대한안과학회지 2008 년제 49 권제 10 호 J Korean Ophthalmol Soc 2008;49(10):1706-1710 DOI : 10.3341/jkos.2008.49.10.1706 = 증례보고 = 외상성공막천공환자에서시행한자가테논낭이식 1 예 배지현 성현경 김하경 남우호 한림대학교의과대학강남성심병원안과학교실 목적 : 외상성공막천공환자에서일차공막봉합이어려운경우에시행한자가테논낭이식 1 예를보고하고자한다. 증례요약 : 81 세남자환자가쇠뿔에찔린뒤발생한우안시력저하를주소로내원하였다. 내원당시교정시력은 0.1 로각막의상부에위치한 5~6 mm 정도의공막천공부위로홍채가돌출되어있었다. 응급수술시공막천공부위의결손으로일차봉합이어려워, 동측의하내측에서테논낭을분리하여자가이식술을시행하였다. 술후안구는잘유지되었으며, 수술 3 개월후초음파수정체유화술및후방인공수정체삽입술을성공적으로시행하였다. 결론 : 외상으로발생한공막결손부의치료방법으로시행한자가테논낭이식후안구가성공적으로유지되었다. 따라서공막결손시여러치료방법중하나로자가테논낭이식을고려해볼수있겠다. < 대한안과학회지 2008;49(10):1706-1710> 외상성공막천공환자에서일차봉합이어려울경우, 천공부위의크기와위치에따라자가결막이식술, 1 동종공막이식술, 2-5 자가공막이식술, 6,7 자가측두근막이식술, 8 대퇴근막이식술, 5 골막이식술, 9 부분진피이식술 10 등이이용될수있다. 본증례는외상성공막천공이발생한환자에서공막결손으로일차봉합이어려운상태에서동측하내측에서분리한테논낭을자가이식하였다. 본증례에서술후안구가성공적으로유지되고초음파수정체유화술및후방인공수정체삽입술도성공적으로시행하여시력개선을보였기에자가테논낭이식술이공막결손시여러치료방법중하나로고려될수있음을보고하고자한다. 증례보고 81 세남자환자가내원당일오후 5 시경쇠뿔에찔린뒤발생한우안시력저하를주소로내원하였다. 과거력에서당뇨나고혈압등의전신질환은없었으며, 안과 < 접수일 : 2008 년 1 월 23 일, 심사통과일 : 2008 년 7 월 7 일 > 통신저자 : 남우호서울시영등포구대림 1 동 984-1 한림대학교강남성심병원안과 Tel: 02-829-5193, Fax: 02-848-4638 E-mail: eyedrnam@naver.com * 본논문의요지는 2007 년대한안과학회제 98 회추계학술대회에서포스터로발표되었음 적으로좌안은어릴때부터잘보이지않았다고진술하였다. 수상 6 시간후검사에서교정시력은우안 0.1, 좌안은광각이었다. 세극등검사에서우안은결막충혈과각막부종소견을보이고, 상부각막윤부에근접하여발생한 5~6 mm 길이의공막천공부위가관찰되었다. 천공부는불규칙한변연부와약간의이물이관찰되었으며, 외상후시간경과에따른조직의위축및탈수소견이관찰되었다. 또한천공부로홍채가탈출되어있으며일부홍채조직은소실된상태였다. 이와더불어전방허탈, 전방출혈, 성숙백내장소견을보였으며안저는관찰되지않았다. 좌안은세극등검사에서각막혼탁과홍채유착소견을보였다. 안와전산화단층촬영에서우안의안구내이물은관찰되지않았고, 안구초음파검사에서유리체출혈이나망막박리등의특이소견은관찰되지않았다 (Fig. 1). 세균배양을실시한뒤, fortified vancomycin (500 mg/v 5% solution), fortified tobramycin (80 mg/2 ml/v 2% solution) 점안액을매시간마다점안하며, vancomycin 500 mg, ceftazidime 1 g 을각각 1 일 2 회씩정맥주사하였다. 내원당일전신마취하에공막천공에대한일차봉합술을시행하였으나, 이미수상후 16 시간이경과한후였다. 공막천공부의이물을평형염액 (Balanced Salts solution) 으로세척한뒤, 8-0 vicryl 로공막봉합을시도하였다. 그러나불규칙한변연부로인한공막결손부로일차봉합술은일부만가능하였다. 공막결손부를재건하기위해동측하내측에서결막박리후 8 8 mm 의테논낭을분리하여 1706

It showed diffuse corneal edema and 5~6 mm full-thickness scleral laceration with protruded uveal tissue. 얻은이식편을채취하였다. 이식편으로공막결손부를덮은뒤 10-0 nylon 으로주변각공막에봉합고정하였다. 이식편의수여부및공여부의결막을 8-0 vicryl 로봉합한뒤수술을마쳤다 (Fig. 2). 수술종료후치료용콘택트렌즈를착용시키고 1% atropine sulfate (Ocutropine ), fortified vancomycin (500 mg/ V 5% solution) 과 fortified tobramycin (80 mg/ 2 ml/v 2% solution) 점안액을모두 6 시간간격으로점안하였다. 술전에시행한세균배양검사는음성소견을보였다. 술후 3 일째 1% prednisolone acetate (Pred forte 1%, Allergan, Irvine, CA) 점안액을 6 시간간격으로추가점안하였다. 술후전방이얕은소견이보였지만점차전방이형성되었고, 술후 7 일째부터는비교적안정된상태로회복되어 fortified vancomycin 과 fortified tobramycin 점안액의사용을중지하고, 0.5% moxifloxacin hydrochloride (Vigamox, Alcon, Fort Worth, TX) 점안액을 6 시간간격으로점안하였다. 술후 13 일째퇴원시 1% atropine sulfate (Ocutropine ), 0.5% moxifloxacin hydrochloride (Vigamox ), 0.1% fluorometholone (Ocu metholone ) 점안액을 6 시간간격으로점안하도록하였다. 