dle meningeal artery)이 손상되지 않게 조심스럽게 타원형 의 골편을 만들어 개두술을 시행한다. 경막이 노출되면 중간 경막동맥이 손상되지 않게 중간경막동맥의 경로에 따라 종으 로 길게 경막을 절개하고 절개면을 따라 다시 수직으로 5mm 길이로 잎사귀 모양으로 절개를 한다.

4 허승호 나영신 손문준 김정훈 노성우 김창진 권 양 임승철 이정교 권병덕 도로 두피판과 측두근(temporalis muscle)의 근막으로부터 하여 벌리고 개두술을 시행하여 경막을 노출시킨다. 경막을 박리한 후 측두근을 절개하여 두개골로부터 박리하고 측두 중간경막동맥이 손상되지 않게 종으로 절개하여 대뇌 피질표 근피판(muscle flap)을 만들어 측두부 하방으로 젖힌다. 이 면을 노출시킨 후 천측두동맥을 포함한 주위조직을 경막과 후에 박리된 천측두동맥과 골피판 밑의 중간경막동맥(mid- 봉합한 후 골편을 부착하였다. dle meningeal artery)이 손상되지 않게 조심스럽게 타원형 의 골편을 만들어 개두술을 시행한다. 경막이 노출되면 중간 경막동맥이 손상되지 않게 중간경막동맥의 경로에 따라 종으 로 길게 경막을 절개하고 절개면을 따라 다시 수직으로 5mm 길이로 잎사귀 모양으로 절개를 한다. 초기에는 대뇌 피질 이 노출되면 피질의 혈관들이 손상되지 않도록 주의하면서 조심스럽게 지주막을 열어 연막면을 노출하였으나 뇌척수액 의 유출이 많아져서 뇌표면이 가라앉으면서 경막과 대뇌 피 질사이의 빈 공간이 커지는 단점과 수술후 상처부위의 가성 수막류를 형성하는 경우가 있어서 최근에는 지주막을 절개 하지 않고 보존하였다(Fig. 2). 이후 박리된 천측두동맥의 주 위조직을 절개된 경막에 봉합하여 천측두동맥이 대뇌피질에 직접 접촉하도록 해준다. 또한 잎사귀 모양으로 절개된 경막 을 접어서 안쪽으로 말아 넣고 경막의 바깥쪽면이 경막 아래 로 접히도록 하여 뇌피질부위와 접촉하도록 한후 근피판을 접혀진 경막의 절개면에 따라 노출된 대뇌피질과 접촉되도록 주변 경막의 절개면을 따라서 봉합하고 뇌척수액의 유출을 최 소화하기 위하여 fibrin glue를 사용하여 봉합 부위를 접착 시킨다. 근피판의 근위부와 천측두동맥의 원위 및 근위부 pedicle이 압박되지 않도록 골편을 다시 부착하고 수술부위의 배 5. 치료결과 대상환자중 EDAMS수술은 16명에서 18부위에 시행되었 고, 직접 혈관문합술인 EIAB를 시행한 경우는 7명의 환자에 서 12부위였으며, EDAS를 시행한 환자는 2명으로 2부위의 시술이 시행되었다. 환자의 추적기간은 3에서 39개월로 평 균 16개월이었다. EDAMS를 시행받은 환자중 7명에서 평균 10개월 후 추 적뇌혈관 조영술을 시행한 결과 재혈관화를 보인 정도는 다 음과 같다. 개두술을 한 전체 부위중 약 2/3정도의 부위에서 재혈관화를 나타낸 환자는 4명이었으며, 1/3에서 2/3의 재 혈관화를 보인 경우는 2명이었고, 1/3미만의 재혈관화를 보 인 환자는 1명이었다(Table 3). 수술후에 시행한 뇌관류에 대한 핵의학검사(99mTc perfusion SPECT)는 대상환자 중 15명에서 시행하였는데, EDAMS를 시행받았던 10명의 환 자에서는 5명에서 뇌관류가 증가하였으며, 4명에서는 수술 전과 차이가 없었으며, 1명에서는 오히려 감소하였다.
