CBRNE disaster has the potential to cause specific physical symptoms and psychological distress in victims ; moreover, various toxic symptoms and carcinogenesis/mutation would be an important issue. Bioterrorism can cause localized outbreaks of infectious disease or pandemic disaster. Somatization as well as posttraumatic stress symptoms and depression are the characteristic psychological symptoms in CBRNE disaster s victims. CBRNE disasters could lead to large-scale public fear and social chaos due to the difficulties involved in verifying the extent of exposure and unfamiliar area to the common people. In the evacuation process, problems associated with adjustment and conflict between victims and residents should be considered. 2 화학, 생물, 방사능사고의정신적영향 집하였다 대표적인 유해화학물질사고 방사능 재난 생물테러 사건에 대해서는 연구 논문 보고서 도서 인터넷 자료 신문 기사 등을 추가로 검색하였다 본론 유해화학 물질 방사성 물질 감염원은 모두 실체를 볼 수 없어 노출 여부와 노출 정도를 알기 어렵고 일반인들에게는 생소한 전문 영역이라는 점에서 많은 불확실성을 내포하고 있다 이와 같은 불확실성은 위험요소를 더욱 과장시켜 비논리적인 추측과 소문을 양산함으로써 사회적 공포와 혼란을 가중시키곤 한다 극대화된 공포와 불안은 집단 히스테리 현상을 불러 일으키기도 하는데 방사성 물질에 노출된 피해자들은 오히려 타 지역에서 심한 배척과 차별에 시달리기도 했다 또한 재난의 범위나 종식이 불분명함에 따라 광범위한 대상자들에서 오심 피로 두통과 같은 신체화 반응이 장기화되는 경향이 있다 유해화학물질누출사고화학물질은 석유화학 정밀화학 전자 반도체 식약품 등 거의 모든 산업분야에서 주요 소재로 사용되고 있으며 국내 산업 구조가 고도화되면서 화학물질 유통량은 꾸준히 증가하고 있다 이와 함께 화학물질의 누출 사고로 인한 환경오염 직업병 등 유해 가능성이 제기되었는데 지난 년 동안 화학사고는 년까지 연평균 건을 유지하다가 년에 건 년에 건 년에는 건으로 최근 수년 사이에 발생추이가 급격히 증가하였으며 화학사고의 가 화학물질 누출 사고이다 년에 발생한 구미 불산 누출사고는 화학사고의 대표적인 사례로 당시 현장에 있던 직원 중 명이 사망하고 명이 부상을 입었으며 대기 중으로 확산된 불산 증기가 주변으로 퍼져 인근 헥타르의 농작물이 고사하고 가축 폐사 차량과 건물 부식으로 약 억원에 달하는 엄청난 경제적 피해가 발생하였다 이 사고로 불산에 노출된 근로자들은 개월이 지난 후에도 가 불안증상을 보였고 는 외상후스트레스장애 위험군에 해당하였으며 지역주민 명 가운데 가 유의한 외상후 스트레스 증상을 보였다 년 태안 기름유출사고 후 지역 주민들을 조사한 결과 이상이 외상후스트레스장애 고위험군에 해당했고 에서 자살사고가 나타나는 등 상당수 지역 주민들의 정신건강문제가 심각한 수준에 달했고 의 주민들은 피해사고와 적대감을 보이는 것으로 나타났다 화학사고는 이처럼 직접적인 누출 뿐 아니라 독성물질 확산 환경오염 등 복합적인 형태로 발생하기 때문에 피 해자의 범위도 사업장 근로자 뿐 아니라 인근 