Methods: Data were collected from April 1 through September 30, 2005 using a double blind method. Ondansetron 4 mg and dexamethasone 10 mg were administered to the experimental group (25 patients), and ondansetron 4 mg only to the control group (25 patients). The medications were administered through an intravenous line at the beginning peritoneum suture. PONV by Index of Nausea Vomiting and Retching (INVR), nausea by Visual Analogue Scale (VAS), and pain (VAS) were assessed at postoperative 1 hr, 3 hr, 6 hr, 24 hr, and 48 hr. Data were analyzed using repeated measures ANOVA, and Bonferroni methods. Results: The experimental group that received ondansetron combined with dexamethasone had less PONV (p=.048), and nausea (p=.012) than control group that received ondansetron alone. However, there was no difference in pain (p=.557) between the patients in the two groups. Conclusion: We conclude that the administration of ondansetron combined with dexamethasone is more effective than the administration of ondansetron alone to reduce PONV in patients with laparoscopic hysterectomy. Key words: Ondansetron, Dexamethasone, Postoperative nausea and vomiting, Postoperative pain 서론 1. 연구의필요성복강경수술은개복술과비교하여기복에의한복압상승으로복강내출혈을저하시키고, 피부절개를최소화하여통증이 적으며, 수술후입원기간을단축시키는이점이있어최근일반화되고있다 (Choi et al., 2004). 그러나복강경수술은투관침삽입에따른장기천공, 기복에의한피하기종및공기색전증등의합병증이따르며복강경수술후오심및구토를호소하는경우가많다. 개복술을받은부인과수술환자의 38-50% 가수술후오심및구토를경험하는반면복강경에의한수술환자의 73% 가 주요어 : 온단세트론, 덱사메사존, 수술후오심및구토, 수술후통증 * 본논문은석사학위논문을수정한내용임. *This study is a part of Master s thesis. 2 Ondansetron 과 Dexamethasone 의병합투여가복강경하질식전자궁적출술환자의수술후오심및구토, 통증에미치는효과 45 수술후오심및구토를경험하는것으로알려져있다 (Choi, Ko, Ahn, & Kim, 2005; Koivuranta, Laara, Snare, & Alahuhta, 1997). 오심은토하고싶은불쾌한주관적인식이며, 구토는입을통해위, 십이지장및공장의내용물을강하게배출하는것을의미한다 (Rhodes & McDaniel, 1999). 수술후지속적구토는탈수및전해질불균형을초래하고, 복압을상승시켜창상봉합을파열시키거나혈종과고혈압을야기하며토물에의한흡인성폐렴으로수술로부터회복을지연시키기도한다 (Watcha & White, 1992). 오심및구토에대한간호중재는수술후오심및구토로인한불편감을완화시키고, 수술후합병증을예방하며, 심리적안정과생리적회복을촉진시켜준다는점에서중요하다. 마취전문간호사는외과의감독하에마취를시행함은물론수술후마취로부터의각성및회복을도와주고수술후통증, 오심및구토완화를위한마취관련보조약물을투약할수있는중요한위치에있다. 