In Korea, over 15 instruments have been used for measuring PSA. In this study, chemiluminescence microparticle immunoassay Architect total PSA (Abbott Lab., Abbott Park, IL, USA) was evaluated for analytical performance and diagnostic usefulness as a marker for prostate cancer. Methods : The within-run and between-run precision, lower detection limits, correlation with AxSYM total PSA (Abbott Lab., Abbott Park, IL, USA) and clinical investigation were evaluated. Three level control serums (0.5, 4.0, and 23.0 ng/ml) were used for a precision test. The linearity was evaluated using a patient serum sample with a PSA concentration of 100 ng/ml. Functional and analytical sensitivities were tested using a patient serum sample with a PSA concentration of less than 0.1 ng/ ml and saline. A correlation study with AxSYM total PSA was done with 42 serum samples. Clinical evaluation was done with 230 patients of whom 17 had prostate cancer. Results : The total PSA showed a good precision result with less than 5 % of CV and showed linearity to 100 ng/ml. The functional sensitivity was ng/ml and analytical sensitivity ng/ml. The correlation evaluation showed Y (Architect)=1.0575X(AxSYM) , r= A Cut-off value of 8.35 ng/ml showed 88.2% sensitivity, 80.3% specificity as a diagnostic marker for prostate cancer. Conclusions : Architect total PSA showed an acceptable analytical performance with its high sensitivity and could be a useful marker for early detection and recurrence of prostate cancer. PSA 측정법은방사선면역검사, 효소면역검사, 화학발광면역분석법으로점차변화함으로써정확도및정밀도가증가하고, 최저검출한계를낮출수있도록발전하고있다. 대한정도관리협회에서주관한 2003년 PSA 외부정도관리결과에의하면 15종이상의다른기종을사용하여 PSA를측정하고있었고화학발광면역검사법을가장많이사용하고있었다 [7]. 본원에서도효소면역분석법인 IMx, AxSYM total PSA (Abbott Lab., Abbott Park, IL, USA) 에서화학발광면역분석법인 Architect total PSA (Abbott Lab., Abbott Park, IL, USA) 로검사방법이변화되었다. 이에저자들은 Architect 총 PSA 자동면역검사법의정밀성, 정확성및검사의유용성을 National Committee for Clinical Laboratory Standards (NCCLS) 에서제시한지침 [8-10] 에따라평가하였다. 저농도 (1.3 ng/ml) 를 0:4, 1:3, 2:2, 3:1, 4:0 비로다섯단계의농도로제조하여각각에대하여 4회씩반복측정하여구한평균치로부터측정치와계산치사이의회귀직선식을구하였다 [9]. 3) 최저검출한계기능적예민도측정은 0.1 ng/ml 이하의다섯단계농도에서각각 10회측정하여 CV값이 20% 를보이는 PSA 최저농도를구하였다. 분석예민도측정은 PSA 0 ng/ml 물질인생리식염수를 20회측정한평균값에표준편차의 2배를더한값으로하였다 [10]. 4) 상관성평가 EP9-A2 방법에준하여환자 42명으로부터얻은검체로 AxSYM 총 PSA와의비교를시행하였다. AxSYM 총 PSA 검체의측정범위가 50 ng/ml 이하이므로이농도이하의검체들만비교하였다. 정상농도, 낮은농도, 높은농도를모두포함하였다 [11]. 대상및방법 1. 대상임상적평가는 2003년 10월부터 2004년 3월까지이대동대문병원진단검사의학과에 PSA 검사를의뢰한 230명으로서전립선암 17명, 전립선양성질환 213명환자를대상으로하였다. 