Liver function test is rarely abnormal Treatment is surgical removal and the prognosis is good Malignant changes or recurrence after removal are not reported Index Words: Liver neoplas ms, diagnosis 76,31 Children, GI tract 서 유소아의간종괴의대부분은건이성종양으로원빌 성악성종양은그빈도가석으며양성종양또한 " 1 교 적드물마 1, 2 ) 양성종양은대부분복부의종괴를주소로내원하여 1956 년 Edmondson 은이중간엽성과오종 ( mesenchymal harmatoma ) 을간의 였마 2 ) 르르 다흔종양과감벨보고하 순천향의과대학영리학교실 Department 01 Pathology, Soon Chun Hyang Uni versity Hospital 이논운은 1989 년 10- 월 3 1 일접수하여 년 11 월 4 일 에 채택되였음 간의간엽성과오종은드운양성종양으로다른간종양, 즉원딸성간암, 육종, 국소성결절성증식증 (fo cal nodular hyperplasia ), 바기생충성낭종 (s이 ltar y nonparasitic cys t ) 이나선종 (adeno m a), 고립종 ( h epa tic h etero poi es i s ) 등파감벨을요한다 1, 3, 4 ) 간의간엽성과오종은풍부한결합조직간질 (connective tissue stroma) 내의판조직의 (ductal structure) 의혼합으로특정지워지며, 간세포나조혈세포퉁이포함되는경우도있마 3, 5 ) 간염조직은낭포를만들기에충분한액체를포함하는경향이있으여, 이로인해종괴의빠른크기증가를보인마. 최근저자들은순천향대학영원에서간의낭포성간엽성파오종 1 예를경험하였기에문헌고찰파함께보고하는바이다

2 大障放射線훨學용註 : 第 26 卷第 1 號 증 례 환아는 2 세된남아로 2, 3 개월천부터시작되는복 부팽얀파복부종괴를주소로내원하였다. 환아는정상만삭산도분만으로출생하였고, 출생 시체중은 3. 9 k g 로과거력이나가속력상특이사항은 ~ o-:l r- I BÁ λλ -r 이학적검사상우측복부를거의채우는통증이없 고움직임이없는 (not tender, not movable ) 펑딸한 표연을갖는종괴가촉지되었고복부팽만이있었으냐 호흡곤란이나사지의부종은없었다. 헬액검사상 Hb, Ht 와간기능검사소밴검사는모두 정상소견을보였다. 단순복부사진상 ( Fi g 1) 커다란연부조직종괴에 의해장내공기가왼쪽으로전위되어있었으며, 위장 판검사 ( Fi g. 2a) 와소장검사상 ( Fi g.2b) 위는종괴 에의해왼쪽후측으로전위되었고, 소장은왼쪽 A 로 전위되어있었으여, 대장의우걸장곡 (hepat i c fl e. 3 - 홍현숙외 : 간의간엽성과오종 l 례보고 - xure ) 의히 ( 부전위를보였다. 초음파검사상 ( Fig.3 ) 복부전체를거의채우는다발성격악의낭포성종괴가약 14X12 c m 크기로보였으며 CT 소견상 ( Fig.4 ) 커나란낭포성종괴가한쪽옆으로마발성소낭포를가지며간중간에서부터신장위치에까지걸쳐있였고그외다른복막내액체저류 ( fluid coll e ction ) 나이상소견을보이지않았다. 수술소견상종괴는경 ( 혈 pedicl e) 에의해간우엽하방에연결되어있였으여주위조직으로의침윤이나그외의간조직에이상소견은없었다. 종괴는완전히절제되었고환아는수술후정상석안 회복을보여주었다. 육안소견상종괴는크기가 15X 15X8 cm, 우게가 1350 gm 이었고낭포성으로연녹색 의 장액성액체로채워져있었다. 표연은비교적정활하였고정상간과같은피막으로 둘러쌓여있었으며절단연상다양한크기의낭포호구 성되어 있었는데 가장큰낭포의장경은 13 c m 이 었마. 낭포의내변은다소주릎이잡혀있었고담즙파유 사한색소첨착을보였마. 낭포와낭포사이는점액성 의 간엽조직으로구성되어있었고종괴의변연부는 간조직과바숫한조직으로구성되어있었다 ( Fig.5 ) Fig. 5. Ultrasonographic examination shows mul. tiseptated homogenous cystic mass in abdomen between the liver and right kidney, which occupies almost entire abdomen 현미경소견상낭포는주로한층의원주또는입방상피로파복되어있었고낭포백은두꺼운섬유조직으로구성되어있었다 ( Fig. 6a ) 낭포사이의기질은소성의정액성간엽조직으로구성되어있었는데확장된혈관, 임파판및간세포도를포함하고있었다 ( Fig. 6b) 주변부의간조직은정상간조직의구조와상당히유사하였으나증식펀담판들이간세포와우질서하게혼재되어있었고섬유화가심한운액관에렬판파임파판이 8 ] 정상석으로증식되어있었마. 