We aimed to evaluate the clinical practice patterns of gastroenterologists for initial and maintenance therapy of GERD in Korea. Methods: We administered a nationwide, multi-center, and questionnaire-based online survey between December 2007 and January The questionnaire consisted of 15 questions about prescribing patterns of initial and maintenance therapy for GERD. Results: A total of 371 gastroenterologists participated in the survey with the response rate of 77%. For mild cases of GERD, the most common choice of initial therapy was full dose proton-pump inhibitors (PPIs) (59%), followed by half dose PPIs (20%), and H 2 receptor antagonists (4%). For severe cases, full dose PPIs were prescribed in 99%. Almost all gastroenterologists agreed to the need for maintenance therapy. For both mild (95%) and severe (99%) cases of GERD, gastroenterologists preferred the use of PPI-based maintenance regimen. The preferred maintenance strategy for GERD was continuous therapy in erosive esophagitis (67%), and on-demand therapy in non-erosive reflux disease (68%). The overall duration of the therapy (initial+maintenance) was 7.7±5.1 wk in mild cases and 15.0±9.4 wk in severe cases. The duration of maintenance therapy was affected by symptom severity, followed by symptom frequency and endoscopic finding. Conclusions: In this study, a majority of the gastroenterologists is aware of importance on PPI-based pharmacological treatment for GERD. Further studies are needed to clarify the appropriate strategy and duration of maintenance therapy. 2 정수진외 4 인. 위식도역류질환의약물치료에관한설문조사 365 서론위식도역류질환 (gastroesophageal reflux disease) 은위산이식도로역류됨으로써증상이나점막손상이발생하는경우로정의된다. 1 동양에서는서구와대조적으로유병률이낮고, 중증의미란성식도염이나이에따른식도협착, 바렛식도등의합병증도드물다. 최근식생활양식의서구화와더불어국내에서도위식도역류질환의유병률은증가하는추세로 2005년에아산시지역주민을대상으로한표본인구설문조사에서 3.5% 로보고되었고, 년발표된건강검진수진자대상의전국적다기관공동연구에서는미란성식도염과비미란성위식도역류질환의유병률이각각 8%, 5% 로조사되었다. 3 가슴쓰림 (heartburn) 과산역류 (acid regurgitation) 등의역류증상은삶의질을저하시키는중요한건강문제로, 4 역류성식도염뿐만아니라비미란성역류질환 (nonerosive reflux disease) 에서도만성재발성경과를보인다. 