Methods: The intentional self-harm (KCD-6: X60-X84) death data (7,739 deaths: aged 0 years and older) of Korea Statistics from 20 to 205 were used for this analysis. Gender ratio of suicide and 95 percent confidence interval were calculated by the age specific suicide rate and suicide methods. Results: The suicide rate of males (43.5 per population of 00,000) were higher than those of females (9.). The gender ratio of suicide was 2.28 (95% intervals: ). Age specific suicide rate increased with advanced age in both gender. The gender ratio of suicide was highest in aged years (3.38), and lowest in aged 0-9 years (.3). The common methods of suicide was hanging, accounting for 5.3 percent of all suicide deaths. The next most common methods for suicide was jumping from a height (5. percent), self-poisoning by gas (2.0 percent) and pesticides (.4 percent), and drowning (3.5 percent). The gender ratio by suicide methods was considerably high in self-poisoning by gas (4.65), 2.49 for hanging, 2.22 for drowning,.49 for self-poisoning by pesticides, and.25 for jumping from a height. The suicide rates of male were higher than those of female in most of suicide methods and all age groups (0-29 years, years and 60 years), with the exception for self-poisoning by pesticide in age group 0-29 years (gender ratio: 0.8). The gender ratio of self-poisoning by gas and pesticide, hanging, and drowning increased with advanced age, while gender ratio of jumping from a height decreased with advanced age. Conclusions: The gender ratio of suicide increased with advanced age in the majority of suicide methods, with the exception for jumping from a height. There is need to understanding the variations of gender ratio in suicide methods and age group. 2 Sang Hwa Park and Dar Oh Lim (2009년 ) 2.9 [7], 브라질 ( 년) 3.7 [8], 스페인 ( 년 ) 3.53 [9], 북아일랜드 (2005- 년 ) 3.35 [0], 세르비아 (2006-0년 ) 는 2.56 [] 으로자살성비는최소.8에서최대 3.9로국가별로차이가있는것으로보고되었다. 그리고자살성비는시계열적으로변동이있는것으로알려져있는데우리나라의자살성비는 985년 2.9에서 2006년 2.4 [2], 20년 2.6 [3] 으로감소하는것으로보고되었다. 미국의자살성비는 999년 4.45에서 204 년 3.56으로감소하였고 [4], 영국은 966년.5에서 2000년대후반에 3.9로증가 [5], 쿠바는 987년.에서 204 년 3.9 로증가한것으로보고되었다 [6]. 이러한시계열적인자살성비는국가에따라증가또는감소경향을보였다. 과거수십년간자살률의규모를결정하는주요인자는인구의성및연령구조이며 [7], 자살성비는자살방법이나연령에따라서로다른양상을보이는것으로보고된바있다 [3,8,9]. 