1 ORIGINAL ARTICLE Otgonbold Jamiyandorj 김지환 김무성 박영범 심준성 * 연세대학교치과대학치과보철학교실 연구목적 : 본연구의목적은임플란트지대주연결에사용되는기성품나사와맞춤형나사의풀림토크를비교하는것이다. 연구재료및방법 : 세가지임플란트시스템에 (Osstem, Astra, Zimmer) 대해고정체와지대주의연결에기성품나사군 ( 대조군 ) 과맞춤형나사군 ( 실험군 ) 으로총 6 군 (n = 6) 으로나누었다. 조임토크조절은디지털토크측정기를이용하였으며각임플란트제조사가추천한조임토크값을적용하였다. 체결 10 분후동일한조임토크를다시적용하고 5 분후에풀림토크력을측정하였다. 이과정을 10 회반복측정하였다. 반복하중실험에서는 6 개군 (n = 3) 에대해체결 10 분후 2 차조임토크를적용하고 30 경사로 50 N 하중으로총 1,000,000 번반복하중을가하였다. 반복하중적용이후풀림토크값을측정하였다. 통계방법으로는 10 회반복측정에서풀림토크값의차이를비교하기위해 repeated measures of ANOVA test (α=.05) 를사용하였고, 반복하중후풀림토크값의차이를비교하기위해서각시스템별 Independant t-test 로통계처리하였다. 결과 : 모든군에서반복횟수가증가할수록풀림토크값이유의성있게감소하는것으로나타났다 (P<.05). 10 회반복측정실험에서는세종류의임플란트에서대조군 ( 기성품나사 ) 과실험군 ( 맞춤형나사 ) 간에풀림토크값은유의차가없었다 (P>.05). 반복하중실험에서세가지시스템에서대조군과실험군간의풀림토크력은유의한차이가없었다 (P>.05). 결론 : 반복측정된풀림력검사와반복하중을적용후풀림력검사에서맞춤형나사와기성품나사의풀림력은유의한차이가없었다. ( 대한치과보철학회지 2012;50:243-8) 주요단어 : 맞춤형나사 ; 지대주나사 ; 풀림토크 ; 치과용임플란트 ; 나사풀림 서론 임플란트의고정체와지대주의연결방식은다양하다. 임플란트의치관면상방으로기하학적형태가돌출되었는지여부에따라내측연결또는외측연결로분류된다. 연결의형태에따라 slip fit joint, friction fit joint 으로분류된다. Slip fit joint 는연결부위에약간의공극이존재하여, 수동적으로연결되며 friction fit joint 는연결되는짝부품사이에공간이존재하지않아서로약간의힘이가해지는상태로연결된다. 1 임상적으로많이사용되는방식은 slip fit joint 이며주로나사로연결되는방식이이용되고있다. 나사로연결되는임플란트보철물에서나사풀림현상은흔히발생하는합병증이다. 2.3 이러한임플란트지대주의나사풀림의빈도에대한많은연구가있었다. 6-8 Jemt 4 에의하면 87 명의부분무치악환자에게사용된고정성임플란트보철물에서일년동안가장많이나타난문제점은나사풀림이고상악에서 49%, 하악에서 20.8% 가발생하였다고보고하였다. Henrye 등 7 에의하면금나사를사용하여풀림토크값을측정하여나사풀림의빈도를알아본결과나사의풀림빈도가 18.3% 라고하였다. Jung 등 5 에의하면 년동안의리뷰저널에나사의 풀림빈도가 5.8% 라고하였다. 나사풀림의빈도가줄어드는추세이기는하나, 나사풀림은임플란트보철물에서흔하게발생하는합병증중의하나이다. Bickford 9 는지대주나사풀림의과정을두단계로설명하였다. 일차적으로나사결합부위에외력 ( 저작력 ) 이가해져서나사산사이에미끄러짐이생겨나사에가해진전하중의상실이야기된다. 이차적으로전하중이일정한값이하로줄어들게되고외력과진동에의해마주보는나사산이회전을일으킨다고하였다. 나사의풀림을유발하는원인은외부적요인과내부적요인으로나누어볼수있다. 연구재료 1) 임플란트고정체및지대주오스템 (GS II, Osstem Implant Co. Ltd., Busan, Korea), 짐머임플란트시스템 (Zimmer system ; Zimmer Inc., Warsaw, Germany), 아스트라 (Astra Tech AB, Mölndal, Sweden) 의세시스템을사용하였다 (Fig. 1). 시스템별로사용된지대주는시멘트유지형지대주를사용하였다.