5,6 따라서, 초치료로증상이소실되고식도염이치유되더라도재발방지를위한장기간의유지치료가필요하며, 1,7-10 이에따른개인의경제적손실및사회적으로막대한의료자원의소모가초래된다. 위식도역류질환에서유지치료의목표는환자의증상과식도염의재발을방지하여궁극적으로는합병증을예방하는것으로, 이에대해국내외적으로다양한가이드라인이제시되었다. 이번연구는전국 2, 3차의료기관소화기내과전문의를대상으로위식도역류질환의초치료및유지치료에대한최근의인식과실제임상에서의적용양상을알아보고자하였다. 대상및방법 2007년 12월 5일부터 2008년 1월 9일사이 1개월동안전국의 2, 3차의료기관소화기내과전문의 475명을대상으로하였다. 정부통계자료의인구연령비를고려하여지역별대상자수를할당하였고, 설문조사시행전설문자는직접방문또는전화로설문대상자에게조사목적을설명하고참가를요청하여구두로동의를얻은후에진행하였다. 인터넷웹페이지에있는설문문항에답변하는온라인설문조사형식으로하였다. 설문지는 2004년에발표된아시아- 태평양지역 FAST survey 11 에서의유지치료에대한항목을근거로우리나라실정에맞도록개발하였고, 본연구진이수차례토의하여적정함을확인하였다. 설문내용은위식도역류질환의초치료방법및기간, 유지치료방법및기간, 총치료기간및치료기간선택에영향을미치는요소등총 11문항으로구성하였다 ( 부록 : 설문지전문 ). 역류증상이일상생활에지장을줄정도로심하거나, 빈도가주 2회이상, 또는 LA 분류 C 이상의역류성식도염인경우를중증으 로, 일상에영향을미치지않는경미한증상, 주 1회미만의빈도또는 LA 분류 A, B인식도염에해당되면으로정의하였다. 12 설문은대부분보기를제시하고그중하나또는 3순위까지를고르는형식이나, 일부문항은응답자가자유롭게서술할수있도록하였다. 각문항별로답변의빈도는백분율로표시하였고, 중증도에따른평균유지치료기간의비교는 t-test를사용하였다. 일부항목에대해질환의중증도 ( 경증또는중증 ), 응답자의진료경력 ( 전문의취득후연도 ) 및소속기관의종류 (2차또는 3차의료기관 ) 등에따라카이제곱검정 (chi-square test) 을이용하여교차분석을시행하였다. p값은 0.05 미만인경우를통계적유의성이있는것으로판단하였다. 결 1. 응답자의일반적인특성 설문응답자는총 371명으로응답률은 78% 였다 (371/475). 응답자의성별, 연령별, 지역별분포는조사대상자의성별, 연령별, 지역별분포와유의한차이를나타내지않았다 ( 각 p>0.05) (Table 1). 평균연령은 42.4±4.8세 ( 범위 32-68), 남녀비는 6:1이었다. 근무지역은대도시가 161명, 중소도시가 210명이었다. 진료경력은 5년미만 95명, 5-9년 164명, 년 67명, 15년이상 45명이었다. 응답자의 63% 가 2차의료기관 ( 소화기전문병원및 500병상미만의준종합병원 ) 에서 Table 1. 3 366 The Korean Journal of Gastroenterology: Vol. 54, No. 6, 2009 근무하고있었으며 3차의료기관 ( 대학병원및 500병상이상의대형종합병원 ) 종사자는 38% 였다. 2. 위식도역류질환의초치료에관한설문전체응답자의 54% 인 200명에서증상을위주로즉, 증상의심한정도 (185명) 또는빈도 (15명) 를기준으로위식도역류질환의중증도를구분하였고, 46% (171명) 는내시경소견을근거로하였다. 초치료시은거의모든경우에서표준용량의프로톤펌프억제제 (proton pump inhibitor, PPI) 를사용하였다 ( 병용요법인경우를포함하여 99%). 경증은 59% 에서표준용량 PPI를, 20% 에서저용량 PPI를주치료제로선택하였고, H 2 수용체차단제를단독으로사용하는경우는 4% 에불과했다. 초치료기간에따른평균환자비율을분석한결과, 경증인경우 4주미만 43%, 4-7주 47%, 8주이상 16% 로대부분 4주전후로투약하였다. 중증에서도 4-7주가 49% 로절반을차지했고, 8주이상의경우도 34% 에달했다. 3. 위식도역류질환의유지치료에관한설문응답자거의모두에서일부또는모든위식도역류질환환자를대상으로유지치료를실시하고있었다 (95%). 유지치료시투여약제에대한설문에경증인경우대부분에서저용량 PPI 단독요법을주치료로하였고 (66%), 다음으로표준용량의 PPI 제제를선호하였으며 (18%), H 2 수용체길항제를사용하는경우는 5% 에불과했다. 