We compared the results of Treponema pallidum latex agglutination and RPR tests on the automated analyzer with those of TPPA and RPR card tests. Methods: A total of 1,000 samples with negative TPPA results and 103 samples with positive TPPA results ( 1:80 titers) were evaluated in this study. HiSens Auto TP, RPR (HBI, Anyang, Korea) and Mediace TPLA, RPR (Sekisui, Tokyo, Japan) reagents were used on the automated analyzer. FTA-ABS test was performed as a confirmatory test to evaluate the sensitivity and specificity of HiSens Auto TPLA, RPR and Mediace TPLA, RPR reagents. Results: The concordance rate between HiSens Auto TP, Mediace TPLA and TPPA was 95.5% and 95.4%, respectively. The concordance rate between HiSens Auto RPR, Mediace RPR and RPR card was 79.6% and 80.6%, respectively. Sensitivity of HiSens Auto TP and Mediace TPLA was 87.7% and 90.8%, respectively, and specificity was 99.5% and 99.0%, respectively. Received on July 18, 2014. Revised on August 22, 2014. (Korean J Blood Transfus 2014;25:226-234) Key words: Syphilis, TPPA, TPLA 서론 매독의원인균인 Treponema pallidum을검출하기위해서일반적인배양법으로는분리되지않기때문에병소에서균체를직접관찰하는방법이있으나효용성이떨어져혈청학적검사가흔하게이용되고있다. 1) 매독검사의혈청학적방법에는비트레포네마검사 (nontreponemal tests) 와트레포네마검사 (treponemal tests) 가있으며기존의매독진단알고리즘은 VDRL (venereal disease research laboratory), RPR (rapid plasma reagin) 등의비트레포네마검사를먼저시행한후 TPHA (Treponema pallidum hemagglutination assay) 나 FTA-ABS (fluorescent treponemal antibody absorption test) 등의트레포네마검사로확진검사를실시하고있다. 2-4) 혈액관리법시행규칙제8조의혈액의적격여부검사에따라헌혈자선별검사에는매독검사가포함이되어있다. 대한적십자사혈액검사센터는헌혈자매독선별초회검사로혈액형자동화장비 PK7300을통하여트레포네마검사인 TPPA (T. pallidum particle agglutination) 검사를시행하고있으며선별초회검사에서양성인경우에수기법으로 TPPA 반정량검사를시행하고 TPPA 반정량검사가 1:80 이상양성이면 RPR (rapid plasma reagin) card 검사를부가검사로시행하고있다. 최근에매독의진단에주로이용되던 VDRL, RPR card, TPHA 등의수기법을대신하여임상화학장비를이용한라텍스응집비탁법의도입이늘고있는추세이다. 라텍스응집비탁법검사는비 트레포네마검사인 RPR검사 (auto RPR) 와트레포네마검사인 TPLA (T. pallidum latex agglutination) 검사가가능하고임상화학장비를이용하여검사의소요시간이짧으며객관적인검사결과의판독이가능하며한번에다량의검사를시행할수있는장점이있다. 