오심및구토완화에이용하는비약물중재의대부분은약물요법과함께병행된것으로비약물요법만으로진토효과를기대하기는어렵다. 수술후의오심및구토는남성보다는여성에게, 연령이적을수록, 불안과긴장감이클수록, 비흡연가인경우, 오심및구토과거력이있는경우, 이비인후과수술, 유방성형술, 사시수술 및복강경수술후에발생빈도가높다고한다 (Apfel, Laara, Koivuranta, Greim, & Roewer, 1999; Koivuranta et al., 1997). 또한수술소요시간이길어질수록, 흡입마취제 N 2 O 투여에서오심및구토의발생빈도가높으나정맥성마취제 propofol 및고농도의산소투여는오심및구토를완화시킨다고한다 (Goll et al., 2001; Kim, Han, Kil, Lee, & Kim, 2000; Koivuranta et al., 1997). 그외수술후의오심및구토는아편양제제, 탈수, 빈혈, 저혈압, 급격한움직임및체위변경그리고통증에의해초래된다 (Apfel et al., 1999; Choi et al., 2005; Roberts et al., 2005). 오심및구토는구토중추의직접적자극또는화학수용체유발영역의자극으로발생하며, 치료약물에는항콜린성제제, H 1 수용체차단제, 도파민수용체길항제, 세로토닌길항제그리고스테로이드제제가있다 (Mitchelson, 1992). Scopolamine, atropine 및 trihexyphenidyl 등의항콜린성제제는중추의무스카린수용체를차단하여수술후구토와멀미에투여하는데특히 trihexyphenidyl은멀미에효과가좋다. H 1 수용체차단제는히스타민성또는콜린성경로를차단하여항구토효과를가져오며주로수술후구토, 멀미, 임신, 메니에르질환및요독증에동반하는오심및구토에투약한다. 화학수용체유발영역의 도파민수용체를차단하는도파민수용체길항제에는 chlorpromazine, butyrophenones와 metoclopramide 등이있다. Chlorpromazine과 butyrophenones은메니에르질환, 방사선치료및항암요법관련구토, 그리고수술후구토에투약하나 metoclopramide는멀미및내이장애를제외한모든유형의구토에투약한다. 1980년중반부터연구하기시작한세로토닌길항제는위장관점막이나미주신경말단부및화학수용체유발영역의 5- HT 3 를차단하며 ondansetron, granisetron과 tropisetron이있다. Ondansetron과같은세로토닌길항제는항암요법과관련한구토에투약하나두통, 과도진정, 설사또는변비등의부작용이따를수있다. 반면스테로이드제제는프로스타글란딘의생성을억제하고세로토닌생성을감소시켜오심및구토완화효과가있으나불면증등의부작용이있다 (Allen, Jorgensen, & Sims, 1999; Mitchelson, 1992). 따라서세로토닌길항제에스테로이드제제의병용은수술환자에게수술후오심및구토완화는물론수술후통증완화에도움이될수있을것으로생각한다. Ondansetron과타약물과의병합요법에대한진토효과및부작용을살펴보면, McKenzie, Uy, Riley와 Hamilton (1996) 은난관결찰수술에서 droperidol의단독투여에비해 droperidol 과 ondansetron의병합투여가오심및구토완화에효과가있는것으로보고하였다. 그러나 droperidol은심전도에서 QTC 연장과부정맥을초래하는것으로알려진후사용을제한하고있다. 스테로이드제제가세로토닌길항제의오심및구토완화효과를상승시키고, 신경조직의세로토닌생산및장관조직의세로토닌유리를방해한다는연구보고후 (Rudd, Bunce, & Naylor, 1996) 세로토닌길항제와스테로이드제제의병합투여를시도하기시작하였다. Fujii, Saitoh, Tanaka와 Toyooka (2000) 와 Szarvas 등 (2003) 은 ondansetron과 dexamethasone의병합투여가 ondansetron 단독투여와비교하여오심및구토의완화효과가큰것으로보고하였다. 반면, Kim (1999) 은 ondansetron과 dexamethasone의병합투여그리고 Lee 등 (2001) 은 tropisetron 과 dexamethasone의병합투여가 ondansetron 단독투여와비교하여오심및구토완화효과에차이가없는것으로보고하고있다. Smith 등 (1991) 은항암요법환자들을대상으로 ondansetron 8 mg과 dexamethasone 8 mg, McKenzie 등 (1994) 은부인과개복수술환자들을대상으로 ondansetron 4 mg과 dexamethasone 8 mg, Szarvas 등 (2003) 은척수마취하에정형외과수술환자를대상으로 ondansetron 8 mg과 dexam-

