We performed this study to identify predictors of mortality in critically ill ARF patients treated with CRRT. Methods : We analyzed the data of 128 patients who were treated with continuous veno-venous hemofiltration (CVVH) or continuous veno-venous hemodiafiltration (CVVHDF) from May, 2002 to March, We compared the clinical data of survivors with non-survivors. Results : On univariate analyses of prognostic factors of patients treated with CVVHDF, APACHE II scores (p=0.004), prothrombin time (INR) (p=0.033) and the number of inotropics used (p=0.005) were significantly lower in survivors than those of non-survivors. MAP (p=0.027), diastolic BP (p=0.015) and fibrinogen level (p=0.007) were significantly higher in survivors than those of non-survivors. Multivariate analysis revealed that APACHE II scores and fibrinogen level were the independent factors for the prediction of mortality. And on univariate analyses of prognostic factors of patients treated with CVVH, APACHE II scores (p=0.002) and the number of inotropics used (p=0.006) were significantly lower in survivors than in non-survivors. MAP (p=0.03), systolic BP (p=0.02) and diastolic BP (p=0.03) were significantly higher in survivors than in non-survivors. Multivariate analysis also revealed that APACHE II scores was the only independent factor for the prediction of mortality. Conclusion : This study showed that the independent prognostic factor for mortality in ARF patients treated with CRRT was the APACHE II score. 2 Eui Sik Kim, et al. : Prognostic Factors of ARF Patients Treated with CRRT 며, 다른심각한중증질환에대한생존율이늘고있어, 이와동 반된급성신부전증환자들이증가했기때문으로추정된다 3-5). 따라서투석요법의발달과더불어이와같은혈류역동학적으로 불안정한급성신부전증환자들에대해지속적신대치요법 (contineous renal replacement therapy, CRRT) 의적응 증이지속적으로늘어나는추세다. 석 최근까지의여러연구들은 CRRT가통상적인간헐적혈액투 (intermittent hemodialysis, IHD) 에비하여유의한사 망률의감소효과는없었다고보고하였다 6-8). 그러나, 혈류역동 학적안정성, 신기능의회복, 세포활성인자의제거, 생체적합성, 대사성산증이나영양결핍의교정, 용질의제거등에있어서는 CRRT가 IHD에비하여우수하다고알려져있어서급성신부 전증이동반된중환자의투석요법으로최근많이사용하고있다 9). CRRT 는대부분중환자실의혈류역동학적으로불안정한환 자들에게적용되는것으로불량한예후를개선하는데기여하지 만고비용의문제가있어서의료자원을효율적으로이용하기 위해예후인자의확인이필요할것으로생각된다. CRRT 치료를받는급성신부전증환자들에서예후에영향 을주는인자들에관한연구로 Barton 등 10) 은환자들의나이, 인공호흡기사용여부, 치료중사용된승압제의수, 소변양, 혈 중빌리루빈치, 혈중크레아티닌치등이예후와밀접한관련이 있다고보고하였다. Sasaki 등 11) 은나이, 혈중크레아티닌은 예후와연관이없고, 장기부전의개수증가, APACHE II 점수 의상승, 간부전의동반, 동맥혈유산 (lactate) 치의상승등이 예후와유의한연관이있다는보고를하였다. 