Methods: This study was conducted retrospectively using the medical records of patients under 15 years who visited a training hospital Emergency Department (ED) for 5 years. The study subjects were divided into the correct group (registered as anaphylaxis codes correctly) and the incorrect group (registered as other anaphylaxis related codes). Results: Of the 133 patients, 14 belonged to the correct group and 119 to the incorrect group. The median age of the correct group was 9 years old and that of the incorrect group was 2 years old. Sex, transportation to the ED, elapsed time from exposure to ED arrival, past history of allergy, causes of anaphylaxis except drug, severity of symptom, mental status, and antihistamine use were not different between the 2 groups. Drugs as the cause of anaphylaxis and cardiovascular/neurologic symptoms were more common in the correct group. Gastrointestinal symptoms were more frequent in the incorrect group. Intravenous fluid, steroid, bronchodilator, and epinephrine were more commonly used as the treatment for anaphylaxis in the correct group. The pediatric patients treated with epinephrine tended to be registered anaphylaxis correctly. Conclusion: More patients were registered incorrectly as other anaphylaxis-related disease codes rather than correctly as the anaphylaxis disease codes in pediatric anaphylaxis. Epinephrine use was the associated factor for being registered correctly as the anaphylaxis disease codes in pediatric anaphylaxis. ( 2017;5: ) Keywords: Anaphylaxis, Pediatrics, Accuracy 서론아나필락시스는전신적으로나타나는알레르기반응으로, 짧은시간에중요한장기들을침범하며심하게발현될경우사망에까지이르게할수있는질환이다. 1,2 아나필락시스의증상은다양하게나타날수있으나급작스럽게발현되어단시간에악화되므로대부분의환자들이처음으로내원하는곳이응급실이다. 따라서아나필락시스환자를처음접하게되는응급실의의료진은정확한진단을내려야하며신속하고적절한치료를하는것이중요하다. 이전논문에따르면아나필락시스는전체인구에서 0.05% 2% 의유병률을보였으며국내연령별분류에서는 18세이하의소아에 서약 10만명당 건으로보고되었다. 3,4 소아의경우아나필락시스의가장흔한원인이식품으로알려져있는데, 미국에서조사한식품알레르기유병률은전체인구에서약 1% 10% 로보고되었고미국국민건강영양조사에서실시한소아식품알레르기유병률은 2007년 2010년까지 6.5% 로나타났다. 5,6 국내에서전국초등학생을대상으로시행한조사에서식품알레르기유병률은 1995년과 2000년에각각 4.2%, 4.7% 였고서울지역초등학생의식품알레르기유병률은 1995년과 2000년에각각 4.6%, 5.2% 였다. 7 국내외연구에서보듯이아나필락시스의유병률은증가하고있으며최근아나필락시스에대한연구가활발하게진행되고있으나, 많은경우상병명이아나필락시스로등록된환자로연구대상이 Correspondence to: Sun Hyu Kim Department of Emergency Medicine, Ulsan University Hospital, University of Ulsan College of Medicine, 877 Bangeojinsunhwan-doro, Dong-gu, Ulsan 44033, Korea Tel: , Fax: , stachy1@paran.com Received: February 9, 2017 Revised: March 13, 2017 Accepted: March 17, The Korean Academy of Pediatric Allergy and Respiratory Disease The Korean Academy of Asthma, Allergy and Clinical Immunology This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License (

