This report will describe the characteristics of the HIV-1 positive reactive donors. Methods: We attempted to determine the number of HIV-1 positive reactive donors for each NAT center, and attempted to characterize the donors by gender, age, RNA viral load, and the distribution of HIV-1 subtype. Results: Among the 80 HIV-1 positive reactive donors determined by the NAT, 57.5% of the donors were in their twenties and all but one of the donors was male. Of all of the donors, 82.5% were repeated donors and four donors showed antibody negative window periods. The average quantity of HIV-1 RNA for 78 donors was copies/ml and for the four donors that showed the antibody negative window periods was copies/ml. The HIV-1 subtypes of 76 cases were all B of group M. Conclusion: NAT screening contributes to the safety of the domestic blood supply. Therefore, it is necessary to continue to study the characteristics of the blood that was found to show HIV positivity by the NAT. 2 핵산증폭검사에서 Human Immunodeficiency Virus Type 1 양성결과를보인헌혈자의특성 서론 2006년 12월말현재국내에이즈누적감염인수는총 4,580명이확인되었으며, 감염경로가밝혀진사람은 3,849명이다. 성접촉에의한감염비율이 98.6% 로가장많으며국내수혈에의한인한감염자수는 16명이다. 1) 대한적십자사에서는수혈에의한에이즈감염을예방하기위해 1987년 7월부터모든헌혈자를대상으로 HIV 항체선별검사를시행하고있다. 2005년 2월부터는항체검사보다 11 15일그리고항원검사보다 5 9일더빨리바이러스를검출할수있는핵산증폭검사 (nucleic acid amplification test, NAT) 를 3개혈액검사센터에서시행하고있다. 중앙 ( 서울 ) ㆍ남부 ( 부산 ) 혈액검사센터에서는 Roche system (Roche Diagnostics, Branchburg, NJ, USA) 의 Reverse Transcription-Polymerase chain reaction (RT-PCR) 방법을이용하여 NAT를시행하고있으며, 중부 ( 대전 ) 혈액검사센터에서는 Chiron system (Chiron/Gen Probe, Emeryville, CA, USA) 의 Transcription Mediated Amplification (TMA) 방법을이용하여 NAT를시행하고있다. 2) 본연구에서는 2005년 2월부터 2006년 7월까지 18개월간 3개혈액검사센터에서 NAT 시행결과 HIV-1 양성으로밝혀진헌혈자의특성을분석하고자하였다. 1. 연구대상 대상및방법 2005년 2월부터 2006년 7월까지대한적십자사 3개혈액검사센터에서실시한총 3,481,972건의 NAT 시행결과 HIV-1 양성으로밝혀진 80건을 대상으로헌혈자와헌혈혈액의특성을분석하였다. 2. 방법 1) NAT 선별검사 중앙과남부혈액검사센터에서는 COBAS Ampliscreen (Roche Diagnostic, Mannheim, Germany) 을, 중부는 Procleix HIV-1/HCV multiplex assay (Chiron Cooperation. Emeryville CA, USA) 를이용하여핵산증폭검사를수행하고있다. 전자의경우 24 개의검체를혼합 (pooling) 하는방법이며, pooling 에는 TECAN Genesis RSP 150 (Tecan, Mannedorf, Switzerland) 을, 핵산의추출에는 NucliSens Extractor (Biomerieux, Geneve, Switzerland) 를이용하고있다. 