경험자와비교한교차비로나타내었다. 단변량분석을시행하였고, 모델 1에서는나이군과성별을보정하였고, 모델 2에서는나이군, 성별, 총칼로리섭취, 비만군을보정하였고, 모델 3에서는이에더하여운동, 흡연, 음주, 교육정도를같이보정하였다. 균형잡힌식이여부는비암경험자에비하여암경험자에서의교차비 (95% 신뢰구간 ) 는단변량분석에서는 0.96 ( ) 였고, 모델 1에서는 1.12 ( ), 모델 2 에서는 1.11 ( ), 모델 3에서는 0.88 ( ) 로모두통계적으로유의하지않았다. 하루 5회이상과일 / 채소섭취여부는암경험자에서비암경험자와비교하였을때교차비 (95% 신뢰구간 ) 가단변량분석에서는 1.00 ( ), 모델 1에서는 1.09 ( ), 모델 2 에서는 1.09 ( ), 모델 3에서는 1.22 ( ) 로통계적인유의성은관찰되지않았다. 지방섭취가총칼로리섭취의 25% 미만인지여부에대하여는암경험자에서비암경험자와비교하였을때교차비 (95% 신뢰구간 ) 가단변량분석에서는 1.28 ( ), 모델 1 에서는 1.27 ( ), 모델 2에서는 1.25 ( ), 모델 3에서는 0.99 ( ) 로확인되었다. 하루나트륨섭취량 2,000 mg 미만여부를확인하였을때단변량분석에서비암경험자에비하여암경 험자군의교차비 (95% 신뢰구간 ) 가 1.34 ( ) 로통계적인유의성이관찰되었으나, 나이와성별을보정한모델 1, 총칼로리섭취와비만군을보정한모델 2, 그외생활습관과교육수준을보정한모델 3에서는모두통계적유의성이관찰되지않았다. 또한, 현재흡연과음주모두단변량분석에서통계적으로유의하게암경험자군이건강한습관을가지는것으로보였으나 ( 비암경험자군에비하여암경험자군의현재비음주자의교차비 [95% 신뢰구간 ]=1.97 [ ], 현재비흡연자의교차비 [95% 신뢰구간 ]=1.66 [ ]) 현재흡연여부는보정이후모델 2, 3, 4, 모두에서통계적인연관성이관찰되지않은반면, 현재음주여부는모델 2, 3, 4 모두통계적으로유의하게암경험자군이술을마시지않는다고대답여부에대한교차비가높았다 ( 교차비 [95% 신뢰구간 ]=1.66 [ ] in 모델 1, 1.41 [ ] in 모델 2, 1.42 [ ] in 모델 3). 3. 진단 5년이내암경험자와 5년이후의암경험자로나누었을때의건강한식습관비교이번연구에서는암경험자를진단 5년이내와 5년이후로나누어건강한식습관여부에차이가있는지를확인하였다. 로지스틱회귀분석을이용하여비암경험자군에비하여 5년이내암경험자 /5년이후암경험자군의교차비와 95% 신뢰구간을구하였다. 결과는 Table 2. Table 3에정리되어있으며, 이번분석에서는나이와성별을보정한모델 1, 나이, 성별, 총칼로리섭취, 비만군, 생활습관및교육수준을보정한모델 3을표에나타내었다. 진단 5년이내의암경험자는비암경험자에비하여모델 3의교차비 (95% 신뢰구간 ) 가균형잡힌식사에서는 0.98 ( ), 하루 5회이상채소 / 과일섭취여부는 0.86 ( ), 지방섭취 25% 미만여부는 1.25 ( ), 나트륨섭취 2,000 mg 미만 0.78 ( ), 현재비음주자 1.20 ( ), 현재비흡연자 1.43 ( ) 으로모두통계적인유의성이관찰되지않았다. 반면 5년이후암경험자에서는비암경험자에비하여모델 3의교차비 (95% 신뢰구간 ) 가균형잡힌식사에서는 0.84 ( ), 하루 5회이상채소 / 과일섭취여부는 1.53 ( ), 지방섭취 25% 미만여부는 0.85 ( ), 현재비흡연자 0.73 ( ) 으로통계적으로유의하지않았던반면, 현재비흡연자는 1.52 ( ) 로통계적으로유의하게 5년이후암경험자에서높았고, 나트륨섭취 2,000 mg 미만 1.44 ( ) 로한계유의성을보였다. 4. 