고 찰 간의간엽성파오종은대생기의발생과정의찰옷으 Fig. 4. The stroma is comprised of loose mesenchymal tissue containing vascular spaces and liver cell islands.(he, X40) 척루 (emaciation), 복부통증, 구톡냐밴 B1 혹은설사 6, 8, 9) 황달과함께울혈성심마비 (congestive Heart 종양내석회화는보고된바가없으며수술전간종양신갱아세포종 (neuro blastoma ), 윌릉씨종양 (wilm s tumor), 대망낭종 (omental cyst) 과 bowel duplication 파의감벨이필요하마. 초음파검사상격막을가진커다란낭종종괴로보이고 13, 14 ) 19'Au 이냐 99m Tc-sulfur colloid를이용한간주사 (R adioisotope liver scan) 에서는종괴내의앙사성의약품 (radiopharmaceuticals) 의첨석은없으며이는종양의위치, 형태, 크기및액관조직계등을관찰하는데유용하다. 초음파검사상낭성종괴의강벨은기형종이나기생충성, 바기생충성낭종은단순복부사진상석회화가보이으로강별에도움이되고당관낭션암 (Biliary cystadenoma) 은대부분 30세이상의중년여성에호발하며낭종의코기가더큰것으로강별에도움이쉴수있다. 경정액요로조영술이나위장판검사로는종괴에의한신장, 요판의전위를보이고, 대부분간우엽에서발생하으로장을후땅으로전위시켜서선방으로선위시키는후복막종양과의강별에도움을줄수있으며, 간주사상 vascularity 가없는것이혈관종 (hemangi ma) 과의감별에도웅을줄수있마. 헬판촬영상간운맥헬판 (hilar vessel) 의전위와말단지엽분지 (terminal hepatic branch) 의신전은보이지만종양음영 (tumor s taining ) 은없는것으로알려져있으나때로중정도의신생혈판이보이는경우가 있다고하는보고도있다 5, 9, 10, 16 ) fail ure ) 냐복부압력증가로인한호흡곤란이나대정 액앙박증상도보고되어있다 3, 1 이. 말현시기는신생아에서부터 21 세에이르기까지마 양하지만대부분의경우생후첫 3 년내에발견되여평 균 개월에발견된다 2, 3, 8, ) 나이든소아에서 coma 와감별해야한다. 발견되는경우는 embryonal sar 남녀의벤도는남자가 2 : 1 로않으며대부분의경우 유소아기에서의증상말현이선천성원인에의한종양 임을강하게시사한마. 대부분간기능검사는정상이지만때로헬장단액 병리학적소견상종괴는경계는좋우냐고l 막은없으 며평균 16 cm 이상의크기를가지고 80 % 에서는육 안소견상낭포성변화를보이며표면은매끄렵고암적 색이냐석칼색을보이고드물게는다말성결절로서간 내에서말생하기도한마내부에는호박색이냐황색의 아교질성또는액체성내용물흥가지며괴사냐출헬은 없다. 종괴의급작스러운크기의증가는이러한낭포성조 직에헬장과비슷한액체가빨리고이기때운으로생 각된다 3-6 ) 판미경소견으로는느슨하게배영된원시간엽조직

5 - 홍현숙외간의간엽성파오종 l 례보고 내에담판, 혈관, 임파판및간세포가증식하며분포되 어있으여당판이나병변의주변부위에서골수외조 혈세포나염증세포의침윤을볼수도있다. 낭포는장 액변성펀결합조직으로구성된위낭포와중파세포또 는담판상피세포와유사한세포로파복된진낭포 (true cys t ) 로구성된다. 치료로는외과적우로종괴블경 ( p e di cl e ) 과함께절 제 (excision or enucleation ) 하거나간엽절제술 O o be c t o m y ) 을시행한다. 간엽성과오종의대부분은 우엽에 6 배정도않고, 양엽을모두첨벙하는경우는 10 % 이고, 20 % -1 / 3 의경우경을가지고있다 1, ) 전이나주변간조직에로의침윤이나악성변화에대 한보고는없다. 7~ <= 론 저자들은내원 2-3 개월전부터시작펀복부팽만파 종괴를주소로내원한 2 세된남아에서간의낭성간엽 성과오종을경험하였기에문헨고찰과함께보고하는 바이다. 5.