5,6) 이에본연구자들은국내에서많이사용되는시약인 HiSens Auto TP (HBI, Anyang, Korea), HiSens RPR (HBI, Anyang, Korea) 과 Mediace TPLA (Sekisui, Tokyo, Japan), Mediace RPR (Sekisui, Tokyo, Japan) 을헌혈자선별검사에시행하는 TPPA와 RPR card와비교해보고자하였다. 1. 대상 재료및방법 중앙혈액검사센터에서 2013년 7월부터 8월까지의헌혈자검체중에서매독선별재검사결과 Table 1. Study flow diagram. 1:80 이상의양성인혈청검체 103건 (RPR card 양성 43건, 음성 60건 ) 과성분채혈혈장헌혈자검체중에서 TPPA 선별초회검사음성인혈청검체 1,000건을대상으로 4주간연구를시행하였다 (Table 1, Fig. 1). 양성검체는냉장보관하여 1주일이내에검사를시행하였으며, 음성검체는검사당일의중앙혈액검사센터의매독선별초회검사를완료한검체로검사를시행하였다. 2. 방법 1) 혈액검사센터 TPPA 및 RPR card 본원의매독선별초회검사는혈액형자동화장비인 PK7300 (Beckman Coulter, USA) 을이용하여 TPPA 검사를실시하였으며시약은 SERODIA- TPPA Auto (FUJIREBIO INC., Tokyo, Japan) 를사용하였다. 선별초회검사가양성인검체는수기법 TPPA 반정량검사로선별재검사를실시하고, TPPA에서 1:80 이상인검체는부가검사로 RPR card (Asan, Seoul, Korea) 검사를실시하였다. 2) 임상화학장비 TPLA, RPR 검사임상화학장비 AU2700 (Beckman Coulter, USA) 에 HiSens Auto TP, HiSens RPR 시약과 Mediace TPLA, Mediace RPR을장착하여검사를동시에실시하였다. HiSens Auto TP는 recombinant TP항원이감작된라텍스를사용하고 Mediace TPLA는 T. pallidum에서유래한항원성분감작라텍스를사용하였으며혈청중의매독항체가감작라텍스와특이적으로항원-항체반응이일어나형성된응집덩어리에의한탁도를측정하여검체중의항체가를구한다. HiSens RPR과 Mediace RPR은 cardiolipin과 lecithin을부착시킨라텍스를사용하여매독의항지질항체를측정한다. Hisens Auto RPR과 Mediace RPR은검사결과가시약사에서제시한 1.0 R.U. 을넘는경우를양성으로하였다. Hisens Auto TP는시약사에서는 10 U/mL 이상 20 U/mL 미만은판정보류이며 20 U/mL 이상은양성이나 10 U/mL 이상부터양성으로하였다. Mediace TPLA의검사결과가시약사에서는 10 T.U. 이상이양성이었으며이를양성으로하였다. 3) FTA-ABS IgG 및 IgM 검사중앙혈액검사센터의 TPPA 1:80 이상인양성결과와 TPPA 음성검체중임상화학장비로실시한 TPLA 또는 RPR 검사결과의양성결과를객관적으로비교하기위하여외부기관 ( 서울의과학연구 - 228 -
임가영외 : 헌혈자매독선별검사법비교 소 ) 에 FTA-ABS IgG 및 IgM (Zeus, Raritan, USA) 검사를의뢰하였다. FTA-ABS IgG 및 IgM의결과는약양성과양성을양성으로판정하였으며 IgG, IgM 중어느한가지만양성이어도양성으로판정하였다. 3. 통계및결과분석 TPPA와 RPR card 검사결과를임상화학장비 TPLA와 auto RPR 검사결과와비교하여두검사의일치율과 HiSens Auto 시약과 Mediace 시약의각각 TPLA와 RPR의민감도및특이도를평가하였다. FTA-ABS 검사를현재검사센터에서시행하는 TPPA와임상화학자동화장비의 TPLA 결과를비교하였다. 결과 1. 일치율 1) TPPA와 TPLA 일치율 TPPA 1:80 이상의양성인검체 103건과 TPPA 선별초회검사음성인검체 1,000건을대상으로검사를시행하였으며 TPPA와 HiSens Auto TP, Mediace TPLA의일치율은각각 95.5%, 95.4% 였 다. TPPA에서양성인검체 103건중에서 HiSens Auto TP는 46건이음성, Mediace는 40건이음성으로양성일치율은각각 55.3% (57/103), 61.2% (63/103) 이었다 (Table 2). 