암종별건강한식습관여부암경험자를위암경험자 (89명 ), 대장암경험자 (36명), 유방암경험자 (71명 ), 자궁경부암경험자 (94명), 갑상선암경험자 (106명 ) 로나누어건강한식습관을확인하였다. 나이, 성별, 총칼로리섭취, 비만군, 생활습관및교육수준을보정한모델 3만 Table 4에나타내었다. 비암경험자에비하여대장암경험자에서는균형잡힌식사여부, 현재비흡연자여부의교차비 (95% 신뢰구간 ) 가각각 0.12 ( ), 0.31 ( ) 으로대장암경험자에서비암경험자에비하여균형잡힌식사를덜하고, 현재흡연자가높은것으로보였다. 유방암경험자에서는현재비흡연자의교차비 (95% 신뢰구간 ) 가비암경험자에비하여 2.32 ( ) 로확인되었고, 자궁경부암경험자와갑상선암경험자에서는통계적유의성을확인할수있는생활습관은관찰되지않았다. 고찰이연구에서암경험자는비암경험자와비교하였을때균형잡힌식사, 적정지방의섭취, 나트륨섭취, 과일및채소의섭취는큰차이가없었다. 반면현재흡연의경우단변량분석에서는유의한차이가있는것으로확인되었으나변수들을보정하였을때큰변화는없었다. 이러한특성은 5년이상의암경험자에서더뚜렷하게관찰되었다. 그러나절대적인흡연, 음주의비율은적정수준에달하진못하였고, 이는역시암경험자들이암의재발과같은추가적인병의진행이나만성질환에노출될가능성이높은것임을충분히고려해야할것이다. 이연구의암경험자를분석했을때, 갑상선암경험자가가장많았고그다음으로자궁경부암경험자, 그다음이위암경험자, 유방암경험자, 대장암경험자순이었다. 우리나라의암발생률은갑상선암, 위암, 대장, 폐암, 유방암, 간암, 전립선암순이며, 상대적으로 5년암생존율이높은갑상선암, 유방암, 자궁경부암이이연구에많이포함된것으로생각된다. 17) 특정영양소의섭취가암의전이와관련되어있다는보고는현재까지는없다. 그러나, 미국암학회와미국스포츠의학회에서는채소와과일을하루 5번이상섭취하도록권고하고있다. 14) 채소와과일은그자체에암진행을억제하는여러영양소가포함되어있을뿐만아니라열량이적어적정체중유지에도움이된다. 18) 메타분석에서하루 3 serving 미만의채소 / 과일을먹은군에비하여, 하루 3 5 serving을먹은군과 5 serving 이상을먹은군에서뇌졸중의위험도가낮음을보고하였다. 19) 따라서하루 2 3컵의채소와 1.5 2컵의과일을매일섭취하도록권장하고있다. 14,20) 암경험자의건강한생활습관순응도와관련된논문들에서암경험자가비암경험자에비해생활습관을건강하게가질가능성이높지않다는연구들은지속적으로있어왔다. 2,8-10) 이연구에서는채소및과일섭취횟수를이용하여암경험자와비암경험자를비교하였고, 두군사이에통계 29

Unhealthy Eating Habits among Cancer Survivors 적으로유의한차이가없음을확인하였다. 특히유방암의경우유방암경험자를대상으로한연구에서채소, 과일, 콩곡물의섭취를늘리고단순당, 정제곡물, 동물성식품을적게섭취할경우사망률이낮음을보고하였고, 21) 유방암과대장암환자에서적정체중의유지가권고되고있으나유방암과대장암경험자모두비암경험자와비교했을때 5회이상섭취여부에유의한차이가없었다. 고지방섭취는열량이높아비만을유발하는것으로알려져있다. 지방의섭취와암의관계는명확하지않지만암경험자에서일반적으로포화지방의섭취를제한할것을권고하고있다. 14) 이연구에서는적정지방섭취여부를확인하기위하여총에너지원의 25% 미만인경우를확인하였고, 암경험자와비암경험자사이에유의한차이는발견되지않았다. 균형잡힌식단의경우암경험자에대한기준이따로있지는않다. 한국영양학회에서는일반성인의경우하루에너지원의구성을지방 15% 25%, 탄수화물 55% 70%, 단백질 7% 20% 로할것을권고하고있다. 따라서이에따라균형잡힌식단여부를확인하였을때암경험자와비암경험자사이에통계적인차이는관찰되지않았다. 