조임토크조절은디지털토크측정기인 Digital strain gauge (DC Strain Gauge Amplifier GM 70, Messtechnik Co., Alfdorf. Germany) 을이용하여 (Fig. 3) 각임플란트제조사가추천한조임토크값을적용하여체결 10 분후다시동일한조임토크를적용하고 5 분후에풀림토크값을측정하였다.

Zembic 등 12 에의하면 3 년동안맞춤형지대주에대한임상관찰결과, 맞춤형 zirconia 지대주와맞춤형 titanium 지대주모두안정적인임상결과를제시한바있다. 그러나아직까지맞춤형나사와기성품나사와의비교연구는이뤄지지않았다. 본연구의목적은임플란트지대주연결에사용되는기성품나사와맞춤형나사의풀림토크값을비교하는것이다. 연구재료및방법 1. 연구재료 1) 임플란트고정체및지대주오스템 (GS II, Osstem Implant Co. Ltd., Busan, Korea), 짐머임플란트시스템 (Zimmer system ; Zimmer Inc., Warsaw, Germany), 아스트라 (Astra Tech AB, Mölndal, Sweden) 의세시스템을사용하였다 (Fig. 1). 시스템별로사용된지대주는시멘트유지형지대주를사용하였다. 본실험에서사용한각임플란트고정체의직경과길이는 Table 1 에제시되어있다. 2) 지대주나사고정체에지대주를체결함에있어서기성품나사로임플란트고정체 - 지대주를연결한것을대조군으로, 맞춤형나사를사용한경우를실험군으로설정하였다. 실험군은각시스템에맞는맞춤형나사를정밀가공업체 (Japan star CNC milling Dellcamsystem, Daeson Co., Seoul, Korea) 에의뢰하여제작하였다. 지대주나사는티타늄합금나사로질량조성은 90% Ti, 6% Al, 4% V 이었다 (Fig. 2). 2. 연구방법 1) 풀림토크값의반복측정세가지임플란트시스템 (Osstem, Zimmer, Astra) 에대해고정체와지대주의연결에기성품나사군 ( 대조군 ) 과맞춤형나사군 ( 실험군 ) 으로총 6 군 (n = 36) 으로나누었다. 조임토크조절은디지털토크측정기인 Digital strain gauge (DC Strain Gauge Amplifier GM 70, Messtechnik Co., Alfdorf. Germany) 을이용하여 (Fig. 3) 각임플란트제조사가추천한조임토크값을적용하여체결 10 분후다시동일한조임토크를적용하고 5 분후에풀림토크값을측정하였다. 이과정을 10 회반복측정하였다. 제조사가추천한조임토크값은 Osstem 임플란트시스템은 30 N 이고, Zimmer 임플란트시스템은 25 N 이고 Astra 임플란트시스템은 30 N 이었다. Ac Ae 최근에맞춤형지대주의보급및확산으로맞춤형지대주의효용성에대한연구가많이되어왔다. 맞춤형지대주는이상적인크기의맞춤형지대주를제작하여상부보철물에들어가는재료의양을줄이고전장도재의두께를일정하게디자인할수있게하여도재파절의가능성을최소화시킬수있는장점이있다. 14 Zembic 등 12 에의하면 18 명환자에게 18 개맞춤형 zirconia abutment, 10 개의맞춤형 titanium abutment 를장착후 3 년간관찰한결과 100% 성공률을보였다고보고하였다. 임상에서나사또한맞춤형으로제작되어사용되는경우가있다. 이러한맞춤형나사에대한비교연구는되어있지않은상태였다. 1 차로반복측정실험에서는 1 회부터 10 회까지반복측정하였을때풀림토크값이감소되는양상을보였다. Haack 등 11 에의하면티타늄재질의지대주나사는잠금및풀림횟수가증가할수록풀림토크가감소한다고보고하였다. 그러나금재질의지대주나사는잠금및풀림횟수가증가해도풀림토크에는유의한차이가없다고하였다. 또한, Lee 등 15 에의하면티타늄지대주나사의 20 회정도의조임과풀림반복시그횟수가증가함에따라풀림토크가유의성있게다소감소했다고하였다. 본연구에서는티타늄재질의지대주나사를사용하였으며이는기존의연구와같은결과이다. 이실험에서는모든군의지대주나사에조임회전력을가하고 10 분후동일조임회전력을한번더적용하였으며, 5 분경과시간을두고풀림토크값을얻었다. Siamos 등 16 에의하면임플란트지대주나사를조인후 10 분후다시 retightening 을제안했다. 이는정착효과에의한초기전하중상실을보상하기위함이다. 2 차실험에서는반복하중을 1,000,000 번가한후측정하였는 246 대한치과보철학회지 2012 년 50 권 4 호