3 46 남미옥 윤혜상 ethasone 8 mg, 그리고 Fujii 등 (2000) 은 granisetron 40 μg/ kg과 dexamethasone 8 mg을투약하여세로토닌길항제와 dexamethasone의병합투여가오심및구토완화에효과가있는것으로결론을내렸다. 반면, Kim (1999) 은개복하에자궁절제술을받는환자를대상으로 ondansetron 4 mg과 dexamethasone 8 mg, 그리고 Lee 등 (2001) 은갑상선절제술환자를대상으로 tropisetron 30 μg/kg과 dexamethasone 5 mg 의병합투여가오심및구토완화에효과가없는것으로결론을내렸다. 위에서와같이오심및구토완화에대한세로토닌길항제와 dexamethasone의병합투여효과와관련된국내외선행연구결과가상반되어있지만선행연구가많지않고, 연구대상은물론투약용량및투약시기가다양하여상반된이유에대해뚜렷한결론을내리기가용이하지않다. Ondansetron 4 mg과 dexamethasone 8 mg의투약이오심및구토완화에효과가없는것으로보고한국내선행연구 (Kim, 1999; Lee et al., 2001) 를토대로 ondansetron 및 dexamethasone 병합투여가오심및구토, 그리고수술후통증완화에미치는효과를파악하는연구가필요하다고생각한다. Ondansetron은항구토효과가우수하나가격이비싸며, dexamethasone은가격이저렴하고항오심효과가우수하다는선행연구 (Mitchelson, 1992; Rung et al., 1997) 를토대로비용을최소화하고약물의효과를극대화시킬수있는방안을찾기위해본연구를시도하였다. 본연구에서는복강경하질식전자궁절제술을받은환자들을대상으로 ondansetron의단독투여와 ondensetrone 및 dexamethason의병합투여간에수술후오심및구토와통증에차이가있는가를비교하고자한다. 2. 연구목적본연구의목적은전신마취하에서복강경하질식전자궁적출술을받은환자를대상으로 ondansetron의단독투여와 ondansetron 및 dexamethasone의병합투여간에수술후오심및구토와통증에차이가있는가를파악하여마취간호실무를위한기초자료를제공하는데에있다. 구체적인연구목적은다음과같다. 첫째, ondansetron의단독투여와 ondansetron 및 dexamethasone의병합투여간에수술후오심및구토에차이가있는가를파악한다. 둘째, ondansetron의단독투여와 ondansetron 및 dexamethasone 병합투여간에수술후통증에대한차이가있는 가를파악한다. 3. 용어정의 1) 복강경하질식전자궁적출술본연구에서는복강내에 CO 2 gas를주입하여기복 (pneumoperitoneum) 을만들고복벽에광원을투시하여시야를확보한후자궁동정맥상방까지는골반경하에서수술을하고나머지부분은질식으로수술하는방법을의미한다 (Choi et al., 2004). 2) 오심및구토오심은토하고싶은주관적인식이며, 구토는구강을통해위, 십이지장및공장의내용물을강하게배출하는것으로 (Rhodes & McDaniel, 1999), 본연구에서는 Rhodes와 McDaniel (1999) 의 Index of Nausea Vomiting and Retching (INVR) 을 Kim, Kim, Choi, Kim과 Rhodes (2000) 가한국어로번역한 INVR 도구로측정한점수이다. 3) 통증통증이란조직손상의크기나범위에대한생물학적인신호로신체에상처나손상을입었을때느끼는감각및고통으로 (APS, 1995) 본연구에서는시각적상사척도 (Visual Analogue Scale, VAS) 를이용하여측정한점수이다. 연구방법 1. 연구설계본연구는전신마취하에복강경하질식전자궁적출술을받은수술환자 50명을실험군과대조군에 25명씩무작위로배정한동등성대조군전후실험연구설계이다. 자궁적출후복막봉합이시작되면실험군에게는 ondansetron 4 mg과 dexamethasone 10 mg, 그리고대조군에게는 ondansetron 4 mg을정맥으로투여하였다. 2. 연구기간및연구대상본연구는인천광역시에소재하는 1,300병상규모의 G의대병원에서 2005년 4월 1일부터 9월 30일까지복강경하질식전자궁적출술을받은환자 50명을대상으로하였으며연구대상