2003년본원에도입된 Architect (Abbott Laboratories, Abbott Park, IL, USA) 을이용하여총 PSA를측정하였다. 원리는화학발광미세입자면역검사로서총 PSA는유리 PSA와 alpha-1- antichymotrypsin과결합된 PSA를측정한다. 이는두단계면역검사로서첫번째단계는검체와 anti-psa coated paramagnetic microparticle을넣고반응후세척한다. 두번째단계는 anti-psa acridium labeled conjugate 첨가후 pre-trigger와 trigger 용액을넣고화학발광체를 relative light unit (RLU) 로측정하며이는농도에비례한다. 상관성은미세입자효소면역검사인 AxSYM total PSA (Abbott Laboratories, Abbott Park, IL, USA) 와비교평가하였다. 정밀도평가, 직선성평가, 최저검출한계, 상관성평가및임상적평가를하였다. 5) 임상적검토환자군의연령은남자 34세에서 100세까지분포하였고평균연령은 67.6세였다. 전립선암여부는의무기록지의경직장전립선초음파소견및전립선조직검사소견조회로확인하였다. 6) 통계 SPSS 11.0 (SPSS Inc. Chicago, IL., USA) 을이용하여 Pearson 상관계수를구하고, 비모수검정으로평균치비교시 Mann- Whitney test를실시하였으며, Receiver operating characteristic (ROC) curve에서 Area under the curve (AUC) 를구하였다. 결과 1. 정밀도평가 PSA 저농도 (0.5 ng/ml), 중간농도 (4.0 ng/ml) 및고농도 (23.0 ng/ml) 의검사내정밀도 (CV) 는 5.4%, 4.2%, 4.5% 이었고검사간정밀도는 2.4%, 4.6%, 3.2% 이었다 (Table 1). 2. 방법 2. 직선성평가 1) 정밀도평가상품화된세단계농도의정도관리물질 (Architect total PSA controls 0.5, 4.0, 23.0 ng/ml) 을사용하여 NCCLS EP5-A의방법에준하여 20일간의검사일내, 검사일간정밀도평가를시행하였고평균과표준편차, 변이계수를구하였다 [8]. PSA 100 ng/ml까지직선성을유지하였고측정치와기대치값사이의결정계수 (R 2 ) 은 , Y ( 측정치 )=1.0178X ( 기대치 ) 이었다 (Fig. 1). 3. 최소검출한계 2) 직선성평가 NCCLS EP6-A 에따라환자검체중고농도 (94.0 ng/ml) 와 기능적예민도는 ng/ml를보였다. 기능적예민도는 y= Ln(x) 공식에서 y에 CV 값인 20(%) 를대입하여

The sensitivity, specificity and area under the curve (AUC) of PSA in prostate cancer by receiver operating characteristic (ROC) curve. 계산한 x 값 (ng/ml) 이었다. 분석예민도는 ng/ml 이었다. 4. 상관성 두기기간의상관관계는 Y (Architect)= X (AxSYM) , 상관계수 (r) 는 이었다 (Fig. 2). 5. 임상적검토 전립선양성질환에서의 PSA의평균 (Mean±SD) 은 6.6±5.01 ng/ml, percentile은 ng/ml이었다. 전립선암에서는 28.6±29.46 ng/ml, ng/ml이었다 (P<0.05) (Table 2). 전립선암에서의 PSA의결정치를 8.35 ng/ml으로 Table 2. Comparision of PSA in benign prostatic diseases and prostate cancer PSA (ng/ml) Benign (n=213) 정했을때, 예민도는 88.2%, 특이도는 80.3%, AUC는 0.909이었다 (Fig. 3). 고 PSA는전립선암표지자중하나로 19번염색체의 human kallikrein family에서기원하는물질이며 kallikrein-like serine protease이다. 혈액내 PSA는 70-90% 가 1-antichymotrypsin (ACT) 와결합되어있고소량이 1-antitrypsin, protein C와결합되어있다. 2-macroglobulin (AMG) 과결합할수있으나거의발견되지않는다 % 는혈청단백과결합되지않은유리형으로존재한다. 현재사용중인 PSA 키트의대부분은 ACT와 찰 Prostate cancer (n=17) P value Mean±SD 6.6± ±29.46 < percentile <0.05