후자의경우 16개의검체를혼합하는방법이며, pooling에는동일한 TECAN Genesis RSP 150을이용하고있다. 2) 2) 항체검사결과비교및 HIV-1 NAT 양성의특징분석대한적십자사의혈액정보관리프로그램인 Blood Information Management Service (BIMS) 를조회하여 NAT system별 HIV-1 양성검출수와 HIV 항체검사결과및성별, 연령별분포와헌혈횟수그리고매독항체검사결과등의특징을분석하였다. 3) HIV-1 RNA 정량및아형분석 HIV-1 RNA 정량과아형분석은 80건중원혈장을얻지못한 2건을제외한 78건에대해서실시하였다. HIV-1 RNA 정량은 COBAS Amplicor HIV-1 Monitor Test version 1.5 (Roche Molecular Systems Inc. Pleasanton, CA, USA) 를사용하였으며그결과를항체검사결과와 NAT system에따라분석하였다. HIV-1 아형분석은 HIV-1 env 유전자의 V3 domain 부위를증폭하여염기서열을분석하여결정하였다. 혈장에서 QIAamp Min- Elute Virus Spin Kit (Qiagen, Hilden, Germany) 로

Nested PCR에는 HIV- F1 (5 -CACAGTACAATGTACA- CATG-3 ) 와 HIV- R1 (5 -ACAGTAGAAAAATTC- CCCTC-3 ), HIV- F2 (5 -GCTGTTRAATGGCA- GTCTAGCAGAAG-3 ) 와 HIV-R2 (=3'C2V3) (5 - ATTTCTGGGTCCCCTCCTGAGG-3 ) 를사용하였고증폭산물의크기는 336 base pairs이다. 3) 최종 PCR 산물은 2% agarose gel (Seakem LE, Cambrex, Rockland, ME, USA) 에서전기영동하여 100 base pairs DNA ladder (Promega, Madison, WI, USA) 로비교하여확인하였고 QIAquick PCR purification kit (Qiagen, Hilden, Germany) 로정제한후염기서열분석에사용하였다. ABI 3730 DNA Analyzer (Applied Biosystems, Foster City, CA, USA) 를사용하여염기서열을분석하였고 National Center for Biotechnology information (NCBI) [ nih.gov/projects/genotyping/formpage.cgi] 의 BLAST 프로그램을이용하여아형을결정하였다. 4) 4) 통계분석 P값은 chi-square test로분석하였고 P값이 0.05 미만이면통계적으로유의한것으로판단하였다. 결과 1. Roche system에서는 67건, Chiron system에서는 13건의양성반응이검출되었다. 이들 80명중 HIV 항체검사에서음성이었던윈도우기는총 4건이었다. 2. HIV-1 NAT 양성헌혈자의특성 HIV-1 NAT에서양성결과를얻은헌혈자들의연령분포는 Table 2와같다. 20대가 57.5%, 30대가 27.5% 로 20 30대가전체양성자의 85% 를차지하고있다. 전체헌혈자의분포는 Table 2와같이 10 20대에집중되어있는데반해, HIV-1 NAT 양성자는 20대의헌혈자에서가장많으며 20 30대에집중되어있었다 (P<0.05). 전체헌혈자연령별통계는 BIMS에집계된적십자혈액원채혈일기준의통계이므로의료기관을포함한혈액검사센터의접수일기준핵산증폭검사건수와차이가있다

HIV-1 NAT 양성 80명의중 79명은남자였고, 1명만여성이었다. 이는전체헌혈자에서의남성대여성의분포가약 8:2인것을감안할때남성의비율이통계학적으로유의하게높았다 (P< 0.05). NAT에서양성반응을보인 HIV-1 헌혈자들의헌혈력은초회헌혈자가 14명 (17.5%), 2회이상다회헌혈자가 66명 (82.5%) 이었다. 다회헌혈자들의헌혈횟수별분포는 2 9회가 45명, 10 19회가 16명, 20 29회가 1명, 30 39회 0명, 40 회이상이 4명이었다. 연구대상기간의전체헌혈자에서의매독항체검사양성은 0.04% 였고, HIV-1 NAT 양성자에서의매독항체검사양성은 13.9% 였다 (P<0.05). 매독항체검사결과통계는특별검사를포함한적십자혈액원채혈일기준의통계자료이다 (Table 3). 3. HIV-1 RNA 정량및아형분석결과 HIV-1 RNA 정량결과는최소 copies/ ml에서최대 copies/ml였으며, HIV-1 RNA 정량평균치는 copies/ml였다. 