지역의 주민들까지 확대되어야 한다는 것을 알 수 있다 몇몇 연구들에서 화학사고 피해자의 가 유의한 정신적 문제를 갖는 것으로 보고하고 있다 년에 인도 보팔에서 발생한 메틸 이소시안염 유출사고는 명의 사망자가 발생하였는데 사고 직후에 병원에 내원한 사람들 가운데 가 불안신경증 가 우울증 가 적응장애에 해당하는 증상을 보였다 사고 발생 년 후 가스에 노출되었던 명 중 가 불안 가 인지능력 저하 가 우울증상을 호소하여 피해자들의 정신적 후유증이 장기간 지속될 수 있음을 시사하였다 도쿄 지하철 사린가스 테러 피해자들 중 가 외상후 스트레스 장애 가 긴장 가 건망증을 보고하였는데 주목할 점은 사린 가스와 같은 신경독성물질들은 자체적인 유해성만으로도 신경계 정신계 증상을 유발할 수 있기 때문에 노출 정도에 대한 세심한 평가와 연구가 필요하다는 것이다 화학물질은 산업현장에 국한되어 있는 것이 아니라 이미 우리 생활 전반에 깊숙이 침투해있다 국내에서 발생한 가습기 살균제 사고 역시 화학물질로 인해 인체에 위해가 발생한 화학사고로써 가습기 살균제 성분인 로 인해 명에서 폐질환이 발생하였다 정신건강 평가를 시행한 명의 가습기 살균제 피해자 가운데 는 경도 이상의 우울감 는 유의한 외상후 스트레스 증상을 보여서 신체적인 문제와 함께 정신적인 후유증이 지속되고 있음을 알 수 있다 등은 재난의 원인 자연적 인위적 회복 정도 주관적인 통제감 수준이 심리적인 충격에 영향을 미치는 요인이라고 하였다 국내 화학사고의 대부분이 작업자 부주의나 시설관리 미흡 운송차량 사고와 같은 인위적 요인으로 발생하며 짧은 노출만으로도 폐부종 등 치명적인 위해를 일으킬 뿐 아니라 화학물질로 인한 장기적인 합병증이나 환경오염에 의한 이차적인 피해는 쉽게 드러나지 않아서 불안과 두려움과 같은 심리적 반응을 더 많이 유발하는 것으로 생각된다 이는 화학물질에 대한 노출 정도가 클수록 정신과적 증상이 더 증가한다는 기존의 연구 결과들에 의해서도 뒷받침된다 또한 화학사고 피해자들은 신체화 증상을 많이 보이는 특징이 있는데 이들의 신체증상은 화학물질에 대한 노출 정도뿐 아니라 불안 우울 분노와 같은 정신과적 증상과도 연관성을 보여 기질적인 증상과 심인성 증상 모두에 해당할 수 있다는 점을 고려해야 한다 방사능사고흔히 방사성 물질이라 하면 원자력발전이나 핵무기 같은 2 Anxiety and Mood Volume 13, No 1 April, 2017

4 화학, 생물, 방사능사고의정신적영향 거대 산업을 떠올리나 실제로는 과학 의료 산업 군사기관 등에서 널리 사용되고 있어 우리 생활과 밀접한 관계에 있다 실제로 국내에서 방사성 물질 사용이 신고 및 허가된 기관수는 년 현재 곳으로 꾸준히 증가하고 있으며 이중 인 기관이 서울과 경기 인천의 수도권에 밀집되어 있어 사고 발생 시 대규모 인명 피해로 이어질 수 있다 또한 우리나라는 년 고리 호기 가동을 시작으로 월성 영광 울진 네 개 지역에 기의 원자력 발전소를 가동 중이며 원전이 밀집해 있고 설비의 노후화가 진행되고 있어 주의가 필요하다 방사선 노출의 신체적 영향을 보면 단기간에 고선량 방사선에 노출되었을 때 나타나는 급성 방사선 노출 증후군은 수시간에서 수일 내에 오심 구토 설사 발열 전해질 불균형 피부 병변 혈구감소 출혈 등의 증상을 동반하며 심할 경우 쇼크 의식저하 심혈관계 허탈로 수일에서 수주 내에 사망에 이를 수 있다 방사선 노출에 의한 지연 신체 반응은 노출 후 수개월에서 수년 후에 발생하며 혈관 손상 및 세포분열 저해 등에 의해 신장염 폐렴 백내장 불임 중추신경계 장애가 나타날 수 있고 장기적으로는 갑상선암 유방암 뇌종양 백혈병 등의 각종 암 발생 위험을 높일 수 있다 방사능 사고는 신체적 문제 뿐 아니라 개인과 집단의 심각한 정신적 고통을 유발할 수 있다 히로시마 나가사키 원폭 생존자를 대상으로 한 연구에서 가 신경증적 증상을 보였으며 