4 Ondansetron 과 Dexamethasone 의병합투여가복강경하질식전자궁적출술환자의수술후오심및구토, 통증에미치는효과 47 자선정기준은다음과같다. 첫째, 본연구의목적을이해하고연구에참여하기로동의한자둘째, 35-55세여성으로 American Society Anesthesiologist (ASA) 신체상태 II 이하에해당되는자셋째, 복강경하질식전자궁적출술을받은환자로수술소요시간이 2시간을초과하지않는자넷째, 복강경하질식전자궁적출술을받은환자로오후 4시전에수술이종료된자다섯째, 수술 24시간전에진토제를투여받지않은자여섯째, 최근 3개월내에스테로이드나면역억제제를복용하지않은자일곱째, 의사소통이가능하며, 지남력이있는자동전던지기를이용하여 50명의연구대상자를실험군과대조군에무작위로배정하였다. 또한수술종류, 수술자극강도, 수술시간, 마취방법및마취제등이수술후오심, 구토에미치는영향을최소화하기위해 1인의의사가집도하는복강경하질식전자궁적출술을받은환자를연구대상자로선정하였다. 연구대상자의수는 Cohen의 power analysis를근거로 α=.05, β=.20, 효과크기 d=0.5를기준으로실험군과대조군을각각최소 25 명으로하였다. 3. 연구도구 1) 오심및구토 Rhodes와 McDaniel (1999) 의 INVR를 Kim 등 (2000) 이한국어로번역한후신뢰도와타당도를검증한 INVR을이용하였고이때의 Cronbach s alpha는.84이었다. INVR은오심, 구토와헛구역질에관한 8개항목으로구성된 5점척도도구로 8 개항목중 4개항목은역문항으로구성되어있다. 전혀그렇지않다 에0점, 매우심하게그렇다 에 4점을배정하여총점은최저 0점에서최고 32점이며점수가많을수록오심및구토가심한것을의미한다. 본연구에서 1차, 2차의 Cronbach s alpha 는각각.95와.94로나타났다. 2) 오심 VAS를이용하여환자자신이지각하는오심정도를측정하였다. VAS는왼쪽끝에 오심없음 (0점) 과오른쪽끝에 매우심한오심 (10점) 이적힌 10 cm의수평선상에대상자가느끼는오심에해당하는지점을표시하게한후, 왼쪽점을기준으로측정한길이를 mm로점수화하였으며점수가높을수록오심이심한것을의미한다. 3) 통증 VAS를사용하여환자자신이지각하는통증정도를측정하였다. VAS는왼쪽끝에 통증없음 (0점) 과오른쪽끝에 매우심한통증 (10점) 이적힌 10 cm의수평선상에대상자자신이느끼는통증에해당하는지점을표시하도록하며, 왼쪽점을기준으로측정한길이를 mm로점수화하였으며점수가높을수록통증이심한것을의미한다. 4) 자가통증조절장치누름회수자가통증조절장치기의버튼을누르는횟수를의미한다. 4. 연구절차및자료수집방법본연구의자료수집은 2005년 4월 1일부터 9월 30일까지연구자와 2인의연구보조자가하였다. 본연구자는대조군에게 ondansetron 4 mg, 실험군에게 ondansetron 4 mg과 dexamethasone 10 mg을투여하였다. 수술종료후 1시간에서의자료수집은회복실간호사, 그리고수술후 3시간부터 48시간까지의자료수집은병동간호사가하였다. 연구대상자는물론자료를수집한회복실간호사및병동간호사는연구대상자가어느집단에속해있는가를알지못하는이중맹검법으로자료수집을하였으며자료수집절차는다음과같다. 첫째, 본연구가이루어진병원의연구윤리위원회의승인을얻었다. 둘째, 복강경하질식전자궁적출술을받기위해입원한환자를수술전날연구자가방문하여연구목적을설명하고서면동의서를받았다. 셋째, 연구대상자에게 VAS, 자가통증조절기사용법과 INVR 작성법에대해설명하고오심, 구토의과거력 ( 임신시, 차멀미유무 ), 기질불안, VAS에의한오심과통증, INVR, 혈압및맥박을측정하였다. 넷째, 수술당일마취전처치로병실에서 midazolam 3 mg 과 glycopyrrolate 0.2 mg을마취시작 30분에서 1시간전에근육주사하였다. 다섯째, 연구대상자가수술실에도착하면앙와위상태에서 EKG monitor (Dash 4000, GE, Bloomfield, CT, USA) 을적용한후혈압과맥박을측정하였다. 여섯째, Alfentanil 1,000 μg, thiopental sodium (5 mg/ kg) 과 esmerone 0.8 mg/kg으로마취유도를하였고, 기관내삽관후마취유지는 enflurane 1.5-2%, 또는 isoflurane 1-1.5%, N 2 O와 O 2 를각각 2 L씩흡입시키고, 수술중근이완제로 roc-