4 392 서소연 조성은 홍기숙 결합한 PSA를측정한다. 1979년 Wang 등에의해혈청 PSA가전립선암의표지자로서보고된이래혈청 PSA가전립선암의선별과치료중추적에효과적인것으로알려져있다 [12-14]. PSA의변화치를이용한전립선암진단에서는정상 PSA치를가진남자에서 1년에 20% 이상증가하거나 0.5 ng/ml 증가시전립선암을의심하고비뇨기과진찰을할필요가있다고하였다 [15, 16]. PSA와수지직장검사로직장암을선별할때는 PSA 가 ng/ml이면 1-2년마다, ng/ml이면 1년마다재검을하고, PSA 단독으로사용시에는 ng/ml이면 1년마다재검을하고 ng/ml시는 3-5년사이에재검을권고하고있다 [17]. 본연구에서는전립선양성질환에서의 PSA치는 6.6± 5.01 ng/ml, 전립선암에서는 28.6±29.46 ng/ml로통계적으로유의한차이가있었다. 전립선암진단의결정치는 8.35 ng/ml으로예민도 88.2%, 특이도 80.3% 이었고 IMx에의한 7.9 ng/ml ( 예민도 86.7%, 특이도 86.8%)[18] 와큰차이를보이지않았다. PSA의최저검출한계는종양치료의치료반응및재발추적을판단하는데중요하다. 진행된전립선암의호르몬치료에서 PSA 치는대개질병진행의임상적증거가있기전 6-12개월전에먼저상승한다고보고되었다. 곽등 [19] 에의하면 PSA 최저치에따른생존율을보면, 최저치가 0.2 ng/ml 미만이었던환자들의중앙생존기간은 53개월이었고, ng/ml, ng/ml, 10.1 ng/ml 이상의환자들의중앙생존기간은각각 42개월, 24 개월, 15개월로 PSA 최저치에따른유의한차이가관찰되었다. Critz 등에의하면전립선암음성치료목표로서 PSA 0.2 ng/ml 을최저치기준으로제시하고있다 [20]. 본연구에서기능적예민도는 ng/ml를보였고, 분석예민도는 ng/ml로서매우낮은값도측정할수있었다. 같은회사의효소면역검사법인 AxSYM의분석예민도는 0.04 ng/ml이었고, IMx에서는 ng/ml 로점차최저검출치가검사방법의발전에따라예민해지고있었다 [21-23]. Klee 등에의한화학발광면역법에서는기능적예민도는 0.02 ng/ml, 분석예민도는 ng/ml로보고하고있다 [24]. 본결과에의한최저검출한계라면치료반응및재발추적에유용할것으로판단된다. PSA 검사방법에서사용하는항체가다클론항체에서단클론항체로, 방사선면역검사에서효소면역검사로, 또화학발광면역검사로빠르게대치되어지고있다. 이에따라검사결과의예민도와특이도가높아지고, 시약의유효기간이더길고, 자동화되고있어서검사의정밀도및정확도가증가한다 [24, 25]. 우리나라 PSA 외부정도관리에참여한총 202 기관중화학발광법을사용하는기관이 114 기관으로 56.4% 를차지하였고그다음으로효소면역검사, 형광면역검사법등을사용하고있었다. 본연구에서사용한 Architect 측정의외부정도관리정밀도는 0.2 ng/ml에서 %, 19 ng/ml에서 % 를보였다 [7]. 본연구에의하면 PSA 저농도에서고농도까지의검사내및검사간정밀도는모두 5.0% 정도로만족할만한결과였다. 직선성평가에서 PSA 100 ng/ml까지측정치와기대치값사이의 상관은우수하였다. AxSYM 기기와 Architect 기기간비교에서도우수한상관을보였다. 같은방법사이에서도기기마다 PSA 치의차이가있어 Architect와 Elecsys 2010 사이에 11% 정도의차이가있다고하였고 [26], 전립선암환자의진단및추적검사시는같은방법으로검사해야한다고하였다 [27]. 위의결과들을종합할때 Architect 총 PSA 검사는전립선암조기진단, 치료후최저치확인및재발판정에적절한검사로판단된다. 요약배경 : 혈청 prostate specific antigen (PSA) 검사는전립선암조기진단, 치료추적및재발의판정에절대적으로중요하다. 한국에서는 15종이상의 PSA 측정기기가사용되고있다. 이에최근에많이사용하고있는화학발광면역분석법인 Architect total PSA (Abbott Lab., Abbott Park, IL., USA) 의분석력및전립선암진단의유용성을평가하고자한다. 재료및방법 : 검사내및검사간정밀도평가, 최저검출한계는세단계 (0,5, 4.0, 23.0 ng/ml) 의정도관리물질을사용하였다. 직선성검토는 PSA 100 ng/ml의환자검체로, 기기상관성검토는환자검체 42개로시행하였다. 임상적검토는 2003년 10월부터 2004년 3월까지이대동대문병원진단검사의학과에 PSA 검사를의뢰한 230명으로전립선암 17명, 양성질환 213명환자를대상으로하였다. 전립선암여부는의무기록지의경직장전립선초음파소견및전립선조직검사보고서로확인하였다. 결과 : PSA의정밀도는 5% 정도를보였고, 100 ng/ml까지직선성을유지하였다. 최저검출한계는기능적예민도는 ng/ ml, 분석예민도는 g/ml이었다. 기기간비교는 Y (Architect)=1.0575X (AxSYM) , r=0.9960이었다. 전립선암진단에있어서 PSA 결정치는 8.35 ng/ml으로예민도 88.2%, 특이도 80.3% 이었다. 결론 : Architect 총 PSA를이용한검사는전립선암의조기진단과치료후최저치및재발판정추적검사에적절한검사로판단된다. 참고문헌 Annual report of the Korea central cancer registry ( ) (based on registered data from 139 hospitals) ( 보건복지부한국중앙암등록본부. 한국중앙암등록사업연례보고서 ( )) 2.