또한 Van Bom- mel 등 2) 은 APACHE II 점수와, 승압제의사용, 호흡기의사 용, 패혈증이예후와연관이있다는보고를하여예후인자들이 서로일치되지않는결과를보이는부분이많이있었다. 국내에서는이에대한연구들이거의없었고 Park 등 12) 이 Cleveland Clinic Foundation Score 와수축기혈압을바탕 으로예후를추정하여적극적인 CRRT의시행이필요하다고보 고한바있다. 이에저자들은중환자실에서급성신부전증에대 하여 CVVH 또는 CVVHDF 등 CRRT 치료를받은환자들을 대상으로투석종료시생존에미치는각각의예후에영향을주 는인자들을분석하고자하였다. 1. 대상 대상및방법 급성신부전증으로중환자실에서 CRRT 치료를받은 128명 을대상으로했다. 이중지속적정정맥혈액여과법 (continuous veno-venous hemofiltration, CVVH) 을이용한환자 는 2002년 4월부터 2007년 11월까지 48 명이었으며, 지속적 정정맥혈액여과투석법 (continuous veno-venous hemo- diafiltration, CVVHDF) 를이용한환자는 2005년 2월부터 2008년 3월까지 80 명이었다. 혈청크레아티닌치가 50% 이상증가하고소변량이하루 400 ml 이하로감소하는경우를급성신부전증으로정의하였고 CKD IV 이상이거나투석중인만성신부전증환자들은제외하 였다. CRRT는대상환자들의전신상태가불량하고혈류역동학 적으로불안정하며혈청크레아티닌, BUN의증가와함께전해 질불균형이교정이되지않거나, 폐부종과같은수분저류에대 해이뇨제투여해도핍뇨가지속될때를적응증으로하였다. CVVHDF 및 CVVH 치료의적응증은별도로정하지않았으며 해당장비의사용가능여부에따라서결정하였다. 2. 방법 대상환자들의분석방법은의무기록을후향적으로조사하였 다. 대상환자들의급성신부전증의원인질환, CRRT 시작당 시의혈압, 맥박수와체온등의활력징후를조사하였으며, 혈중 요소질소 (BUN), 크레아티닌, 알부민, 섬유소원 (fibrinogen), 전해질, FDP (Fibrinogen Degradation Products), 백혈구, 혈소판과동맥혈가스검사등의검사결과들을평가하였다. 이를 바탕으로환자의중증도평가는 CRRT 시작시점의 APACHE II score 6) 를측정하여비교분석하였다 (Table 1). 혈액투석을위한일시적혈로 (vascular access) 는통상적 인 modified Seldinger 방법으로내경정맥이나대퇴정맥을이 용한 dual luminal catheter 를유치하여 CVVH 또는 CVVHDF 를시행하였다. CVVH는 AK-10 혈액투석기 (Gambro, Sweden) 의혈류펌프를이용하였고, 투석막은 HF 6S (Gam- bro, Sweden) 를사용하였다. 그리고 CVVHDF는 PRISMA (Gambro, Sweden), 투석막은 M60 (surface area 0.6 m 2 ) 또는 HF1000 (surface area 1.15 m 2 ) 을사용하였다. 3. 통계분석 대상환자들에서 CRRT 방법으로 CVVH치료를받았던환자 군과 CVVHDF 치료를받은환자군을나누어서각군에대한 예후인자들을비교분석하였다. 데이터는평균값과표준편차 (Mean± SD) 를사용하여표기하였다. 통계분석은 CVVH와

CVVH와 CVVHDF간의생존기간에대한비교는생 명표법 (Life-table 분석은 SPSS 프로그램 analysis) 를통하여분석하였다. 데이터 (SPSS, version 12.0 for win- dows) 을이용하였고통계적인유의수준은 p 값 0.05 미만으 로하였다. 결 과 Table 2. CRRT의평균치료기간은 4.9일로최소 1일에서최대 28일 이었다. 동반된질환으로당뇨 44 명 (34.4%), 고혈압 30명 (23.4%), 심혈관질환은 9 명 (7%) 이었다. 전체대상환자들의 급성신부전증의원인으로내과적질환에의한경우가 90명 (76.7%), 수술과연관되어발생한경우는 12 명 (9.3%), 외 상과관련된경우가 18 명 (14.0%) 이었다. 전체대상환자중 질환별급성신부전증의원인은패혈증 80 명 (62.5%), 외상 18 명 (14.0%), 수술 12 명 (9.3%), 급성심근경색증 11명 (8.5%), 약물중독 3 명 (2.3%), 횡문근융해증 4 명 (3.1%) 등이었다 (Table 2). CVVHDF 로치료받은급성신부전증환자들의평균연령은 63세로 CVVH 군의 56.3 세보다많았다 (p=0.013). CRRT 치료기간은 CVVHDF로치료받은군이평균 5.4일이었고 CVVH 군은 4.0일이어서양군사이에통계적으로유의한차 이는없었다 (p=0.08). 두가지치료군간에남녀비의차이는 없었다. CVVH로치료받은군과 CVVHDF로치료받은군사 이에급성신부전증의원인별차이는없었다. APACHE II