5 48 남미옥 윤혜상 uronium (0.8 mg/kg) 을투여하였다. 일곱째, 자궁적출후복막봉합을시작하면대조군은 morphine (15 μg/kg/hr), ketorolac (25 μg/kg/hr) 과 ondansetron 4 mg, 실험군은 morphine (15 μg/kg/hr), ketorolac (25 μg/kg/hr), ondansetron 4 mg과 dexamethasone 10 mg을생리식염수와각각혼합하여총 100 ml를만든후자가통증조절기 (Accufuser plus, Woo Young Med, Jincheon, Korea) 에연결하였다. 자가통증조절기는시간당 2.0 ml씩지속적으로주입되며, 잠금시간은 15분이고, 자가통증조절기의단추를누르면 1회에 0.5 ml가추가투여되도록설정하였다. 여덟째, 수술종료후연구대상자가회복실에도착하면기본사항들을기록한후수술후 1시간에오심, 통증, 진토제및진통제의추가투여를기록하였다. 아홉째, 병동으로이동후수술후 3시간, 6시간, 12시간, 24 시간및 48 시간에각각오심, 통증, 진토제및진통제의추가투여와자가통증조절기버튼누름횟수를기록하였다. 열번째, 수술후 24시간과 48시간에 INVR을측정하였다. 수술후회복실과병실에서 VAS에의한 7점이상의통증이나오심을호소하거나환자의요구가있으면 ondansetron 4 mg과진통제로 ketorolac 30 mg을추가투여하고두통, 어지러움, 진정, 가려움증및안면홍조등의부작용발생을관찰하였다. 5. 자료분석방법수집된자료는 SPSS WIN 10.0 프로그램을이용하여분석하였고, 분석방법은다음과같다. 첫째, 연구대상자의일반적특성은실수와백분율, 평균및표준편차를구하였다. 둘째, 실험군과대조군간의동질성검증은 t-test와 2 -test 를이용하여분석하였다. 셋째, 실험군과대조군간에추가진토제, 추가진통제투여및자가통증조절기누름횟수의차이는 t-test로분석하였다. 넷째, 실험군과대조군간의실험처치후 INVR, 오심, 통증과 PCA 조절버튼누름횟수는반복측정분산분석을이용하였고, 실험군과대조군의사후분석은 t-test, 시점간사후분석은 Bonferroni 다중비교법을이용하였다. 연구결과 1. 연구대상자의일반적특성및생리적특성에대한동질성검증연구대상자의일반적특성및생리적특성에대한동질성검증결과는다음과같다. 결혼상태, 종교, 오심및구토과거력, 금연및수술경험유무에대한동질성검증에서대조군과실험군간에유의한차이가없는것으로나타났다 (Table 1). 또한대조군과실험군간에연령, 체중, 수술시간, 마취시간, 금식시간, 수술중출혈, 혈색소, 기질불안, INVR, 오심, 통증에는유의한차이가없는것으로나타났다 (Table 2). 2. 오심및구토실험군과대조군의 INVR 지수에는차이가있으며 (p=.021) 사후분석결과수술후 48 시간에서실험군은대조군과비교하여 INVR 지수가낮았다 (p=.022). 수술전, 수술후 24시간및 Table 1. 6 Ondansetron 과 Dexamethasone 의병합투여가복강경하질식전자궁적출술환자의수술후오심및구토, 통증에미치는효과 49 48시간시점에서반복측정한 INVR 지수간에는차이가있는것으로나타났다 (p<.001). 시점간 Bonferroni 다중비교에서수술후 24 시간과수술후 48 시간 (p<.001) 의 INVR 지수에는차이가있는것으로나타났다 (Table 3). 대조군과실험군간에수술전, 수술후 24 시간및 48시간에걸친 INVR 지수의변화양상이서로달라교호작용은통계적으로유의하며 (p=.048) ondansetron 및 dexamethasone의병합투여가 ondansetron 단독투여보다 INVR 지수의저하효과가큰것으로나타났다. 실험군과대조군의오심에는차이가있으며 (p=.002), 사후분석에서수술후 3시간 (p=.012) 과수술후 24 시간 (p=.002) 에서실험군은대조군과비교하여오심이적었다. 수술전, 수술후 1시간, 3시간, 6시간, 24시간및 48 시간시점에서반복측정한오심간에는차이가있는것으로나타났다 (p<.001). 