수술 3 주후우안교정시력 0.15, 안압은 9 mmhg 이었고, 이식편위로섬유혈관막이자라들어가는것이관찰되었다 (Fig. 3A). 수술 3 개월후교정시력은 0.3 이고, 이식편이생착되어가는소견보여초음파유화술로백내장을제거하고, 6.0 mm 직경의 hydrophilic acrylic IOL (I-Flex, I-Medical intraocular Lenses) 을성공적으로수정체낭내삽입하였다. 자가테논낭이식 6 개월후우안교정시력 1.0, Figure 2. It showed autologous Tenon s capsule patch graft (5 4 mm 2 ), which was covered by a conjunctival flap at the superior limbal area. 안압 8 mmhg 이고자가테논낭이식편이성공적으로생착되고각막내피세포검사에서평균세포면적 (cell density) 도 1030/mm 2 로유지되었다 (Fig. 3B, 4). 고 찰 공막결손이있을경우안내염, 시력저하, 전방허탈, 저안압증, 맥락막박리등의합병증이동반될수있다. 따라서결손부의크기와위치, 기저질환의종류, 결막의상태, 환자의나이, 과거력, 시력, 수술후기대치등을고려하여적절한방법을사용하여결손부위를재건하여야한다. 작은결손부의경우치료용콘텍트렌즈, 압박안대, symblepharon ring, 11 조직접착제 (cyano acrylate glue), 일차봉합술, 결막이식술등을이용하여재건할수있으나큰공막결손의경우판이식 (patch graft) 을시행하여결손부위를재건해주어야한다. 이때이용될수있는방법으로자가결막이식술, 1 동종공막이식술, 2-5 자가공막이식술, 6,7 자가측두근막이식술, 8 대퇴근막이식술, 5 골막이식술, 9 부분층진피이식술 10 등이있다. 여러가지이식편중공막은유연성이있는무혈관조직으로동종이식의경우기증공막을보관하기쉽고전층을이식할수있다는장점이있다. 3 자가공막이식의경우동종이식에비해면역반응이적고, 다른이식자의기증을거부하는경우유용히사용될수있다. 7 그러나공막은무혈관조직으로이식후혈관화가잘이루어지지않을경우이식편의괴사의가능성이있다. 자가결막이식은이식편의섬유혈관화가잘이루어진다는장점이있으나공막지지정도가부족하여큰공막결손에는사용되기어렵다. 5 이외에자가측두근 1707

(B) Anterior segment photograph 3 months after phacoemulsification and IOL implantation in the bag. 막, 자가대퇴근막, 자가골막, 부분층진피등도이식편으로서그효과가좋은것으로알려져있으나눈이외의다른조직에서이식편을채취하여야한다는단점이있다. 본증례에서는외상성공막천공환자에서응급수술시자가테논낭이식술을시행하여공막결손을재건하였다. 자가테논낭이식편은다른합병증없이성공적으로생착되어술후 3 개월째백내장제거술및후방인공수정체삽입술을시행하여술후 6 개월째교정시력 1.0 으로개선되었다. 1997 년 Lee et al 1 이국소공막괴사환자 15 안을대상으로자가결막 / 자가테논낭이식술을시행하여성공적인결과를보고한바있으나외상성공막천공에서시행한자가테논낭이식술에대한보고는현재까지국내에없는상태이다. 테논낭은각막을제외한안구전체를 둘러싸는막으로탄력섬유조직으로구성되며혈관조직이풍부한특징을가지고있다. 따라서자가테논낭이식은자가결막이식에비해공막지지력이크면서자가공막보다생착력이좋고, iatrogenic staphyloma, choroidal injury 의가능성을낮출수있는유용한방법이라고생각된다. 또한응급수술시공여조직이준비되지않을경우비교적쉽게이식편을얻을수있고눈이외의다른부위의수술을필요로하지않는장점이있어좋은이식편이될수있을것으로사료된다. 참고문헌 1) Lee CO, Jong SH, Lee JJ. Case summary: An 81-year-old man presented with loss of vision in his right eye after a perforating injury caused by a cow horn. Examination showed a laceration of the sclera at 12 o clock approximately 5~6 mm in length, and a uveal tissue was prolapsed into the wound. The best corrected visual acuity was 0.1. Primary repair of the eye was insufficient because of tissue loss. The inferonasal Tenon s capsule graft was carefully dissected from the sclera and tailored to fit the defect. The graft was covered with a conjunctival flap. The scleral defect was successfully closed with the autologous Tenon s capsule graft. Three months after grafting, phacoemulsification with intraocular lens implantation was performed. Conclusions: Autologous Tenon s capsule graft is an effective measure to repair traumatic scleral defects and is useful when patch grafts are unexpectedly needed.