Accuracy for registration of disease codes in pediatric anaphylaxis 국한되어있어상병명이아나필락시스로정확하게등록되지않은 환자는연구대상에서누락되는경우가발생할수있다. 8 특히, 상병 명이두드러기나혈관부종으로등록된소아의경우성인에비해실 제아나필락시스로재진단되는비율이높은것으로보고되고있 어, 9 이는소아환자에서아나필락시스의진단기준에는맞지만아 나필락시스로등록되지않는경우가많음을의미한다. 따라서, 이연구에서는응급실로내원한소아아나필락시스환 자중에서실제아나필락시스진단기준을만족함에도불구하고 아나필락시스이외의다른상병명으로등록된경우의빈도와그 임상적특성에대해조사하고자하였다. 1. 연구대상및방법 대상및방법 이연구는 2011 년 1 월부터 2015 년 12 월까지 5 년동안일개 3 차병 원응급실로내원한 15 세이하의소아아나필락시스환자를대상 으로하였다. 전체아나필락시스환자를검색하기위해기존의국 내외소아아나필락시스에대한논문을검토하여아나필락시스와 관련성이있는상병명과상병코드를수집하였고 (Table 1), 10 해당 상병명과상병코드가입력된모든소아환자의의무기록을후향적 으로검토하여실제아나필락시스의진단여부를재평가하였다. 이 를아나필락시스가직접언급된상병명으로등록된경우를정확 군 (correct group) 으로, 아나필락시스이외의관련상병명으로등 록된경우를누락군 (incorrect group) 으로분류하였다. 환자의일반적특성, 아나필락시스의원인, 임상적특성, 치료에 대해자료를수집하였고, 환자의일반적특성으로나이, 성별, 응급 실내원경로, 원인물질노출로부터증상발생까지경과시간, 증상 발생으로부터응급실도착까지경과시간, 알레르기병력, 동반질 환유무에대해조사하였다. 아나필락시스의원인이약물일경우 비스테로이드항염증제 (nonsteroidal anti-inflammatory drugs), 항 생제의일종인페니실린, 세팔로스포린, 예방접종, 아세트아미노펜 Table 1. International Statistical Classification of Diseases 10th Revision (ICD- 10) codes related anaphylaxsis Code Anaphylaxis codes Anaphylaxis Anaphylactic shock Other related codes Allergy Food allergy T78.1 Drug allergy Insect bite Angioedema T78.3 T78.2B, T78.2C T78.0, T78.2,T88.6 T78.4, T78.4A, T78.4B ICD-10 code T88.7, T88.7B, T88.7C, T88.7D, T88.7E, Z88 S-codes, T14.0G, T63.4A, W57, X23 Urticaria L28.2C, L50.0, L50.88, L50.9 으로세부원인을분류하였고, 곤충이원인일경우벌, 개미, 기타곤충으로세부원인을분류하였으며, 음식은해산물, 밀가루, 메밀, 번데기, 견과류, 계란, 돼지고기, 우유및유제품으로세부원인을분류하였다. 그외운동유발성, 음식의존운동유발성, 비특이적원인에대해서도조사하였다. 임상적특성으로는계통별증상, 과민반응의중증도, 내원당시생체징후, 쇼크발생여부, 의식상태에대해확인하였다. 과민반응의중증도분류는 Brown 11 이보고한방법을이용하여저산소증, 저혈압, 신경학적증상이있을때를중증으로정의하였다. 응급실에서시행된치료로산소투여여부, 수액투여여부, 항히스타민제사용, 스테로이드사용, 에피네프린사용, 기관지확장제사용여부를조사하였다. 2. 아나필락시스진단기준아나필락시스의진단기준은 2011년 World Allergy Organization Guidelines for the Assessment and Management of Anaphylaxis에서제시한임상적진단기준을이용하였다. 다음 3가지항목중 1가지항목이라도만족하는경우아나필락시스로진단하였다. 1. 갑자기 ( 수분에서수시간이내 ) 발생한피부, 점막 ( 전신두드러기, 가려움증, 홍조, 입술-혀 -목젖의부종 ) 증상과함께적어도다음증상중한가지이상동반할때 A. 호흡기증상 ( 호흡곤란, 천명, 기도수축, 협착음, 최고호기유속감소, 저산소증 ), B. 