염분의과다섭취는위암 20) 과관련성이있으며, 고혈압및심혈관질환의위험인자로널리알려져있다. 22) 적정나트륨섭취여부와암경험자 / 비암경험자의차이를확인하였을때두군사이에뚜렷한차이는없었으며, 개별암분석에서위암경험자도특별한차이는관찰되지않았다. 이연구에서는단변량분석상암경험자에서비암경험자에비해현재흡연율은적었으나, 보정인자들을보정한후에통계적인유의성은사라졌다. 현재비음주자는암경험자에서비암경험자보다더높았다. 흡연은암의가장잘알려져있는위험인자의하나로꼽히고있으며, 23) 암의발생뿐아니라암의재발 24) 이나이차암발생 25) 과도연관이있다는보고가있다. 그러나, 이연구에서는흡연의경우혼란변수들을보정한후통계적인유의성이사라지는것으로보이며, 흡연하는암경험자가 24.54% 로확인되었다. 따라서암경험자의건강을위한보다적극적인금연교육및캠페인과정책이필요할것으로생각된다. 암경험자의음주는비암경험자의음주에비해적게관찰되었으며, 통계적인유의성도있었다. 그러나, 음주율역시 70.74% 로높게확인되었다. 이전연구에서흡연과음주는우리나라암경험자에서는비암경험자에비해유의하게낮았으나, 그절대적인비율은여전히높다는보고가있었다 ) 음주역시암으로인한사망률증가, 26) 재발, 27) 이차암발생 28) 과관계있다는보고들이있기때문에금주에대한교육및캠페인이적극적으로필요할것으로생각된다. 이연구는여러개의한계점을갖고있다. 첫번째로국민건강영양 조사의설문이자기기입식설문으로보고바이어스를배제할수없다는점이다. 두번째로단면연구이기때문에명확한인과관계를제시할수없다. 세번째, 24시간식이회상과 FFQ는명확한식이패턴을아는것에한계가있을수있다. 네번째, 암경험자의수가적으며, 개별암의경우경험자의수가더적게관찰되어정확한관계를파악하는데한계가있을수있다. 따라서많은수의인구를대상으로하는연구가필요하다. 다섯번째, 암이매우드문질환으로가중치를두지못하였고, 이로인해우리의데이터가한국인의일반인구를대표했다고하기에부족할수있다. 그러나이연구는암경험자와비암경험자의생활습관중특히식이패턴을분석한연구로의미가있겠다. 또한, 이연구는국가기반의일반인구를대상으로한국민건강영양조사를이용하였다는점에의미를둘수있다. 이연구에서암경험자들의식이패턴, 흡연은비암경험자와큰차이가없었다. 다만, 암경험자에서비암경험자에비해음주는덜하는것으로관찰되었다. 이연구로인하여암경험자의영양에대한추가적인연구의초석이되고, 암경험자의건강증진을위한프로그램의개발에도움이될수있는기반이될수있을것으로기대한다. 요약연구배경 : 암경험자들에게흔히식이습관관리, 금연및금주가권고되고있으나, 이의순응도는높지않은것으로알려져있다. 따라서우리는이연구에서암경험자와비암경험자를비교하여식이습관, 흡연및음주에대해알아보고자한다. 방법 : 국민건강영양조사 4기, 5기에참여한사람중 24시간식이회상, 식품섭취빈도조사및인터뷰에참여한사람을대상으로하여총 14,832명이참여하였다. 이중 434명이암경험자였다. 우리는이연구에서비암경험자와비교하여암경험자의건강한생활습관 ( 균형잡힌식단, 하루 5회이상과일 / 채소섭취, 지방섭취총칼로리의 25% 이내, 나트륨섭취 2,000 mg 이내, 금연여부, 금주여부 ) 과의상관관계를알아보고자한다. 이를위해로지스틱회귀분석을이용하였다. 결과 : 비암경험자와비교하였을때혼란변수보정후에는암경험자의식이습관 ( 균형잡힌식단, 하루 5회이상과일 / 채소섭취, 지방섭취총칼로리의 25% 이내, 나트륨섭취 2,000 mg 이내 ) 에대한교차비가모두통계적으로의미가없었다. 비음주자와비흡연자의교차비는각각 1.42 ( ) 와 0.94 ( ) 였다. 결론 : 이연구에서암경험자에서비음주자는비암경험자에비하여통계적으로유의하게적었다. 그러나현재흡연과건강한식이습 30
