홍석호 곽호신 정희원 안이 1.2로 우안 시력의 악화가 있었고, 우측 측두 반맹 및 압박하고 있는 것이 관찰되었다. 시신경과 시각교차, 양측 좌측 상측두 사분구의 시야 결손 소견이 관찰되었다.

미세 수술기법으로 낭종 심되었으나 좀 더 경과를 관찰하기로 하였다. 수술 후 8개 을 모두 제거하였고, 시신경과 내경동맥, 전뇌동맥 등은 손 월 뒤 시행한 추적 검사에서 시력은 우안이 0.02, 좌안이 상 없이 모두 보존하였다. 수술 후 환자는 시력의 호전을 보 0.

환자의 임상 경과가 기왕에 알려진 라스케열 낭의 양상과 일치하지 않는 것을 확인하기 위하여 병리과에 의뢰하여 조 2차 수술은 개두술로 테리온 접근법을 시행하였다.

2 홍석호 곽호신 정희원 안이 1.2로 우안 시력의 악화가 있었고, 우측 측두 반맹 및 압박하고 있는 것이 관찰되었다. 시신경과 시각교차, 양측 좌측 상측두 사분구의 시야 결손 소견이 관찰되었다. 당시 내경동맥과 양측 전뇌동맥에 낭종 벽과의 유착이 관찰되었 시행한 sellar MRI에서는 이전의 낭종이 터어키안 상부로 으나, 뇌하수체 줄기와는 유착이 심하지 않았다. 낭종은 터 진출하여 양측의 시신경과 시각교차를 압박하는 소견이 의 어키안 내부까지 연장되어 있었다. 미세 수술기법으로 낭종 심되었으나 좀 더 경과를 관찰하기로 하였다. 수술 후 8개 을 모두 제거하였고, 시신경과 내경동맥, 전뇌동맥 등은 손 월 뒤 시행한 추적 검사에서 시력은 우안이 0.02, 좌안이 상 없이 모두 보존하였다. 수술 후 환자는 시력의 호전을 보 0.5로 더욱 감소하였고, sellar MRI에서도 낭종이 양측 시 였고, 신경학적 결손 없이 회복되었다. 신경을 압박하는 소견이 진행하여(Fig. 4) 2차 수술을 계획 하였다. 환자의 임상 경과가 기왕에 알려진 라스케열 낭의 양상과 일치하지 않는 것을 확인하기 위하여 병리과에 의뢰하여 조 2차 수술은 개두술로 테리온 접근법을 시행하였다. 수술 직연역화학 기법으로 측정한 세포증식지수 검사에서 상피세 소견 상 얇은 낭종 벽과 연갈색의 균질한 크림성상의 내용 포의 20%에서 Ki-67 항체에 염색되는 것을 관찰할 수 있 물을 가진 낭종이 뇌조 공간을 채우며 시신경과 시각교차를 었다(Fig. 2. ) Fig. 1. 병리조직학적으로 라스케열 낭은 단층 또는 위다층의 상 피와 그를 지지하는 결체조직으로 구성되어 있다. 상피는 섬모 세포, 배상 세포, 편평 세포, 기저 세포로 이루어져 있 으며, 면역조직화학검사에서는 GFP, vimentin, cytokeratin 에 양성의 반응을 보인다.