노출 초기에 급성 방사선 증후군 증상이 있었거나 폭탄이 투하된 도심지에 살았거나 근거리에서 노출 된 경우 등 노출 정도가 높을수록 불안 신체화 증상이 많이 나타나 만성적 경과를 보였다 방사선 노출로 인한 후유증은 수년에서 수십년 후에 나타나는 경우가 많고 자손에게도 영향이 갈 수 있다는 특징 때문에 원폭 피해자들의 정신적 고통은 시간이 경과할수록 증가하며 사회적 기능이나 대인관계의 붕괴와 같은 증상이 두드러져 삶의 전반에 영향을 미치게 된다 년의 체르노빌 원자력 발전소 사고 시에는 생활터전의 파괴 피난과정에서의 혼란 사회적 낙인 등으로 인한 피해자들의 이차적 고통 또한 문제가 되었다 이들은 소위 체르노빌레츠 로 불리며 타 지역 사람들이나 의료기관에서도 배척되거나 차별에 시달렸고 경제적 어려움을 겪기도 하였다 방사선 오염지역의 주민들은 에서 우울 불안 에서 신체화 증상을 보이며 비오염지역 주민에 비해 각각 배 배 증상 발생의 위험이 높았고 년 후의 조사에서도 사고 후 피난민들의 가 외상후스트레스 장애 가 주요우울삽화를 겪은 것으로 나타났다 또 오염지역의 주민들은 경제적 어려움이나 미래에 대한 불확실성 사회적 지지가 부족할 때 정신적인 고통의 위험이 높아졌는데 이와 같이 재난 그 자체 뿐 아니라 이후의 혼란스러운 피난 및 재정착과정도 정신건강의 위험요소가 될 수 있다 또한 시간이 지나면서 외상후 스트레스 장애나 우울증은 점차 감소함에도 신체화 증상과 만성적 건강 문제는 지속되는 등 방사선 노출에 대한 불안과 신체적 문제가 스트레스 요소가 되기도 한다 상대적으로 방사선 노출이 많았던 제염 작업에 참여한 인부들은 사고 이후 가 우울증 가 불안장애 가 외상후 스트레스장애를 겪었으며 자살 위험도 같은 시기의 일반인들보다 배 증가했다 또한 이들 인부를 포함한 방사선에 노출된 사람들을 대상으로 한 연구에서는 조현병 발생률이 인구 명당 명으로 일반인의 명에 비해 훨씬 높았으며 뇌파상에서도 이상이 나타나 방사선 노출이 정신신경계에 영향을 미쳐 정신질환의 위험을 높일 가능성을 배제할 수 없다 방사선은 육안이나 감각으로 확인되지 않기 때문에 일반인들이 방사선에 대한 정보가 전무한 상태에서 감지할 수 없는 위험을 마주하게 되면 사회적 공포와 혼란은 더 커질 수 있다 년 미국 원자력 발전소 핵연료 누출 사고는 피폭선량이 매우 적고 알려진 인명 피해는 없었음에도 사고가 알려진 후 주변지역 주민들이 신체증상 불안 우울이나 침습적 사고와 회피 같은 외상후 스트레스 증상에 시달렸으며 집중력 저하나 높은 소변 내 노르에피네프린 코티졸 및 혈압과 심박수의 상승과 같은 스트레스 관련 생리적 증상이 나타나기도 하였다 보이지 않는 적 에 대한 대중의 불안은 개인과 사회 전반에 실제적인 위협이 될 수 있어 방사능 사고 시 정확한 정보 공유와 지속적인 사후 조사 및 지원이 중요하다 생물학적테러생물테러는 계획된 목적을 성취하기 위해 바이러스 세균 곰팡이 독소 등 생물 또는 생물에서 유래된 물질을 사용해 살상이나 질병을 일으키는 행위를 말한다 생물테러의 역사를 보면 과거 전쟁에서 죽은 시체로 식수를 오염시키는 단순한 형태에서 시작해 년 크리미아 전쟁에서 페스트 환자의 사체를 투석기를 이용해 성벽 너머로 던짐으로써 결과적으로 유럽 전역으로 페스트를 확산시키게 되었으며 아메리카 대륙 개척시대의 유럽 군대에 의한 아메리카 원주민 천연두 전파와 같이 인류의 역사에 지대한 영향을 미치기도 하였다 생물무기의 반인류적 위험성으로 인해 년 제네바의정서 및 년 세균학적 및 독소 무기의 개발 생산 비축금지 및 파괴에 관한 협정을 통해 국가적인 생물학적 제제의 사용은 금지되었으나 정치 종교 단체나 개인에 의한 생물 테러나 의심 사고는 꾸준히 있어 왔다 세기 동안 건의 확 4 Anxiety and Mood Volume 13, No 1 April, 2017

