However, occasionally it causes bacteremia and localized infection requiring antibiotic treatment. A third-generation cephalosporin or fluoroquinolone is often the first choice of antibiotic. However, there has been an increase in nalidixic-acid-resistant nontyphoidal Salmonella with decreased fluoroquinolone susceptibility. Although there have been many cases reported of nontyphoidal Salmonella, no cases of nalidixic-acid-resistant nontyphoidal Salmonella have been reported in Korea. Here, we report on the case of a 61-year-old man with a diagnosis of acute osteomyelitis and periosteal abscess of the femur caused by nalidixic-acid-resistant Salmonella enteritidis. S.enteritidis osteomyelitis - 않으나균혈증및국소감염이발생한경우 fluoroquinolones 혹은 3세대 cephalosporins로치료하는데 1990년대부터낼리딕산 (nalidixic acid) 내성 (fluoroquinolone에대한감수성저하 ) 을보이는비장티푸스성살모넬라균이전세계적으로증가하고있어문제가되고있다 [1]. 국내에도비장티푸스성살모넬라균의낼리딕산내성률이높은것으로알려져있으나 [2] 낼리딕산내성균주에의한증례는보고된바없다. 이에저자들은건강한환자에서발생한낼리딕산내성비장티푸스성살모넬라균에의한대퇴골골수염및골막농양을 16주간고용량의 fluoroquinolone 투여로치료한증례를경험해보고하는바이다. 증례환자 : 61세남자주소 : 우측대퇴부통증현병력 : 2주전부터우측대퇴부통증이발생했으며 1주전부터발열과함께걷지못할정도로대퇴부통증이심해져서입원했다. 과거력 : 20년전갑상선기능항진증을진단받았으나 3년간약물복용후호전되어이후로중단했다. 8년전대퇴골두괴사로양측고관절치환술을받은과거력이있으나이후대퇴부에외상이나침습적시술을받은일은없었다. 가족력 : 특이사항은없었다. 여행력 : 특이사항은없었다. 신체검사소견 : 내원당시혈압 120/70 mmhg, 맥박수 80 회 / 분, 호흡수 20회 / 분, 체온 38.5 였다. 급성병색을보였고의식은명료했으며우측대퇴부에통증, 압통, 열감이동반되어있었다. 두경부및흉부진찰, 복부진찰에서이상소견을보이지않았다. 간과비장은촉지되지않았고임파절비대도없었다. 피부에발진등은보이지않았다. 검사실소견 : 내원당시시행한말초혈액검사에서백혈구 5,400/mm 3 ( 호중구 72.7%, 림프구 20.0%, 단핵구 6.9%, 호염기구 0.3%, 호산구 0.1%), 혈색소 12.0 g/dl, 혈소판 223,000/ mm 3 였고혈액응고검사는 PT 13.7초 (international normalized ratio [INR] 1.04), aptt 47.9초였다. 생화학검사에서혈당 115 mg/dl, BUN 18.1 mg/dl, creatinine 0.83 mg/dl, 총단백 6.9 g/dl, 알부민 3.7 g/dl, AST 45 IU/L, ALT 35 IU/L, 총빌리루빈 0.7 mg/dl, Na/K/Cl 132/3.8/98 mmol/l 였다. 적혈구침강 속도 76 mm/hr, C-reactive protein (CRP) mg/l 로상승되어있었고 creatine phosphokinase 973 IU/L, myoglobin ng/ml로상승이있었다. 요검사에서농뇨, 혈뇨, 단백뇨는없었다. 감염혈청검사에서 HBs Ag, anti-hbv Ab, anti-hcv Ab, anti-human immunodeficiency virus Ab, venereal disease research laboratory (VDRL) 모두음성이었다. 방사선소견 : 단순흉부방사선촬영에서이상소견은없었고고관절및대퇴골방사선촬영에서고관절수술후상태로특이소견은보이지않았다. 치료및경과 : 입원당일혈액, 소변, 대변배양검사실시후경험적으로정주 cefazolin 하루 6 g과정주 netilmicin 하루 435 mg (6.5 mg/kg/day) 을투여하기시작했다. 입원 2일째골반전산화단층촬영검사에서양측고관절수술부위에이상소견은없었고입원 3일째하지초음파에서대퇴직근 (rectusfemoris) 내측근막의찢김 (tearing) 소견이있었다. 입원 4일째입원당일실시한혈액배양검사한쌍에서살모넬라 D군 (Salmonella, Group D) 이동정되었다. 4 cm 크기의심방중격결손외특이소견은없었다. 입원 3일째에추가로시행한혈액배양검사두쌍에서는최종적으로균이동정되지않았으며항생제와함께 nonsteroidal anti-inflammatory drugs (NSAIDs) 를투여했고발열및대퇴부통증이점차호전되어퇴원했다.