2) RPR card와 HiSens RPR, Mediace RPR의일치율 TPPA 1:80 이상인양성검체에 103건에대하여 RPR card 검사를시행하였으며 RPR card와 HiSens Auto RPR과 Mediace RPR와의일치율은각각 79.6%, 80.6% 였다 (Table 3). 2. TPLA 민감도및특이도 TPPA 1:80 이상이면서 FTA-ABS IgG 또는 IgM 결과양성인 65건의검체를진양성으로하여민감도를평가하였다. HiSens Auto TP 양성인검체는 57건이었으며, Mediace TPLA 양성인검체는 59 건으로민감도는각각 87.7%, 90.8% 이다. TPPA 선별초회검사결과가음성인검체 1,000 건중에서 TPLA가양성이면서 FTA-ABS IgG 또는 IgM양성인경우 2건과 TPLA 양성이지만 FTA-ABS 검사를시행하지못한 1건을제외하여 997건에대하여특이도를평가하였다. HiSens Auto TPLA 음성인검체는 994건이었으며, Mediace TPLA 음성인검체는 989건으로특이도는각 Table 2. HiSens Auto RPR, Mediace RPR 민감도및특이도 진양성검체는 TPPA 1:80 이상이면서 RPR card 양성인 43건중에서 FTA-ABS IgG 또는 IgM 이양성인 41건으로 auto RPR의민감도를평가하였다. HiSens Auto RPR 양성인검체는 24건이었으며, Mediace RPR 양성인검체는 22건으로민감도는각각 58.5%, 53.7% 이다. TPPA 1:80 이상이면서 RPR card 음성인 60건중에서 auto RPR 양성이면서 FTA-ABS IgG 또는 IgM 양성인 1건을제외한 59건으로특이도를평 가하였다. HiSens Auto TPLA 음성인검체는 58건이었으며 Mediace TPLA 음성인검체는 59건으로특이도는각각 98.3%, 100% 이다. 4. FTA-ABS 와 TPPA, TPLA 결과의비교 TPPA 1:80 이상의양성인검체 103건을대상으로결과를비교하였다. TPPA가양성인 103건중에서는 65건이 FTA-ABS가양성이었다. HiSens Auto TP는 TPLA 양성 57건중에서 57건이 FTA-ABS가양성이었고 Mediace TPLA 양성인검체는 63건중에서 FTA-ABS가 59건이양성이었다 (Table 4). TPPA 음성검체 1,000건중에서 HiSens Auto Table 3. TP와 Mediace TPLA중에 1건이라도양성인경우는 14건으로 HiSens Auto TP는양성의 4건중 FTA-ABS IgG가양성은 1건이었으며, Mediace TPLA는양성의 11건이었으나 1건은 FTA-ABS 검사를시행하지못하였으며 FTA-ABS 검사를시행한 10건중에서 FTA-ABS IgG가 2건이양성이었다 (Table 5). 5. TPLA와 RPR 결과의비교 1) HiSens Auto TP와 HiSens Auto RPR 결과비교 TPPA 1:80 이상의양성인혈청검체 103건과 TPPA 선별초회검사음성인혈청검체 1,000건에대하여 HiSens Auto TP와 HiSens Auto RPR의결과를비교하였다. HiSens Auto는 TPPA 음성검체중에서 TPLA는 4건이양성이었고, HiSens Auto RPR 17건이양성이었다. 2) Mediace RPR과 Mediace TPLA 결과비교 TPPA 1:80 이상의양성인혈청검체 103건과 TPPA 선별초회검사음성인혈청검체 1,000건에대하여 Mediace TPLA와 Mediace RPR의결과를비교하였다. Mediace는 TPPA 음성검체중에서 TPLA에서는 11건, Mediace RPR은 3건이양성이 었다. 고찰헌혈자매독선별검사인 TPPA 검사와임상화학장비로시행하는 HiSens Auto TP, Mediace TPLA의일치율은약 95% 로높은일치율을보였다. RPR card와 HiSens Auto RPR과 Mediace RPR 의일치율은약 80% 의일치율을보였다. HiSens Auto TP와 Mediace TPLA의민감도에비하여 HiSens RPR과 Mediace RPR의민감도는약 50% 대로낮게나왔다. 