시점간 Bonferroni 다중비교결과수술후 1시간과수술후 3시간 (p=.004), 수술후 6시간 (p=.003), 수술후 24 시간 (p<.001), 그리고수술후 48시간 (p=.001) 에차이가있는것으로나타났다 Table 2. Frequency of Administration of Antiemetics, Analgesics, and Pushing PCA Button for Postoperative 24 hr Variables Group t p Antiemetics Con (n=25) 0.4± Exp (n=25) 0.1±0.3 Analgesics Con (n=25) 0.3± Exp (n=25) 0.3±0.6 Pushing PCA botton Con (n=25) 3.2± Exp (n=25) 2.7±2.3 PCA=Patient Controlled Anesthesia; Con=control; Exp=experimental. 교호작용의통계적유의성이없었다 (p=.557). 따라서 ondansetron 및 dexamethasone의병합투여와 ondansetron 단독투여가통증에미치는영향에는차이가없는것으로나타났다. 4. 진토제및진통제추가투여와자가통증조절기누름횟수 실험군과대조군간에진토제추가요구량 (p=.760) 과진통제추가요구량 (p=1.00) 그리고 PCA 조절버튼누름횟수 (p=.424) 에는차이가없는것으로나타났다 (Table 5). 논 마취전문간호사는법적으로수술집도의사의지시및감독하에마취를시행할수있으므로 (Ryoo, 2008), 마취제는물론수술후통증, 오심및구토관련약물의효과및부작용의파악은마취간호실무의핵심적영역이라고볼수있다. 즉, 마취전문간호사가의료법상직접적으로마취및마취보조약물을선택하여투약할수는없으나 (Ryoo, 2008) 수술집도의사에게마취및마취보조약물에대한정보를제공하여수술집도의사의마취관련약물처방에간접적으로깊이관여하고있다고볼수있다. 본연구는수술후항구토제투약에대한기초자료를제공하여마취간호실무를향상시키기위하여시도하였다. 의 본연구에서 ondansetron 및 dexamethasone 병합투여가 ondansetron 단독투여에비해오심및구토완화효과가우수한것으로나타났다. 부인과환자를대상으로한 McKenzie 등 (1996) 과정형외과수술환자를대상으로한 Szarvas 등 (2003) 은마취유도직후투여한 ondansetron 및 dexamethasone의병합투여가 ondansetron의단독투여에비해항오심및항구토에효과적인것으로보고하여본연구의결과를지지해주고있다. 또한복강경담낭절제술환자를대상으로한 Fujii 등 (2000) 과편도선절제술환자를대상으로한 Sukhani 등 (2002) 도전신마취유도전에 granisetron 및 dexamethasone의병합투여가 granisetron 단독투여보다오심및구토완화효과가우수한것으로보고하여본연구의결과와거의동일하다. 본연구에서 ondansetron 및 dexamethasone 병합투여군과 ondansetron 단독투여군은모두 INVR과오심이수술후 24 시간에서가장높았으며 48시간에저하되어복강경수술후의오심및구토가수술후 24 시간에가장심하며수술후 48 시간에현저히완화됨을볼수있다. 반면, Kim (1999) 은개복술에의한전자궁적출술환자를대상으로수술종료후, 그리고 Lee 등 (2004) 은개복술에의한부인과수술환자를대상으로마취유도전의 ondansetron 및 dexamethasone 병합투여와 ondansetron 단독투여간에항오심및항구토효과는차이가없는것으로보고하였다. Kim (1999) 과 Lee 등 (2004) 의연구결과를통해동일한용량의 ondansetron 및 dexamethasone을투약할때투약시기는오심및구토완화에영향을미치지못하는것으로생각한다. Lee 등 (2001) 도갑상선절제술환자를대상으로마취유도전 tropisetron 및 dexamethasone 병합투여와 tropisetron 단독투여간에항오심및항구토효과는차이가없는것으로보고하였다. Kim (1999) 은 ondansetron 4 mg 및 dexamethasone 8 mg 병합투여와 ondansetron 4 mg 간에오심및구토완화효과에차이가없는이유를세로토닌수용체자극으로발생하는복강내