4 Eui Sik Kim, et al. : Prognostic Factors of ARF Patients Treated with CRRT score는 CVVHDF로치료받은환자들이평균 28.7로 CVVH 로치료받은환자군의 31.1 에비하여유의하게낮았다 (p= 0.025). 수축기혈압, 이완기혈압및평균혈압은모두 CVVHDF 로치료받았던군에서 CVVH 군보다유의하게높았다 (p= 0.009, p=0.008, p=0.005). 그러나치료중사용된승압제 수는두군간에차이가없었다 (Table 3). 2. CVVHDF 치료군의결과및예후인자 128명의전체대상환자중 CVVHDF 방법으로치료받은환 자는 80 명이었고이중 CRRT 치료중에사망한환자들은 46 명 (57.5%) 이었으며, 치료종료시점까지생존한환자들은 34 명 (42.5%) 이었다. CVVHDF 치료기간내사망한환자들 에서 APACHE II score는 30.2± 5.0이었고치료종료시점 까지생존한환자들에서는 26.7± 5.2로생존한환자들에서유 의하게낮았다 (p=0.004). MAP는 CVVHDF 치료기간내 사망한환자들이 67.5± 13.5 mmhg였고 CVVHDF 치료종 료시까지생존한환자들에서는 74.1± 12.1 mmhg로생존한 환자들에서유의하게높았다 (p=0.027). 치료기간중사용된 승압제의수는 CVVHDF 치료종료시점까지생존한환자들에 서 2.0± 1.1로투석기간내사망한환자들의 2.8± 1.3에비해 유의하게낮았다 (p=0.005). 혈청섬유소원은 CVVHDF 치 료기간내사망한환자들에서 300.1± mg/dl이었고생 존한환자들에서는 440.4± mg/dl로 CVVHDF 치료 종료시까지생존한환자들에서유의하게높았다 (p=0.007). Prothrombin time (INR) 은 CVVHDF 치료기간내사망한 환자에서 2.0± 1.5에비해투석종료시까지생존한환자들에서 는 1.5± 0.5로투석종료시까지생존한환자들에서유의하게 낮았다 (p=0.033). 그러나 BUN 치, 혈청크레아티닌치, 알부 민치및 (Table 4). FDP는생존군과사망군사이에유의한차이가없었다 두집단간의차이를보이는예후인자들사이의연관성을배 제하고사망률에실제적인영향을미치는인자를확인하기위해 로지스틱회귀분석을시행하였을때 APACHE II score (Odds ratio=1.19, 95% confidence interval , p=0.034) 와혈청섬유소원 (Odds ratio=0.99, 95% confidence interval , p=0.015) 치가각각독립 적으로환자들의생존에영향을미치는인자임을확인하였다 (Table 6). 3. CVVH 치료군의결과및예후인자 전체대상환자중 CVVH 방법으로치료받은환자들은모두 48명이었으며 CVVH 치료중에사망한환자들은 38 명 (79 Table 3. APACHE II score는 CVVH 치료종료 시까지생존한환자들에서 24.5± 6이었고 CVVH 치료중사 망한환자들에서는 32± 5.6으로생존한환자들에서유의하게 낮았다 (p=0.002). MAP는 CVVH 치료중사망한환자들이 60.6± 14.0 mmhg 이었고 CVVH 치료종료시점까지생존 한환자들에서는 71.6± 15.7 mmhg로생존한환자들에서유 의하게높았다 (p=0.03). 치료기간중사용된승압제수는투 석종료시까지생존한환자들에서 1.7± 1.4개였고 CVVH 치 료기간내사망한환자들의 2.8± 1.0에비해유의하게낮았다 (p=0.006). BUN, 혈청크레아티닌치, prothrombin time (INR), 알부민치및 FDP 는두군간에유의한차이를보이지않았다 (Table 5). CVVH로치료받은환자들의사망과관련된예후인자들에대한로지스틱회귀분석에서는 APACHE II score (Odds ratio=1.33, 95% confidence interval , p= 0.015) 만이독립적으로생존율에영향을미치는인자임을확인할수있었다 (Table 7).