5 Otgonbold Jamiyandorj 김지환 김무성 박영범 심준성 데이는임상적으로약 4 년에해당하는저작횟수이다. 17,18 모든군에서반복하중후에풀림토크력은초기체결된조임회전력에비해상당히감소되는경향이있었다. 하중을준이후에풀림토크값의감소는정착효과에의한것으로사료된다. 지대주나사와임플란트의계면에외력이작용하게되면미세움직임이발생하게된다. 이러한미세움직임으로인하여지대주나사표면의 rough spot 이편평해지면서두계면이더가까워지는것이정착효과이다. Shigley 와 Mischke 19 는초기전하중의 10% 정도가이러한정착효과과정에서상실된다고보고하였다. 이로인하여반복적인조임과풀림을시행할수록나사산의마찰이감소하게된다. 기성품과맞춤형나사에서반복하중후의풀림토크력의감소는유의한차이가없었다 (P>.05). 시편수가많지않았다는단점이있긴하지만맞춤형나사와기성품나사의풀림토크력은유의한차이가없었다. 추가연구로는맞춤형나사를이용한임상평가가필요할것이다. 결론 반복횟수증가에따라모든군에서풀림토크력은유의하게감소하였다. 반복횟수증가에따른기성품나사와맞춤형나사의풀림토크력차이는없었다. 반복측정된풀림력검사와반복하중을적용후풀림력검사에서맞춤형나사와기성품나사의풀림력은유의한차이가없었다. 참고문헌 1. Materials and methods: Three types of implant system (Osstem, Astra, Zimmer) were used. For each system, prefabricated abutment screw (control group) and customized abutment screw (test group) were used to connect the fixture and the abutment (n = 6). Digital torque gauze was used to control the tightening torque and the screws were tightened under each manufacturer's recommendation. 10 minutes after the connection the same tightening torque was applied, and 5 minutes after the second connection, the removal torque was measured. This procedure was repeated 10 times. In the cyclic loading test, 10 minutes after the first connection to the 6 groups (n = 3), the same tightening torque was applied, and a total of 1,000,000 time loading was applied at 30 degree angle to long axis with 50 N load. Repeated measures of ANOVA test (α=.05) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque after cyclic loading. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). In the 10 time repetition test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Also in the cyclic loading test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Conclusion: Within the limitation of this study, there was no significant difference in the removal torque between the prefabricated abutment screw and customized abutment screws.