8 Ondansetron 과 Dexamethasone 의병합투여가복강경하질식전자궁적출술환자의수술후오심및구토, 통증에미치는효과 51 수술후의오심및구토에서는세로토닌수용체길항제인 ondansetron의효과가우선적으로나타나기때문에 dexamethasone의효과가은폐된것으로결론을내리고있다. 그러나 Lee 등 (2001) 과 Lee 등 (2004) 의연구에서 ondansetron 및 dexamethasone의병합투여가오심및구토에대한완화효과가없는것으로나타난현상은 dexamethasone 투약용량과도관련이있을것으로생각한다. 왜냐하면성인의경우 dexamethasone은 8 mg 이상의투약에서항오심및항구토효과가있다고하나 Lee 등 (2001) 과 Lee 등 (2004) 은각각 5 mg 과 4 mg 씩투여하여오심및구토완화효과를가져오기에는미흡했던것으로생각한다. 본연구에서 ondansetron 단독투여군과 ondansetron 및 dexamethasone 병합투여군모두에게수술직후부터수술후 24시간까지오심이증가한것으로나타났다. 이러한현상은복강경복부수술에서수술후회복이신속하므로수술후조기이상이빠른데 (Choi et al., 2004), 걷거나또는급격한자세변경은오심과구토를악화시키기때문에 (Watcha & White, 1992) 나타난현상으로생각한다. 본연구에서 ondansetron 및 dexamethasone 병합투여와 ondansetron 단독투여간에통증에는차이가없는것으로나타났다. Kim (1999) 은복식전자궁적출술환자를대상으로 ondansetron 및 dexamethasone 병합투여와 ondansetron 단독투여간에수술후 48 시간에서의통증에는차이가없는것으로보고하여본연구의결과와유사하였다. 이와같이 ondansetron 및 dexamethasone의병합투여와 ondansetron의단독투여간의통증에차이가없는현상은 ondansetron 단독투여군과 ondansetron 및 dexamethasone 병합투여군모두에게자가통증조절기를적용하였고, ondansetron 자체의통증완화효과 (Liu, Hsu, & Chia, 1998) 때문에나타난현상으로생각된다. 반면, Bisgaard, Klarskov, Kehlet과 Rosenberg (2003) 은복강경담낭절제술환자를대상으로수술시작전 dexamethasone 8 mg 투여집단이생리식염수투여집단에비해통증인지가낮은것으로보고하였다. Bisgaard 등 (2003) 은본연구에서대조군에게투여한 ondansetron 대신에생리식염수를투약하여 dexamethasone의통증완화효과가보다분명하게나타날수있었던것으로생각된다. 반면에 Seo와 Kim (2003) 도갑상선절제술환자를대상으로마취유도직후 dexamethasone 5 mg 투여군과생리식염수투여군간의통증에는차이가없는것으로보고하였다. Bisgaard 등 (2003) 과 Seo와 Kim (2003) 의 dexamethasone에대한통증완화효과가일치하지못하는 것은 Seo와 Kim (2003) 의 dexamethasone 투약량이미흡했기때문이라고생각한다. 한편, Bisgaard 등 (2003) 과 Seo와 Kim (2003) 의통증측정방법이서로다르기때문에통증정도를비교하기가어렵다. 본연구에서 ondansetron 및 dexamethasone 병합투여는 ondansetron 단독투여에비해오심및구토완화에효과적이나수술후통증완화효과는없는것으로나타났다. 이러한연구결과를감안할때복강경하질식전자궁적출술환자에게수술종료 30분전에 ondansetron과 dexamethasone를병합투여하는것은 ondansetron 단독투여에비해수술후오심및구토완화에도움이되며비용도저하시킬수있을것으로기대된다. 결론및제언 본연구를통해복막봉합시에 ondansetron 4 mg 및 dexamethasone 8 mg의병합투여는 ondansetron 4 mg 단독투여와비교하여수술후 1시간, 3시간, 6시간, 24시간그리고 48 시간에서의오심완화에효과가있는것으로나타났다. 또한 ondansetron 4 mg 과 dexamethasone 8 mg의병합투여는수술후 24 시간과 48시간에서의구역완화에효과가있으나통증에는효과가없는것으로나타났다. 이러한연구결과를감안할때복강경하질식전자궁적출술환자에게수술종료 30분전에 ondansetron 및 dexamethasone의병합투여는 ondansetron 단독투여에비해수술후오심및구토완화에도움이되며 ondansetron 투약량을최소화시켜경제적비용도감소시킬수있을것으로기대한다.