CVVH와 CVVHDF 의생존율비교 전체대상환자중 CVVH 방법으로치료받은환자군과 CVVHDF 로치료받은환자군간의생존율을생존함수를통해 비교분석했을때, 시간의경과에따른생존율은두치료군사이 에통계적으로유의한차이를보이지않았다 (p=0.054) (Fig. 1). 고 찰 신부전증에대한치료로 CRRT는 1977년 Kramer 등 13) 에 의해처음으로지속적동정맥혈액여과법 (continuous arteriovenous hemofiltration, CAVH) 이소개된이후단점에 대한보완을거듭해오면서현재중증의급성신부전증환자들에 대한치료로임상에서널리사용되고있다. 환자혈장으로부터 용질의제거는대류 (convection), 확산 (diffusion) 및흡착 (adsorption) 의방법들을통해제거될수있다 14). 지속적정 정맥혈액여과법 (continuous veno-venous hemofiltra- Fig. 1. 7 The Korean Journal of Nephrology 2010;29:54~63 tinuous veno-venous hemodiafiltration, CVVHDF) 은 대류를통한용질의제거방법과함께투석액을혈류의반대방향 으로흐르게하면서필터를통한확산작용을추가로작용하도록 하는방법이다. 급성신부전증환자들의치료방법으로간헐적혈액투석 (intermittent hemodialysis, IHD) 에대한지속적신대치 요법 (Continuous renal replacement therapy, CRRT) 의 장점으로첫째, 치료중혈류역동학적상태를보다안정되게유 지할수있고, 둘째, 축적된전해질, 수분, 요독물질을보다지속 적으로일정하게제거할수있으며, 셋째, 비경구영양주입을위 시한다량의수분을제한없이공급할수있고, 넷째, 패혈증 시유리되는각종사이토카인을위시한염증매개물질의제거 가우수한점등으로인해중환자실의급성신부전증의치료로 CRRT 가우선적으로선호되고있다 15). 그러나 CRRT는 IHD 에비하여항응고제를지속적으로사용해야되므로출혈의위험 요소가있으며고비용의치료비부담과장시간에걸쳐환자가 움직일수없는문제점들이있다 2). 따라서비교적활력징후가 안정적인급성신부전증환자들에서는통상적인 IHD 방법이이 용된다. CRRT가필요한급성신부전증환자들의특성은활력 징후가불안정한환자들로여러장기들의기능부전을동반한경 우가많으며, 이러한다발성장기부전은주로패혈증에기인한 다. Guerin 등 16) 의연구에의하면다발성장기부전의장기로 가장흔한것이호흡부전 질환 (80.6%) 이며, 다음으로급성심혈관 (60.1%) 이흔한데이런이유로혈류역동학적으로유리한 CRRT 방법이우선적으로고려되는신대치요법으로선호된다 고하였다. 이러한이론적인장점에도불구하고 CRRT가 IHD보다생존 율의향상을가져오는지에대해선아직논란이많다. 많은연구 들에서두치료방법간에생존율에차이가없었던것으로조사되 었다 2, 17). 그러나 Kresse 등 1) 은 CRRT 그룹에서사망률이 더높았지만, 질병의심각도정도가높은환자에서 IHD 에비 하여 CRRT 를더선호하는오류를지적하였다. Kellum 등 18) 은 APACHE II score 로질환의중등도를분석하였을때, CRRT 를시행한그룹에서사망률이더낮게나타났다고보고한 바있다. 최근급성신부전증환자들의치료방법으로 IHD, CRRT 외에이들의장점과단점들을상호보완한 slow low efficient daily dialysis (SLEDD) 가제안되고있다. SLEDD는전통 적인투석기를이용하여혈액과투석액의속도를낮추어서하루 에 6시간내지 12 시간동안시행하는방법이다. CRRT와비교 한연구에서상대적으로저렴한비용으로 CRRT와비슷한정도 의요독물질제거의효과가있었으며, 항응고제사용을줄일수 있어서이에대한출혈의위험도감소시킬수있다고보고하였 다 19). 이에대해 Kim 등 20) 은 SLED (sustained low-effi- ciency dialysis) 와 CRRT를비교한연구에서기술적으로쉬 우며, 저비용, 저노동집약적이며환자의신체제약이더적은장 점등을보고하였다. 