혈압감소또는이로인한말초장기기능장애와관련된증상 ( 저혈압, 실신, 실금 ) 2. 의심되는알레르겐노출후빠르게 ( 수분에서수시간이내 ) 다음항목중 2가지이상의증상및징후가나타날때 A. 피부-점막증상 ( 전신두드러기, 가려움증- 홍조, 입술-혀 -목젖의부종 ) B. 호흡기증상 ( 호흡곤란, 천명-기도수축, 협착음, 최고호기유속감소, 저산소증 ) C. 혈압감소또는이와관련된증상 ( 저혈압, 실신, 실금 ) D. 지속적인위장관증상 ( 복통, 구토 ) 3. 알고있던알레르겐에노출된후 ( 수분에서수시간이내 ) 혈압이저하될때 A. 나이에따른수축기혈압이낮거나기저수축기혈압의 30% 이상감소할때 3. 통계분석정확군과누락군에서등록된상병명의종류와빈도를조사하였고, 각군에속한환자들의일반적특성, 아나필락시스의원인, 임상적특성, 치료를 chi-square test, Fisher exact test, Mann-Whitney U-test를이용하여단변량분석을시행하였다. 정확군과누락군간 160

ED, Emergency Department. *Mann-Whitney U-test. 대값이 >30 또는최소값이 < 0.03 인경우는분석에서제외한후, 다변량로지스틱회귀분석진입방법 (entered method) 을통해분 석하였다. 통계는 IBM SPSS Statistics ver (IBM Co., Armonk, NY, USA) 를이용하였고, P<0.05 를통계학적의의를가진다고가 정하였다. 결과 전체의무기록을검토한환자가 2,377 명이었고, 이중아나필락 시스로진단된환자는 133 명이었으며, 정확군은 14 명 (10.5%), 누락 군은 119 명 (89.5%) 이었다 (Fig. 1). 나이분포의중앙값은정확군에 서 9 세, 누락군에서 2 세였다. 두군간의성별차이는없었으며, 응 급실이송수단, 원인물질노출에서부터증상발생까지걸리는시 간과증상발생후응급실도착시간모두차이가없었다. 알레르기 과거력과동반질환의유무역시두군간의차이는없었다 (Table 2). 아나필락시스의원인이약물인경우, 정확군은 36%, 누락군은 11% 였다. 그외벌레물림이나음식이원인인경우, 비특이적원인 에서는두군간의차이는없었다 (Table 3). 아나필락시스의증상은정확군에서누락군에비해심혈관계증 상 (42.9% vs. 0.8%) 과신경계증상 (14.3% vs. 0.8%) 이더많았으며, 위장관계증상은누락군 (58.8%) 에서정확군 (14.3%) 에비해더많 았다. 증상의중증도와의식의명료한정도는두군에서차이가없 었다. 응급실에서의치료에서수액투여를한경우가정확군은 92.9%, 누락군은 34.5% 이었고, 항히스타민제사용유무는두군 Table 3. ED, Emergency Department. *Mann-Whitney U-test. 아나필락시스는과민반응의일종으로두드러기로발현되는피 부증상및점막부종뿐만아니라전신성알레르기반응으로호흡기 계, 심혈관계, 소화기계등다양한기관에서과민반응이나타난다. 대부분아나필락시스의진단은환자의병력및증상에의해이루 어지는데, 소아의경우성인과비교하여병력청취를하는데있어서 증상표현이불명확하기때문에진단기준을만족하더라도정확한 진단을내리기어렵다. 실제로단일증상을호소하는경우가많고 Table 5. Univariate and multivariate logistic regression analysis for factors associated with anaphylaxis registered correctly Variable Univariate logistic regression Odds ratio 원인물질과의인과관계가명확하게나타나지않기때문에아나필 락시스로상병코드가등록될수있지만, 증상과관련된다른상병 코드로등록될수도있다. 아나필락시스환자에서아나필락시스가 포함된상병코드가아니라두드러기나혈관부종과같은증상관련 상병코드로등록된다고해서잘못된것이라고할수는없지만, 아 나필락시스이외의증상관련상병코드로만등록될경우, 아나필 락시스환자의실제유병률을밝히기란매우어렵다. 이전아나필 락시스연구에서도이연구의누락군에해당하는상병코드를포함 함으로써아나필락시스환자가연구에서누락될가능성을최소화 하기위한노력을해왔음에도불구하고, 12 이연구와같이아나필락 시스임에도그외의다른상병코드로등록되고있는현황에대한 연구는없었다. 따라서향후아나필락시스관련연구에서는이연 구의누락군에대한고려를해야할것이다. 95% Confidence interval P-value Age Drug, as anaphylaxis cause Cardiovascular symptom Gastrointestinal symptom Neurologic symptom Diastolic blood pressure Fluid administration Steroid use Epinephrine use Bronchodilator use Multivariate logistic regression Epinephrine use 이연구에서전체 133 명의소아아나필락시스환자중대부분인 119 명의환자가아나필락시스가아닌다른상병명으로등록되었으 며, 그중가장흔한상병명은두드러기 (79 명 ) 이었고, 그다음은혈 관부종 (16 명 ), 음식알레르기 (15 명 ) 로확인되었다 (Fig. 1). 