A huge, heavily calcified unruptured aneurysm originating from the right sinus of Valsalva was detected incidentally in a 61-year-old man. Chest X-ray showed cardiomegaly and 10 cm sized huge calcified mass lesion around the cardiac shadow. Two-dimensional echocardiogrm revealed pericardial effusion with huge calcified mass compressing right ventricular outflow and color-flow Doppler echocardiogram visualized blood flow from aortic root into aneurysm. Chest CT scan and MRI revealed a large thrombosed aneurysm arising from aortic root measuring 1010 cm. After pericardiocentesis cardiac catheterization was performed, which showed elevated right ventricular systolic pressure up to 80 mmhg. Aortic root angiogram revealed huge unruptured calcified aneurysm in the sinus of Valsalva arising from the right coronary sinus. The patient underwent surgical correction for the prevention of aneurysmal rupture and the relief of right ventricular outflow obstruction. 1), 흉부 X- 현병력 심질환이나 외상의 과거력은 없었고 35년 전 선상 심장 음영 증대와 심장 영상 내에 심하게 석회화 에 갑자기 흉통과 호흡곤란이 있어서 개인병원에서 몇 달간 치료한 적은 있었으나 그 뒤에 특이한 심장 증상 은 없었다. 된 종괴 소견을 나타내고 있었다(Fig. 2).

2 Valsalva동 동맥류를 경험하였기에 보고하는 바이다. 이학적 검사 폐동맥 영역과 좌흉골연 상방에서 진전 을 동반하는 제5도의 수축기 심잡음이 청진되었으며, 심 증 례 첨부에서 심음은 약하게 들렸다. 경정맥압은 12 cmh2o 이었고 1.5 횡지의 간비대가 있었다. 환 자 남자, 61세. 심전도 및 흉부 X-선 검사 1도 방실 차단과 우각 주 소 개인병원에서 우연히 발견된 심한 심비대. 차단, 우심실비대소견을 나타내었고(Fig. 1), 흉부 X- 현병력 심질환이나 외상의 과거력은 없었고 35년 전 선상 심장 음영 증대와 심장 영상 내에 심하게 석회화 에 갑자기 흉통과 호흡곤란이 있어서 개인병원에서 몇 달간 치료한 적은 있었으나 그 뒤에 특이한 심장 증상 은 없었다. 된 종괴 소견을 나타내고 있었다(Fig. 2). 경흉부 심초음파 소견 대량의 심낭 삼출액과 우심 방의 압박소견이 있어서 2000 cc의 심낭천자를 실시 Fig. 1. 대동 는 약 10 10 cm 크기의 내부에 혈전으로 충만된 동 맥근 조영술에서 우측 Valsalva동에서 기시하는 파열 맥류소견을 관찰할 수 있었다(Figs. 3 and 4). 되지 않은 동맥류를 관찰할 수 있었다(Fig. 6).

이러한 Valsalva동의 동맥류는 선천성이나 후천성 원 Valsalva동의 동맥류는 흔하지 않은 질환으로서, 대 인으로 발생하며, 선천성인 경우 대개 3개의 Valsalva 개 수년에 걸쳐서 커지며, 일반적으로

Valsalva동에 생기며, 남자에 많고 동반되는 선천성 심 고 안 질환으로는 심실중격결손증이 많다. 이러한 Valsalva동의 동맥류는 선천성이나 후천성 원 Valsalva동의 동맥류는 흔하지 않은 질환으로서, 대 인으로 발생하며, 선천성인 경우 대개 3개의 Valsalva 개 수년에 걸쳐서 커지며, 일반적으로 파열되지 않은 동 중에서 1개의 동에 국한된다. 후천적인 원인으로는 동맥류는 대개 증상을 일으키지 않는 드문 질환이다. syphilis, 세균성 심내막염, 대동맥박리, 외상 등이 원인 대개 심장내로 파열되었을 때 흉통, 심부전, 심잡음 등 이 될 수 있다. 선천성인 경우 대동맥 판륜 위쪽의 Va- 의 증상이나 증후가 나타나며, 파열은 주로 우심실이나 lsalva동의 대동맥벽의 중막의 결손으로 발생하는 것으 우심방내로 일어나며, 대개 20, 30대에 일어나는 것으 로 생각된다.5) 로 알려져 있다.1) 대개의 Valsalva동의 동맥류는 우측 대개의 파열되지 않은 동맥류는 증상이 없고 우연히 117