급성신부전증환자들에서신대치요법의적절한시작시점에 대하여는논란이많다. Bouman 등 21) 의 106명을대상으로한 연구에서는 6시간동안소변량이점차감소하여시간당 30 ml 이내로감소했을때부터투석을시행한경우를조기투석으로 정의하였으나이러한조기투석치료가환자들의사망률에영향을 미치지는못한다는결과를보고하였다. Palevsky 등 22) 은 BUN 치를기준으로조기투석치료 투석치료 ( mg/dl 이내) 와후기 ( mg/dl 이상) 으로나누어사망률을비교 한여러연구들을관찰하면서혈중 BUN치는사람마다생성되 는정도가일정하지않고, 체내분포도개인마다달라서신뢰할 만한근거가되지않는다고지적하면서급성신부전증의경과를 예측할수있는표식자 야한다고제안하였다. (biomarker) 에대한연구가우선되어 패혈증환자들에서신장의기능과상관없이혈액투석치료를 제안하기도하는데이는패혈증상태에서다발성장기부전에 이르게하는다양한염증성매개물질을제거하기위함이다. 특히 Interleukin-6 (IL-6) 와 tumor necrosis factor-alpha (TNF- α) 가패혈증및패혈성쇼크에중요한역할을하고, 이 는생존율과의미있는연관이있다고한다 23). 특히불응성패 혈증의경우패혈성쇼크에대한치료로초기에도파민그리고 노르아드레날린을사용해야했던경우는 85% 의사망률을보였 다 24). 또다른보고에선패혈증환자에서 3개이상의장기부전 을보이는경우에는사망률이 92% 로높았다 25). 유산혈증 (lac- tic acidosis) 의기간은다발성장기부전과연관성이있으며, 초기에유산을제거하는것이패혈성쇼크의예후를향상시킨다 는연구도있다 26). Kellum 등 18) 은중환자실의급성신부전증환자에서투석방 법에따른예후를종합분석한연구들중 1,400명이상의환자 들을포함한 13개논문을분석한연구에서 IHD와 CRRT를시 행한두그룹간에전체생존율에유의한차이가없었으나 APACHE II score로질환의중증도를보정할경우 IHD군에 비해 CRRT 군에서생존율이유의하게높았다고보고하였다. 급 성신부전증환자들의예후는다양한변수에의해서영향을받 기때문에예후인자사이의정확도에대한비교와예측이매우 어렵다. 다양한연구들에서환자들의예후와관련하여 risk,

8 Eui Sik Kim, et al. : Prognostic Factors of ARF Patients Treated with CRRT injury, failure loss and end kidney disease (RIFLE) classification, the acute physiology and chronic health evaluation (APACHE) II scoring system, the SAPS II scoring system, the mortality prediction model (MPM), the sepsis-related organ assessment (SOFA), 그리고 the logistic organ dysfunction system (LODS) 등과같 은지표들이비교되었다 21, 27). 본연구에서는대상환자의평균동맥압, APACHE II score 를조사하였을때, 사망군과생존군간의유의한차이를보였다. 대상환자의동반된질환을객관적으로비교하기는어려웠으나, 평균동맥압, APACHE II score는생체징후및혈액검사결 과등을반영하여간접적으로환자의중한정도를평가할수있 으므로이를통해예후를예측하는인자로사용할수있을것으 로생각된다. Kresse 등 1) 은급성신부전증환자들에서 CRRT 와 IHD간의사망률을비교하여불안정한혈류역동학적상태를 가진환자에서사망률이높다고하며, 조기에신대치요법을시 행할것을언급하였다. Swartz 등 6) 도 CRRT를받은환자군 에서사망률이높았던것은동반된질환의중한정도에영향을 받는다고하였다. 본연구에서섬유소원은 CVVHDF 생존군에서의미있는결 과를보였지만, prothrombin time (INR) 은단변량분석에서 만유의한차이를보였다. 