Purpose: Hand fractures can be treated conservatively in many cases, but rotation malalignment is one of the important indications for surgical treatment because of dysfunction. We performed open reduction and internal fixation in these malalignment fractures and report clinical and radiological results. Methods: This study included 28 patients (18 male, 10 female) who had metacarpal and phalangeal fractures with rotational malalignment of finger on initial examination. Patients with combined injuries including open soft tissue damage or multiple fractures were excluded. Mean age was 36.1 years and average follow-up period was 14.6 months. Perioperative extent of rotation and correction during the follow-up, union on the radiographs, Range of motion, disability of the arm, shoulder and hand (DASH) score, and pinch power at the last follow-up were evaluated. Results: Average corrected angulation of rotation was 11.9 and no patient showed scissoring appearance of fingers at the last follow-up. All patients showed solid bony union on the radiographs during the follow-up. The average of total active motion of the injured fingers were average 254, average DASH score was 3.2 and average pinch power was 3.0 kg at the last follow-up. Conclusion: Clinical and radiologically satisfactory results were obtained in all patients. Care should be taken not to overlook the rotational misalignment after fracture of the hand, and surgical treatment should be considered to ensure correct reduction and fixation. 2 J Korean Soc Surg Hand Vol. 22, No. 3, September 2017 로어느정도보상이가능한변형과달리회전부정정렬에따른변형은그변형각이원위방향으로전달되어전체수지의운동에장애를일으킬수있고경미한회전부정정렬이라도간과된다면미용적측면뿐만아니라심각한기능장애를유발할수있어진단하여교정하는것이중요하다. 하지만일반적으로단순방사선사진에서각변형과단축등의평면변형은판단이비교적쉬운반면회전부정정렬의유무는파악하기가매우어렵다. 따라서회전변형을판단하기위해서는영상자료뿐만아니라가능한한정확한이학적검사가요구된다. 저자들은회전부정정렬이확인된급성수지골절에서정확한정복을위해수술적치료를시행하였으며이에그임상적및방사선적결과를보고하고자한다. 대상및방법 년 월부터 년 월까지수부중수지또는수지의간부골절환자에서단순방사선사진상수술이필요할정도의전위나각변형을보이지않지만이학적검사에서회전부정정렬을보여관혈적정복및내고정술를시행한환자들을대상으로후향적연구를진행하였다. 제 열골절, 연부조직손상을동반한개방성골절, 다발성수지골절, 부종과통증으로인하여회전정렬측정이어려웠던환자및이전골절의병력이있거나추시관찰이안된환자는제외하였다. 총 명 ( 남자 명, 여자 명 ) 이연구대상에포함되었고평균연령은. 세였으며평균추시기간은. 개월이었다 (Table ). 손상된열은제 열 예, 제 열 예, 제 열 예, 제 열이 예였고골절부위는중수지 예, 근위지골 예, 중위지골이 예였다 (Table ). 수상후수술사이의평균기간은. 일이었고단순방사선사진에대한분석과함께이학적검사를시행하여수술적치료를결정하였다. 초기이학적검사에서환자가통증을인내할수있는범위에서주먹을쥐게하여한수지의조갑판을다른수지가덮는경우를회전부정정렬로판단하고수술적치료의대상으로 정하였다. 회전부정정렬의측정은수술전, 수술후와마지 막외래추시에서 회시행하였다. 수술전마취상태에서골 절된수부와반대편건측수부를수동적으로주먹을쥐는모 양으로수지를굴곡하여하여해당수지와제 수지사이의각 도를수렴각 ( 양성값 ) 또는발산각 ( 음의값 ) 으로정량적으로 측정하여회전부정정렬각으로정의하였다 (Fig. ). 제 열의 중수지및수지골절은제 수지사이의각도를측정하였다. 관혈적정복및내고정을시행하고수동적으로주먹을쥐게 하여각도를다시측정하고외래에서최종추시에능동적으 로주먹을쥐었을때의각도를측정하였다. 