The acquired information about maxillary septum in this study is expected to be of great help for implant treatment, which is a major obstacle for maxillary sinus lift. Materials and Methods: One hundred and fiftyone patients (77 men, 74 women) who visited Hallym University Kangdong Sacred Heart Hospital from January to July 2012 for impacted third molar were examined. The height and length of the septum and angle between the septum and maxillary sinus wall are studied by CBCT. The bottom maxillary sinus was divided into maxillary 1st premolar, 2nd premolar, 1st molar and 2nd molar area. The length, height and angle of the maxillary sinus were measured. pvalues less than 0.05 were considered to be statistically significant. Results: The prevalence of sinus septum was 26.5% (41/151). The mean length of the septum in males and females is mm and mm, respectively. The height of the septum is mm in males and 7.15 mm in females. The angle with the sinus lateral wall is o and that with the basal wall is o. The anatomic location of the septa was distributed in the 2nd molar region (52.8%), 2nd premolar region (24.5%), 1st molar region (18.9%) and 1st and premolar region (3.8%). The loss of the remaining teeth as well as age were not related to the presence of sinus septa. Conclusion: These results will be a good support for sinus elevation in implant installation. However, it is too difficult to accept the notion that age has a positive relationship with the number of septums. Thus, before surgery, taking CT images will provide more information regarding the sinus, including septums. 2 I 서론 상악구치부는예전부터임플란트를통한구강기능의회복이가장어려운부위로여겨졌다. 상악구치부에서의임플란트식립성공을어렵게하는요소로는낮은골밀도 1, 상악동함기화로인한짧은가용골의높이 2,3, 수술을위한접근및시야확보의어려움 4, 그리고다른부위보다큰교합압 5 등이있다. 특히상악동함기화를극복하기위한노력으로상악동골이식은치과임플란트분야에서아주중요한부분을차지하고있다 년, Tatum 6 은골흡수가많이이루어진상악구치부에임플란트를식립하기위해치조정과상악동측벽을통한상악동저거상술을처음으로발표하였다. 그후 Boyne 와 James 7 는상악동측벽의작은골창을통해접근하는술식을보고하였으며, 상악구치부의좋지않은골질을가진경우라도임플란트식립이가능하도록임플란트적응범위를넓히는데공헌하였다. 일반적으로상악동저거상술측방접근법시술시, 상악동중격의존재는골창을형성할때뿐만아니라골창을상악동내로밀어올릴때제한이따르고, 상악동막의천공위험성을직접적으로증가시키게된다. 