혈액응고상태를 FDP, 섬유소원, pro- thrombin time 으로평가할수있으며, 이를간기능의간접적 인평가수단으로볼때간기능부전이동반된환자에서사망률 이증가한다는연구결과에도합당하였다. Sasaki 등 11) 은장기 부전의개수, APACHE II scores, 유산치등의영향외에특 히간부전이동반된경우급성신부전증환자의예후를악화시 킨다고하였다. 불안정한혈류역동학적상태를호전시키기위한 승압제의사용개수도생존군에서유의하게적은것도일부연 구결과와일치하였다 2, 10). 급성신부전증환자의 CRRT 치료에있어서가장중요한예 후인자로는투석당시의혈류역동학적인상태이다 12, 28) 이에. 대한평가방법으로 APACHE II scoring system을이용하여 생존율을예측할수있으며, 혈류역동학적으로양호한시기에치 료를시작하는것이생존율을높일수있는방법으로생각된다. 본연구에서도 CVVH 또는 CVVHDF로치료받은대상환자들 에서 인하였다. APACHE II scores는예후와관련된독립인자임을확 그러나본연구는단일기관연구이며후향적인연구로각대 상환자들에서 CRRT를명확한기준에따라표준화하지못하여 임상적상황에따른선택의오류의가능성을배제할수없으며, 대상환자가충분히많지않기때문에앞으로예후인자분석을 위해서는보다많은환자들을대상으로한전향적인연구들이 필요할것으로생각된다. 요 목적 : 지속적신대치요법 약 (CRRT) 은혈류역동학적으로불 안정한급성신부전증의치료에있어서간헐적인혈액투석치료 (IHD) 보다유리한장점들이알려져있다. 저자들은 CRRT로 치료받은급성신부전증환자들의생존에영향을미치는예후 인자들을분석하기위해본연구를시행하였다. 방법 : 중환자실에서 2002년 4월부터 2008년 3월까지 CVVH를이용하여치료받은환자 48명과 CVVHDF로치료받 은 80명을포함한총 128명의급성신부전증환자들을대상으 로성별, 나이, 원인질환, APACH II score, 혈액검사결과를 조사하였다. 또한 CVVH또는 CVVHDF 치료종료시의생존 여부에영향을미치는각각의인자들에대해서비교분석하였다. 결과 : CRRT로치료받은전체대상환자 128명중남자가 83 명 (64.8%), 여자가 45 명 (35.2%) 이고, 평균연령은 60.5± 13.8 세였다. 급성신부전증의원인으로는패혈증 80명 (62.5%), 외상 18 명 (14.0%), 수술 12 명 (9.3%), 급성심 근경색증 11 명 (8.5%), 약물중독 3 명 (2.3%), 횡문근융해 증 4 명 (3.1%) 등이었다. CVVHDF로치료받은환자들의예 후인자에대한단변량분석에서, APACHE II scores (p= 0.004), prothrombin time (INR) (p=0.033) 압제수 그리고승 (p=0.005) 가사망군에비해생존군에서유의하게낮 았으며, MAP (p=0.027), 이완기혈압 (p=0.015) 그리고 혈청섬유소원치 (p=0.007) 는생존군에서유의하게높았다. 이를다변량분석을하였을때, APACHE II scores (Odds ratio=1.19, 95% confidence interval , p= 0.034) 와혈청섬유소원치 (Odds ratio=0.99, 95% confidence interval , p=0.015) 가각각독립적으로 생존에영향을미치는인자임을확인할수있었다. 한편 CVVH 로치료받은환자들의예후인자에대한단변량분석에서 APACHE II scores (p=0.002) 와이뇨제수 (p=0.006) 가사망군에비해생존군에서유의하게낮았으며, MAP (p= 0.03), 수축기혈압 (p=0.02) 그리고이완기혈압 (p=0.03) 이생존군에서유의하게높았다. 이를다변량분석에서는 APACHE II scores (Odds ratio=1.33, 95% confidence interval , p=0.015) 만이독립적으로생존에영 향을미치는인자임을확인할수있었다.