골절각은근위와원위골절편의종축에의해형성된각으로 Table 1. Surgical Treatment of Metacarpal and Phalangeal Fracture with Rotational Malalignment 측정했는데 중수지와 수지 위치에 따라 정상각이 다르기 때 2). 수술적 치료에 있어 관혈적 정복과 내고정을 통하여 최대 문에 수술 전과 수술 후 방사선 사진에서 관상면 및 사면 골절 한의 해부학적 정복을 시도하였고 골절의 특성과 고정이 가 각 변화를 측정하였다. 전후면 영상에서 관상면 골절각 변화 능한 공간에 따라 금속판 또는 나사를 이용하여 고정을 시행 를 측정하고 측면 영상은 중수골이 겹쳐 측정에 방해가 되기 하였다. 때문에 사면 영상에서 사면 골절각 변화를 측정하였다(Fig. 최종 추시에서 방사선 사진상 골유합 및 임상적 평가로 회 전 부정 정렬각, 운동 범위, disability of the arm, shoulder and hand (DASH) 점수 및 수상 수지의 파지력을 분석하였다. 수술 전, 후 그리고 최종 추시 3회에 걸쳐 측정한 회전 부 정 정렬각을 방향에 따라 수렴군과 발산군 두 군으로 나누어 반복 분산분석(repeated measures analysis of variance, RMANOVA)을 시행하였다. 수술 후 마취가 깨지 않은 상태에 서 측정한 골절된 수지와 건측 같은 수지의 회전 부정 정렬 각을 대응표본 T 검정(paired t-test)을 이용하여 분석하였다. p<0.05의 값을 통계적으로 유의한 것으로 간주하였다. 결과 17예에서 금속판(LCP Compact Hand, Depuy Synthes, Oberdorf, Switzerland)으로, 9예에서 피질골 나사(LCP Compact Hand, Depuy Synthes)를 이용한 지연 나사술(lag screw technique)로 고정하였고(Fig. 3), 2예에서 금속판과 피 질골 나사 모두 사용하였다. 수술로 교정된 관상면 골절각은 평균 4.2 (수술 전 평균 6.0, 수술 후 평균 2.0 ), 회전 부정 정 렬각(제3열과의 교차각)은 평균 11.9 이었고 수술 후 및 추 시 기간 동안 수지의 교차는 관찰 되지 않았다(Table 3). 관 Fig. 1. Postoperative 그러므로정확한회전부정정렬의평가가중요하지만낮은 재연성으로인하여적절한방법에대한합의가제대로되지 않은채, 몇가지방법이제시되고있다. 기존에는중수지및 지간관절굴곡시켰을때, 수지교차내지는중복여부를판단 하거나 -, 모든수지말단이주상골결절을향하는여부를통 해회전부정정렬을파악하였다,. 수상후부종과통증으로 환자가수지를굴곡하기어려운상황에서는신전한상태에서 조갑판면을관찰하여회전평가를시행할수있지만정확한 방법이될수없다. Tan 등 은수지의회전, 교차, 평행도및 주상골돌기수렴에대한정상적인지표를보고하였는데, 골 절로인한회전부정정렬을보이는경우교정각결정에있어 건측수지와의비교를권유하였다. 본연구에서는굴곡된상 태의골절된열의수지와제 수지와의교차각을측정하여회 전부정정렬각으로정의하고회전부정정렬정도를정량적 으로평가하고자하였다. Paired T-test between immediate postoperative fractured hand angle and unaffected healthy control angle. 수술전단순방사선사진에서측정한평균관상면과시상면 의각변형은각각. 와. 이었다. 기능저하가없는각변형 의범위에대해다소의의견차이는있지만제 중수지 이 상, 제 중수지 이상, 나머지중수지및모든근위지에대 하여 이상각형성이있는경우수술적치료의적응증이 된다,. 본연구에서수술전측정한각변형은모두보존적 치료가가능한범위내에있었으나회전부정정렬이동반되어수술적치료를시행하였다. 회전성부정유합은대개회전부정정렬이있어관혈적정복및내고정이필요하였으나부적절한치료방법을선택하였을때발생할수있고특히근위영역에서도수정복및핀고정으로치료할경우좀더발생할수있다고보고되었다. 각변형은인접한관절운동으로어느정도보상이가능하지만, 회전부정정렬은원위방향으로전달되어변형이더증가하게된다. 따라서중수지골절은말단에위치한수지의골절보다회전변형에더큰영향을주기때문에비록적은양의중수지회전변형이라도수술적적응증이될수있다,,. 중수지골절의 회전은수지말단에서 까지증가될수있으며, 와 의회전변형은각각. cm와 cm의수지중첩을일으킬수있다. 이러한이유로수술적치료를시행하는경우에도정확한정복이이루어져야하며여러보고에서이를위하여관혈적정복및내고정술을권유하고있다,,. 본저자들도회전부정정렬에따른합병증을최소화하기위해가능한세밀한이학적검사를토대로치료방향을결정하고있으며, 수술이필요한경우에는다른저자들의주장과같이관혈적정복및내고정을통해정확한정복과조기재활이이루어지도록하고있다. 본연구의한계점으로는보존적치료또는도수정복및핀고정을시행한대조군이없어다른치료방법과결과를비교하기어려운점이다. 그리고전산화단층촬영 (computed tomography) 을이용한골절각측정이더정확할것으로예상되나회전부정정렬을평가하기어렵고대상환자모두에게촬영이안되어분석에사용되지못하였다. 본논문은정량적방법으로회전부정정렬을측정하는방법을제시하여수술후회전부정정렬의회복의정도를객관적으로보고하여적절한치료방법을제시한면은어느정도의의가있다고생각된다. 결론 본연구에서회전부정정렬을보인수지골절에대한수술적치료로임상적, 방사선학적으로좋은결과를얻을수있었다. 비록단순방사선사진상경미한각변형이있는수부골절이어도정확한이학적검사를통하여회전부정정렬이간과되지않도록주의해야하며확인시에는정확한관혈적정복과견고한내고정의수술적치료를우선고려해야할것으로생각된다. 193