때문에상악동중격은임플란트와관련된상악동내골재생술시어려움을일으키는주요원인으로임상적중요성이강조되고있다. 기존문헌에따르면, 상악동격벽은총인구중 16~58% 에서존재한다고보고된바있다 8,9. 상악동격벽이존재하는경우보통하나만존재하지만, 드물게는두개, 혹은아주드물게세개까지존재하기도한다고알려져있으며 10,11 격벽은제1대구치와제2대구치부위에대략 50% 정도, 그리고소구치부위에대략 25% 정도위치한다는보고가있다 12. 격벽의방향은보통협설측이기때문에 상악동막거상시큰장해가되지않지만가끔비스듬하게주행하는경우에는정상적인상악동막거상을불가능하게만들기도한다. 상악동내의적절한골이식술을위해주로방사선학적검사로이러한상악동중격및상악동주변구조물들을관찰하는것이가능하다. 이전문헌에서 Krennmair 등 10 은파노라마방사선사진에서상악동중격을관찰할경우잘못진단할확률을 21.5% 로보고하였다. 이결과에따르면결국상악동거상술시가장주요한어려움중하나인상악동중격을정확히진단하기위해서는파노라마방사선사진보다는전산화단층촬영 (computed tomography, CT) 을이용하여 3차원적인영상으로분석하는것이적합하다고볼수있다 13,14. 현재까지상악동중격에관하여국내외로 CT를이용한여러분석연구가이루어져왔다. 이를바탕으로하여이번연구에서는콘빔전산화단층촬영 (conebeam CT, CBCT) 을통해상악동중격의빈도나, 길이, 높이, 각도등의형태를관찰하여일반적인환자변수인나이및성별, 잔존치아같은다양한변수들과의상관관계및유의성을분석하고자한다. 이를통하여상악구치부임플란트식립시에매우중요한변수인상악동중격의특성에대해좀더이해할수있도록하고자하였다. II 연구재료및방법 1. 대상 2012 년 1월부터 2012 년 7월까지한림대학교강동성심병원구강악안면외과에지치발치를목적으로내원하여 CBCT 를촬영한환자를대상으로하였다. 최종적으로총 151명의환자에서 302개의상악동이조사되었다. 조사대상중남자는 77명, 여자 74명이었으며, 연령은 17세에서 대한구강악안면임프란트학회지 17 권 4 호,
3 Original Article 65세로, 평균 27세였다. 임플란트식립이나골이식병력이있는환자의상악동은제외하였다. 2. Imaging and processing CBCT 기기는 Master 3D (Vatech Korea Inc., Seoul, Korea) 를이용하였다. 계측은 Vatech 사의 Ezimplant software (Vatech Korea Inc.) 를이용하여시행하였다. 촬영범위는 cm, cm, cm, 16 7 cm 중에서한가지로선택하였다. 촬영조건은 voxel size: 0.2 mm; gray scale: 14 bits; focal spot: 0.5 mm; section 간격 : 0.2 mm; image detector: flat panel detector; image acquisition: single 360 o rotation 이다. 선택한 CBCT 영상을 Ezimplant software 를이용하여삼차원적으로재구성한후측정하였다. 3. 측정방법 1) 측정위치상악동중격의위치를제1, 2 소구치와제1, 2 대구치 4 개의치아영역으로구분하였다 (Fig. 1). Implantology ) 측정및참고점선택한 CBCT 영상의 sagittal view 와 axial view 상에서상악동중격의길이와높이, 각도를측정한다. 먼저상악동중격의위치를확인한후 sagittal view 에서중격의높이와상악동저와중격이이루는각도를측정한다 (Fig. 2). 상악동중격의높이는중격전후의상악동저의가장 Fig. 2. Implantology 2013 낮은부위를연결한가상의선에서중격의가장높은부위까지의거리를측정하였다. 상악동저와중격이이루는각도는중격이있는부위의상악동저를수평으로지나는가상선을기준으로하였다. 다음은 axial view 에서상악동중격의길이와상악동외측벽과중격이이루는각도를측정하였다 (Fig. 3). 상악동중격의주행길이는상악동중격전후의상악동내벽, 외벽의가장얇은부위를연결한가상의선을중격과내 외벽이만나는지점으로설정하고그지점사이의거리를길이로측정하였다. 상악동의외측벽과중격이이루는각도는중격이있는부위의외측벽을수평으로 III 연구결과 1. 상악동중격의남녀발생빈도조사한 151명의환자중 26.5% 인 40명 ( 남자 19명, 여자 21명 ) 이상악동중격을가지고있었고, 302개의상악동중 17.5% 인 53개 ( 남자 24개, 여자 29개 ) 의상악동에서중격이발견되었다. 또한좌우상악동을합하여상악동중격의개수가하나도없는경우는 73.5% 로 111명, 1개있는경우는 19.2% 로 29명이었다. 2개있는경우는 6% 로 9 대한구강악안면임프란트학회지 17 권 4 호,