반면중증에서는표준용량의 PPI 단독요법을선택하는경우가 50% 로가장흔했고, 35% 는저용량 PPI 제제로유지치료를하여경증과차이를보였다 (p=0.02) (Fig. 1). 유지치료약제및투여용량선택에있어서응답자의진료경력이나소속기관의종류에따른유의한차이는보이지않았다 ( 각각의 p>0.05). 초치료후지속적유지요법 (continuous maintenance therapy) 을시행하는지에대한설문에모든환자를대상으로 (38%), 초진시심한증상을호소한경우에만 (35%), 내시경에서심한병변이관찰된경우에만 (18%) 등의순으로답하였다. 지속적유지요법에서초치료와동일한투여법을사용하는경우는 25% 에불과했고, 대부분은초치료와다른투여법을선택하였다 ( 항상 55%, 상황에따라 19%). 항상다른투여법을사용한다고답한 188명을대상으로초치료와의차이점을물었을때대부분기존약제의투약용량을감량하였고 (68%), 약제또는투여용법을변경하는경우는각각 17%, 14% 를차지하였다. 위식도역류질환의아형별로선호하는유지치료방법에있어서차이를보였다. 미란성식도염이동반된경우는지속적유지요법이 67% 로가장많은비율을차지하였고, 다음으로필요시유지요법 (on-demand maintenance therapy) 순이었다 (29%). 반면비미란성역류질환에서는필요시유지요법을선호하였고 (68%), 26% 만이지속적유지요법을고려하여미란이있는경우와유의한차이를보였다 (p=0.01). 두아형모두에서간헐적유지요법 (intermittent maintenance therapy) 을시행하는경우는 5% 미만으로드물었다 (Fig. 2). 유지치료시투여방법을결정하는데있어서진료경력이나소속기관의종류에따른차이는보이지않았다 ( 각각의 p> 0.05). 평균유지치료기간은이 8주 (2-52 주 ), 중증인경우 15주 (4-56주) 로질환의중증도에따라차이를보였다 (p=0.01). 초치료를포함한총치료기간은경증인경우 1-3개월이 69% 로대부분을차지하고 6개월이상치료하는경우는드물었다. 중증에서는평균 4-6개월이 57% 로가장많았고, 29% 에서는 7개월이상이라고답하였고평균 1년이상치료하는경우도 12% 였다 (Fig. 3). 총치료기간을 Fig. 1. GERD, gastroesophageal reflux disease. 결정하는데영향을주는인자들중 1순위로는 73% 에서증상의중증도를들었고, 2순위는증상의빈도 (40%), 내시경검사에서병변의심한정도 (25%) 순이었다. 이외에도 3순위로정부의보험정책을고려하는경우가 17% 에서있었다 (Table 2). 고 위식도역류질환은대부분에서만성재발성경과를보이기때문에, 5,6 초치료로증상의소실이나식도염의치유가이루어지더라도재발방지를위한장기간의유지치료가필수적이다. 1,7-10 전국 2, 3차의료기관의소화기내과전문의를대상으로한본연구에서, 2004년발표된아시아-태평양지역 FAST survey 11 의결과와유사하게거의모든소화기내과전문의가유지치료의필요성을인지하고시행하는것으로조사되었다. 위식도역류시가장중요한공격인자는위산이므로 H 2 수용체차단제나 PPI 제제에의한산분비억제는위식도역류질환의가장핵심적인치료이다. 1,7 서구와달리심한미란성식도염이매우드문우리나라에서도위식도역류질환은치료후증상재발이흔하며, H 2 수용체차단제투여만으로는재발방지효과가미흡하다 PPI 제제는가장강력한산분비억제제로, 한국의의료환경에서도초치료뿐아니라유지치료에서도비용-효과면에서우월함이확인되어일차선택약제로권장되고있다 이번설문조사결과응답자의대다수가중증뿐아니라경증역류성식도질환에서도초치료및유지치료모두에서 PPI 제제를근간으로하였고, H 2 수용체차단제를단독투여하는경우는극소수였다. The mean value of percentages corresponding to the 1st-3rd factors in each item. 성식도염에서는초치료로표준용량의 PPI 제제를 2-4주간투여하고, 증상지속시 4주더투약한다. LA 분류 C, D의역류성식도염인경우표준용량의 PPI 제제를 8주간투여해보고증상이지속되면두배용량의 PPI 제제를 8주더투여하며, 증상이호전되면두배용량으로유지요법을시작한다. 이번설문조사결과최근국내 2, 3차의료기관에근무하는의사들은경증환자들의유지치료시표준용량보다저용량 PPI 제제를선호하였고, 초치료에있어서도경증위식도질환의 20% 에서저용량 PPI 제제를투여하였다. 