7 이다영등 인된 생물학적 제제의 사용이 있었으며 이 중 년 사이에 건의 사례가 발생하였다는 보고도 있어 전세계적인 생물학적 제제 사용반대 경향에도 생물 테러의 위협은 끊이지 않을 것으로 보인다 년 미국의 테러 이후 발생한 탄저균 우편물 공격은 생물테러에 대한 경각심을 일깨우는 사례이다 년 월에 걸쳐 미국 정부기관과 언론사로 개의 탄저균 포자를 포함한 우편물이 배달되었고 이로 인해 명의 사망자와 명의 감염자가 발생하면서 미국 사회에 전염병에 대한 불안이 확산되었다 탄저균에 노출되었던 사람들은 그렇지 않은 사람들보다 불안감이 현저히 높았으며 탄저균이 배달된 미국 의회의 직원들을 대상으로 한 연구에서는 가 테러 이후 우울증 외상후 스트레스장애 알코올 사용장애 등에 해당하는 증상이 새로이 발생한 것으로 나타났다 뿐만 아니라 직접 노출 없이 미디어를 통해 탄저균 소식을 접한 사람들에서도 스트레스나 침습 회피와 같은 외상후 스트레스 증상이 나타났고 많은 사람들이 우편물 개봉을 꺼리거나 항생제 수요가 증가하고 탄저균 오인 신고가 속출하는 등 사회적 혼란을 야기하였다 생물테러는 감염원이 유포된 후 잠복기를 거쳐 증상 발병까지의 시간적 차이가 발생하기 때문에 조기 발견과 오염범위의 확인이 어렵고 이상 징후를 감지했을 시에는 이미 대규모 피해로 번져 있을 가능성이 높아 대중의 공포가 크고 사회적 공황 상태를 유발할 수 있다 발생 초기에는 감염에 대한 걱정이 주요 스트레스 인자로 감염원의 종류 잠복기 치명도 등에 따라 심각성은 달라질 수 있는데 적절한 대처가 이루어지지 않을 시에는 사회 전반에 공포 분노 잘못된 정보와 연관된 감염에 대한 마술적 사고나 과민한 반응 편집증적 사고와 불신 등이 퍼져 혼란을 초래하기도 한다 장기적으로는 지연된 신체반응이나 만성적인 건강문제가 나타나거나 감염원이 생태계 변화를 일으켜 풍토병으로 자리잡아 지속적인 문제로 이어질 수 있으며 발암 기형의 위험을 높일 수 있는데 이렇듯 예측하기 어려운 장기적 위험은 기존 정신질환을 악화 시킬 뿐 아니라 만성적인 스트레스나 불안감 우울증 및 외상후 스트레스 장애의 위험을 높일 수 있다 결론 특수재난은 자연재해나 다른 인적 재난에 비해 발생 빈도는 적지만 초반 대처가 늦어질 경우 피해 범위와 규모가 걷잡을 수 없이 확산되고 원인물질이 주변 환경에 축적되어 피해가 장기화될 수 있다 특수재난과 관련된 불확실성은 대중의 공포와 혼란으로 이어지는데 의 피해자들에서 신체 Table 3. 8 화학, 생물, 방사능사고의정신적영향 화 증상 인지저하 등의 스트레스 반응과 불안장애 우울증 외상후 스트레스 장애와 같은 정신질환이 보고되었다 특수 재난 시에는 신속하고 정확한 정보 공유를 통해 사회적 혼란 을 최소화해야 하며 피해자들에게는 원인 물질의 특성과 인 체 및 자연환경에 미치는 영향을 고려하여 장기간에 걸친 면 밀한 평가와 지원을 제공하는 것이 중요하다 산업의 발전과 더불어 국내에서도 특수 재난의 위험은 점점 높아지고 있으 나 이에 대한 인식은 여전히 부족한 실정이다 현재까지는 국 내의 대규모 특수재난 사례가 적을 뿐 아니라 이에 대한 연구 대부분이 신체적인 영향이나 재난 대응 및 관리 체계에 초점 을 두고 있어 정신적인 영향을 알기가 어렵다 원인 물질에 따 른 신체적 영향은 국가나 문화와 상관없이 거의 동일한데 비 해 특수재난에 대한 심리적 반응은 그 나라의 발전 수준이나 문화에 따라 제 각각일 수 있어 외국의 선행 연구를 참고하는 것도 제한점이 있다 또한 특수재난은 피해범위와 유효기간을 특정 짓기가 어렵고 원인 물질의 완벽한 제거가 거의 불가능 하기 때문에 장기적인 영향을 볼 수 있는 코호트 연구가 반드 시 필요하다 따라서 향후에는 특수재난의 정신적 영향에 대 해 국내의 환경적 문화적 특성을 고려한 장기적인 추적 연구 가 수행 되어야 할 것이다 중심단어 : 특수재난 화학물질사고 생물테러 방사능 외 상 정신적 영향 REFERENCES 1.