5 Original Article 명이었고, 3개인경우는여자만 2명 (1.3%) 있었다. 카이제곱검증결과상악동중격의발생여부와남녀성별과의통계적유의성은없었다 (p=0.510) (Table 1). 길이와높이모두남녀별통계적유의성은없었다. 오직제1대구치영역의중격의높이에서만통계적인유의성을보였다 (p=0.032). 2. 상악동중격의길이와높이상악동중격의평균길이는 mm였고, 남자는평균 mm, 여자는 mm였다. 평균수치를봤을때제 1소구치영역 (1st premolar) 에서제2대구치영역 (2nd molar) 으로갈수록더커지는양상을보였다 (Table 2). 상악동중격의평균높이는 9.11 mm였으며, 남자는평균 mm, 여자는 7.15 mm였다 (Table 3). 6 지는양상을보였으나유의성은없었고, 남녀에따른통계적유의성도보이지않았다. 4. 상악동중격의위치분포중격의위치분포는제1소구치에 2개 (3.8%), 제2소구치에 13개 (24.5%), 제1대구치에 10개 (18.9%), 제2대구치에 28개 (52.8%) 가분포하였다. 통계적분석결과남녀성별간중격의위치차이가유의하지않았다. 그리고 case 별격벽의비율은 total 132.5% 로격벽이있는사람은그수가평균 개정도로연구되었다 (Table 6). 6. 상악동중격이있는경우, 중격의높이와길이, 상악동벽과중격이이루는각도간의상관관계상악동중격의길이가클수록상악동중격과측벽이이루는각이커지는경향을보였다. 또한중격의높이가클수록상악동의길이도더커지는양상을보였다 (Table 8). Implantology 2013 상악동중격에대한빈도나양상에관한연구는해부학적으로나방사선학적으로예전부터많이다루어져왔다 년 Underwood 15 는지금으로부터 100 년전사체의두개골을대상으로 33.3% 의빈도로상악동중격이있음을보고하였다. 최근에는파노라마와 CT 등의영상을이용한연구가많이이루어지고있다 년 Krennmair 16 는 CT 를이용하여 200 개의상악동에서 16% 정도의중격을관찰하였으며, Koymen 등 17 은 2009 년 410개의상악동에서 35.4% 의비율로중격을관찰하였다고보고하였다. 한국인을대상으로한연구에서 2006 년 Kim 등 12 은 CT 영상을이용하여 200개의상악동에서 26.5% 발생비율을보고하였고, 2010 년 Lee 등 18 은마찬가지로 CT 영상에서 236 개의상악동중에 24.6% 정도의중격을발견하였다고 Table 8. 7 Original Article 보고하였다. 상악동중격의발생비율은평균 20~40% 정도로여러연구마다다양한결과를보이며이번연구에서는 151명의환자중 26.5% 인 40명 ( 남자 19명, 여자 21명 ) 이상악동중격을가진것으로관찰되었다. 그리고 302개의상악동중에서 17.5% 인 53개 ( 남자24 개, 여자 29 개 ) 의상악동에서중격이발견되었다. 이와유사한연구에서 2011 년 Chae 등 19 은 204명의환자중 31.4% 인 64명에서상악동내중격이관찰되었고, 408개의상악동중 21.3% 인 87개의상악동에서중격을발견하였다. 이연구의결과보다이번연구에서중격의발견빈도가낮게나온것은이번연구대상의더낮은연령대와결손치가거의없는치열때문인것으로보인다. 중격의발생원인에대해서는여러가지가설이주장되고있다 년 Chanavaz 20 는비어있는상악동을지지하기위해서중격이생겨난것이라고주장하였으며, 1999 년 Krennmair 10 는중격의발생을 1차와 2차로나누어설명하였는데 1차중격발생은중안모의성장시에, 2차중격발생은후천적으로치아탈락후상악골의흡수시에생기는것으로보고하였다. 그리고 1995 년 Ulm 등 21 은상악동함기화가상악동중격발생에영향을끼칠수있다고발표하였다. 한편 1998 년 Oh와 Ryu 22 의연구에서는상악동중격의발생과치아상실이나상악동함기화는관련이없는것으로보인다는발표를하였다. 