또한의유지치료기간은평균 15주정도로, 초치료를포함하여 6개월이상장기투여하는경우도 30% 정도에서있었다. 그러나국내에서 PPI 제제는고가이며보험적용에제한이있어처방하기가쉽지않은실정으로, 대다수를이루는경증에서초치료또는유지치료시반드시표준용량의 PPI 처방이필요한지, 적절한유지치료기간이어느정도인지는불명확하다. 22,23 추후을대상으로한약물치료가이드라인을제시하는것이필요하며, 유지치료제로서저용량 PPI 제제의효과를뒷받침할국내자료가요구된다. 위식도역류질환의유지요법으로는 PPI 제제를매일일정기간투여하는지속투여법, 증상이발생하면복용을시작하여증상이완전히없어질때까지유지하는필요시투여법, 증상이재발한경우정해진기간동안 (2-4주간) PPI 제제를사용하는간헐적투여법이있다 이러한치료전략들을비교분석한결과, PPI 초치료에이어필요시투여를하는것이경미한역류증상을보이거나경증미란성식도

5 368 대한소화기학회지 : 제 54 권제 6 호, 2009 염인경우비용-효과적인접근법으로제시되었다 그러나심한미란성식도염의경우, 초치료로증상이성공적으로소실되면연이어서 PPI 제제를지속적으로투여하는유지요법이권장된다. 1,7,10,24 이번조사에서우리나라소화기내과전문의들도기존가이드라인에준해위식도역류질환의아형별로선호하는유지치료방법에있어서차이를보였다. 비미란성역류질환에서는필요시유지요법을선호하였고 (68%), 26% 만이지속적유지요법을고려하였다. 두아형모두에서간헐적유지요법을시행하는경우는 5% 미만으로드물었다. 반면미란성식도염이동반된경우는지속적유지요법을선호하였지만 (67%), 필요시유지요법도 30% 정도에서시행하는것으로조사되었다. 위식도역류질환은우리나라에서도흔한질환임에도불구하고아직까지유지치료에대한국내가이드라인이확립되지않았고보험기준은그해석이모호한상태로위식도역류질환환자의치료에어려움이있다. 이번설문조사에서나타난전국소화기내과전문의들의실제임상적용양상과문헌고찰을통해현실에근거한위식도역류질환의치료지침을제안한다면, 임상적으로경미한경우에는초치료로 PPI 제제를 4주간투여후필요시 PPI 유지요법을시행하고, 증증미란성식도염, 잦은재발등임상적으로심한경우에는 8주간의 PPI 초치료후지속적 PPI 유지요법을고려하는것이재발방지를위해효과적이라생각한다. 이번설문조사의결과, 대부분의 2, 3차의료기관소화기내과전문의들은위식도역류질환의초치료및유지치료시 PPI 제제를근간으로하는약물요법의중요성을인지하고있었다. 또한거의모든응답자에서일부또는모든위식도역류질환환자를대상으로유지치료를시행하고있었다. 앞으로유지치료시의적용대상, 적절한용법및투여기간에대해서많은논의와비용-효과분석등의연구가필요할것으로생각한다. 감사의글설문조사과정에서적극지원해주신 Janssen Korea에감사드린다. 요약목적 : 2003년아시아-태평양지역에서실시한 FAST survey에서위식도역류질환 (GERD) 에대한치료양상을보고하였으나우리나라의실정이잘반영되지않았다. 이번연구는한국에서 GERD의초치료및유지치료에대한최근의인식과실제임상에서의적용양상을알아보고자하였다. 대상및방법 : 우리나라전역의 2, 3차의료기관소화기 내과전문의를대상으로 2007년 12월 5일부터 2008년 1월 9 일까지온라인설문조사를시행하였다. 설문은 GERD 유지치료와관련된 15개의문항으로구성되었다. 결과 : 총 366 명 ( 응답률 366/475명 =77%) 이설문에참여하였으며응답자의지역별분포는조사대상자의지역별분포와유의한차이를보이지않았다. GERD 심한정도의기준은응답자의 54% 가증상이었고, 46% 가내시경소견이었다. 응답자거의모두에서일부또는모든위식도역류질환을대상으로프로톤펌프억제제 (PPI) 를근간으로하는유지치료를시행하였는데, 경증인경우저용량 PPI (66%), 중증인경우표준용량 PPI (50%) 를선호하였다. 역류성식도염은지속유지요법을 (66%), 비미란성 GERD의경우 on-demand 유지요법을 (68%) 주로시행하였고, 유지치료기간은경증 7.7±5.1주, 중증 15.0±9.4주로나타났다. 치료기간을결정하는데영향을주는요인으로는증상의중증도, 증상의빈도, 내시경에서병변의중증도순이었다. 결론 : 대부분의 2, 3차의료기관소화기내과전문의들은 GERD 치료시 PPI 제제를근간으로하는약물요법의중요성을인지하고있으나, 유지치료시적절한용법및투여기간에대해서는더많은논의가필요할것으로생각한다. 색인단어 : 위식도역류질환, 유지치료, 설문조사참고문헌 1.