김등 7), 노등 8) 의연구에서매독환자의경우에는 VDRL이나 RPR card, auto RPR 모두높은민감도를나타냈는데잠복기매독환자의경우에는 VDRL과 RPR card 검사는모두양성이었으나 auto RPR은잠복기매독인경우와매독감염의과거력이있는경우에는 RPR card에비하여양성률이낮게나타났다. 본연구는헌혈자검체를대상으로한것으로 RPR 양성검체의헌혈자들은실제현재매독환자보다는잠복기매독이거나과거력이있는가능성이있을수있다. 또한 RPR card 양성검체는헌혈자에서는확보가어려워 41건만으로자동화 RPR의민 - 231 -

RPR card가 auto RPR보다양성율이더높다고되어있는데이와유사하게 RPR card가음성인검체들은 HiSens Auto RPR이나 Mediace RPR이음성의결과를나타냈다. 7) TPPA 음성이면서 TPLA가양성인검체는 14건으로 FTA-ABS를미시행한 1건을제외하고 2건이 FTA-ABS가양성이었다. 연구대상이된검체들은 TPPA와 RPR card의검사결과에준하여선택한검체들로헌혈자들의임상양상등을확인할수는없어서기존의다른연구를바탕으로유추해볼때 TPPA는음성이고 TPLA가양성인경우는위양성의가능성이높은것으로생각된다. 5) 검사법의변경시에위양성도고려를하여야하나헌혈자선별검사에서는위음성에대한논의가더욱중요시된다. 본연구의한계점은검체의건수에제한이있고특히 TPPA와 RPR card 둘다양성인검체수 (43건) 는부족하였다. 또한검체의정보가 TPPA 와 RPR card 결과외에매독의과거력이나투약력, 헌혈자의다른병력등을알수가없어검사결과를분석하는데제한점이있다. TPPA 음성검체 1,000건중에서 HiSens Auto RPR과 Mediace RPR은각각 17건, 3건이양성이었고이중 FTA-ABS가양성인검체는 HiSens Auto RPR은 4건, Mediace RPR이 2건이었다. 이로볼때현재의 TPPA 선별검사시스템에서 RPR 검사를현재의부가검사가아닌선별검사에포함하는것을고려해볼수있다. 헌혈자선별검사는어느만큼안전한것이충분히안전한것인지에대해서는답하기는어렵다. 매독의원인균인 T. pallidum은열에약하고 20 o C 이하의온도에 72시간이상보관하는경우에는감염성을잃는다고하지만채혈된혈액이 48시간이내에수혈되거나 20 o C 이상에서보관되 는혈소판제제의경우에는매독의감염의위험이있을수있다. 9) 그러나지난 40여년간수혈을통하여감염된사례가없고, HCV, HBV와 HTLV감염의대리표지자가아니며약 1억 4800만혈액중에서 HIV 잠복기 1건정도를검출할수있다고말하고있다. 3) 이에따라헌혈자에대한매독검사의필요성을고려할때가되었다는견해도발표되어추후이에대한논의도필요할것이다. 10) AABB technical manual 17판 11) 에따르면헌혈자의매독선별검사는비트레포네마검사를선별검사로시행하여왔으나최근에는선별검사가트레포네마검사로전환되어자동화장비를사용하고있는추세이다. WHO (world health organization) 에서는매독의수혈전파성감염을최소화하기위하여트레포네마검사를선별검사로권장하고있다. 9,11) 미국 FDA의권고안에따르면 1990 년대이전에는비트레포네마검사를헌혈자선별검사로시행해오다 1990년대가지나면서트레포네마검사를선별검사로시행하고있다고한다. 12,13) 이는트레포네마검사가생물학적위양성율이적고매독이감염이된시기나치료의유무와상관없이검출되어선별검사로시행하면수혈에의한매독의감염의위험성을최소화시켜줄수있기때문이라고한다. WHO 11) 는비트레포네마검사를선별검사로시행하는경우에는민감도와특이도가감소하여유병률이높은경우에한하여고려해볼수있으며헌혈자선별검사로는트레포네마검사를권고하고있다. 대한적십자사혈액검사센터는헌혈자선별검사로트레포네마검사인 TPPA 검사를매독의헌혈자선별검사로시행하고있으며트레포네마검사를선별검사로하는체계를유지해야할것으로생각된다. 본연구는대상검체수가적고헌혈자들에대한임상적정보가없는제한점이있으나 TPPA에서양성이고 FTA-ABS 양성검체중에서 TPLA 결 - 232 -