미국의자살통계 ( ) 에서성에따라연령별자살률의차별양상이존재하는것으로보고되었으며 [20], 남자에서일산화탄소중독, 교수, 추락과같은치명적방법의존율이높은것이자살성비의격차를확산시키는요인이되며 [2], 자살수단의경우고연령남자에서보다치명적인방법에의존하는것으로보고되었다 [22]. 우리나라자살률 (0만명당 ) 은 2000 년 3.6명에서 20년 3.7 명, 204년은 27.3 명으로최근정체수준에있다 [23]. 그리고우리나라 ( 년 ) 의성별, 연령별자살률은남자에서높지만자살률의증가속도는여자에서높은것으로보고된바있다 [24]. 특정집단의자살성비는연령별자살률, 자살수단별자살률의크기에따라영향을받게된다. 우리나라자살률은감소추세에있으나아직높은수준에있고, 그리고인구의고령화현상으로자살규모의성비와같은질적인측면에서검토가필요할것으로보여진다. 본연구는 20-5년우리나라사망통계에서의도적자해에해당하는자살을중심으로연령별, 자살방법별자살률을기초로하여자살성비를비교분석해봄으로써우리나라자살예방중재프로그램에필요한기본적인정보를제공하고자시도하였다. 연구방법 연구자료통계청마이크로데이터서비스 [25] 에서 20-5년사망통계원자료를본연구자료로활용하였다. 원자료에서제공되는사망정보는사망신고일, 사망지역 ( 광역시, 도 ), 사망발생일 ( 시간, 장소 ), 사망원인, 성, 연령, 혼인상태, 직업, 학력등이다. 사망원인에서자살에해당하는의도적자해 (X60-X84) 는비마약성진통제, 마약, 자율신경계통등의약물과약제에의한자의의중독및노출 (X60-X64), 알코올에의한자 의의중독 (X65), 유기용제에의한자의의중독 (X66), 기타가스 ( 일산화탄소등 ) 및휘발성물질에의한자의의중독및노출 (X67), 살충제에의한자의의중독및노출 (X68), 기타상세불명물질에의한고의적자해 (X69) 로구분된다. 그리고목맴, 목졸음및질식에의한의도적자해 (X70), 익사및익수에의한의도적자해 (X7), 총기 ( 권총, 라이플, 엽총등 ), 화기발사, 폭발, 연기및불, 증기등에의한의도적자해 (X7-X75), 예리한물체등 (X77-X79), 높은곳에서뛰어내림에의한의도적자해 (X80), 움직이는물체, 자동차등에의한충돌에의한의도적자해 (X8-X83), 상세불명에의한의도적자해 (X84) 를포함한다. 연구대상원자료에서연도별자살사망수는 20년전체사망자 258,994명중 5,942명, 202년 ( 전체사망자 : 268,980명, 자살사망자 4,62 명 ), 203 년 (268,088명, 4,444명 ), 204년 (269,45 명, 3,867명 ), 205 년에 275,895 명중 3,468명이었으며, 5년간자살사망수는총 7,90명이었다. 또한국외사망자 52명, 연령분류미상 7명, 그리고사망연령이 8-9세경우 3명은통계자료의신뢰도를고려하여연구대상에서제외하였으며그결과최종적으로 7,739 명이집계되었다. 분석방법의도적자해에의한사망률 ( 이하자살률 ) 은 20-5 년 0세이상연령별 (5세계급 ) 주민등록연앙인구 ( 성별, 연령별 ) 를이용하여인구 0 만명당자살률을추정하였다. 0세이상주민등록연앙인구는 20년 45,397,836명, 202년 45,675,607명, 203년 45,95,027명, 204년. 46,57,686 명, 205년 46,357,434 명이었다 [23]. 본연구에서의도적자해 (X60-84) 는자살방법별분포등을고려하여다음과같이재분류하였다. 기타가스및휘발성물질에의한자의의중독및노출 (X67: 이하가스중독 ), 살충제에의한자의의중독및노출 (X68: 이하농약중독 ), 목맴, 목졸음및질식에의한의도적자해 (X70: 이하교수 ), 익사및익수에의한의도적자해 (X7: 이하익수 ), 높은곳에서뛰어내림에의한의도적자해 (X80: 이하추락 ) 로세분화하고, 이를제외한나머지의도적자해수단은기타 (X60-X66, X7-X75, X77-79, X8-X83, X84) 로구분하였다. 본연구에서는연령별, 자살방법별자살률 (0만명당 ) 을이용하여자살성비 ( 남자자살률 / 여자자살률 ) 를비교분석하였고, 여자자살률을비교군 (reference) 으로하여연령별, 자살방법별남자자살률의교차비 (odds ratio) 및 95% 신뢰구간 (confidence interval, CI) 을추정하였다. 원시사망통계자료분석을위해 SPSS 2.0 (IBM Co., Armonk, NY, USA) 프로그램을사용하였다. 4