정량의분포는 Fig. 1과같았다. HIV 항체양성검체의정량평균은 copies/ml이며, 항체음성인윈도우기 4건의평균은 copies/ml 로, 두군의 HIV-1 RNA 정량수치는통계적으로유의한차이는보이지않았다. Fig. 1. Distribution of HIV-1 RNA levels in HIV-1 NAT reactive blood donors. NAT system 별정량비교분석은 Roche system 에서평균치가 copies/ml, Chiron system 에서평균치가 copies/ml으로, 두시스템간의 HIV-1 RNA 역가상의유의한차이는보이지않았다. HIV-1 양성 78건에대해 HIV-1 아형분석을실시한결과 76건이모두그룹 M의 B 형으로확인되었고 2건은증폭실패로아형을확인하지못하였다 (Fig. 2). 고찰 2005년 2월부터 2006년 7월까지 NAT를실시하여검출한 80건의 NAT HIV-1양성의특징에대한연구로 Roche system에서는 67건, Chiron system에서는 13건의양성을검출하였고이는검사점유율이 7:3 정도로수도권의중앙혈액검사센터에집중되어있기때문으로생각되었다. NAT에서양성결과를얻은 HIV-1 헌혈자들의초회헌혈자와다회헌혈자를구분해보면다회헌혈자가 82.5% 로전체헌혈자의분포와일치하

6 핵산증폭검사에서 Human Immunodeficiency Virus Type 1 양성결과를보인헌혈자의특성 였으나다회헌혈자중 40 회이상이 4 명이었고, 윈도우기 4건모두다회헌혈자였다. 이러한사실은다회헌혈자가전체헌혈의 80% 를자치하는상황에서매우주의를요하는부분으로판단되며핵산증폭검사가필요함을의미하는것으로생각된다. HIV-1 NAT 양성자에서의매독양성은 13.9% 로전체헌혈자양성률 0.04% 에비해높은수치를보였다. 매독감염이주로불특정다수와의성접촉으로인해전파되는것으로고려하여볼때, 이는국내에이즈의주된감염경로가성접촉이라는보고 1,5) 를뒷받침해주는것으로여겨지며, 문진에의한이들의채혈배제의중요성을일깨워주는사례라할수있을것이다. 평균 HIV-1 RNA 정량값은 copies/ml 였다. 윈도우기의검체 4건의평균은 copies/ml로 4건모두저역가상태가아니었다. 따라서 NAT에서검출한윈도우기검체는극초기감염시의저역가의바이러스혈증 (low level viremia) 상태가아닌항체형성전단계의 rampup 시기인바이러스의활발한증식기로감염성이매우높은상태의윈도우기라고생각된다. HIV-1 그룹 M의세계적인아형분포는 C형 (47%), A형 (27.2%), B형 (12.3%), D형 (5.3%), CRF01_ AE (circulating recombinant form: 3.2%) 순으로보고되고있다. 또한지리학적인분포는유럽이나아메리카에 B형이주로분포하고아프리카에는 A, C, D형이분포하며동부및동남아시아에는 B형과 E형이분포한다고한다. 6,7) HIV는지리학적및감염경로의차이에따라다양한유전적변이와우세형의빈도가현저히차이가나는것으로알려져있다. 2002년도에발표된 Oh와 Choe 8) 의보고와질병관리본부자료 5,9-11) 에의하면국내유행하는 HIV-1 아형은 B형이라고한다. 본연구에서도 76건의 HIV-1 아형이모두 B형으로확인되 어국내헌혈자그룹에서의우세형임을다시한번확인하였다. 2005년 2월부터 2007년 1월말까지 2년동안 HIV-1 NAT 실시결과윈도우기에있던 4건 ( 검출비율, 1:1,150,122) 을검출하여수혈로인하여발생될수도있었던감염을예방할수있어핵산증폭검사의유효성을입증하였다. 따라서앞으로도 HIV 잠복기헌혈을배제하기위하여는 NAT 가지속적으로필요하다고생각되었다. 핵산증폭검사결과에대한모니터링및바이러스정량및아형분석을포함한연구는임상검체및패널의기초자료가될것이다. 이러한자료는앞으로국내적합한 HIV NAT 검사법의개발및평가시에유용할것이라사료된다. 요약배경 : 대한적십자사는전국에 3개의 NAT 검사센터를설립하고 2005년 2월부터국내헌혈혈액에대하여 HIV-1과 HCV의핵산증폭검사를시작하였다. 그결과 2005년 2월부터 2006년 7월까지총헌혈건수 3,481,972건에대하여 80건 (0.0023%) 이 HIV-1 NAT 양성이었다. 이논문은 NAT HIV- 1양성의특성에대해설명한것이다. 방법 : 헌혈혈액의핵산증폭검사에서 HIV-1 양성을보인헌혈자에서 NAT 검사센터별 HIV-1 양성건수를알아보고항체검사결과, 성별, 연령별등헌혈자의특징, HIV-1 바이러스정량, 그리고 HIV 아형의분포를알아보고자하였다. 결과 : HIV NAT양성헌혈자 80명중 20대가가장많았고 (57.5%) 한명을제외하고모두남성이었다. 다회헌혈자가 82.5% 를차지하였고이중 4건은항체검사에서음성인윈도우기였다. 78건의 HIV-1 RNA 정량의평균치는 copies/ ml였으며윈도우기 4건의평균은 copies/