이번연구결과에서는연령이증가함에따라상악동중격의빈도가낮아지는것으로나타났는데, 사실이러한결과는치아상실후불규칙적인골의흡수나, 상악동함기화가상악동중격의발생원인이라는가설들과는배치되는결과여서좀더연구가필요할것으로보인다. 중격의상악동저에서의평균길이는 mm였고, 남자는평균 mm, 여자는 mm였다. 평균수치를봤을때후방부쪽으로갈수록더길어지는경향을보였다. 이는상악동의부피가후방부로갈수록커져중격의길이도더커지는것으로보인다. 상악동중격의평 균높이는 9.11 mm였고, 남자는평균 mm, 여자는 7.15 mm였다. 평균수치로비교시중격의길이와높이모두남자가높게나타났다. 하지만길이와높이모두남녀별통계적유의성은없었다. 오직제1대구치영역의중격의높이에서만통계적인유의성을보였다 (p=0.032). 이번연구결과를분석해보면, 중격의길이와높이의표준편차에서알수있듯이중격의길이와높이는매우다양한형태와모습을가지고있었다. 상악동중격이상악동벽과이루는각도에대하여이번연구에서는측벽과이루는각도는평균 o 였으며, 남자는 o, 여자는 o 였다. 평균적인수치로비교할때후방부 ( 제2대구치 ) 쪽으로갈수록측벽과이루는각이더커지는것을알수있었다. 그리고상악동중격이상악동저와이루는각도는평균 o 였으며, 남자는 o, 여자는 o 였다. 이수치또한통계적유의성은없었지만, 후방부 ( 제2대구치 ) 쪽으로갈수록상악동저와이루는각이더커지는것으로나타났다. 이같은결과를볼때상악동중격은측벽이나상악동저에대하여평균적으로수직적으로발생한다는것을알수있었으며, 이는상악동거상술기구조작시에좋은참고사항이될것으로생각된다. 보통상악동거상술시격벽이존재하게되면격벽의높이에따라수술방법이변형되게된다. 일반적으로격벽의높이가 5~6 mm를초과하게되면두개의골창을형성하여각각상악동막을거상할것을추천한다. 높이가 2~3 mm 이하라면일반적인골창형성한후상악동막을거상할때에는중격을중심으로앞과뒤의막을각각거상하면서격벽쪽으로접근하여격벽의전방과후방에존재하는상악동막을서로연결하여함께거상한다. 만약격벽의높이가 4~5 mm 정도이면 W형태의골창을형성한다. 이렇게격벽의높이에따라거상술의술식이다르게되는데중격의평균각도와길이를이해하고있으면상악동거상술시이번연구가많은도움이될 264 Implantology Vol. 17 No. 4, 2013
8 것으로여겨진다. 상악동중격발생위치에대해서 Krennmair 10 는상악동전방부에서많이발생한다고하였고, Velásquez Plata 등 11 과 GonzálezSantana 등 23 은상악동의중앙부에서많이발생한다고발표하였다. Underwood 15 는상악동의후방부에중격이많이발생한다고서로상반되는연구결과를발표하였는데이렇게연구들마다다른결과가나타나는것은중격의발생부위에따른기준점이다른것으로여겨진다. 기준점을변화시켜각각의연구결과들을비교해보면대부분의중격이상악동의대구치부에서많이나타난다고설명된다. 이번연구에서확인한상악동중격중 71.7% 의중격이대구치부에서관찰된것으로분석되었고, 이결과는앞서발표된연구결과들과크게다르지않다고보여진다. 상악동중격이있는경우에환자의나이, 중격의높이, 주행길이, 상악동외벽과격벽이이루는사이각, 잔존치아개수간의상관관계를비교했을때, 상악동중격의길이가클수록상악동중격과측벽이이루는각이커지는경향을보였다. 또한중격의높이가클수록상악동의길이도더커지는양상을보였다. 이는 2차원적인영상으로정보를얻을시에좋은참고사항이될것으로보인다. 이번연구에서는상악동중격의발생빈도와형태에대해 CBCT 영상을통해분석하였다. 기존연구들의결과에덧붙여추가적인분석을통해남녀성별에따라중격의형태가다른양상을확인할수있었지만, 통계적유의성을보이진않았다. 또한여러변수와상악동중격과의상호관련성에대해조사하였으나통계적으로유의한관련성은없는것으로나타났다. V 결론 이번연구는상악동거상술시자주관찰되는상악동중격의특성을분석하여상악동거상술시도움이되고자하는목적으로 CBCT 를이용하여측정하였다. 연령에따라중격의수가증가하거나성별등의요인에따라중격의존재여부를예측하기는어려울것으로여겨지며, 많은수의중격이태생적으로발생할수있을것으로생각한다. 그러므로환자나이나다른요인과는상관없이임플란트식립을위한상악동거상술시술전미리 CT 영상으로중격에대한정보를얻고수술을하는것이합병증을줄이고안전하고성공적인임플란트치료를하는데중요할것으로보인다. 참고문헌 1.