이는두 드러기와혈관부종과같이피부증상은육안으로쉽게관찰이가 능하며뚜렷하고객관적인증상인것에비해, 주관적으로호소하 는호흡곤란이나복통, 오심과같은증상이모호하거나경미한경 우에는아나필락시스의기준을만족하더라도아나필락시스대신 비교적증상이명확한두드러기나혈관부종으로등록되었을가능 성역시고려해야한다는것을의미한다. 또한, 소아가응급실을방 문할때울면서보채는경우가흔하므로혈압을포함한초기생체 징후측정이나청진시호흡음을구별하기에어려움이따른다. 뿐 만아니라, 환자의연령에따라본인의증상에대해구체적으로표 현하기어려운경우도있기때문에, 의료진은이러한점을감안하 162

5 조연주외 소아아나필락시스상병등록의적절성 여소아환자를진찰할때환자의해당연령에맞는이해도를가지고동반증상에대해문진을해야하며, 환자의상태에대해잘알고있는보호자에게알레르기의과거력, 원인물질의유무, 증상유발시간및동반되는다른증상에대하여구체적으로병력청취가이루어져야할것이다. 이연구에서정확군과누락군모두음식이아나필락시스의원인으로가장많았는데, 특히누락군에서음식이원인이경우는 72.8% 였고누락군에서등록된상병중두드러기와혈관부종에이어음식알레르기가세번째로많이등록되었던것으로나타났다. 이는기존의많은연구에서소아및청소년에서아나필락시스를유발하는가장흔한원인이식품으로조사되었던것과동일한결과이며, 음식알레르기가소아에서매우흔한질환이므로아나필락시스보다는음식알레르기로등록되는경우가많았던것으로생각한다. 한편, 소아아나필락시스의경우성인과비교시중증의환자비율이적은데, 이연구에서도응급실에서아나필락시스환자를대상으로심폐소생술을시행한경우는없었으며, 중증의과민반응을보이는경우가정확군은 2명 (14.3%), 누락군은 2명 (1.7%) 으로이전연구결과와유사한것을확인할수있었다 아나필락시스의치료에대해살펴보면, 정확군과누락군에서모두산소공급, 수액, 항히스타민제, 스테로이드, 기관지확장제사용이이루어졌으나, 에피네프린의투여는정확군에서 8명 (57.1%), 누락군에서 17명 (14.3%) 으로차이가나는것을볼수있었다 (Table 4). 특히, 에피네프린을사용한환자가에피네프린을사용하지않은환자에비해 5.6배더정확하게아나필락시스로상병명이등록되는것을알수있었다 (Table 5). 에피네프린은아나필락시스의초기치료에서매우중요한약물로혈관을수축시키고기관지점막부종을감소시키며심박출량과박동수를동시에증가시키는작용을한다. 한연구에서국내의료진의아나필락시스에대한인식정도를조사한결과, 아나필락시스의일차약제로에프네프린의중요성을인지하고는있으나미국이나영국과비교했을때에피네프린치료에대한이해도가부족하고, 투여용량, 재투여간격, 투여부위를정확하게숙지하지못해아나필락시스의초기치료에서에피네프린의투여가지연되는것으로나타났다. 19 이연구에서도아나필락시스로정확하게진단된정확군에서조차 57.1% 만에프네프린을투여받았다. 휴대용에피네프린의처방률은더욱낮은것으로알려져있고, 최근국내한연구에따르면휴대용에피네프린의처방률은 7.4% 에불과한것으로보고되었다. 20 아나필락시스의발생이전세계적으로증가하고있는추세이고, 아나필락시스로내원하여입원한건수는지난 10 15년동안 5 7 배증가하였고아토피질환이나벌독알레르기환자들에서아나필락시스의재발은 26.5% 54% 인것으로보고되었다. 21,22 아나필락시스의유병률증가및중증도를고려해볼때, 응급실에서의일회적인치료에서그칠것이아니라전문가에의한추적관찰이이루어 질수있도록교육하여아나필락시스의원인이되는알레르겐을 분석하고, 알레르겐의회피를통한예방과휴대용에피네프린의처 방및사용방법을숙지시켜병원전단계에서응급조치가가능하 도록치료의연속성이유지되어야한다. 따라서응급실에처음으로 환자가과민반응을호소하며내원하였을때정확한문진및진찰 을통하여아나필락시스로정확한상병등록이이루어지는것이 필요할것으로생각한다. 이연구는일개대학병원에서진행한후향적연구로응급실에내 원한환자만을대상으로하여외래를통해내원한아나필락시스환 자는포함하지못하였으며, 누락군에비해정확군의환자수가적 어통계적분석에한계가있었다. 추후연구에서정확군의수를충 분히확보하여두군의특성을비교할필요가있다. 그리고일개대 학병원만의아나필락시스로상병명이등록된행태에대한연구이 므로, 이번연구결과를국내전체로일반화하기에는한계가있다. 하지만이연구결과를토대로타기관에서도아나필락시스로상병 명의등록과연관되어발생할수있는문제를상기시킬수있으며, 또한기존의아나필락시스관련연구에서대상환자의누락가능성 을확인할수있었다는점에서는의의를가질수있다고판단한다. 결론적으로, 응급실에서진료받은소아아나필락시스환자에서 아나필락시스이외의관련상병코드로부정확하게등록된경우가 아나필락시스상병코드로정확하게등록된경우보다많았다. 에피 네프린을사용한경우에는아나필락시스상병코드로등록이될 가능성이높았다.