3 Gender Ratio of Intentional Self-harm Death 연구결과 연도별자살률 (0 만명당 ) 은 20 년 35.0 명에서 205 년 29. 명으로 감소하였고, 남자자살률은 43.5 명, 여자자살률은 9. 명이었고, 연령 별자살률에서는 0-9 세는 4.9 명, 세 4.3 명, 80 세이상은 94.2 명 으로고령에서자살률이높게나타났다. 그리고자살방법분포에서는 교수가 5.3% 를차지하였고, 그다음이추락 (5.%), 가스중독 (2.0%), 농약중독 (.4%), 익수는 3.5% 에불과하였다 (Table ). 연령별자살률은연령이증가함에따라자살률의증가폭은여자보 다남자에서더큰것으로나타났다. 자살성비 ( 남자자살률 / 여자자살 Table. Gender ratio: odds ratio of female death rate to male death rate. *p < 0.05, **p < 0.0. 본연구에서남자자살률 (0 만명당 ) 은 43.5 명, 여자는 9. 명으로 자살성비는 2.28 (95% CI: ) 이었다. 주요국가자살성비는미 국 3.9, 호주 3.3, 일본 2.7 [6], 브라질 3.7 [8]. 북아일랜드 3.35 [0], 말레이 시아 2.9 [7], 영국 3.9 [5], 스페인 3.40 [9] 으로우리나라의자살성비 는비교적낮은것으로나타났다. 자살성비의격차를확산시키는요 인은남자에서치명적인자살방법의존율이높기때문이며 [2], 성별 에따른자살치사율은남자는 23%, 여자는 5% 로보고된바있다 [22]. 자살성비의시계열적인변동에서우리나라는 985 년 2.9, 2006 년 2.4 [2], 20 년 2.6 [3] 으로감소하였다. 미국은 999 년 4.45 에서 204 년 3.56 으로감소 [4], 일본 ( 년 ) 은 2. 에서 2.7 로증가 [2], 영국 은 86 년 3.3, 966 년.5 로최저수준에도달후 990 년대중반부터증 가하여 3.9 로보고되었다 [5]. 스페인 ( 년 ) 은 3.4 수준에정체 현상을보였고 [9], 중국 ( 년 ) 은 0.78 에서 0.9 [26], 쿠바 ( 년 ) 는. 에서 3.9 로증가한것으로보고되었다 [6]. 시계열적인자 살성비의감소현상은남자자살률이상대적으로감소하거나또는여 자자살률의증가속도가상대적으로클때일어날수있다. 우리나라 ( 년 ) 의연령별자살률의증가속도는여자가남자보다큰것으로보고된바있으며 [24], 싱가포르 (989-98년) 의자살성비의증가는여자자살률의감소에기인한것으로보고되었다 [27]. 우리나라자살성비는과거에비해감소하는경향을보이며최근자살성비가 2. 수준에있다. 우리나라의자살성비감소원인을선행연구와본연구결과를기초로추론하기에는원자료의특성상한계점이있으며이부분에좀더체계적인연구가필요할것으로사료된다. 본연구에서연령이증가함에따라남녀모두자살률이증가하였다. 연령별자살치사율의연구에의하면 5-24세의자살치사율은 7%, 60세이상은 34% 이며 [22], 특히 65세이상에서치사율이높다 [28]. 본연구에서연령별자살성비는 0-9세에서.3로가장낮았으며, 연령이증가함에따라자살성비도증가하여 60-69세는 3.38로정점을이룬후감소하여 80세이상의자살성비는 2.75이었다. 청소년기 (5-9 세 ) 의자살성비는미국 (200 년 ) 3.8, 핀란드 (20년 ) 3.6, 일본 (20 년 ).9이었고 [29], 우리나라는 983년 2., 993 년 년.6 202년.2 로보고되었다 [30]. 청년기 (20-29세) 의자살성비는한국.00, 일본 2.23, 호주 4.22, 미국 4.76, 50-59세는각각 3.09, 3.89, 2.98, 3.0, 80세이상은각각 2.3, 2.37, 4.65,.8 로보고된바있다 [6]. 일본 (2006년) 의연령별자살성비는 5-24세.9, 25-44세 2.6, 45-64세 3.7, 65세이상은 2.2로보고되었다 [2]. 본연구에서별도의표로제시하지않았으나자살방법분포에서교수의의존율은남자 52.7%, 여자 48.3%, 가스중독은각각 4.2%, 7.0%, 추락은 2.%, 22.0%, 농약중독은.2%,.7%, 익수는 3.5%, 3.6% 이었으며, 농약중독과익수는성별점유율이비슷한반면, 가스중독, 추락은차이가있었다. 본연구에서교수에의한자살점유율이 50% 를상회하는대표적인자살방법으로교수의경우도구의접근성, 사용법의단순성 [3] 때문에의존율이높다. 자살방법의선택은성, 연령과연관이있으며 [8], 남자는총기, 교수와같은극단적인방법, 여자는약물중독과같은비극단적인방법에의존하는것으로보고된바있다 [4,8,32,33]. 본연구에서자살방법별자살성비는가스중독이 4.65로가장높았으며, 교수 (2.49), 익수 (2.22), 농약중독 (2.9) 순이었고, 추락은.25이었다. 우리나라 (2003-년 ) 중독자살성비는.99에서 2.47로증가, 익수는.43에서 2.0, 교수는 3.63에서 2.36으로감소, 추락은.9에서.20으로변동이없는것으로보고되었다 [3]. 미국 ( 년 ) 60세이상자살성비는총기.6, 교수 4.7, 추락및익수는 2.0으로보고된바있다 [9]. 대만 ( ) 의숯, 가스중독 (charcoal burning) 에의한자살성비는 995년.69에서 2006년 2.28로증가한것으로보고되었다 [34]. 본연구에서농약중독의경우 0-29세의자살성비는 0.8로여 6