(B) A color Doppler US shows a peripheral hyperemia. 항생제감수성결과 ciprofloxacin, levofloxacin 에감수성이확인되어 (Table 1) 정주 levofloxacin 500 mg 1회투여로항생제를변경했다. 소변과대변배양검사에서자라는균은없었다. 같은날시행한심장초음파에서는 0.4 cm 크기의심방중격결손외특이소견은없었다. 입원 3일째에추가로시행한혈액배양검사두쌍에서는최종적으로균이동정되지않았으며항생제와함께 nonsteroidal anti-inflammatory drugs (NSAIDs) 를투여했고발열및대퇴부통증이점차호전되어퇴원했다. 항생제는정주 levofloxacin 4일투여후경구 levofloxacin 500 mg을 10일간투여했다. 이후증상없이지내다가항생제종료후 3주뒤부터간헐적인발열및대퇴부통증, 부종이발생하여재입원했다. 입원당일 38.8 의발열이있었고백혈구 9,600/mm 3 ( 호중구 53.1%), CRP mg/l였으며입원 2일째하지전산화단층촬영에서우측대퇴골에인접하여심부근육내에세로로긴 1.6 cm 6 cm 크기의공동성병변이있었다 (Fig. 1). 다음날하지초음파를실시했고골막농양 (periosteal abscess) 과대퇴골피질골파괴 (cortical destruction) 의골수염을시사하

Cases of osteomyelitis caused by nontyphoidal Salmonella species reported in Korea No. (yr) Sex/Age Site Underlying disease Etiology Treatment Refs. 1 (1990) M/48 Patella DM Salmonella group C Chloramphenicol 1 g/day, Ampicillin 4 g/d [7] for 9 wk 2 (1999) F/35 Femur SLE S.enteritidis Cefazolin 3 g/d, Ciprofloxacin 400 mg/d [8] for 6 wk 3 (2000) F/29 Tibia SLE Salmonella species Ceftriaxone for 6 wk [8] 4 (2005) M/57 Lumbar spine None Salmonella group D Ciprofloxacin for 8 wk [9] 5 (2008) M/34 Thoracic spine None S.othmarschen Ciprofloxacin 400 mg/d for 8 wk [10] 6 (2010) M/61 Lumbar spine CKD Salmonella group D Ciprofloxacin for 4 wk, Ciprofloxacin and Ceftriaxone for 9 wk [9] DM, diabetes mellitus; SLE, systemic lupus erythematosus; CKD, chronic kidney disease. 는소견이있었으며 (Fig. 2) 초음파가이드하에 6 ml의농양을흡인천자했다. 입원당일실시한혈액배양검사에서최종적으로자라는균은없었고농양배양검사에서는이전과동일한감수성을갖는살모넬라 D군이동정되었다. 입원후 9일간정주 cefepime 을하루 6 g 투여했고농양배양검사결과를확인한후낼리딕산내성가능성및골조직투과력을고려하여고용량정주 levofloxacin 750 mg을하루 1회로변경하여 10일간투여했다. 입원 17일째골수염및골막농양의경과확인을위해자기공명영상검사를실시했으며농양크기는 2.5 cm 2 cm 10 cm로큰호전이없어입원 19일째절개및배농수술을했고농양배양검사에서자라는균은없었다. 기존의초음파흡인검체에대해비장티푸스성살모넬라균에대한낼리딕산감수성검사를추가로실시했다. 입원 20일째 MicroScan Neg Combo Panel 53 (Siemens Healthcare Diagnostics Inc.) 을추가로이용하여시행한낼리딕산감수성검사에서최소억제농도 16 µg/ml 이상의내성균임을확인했고고용량정주 ciprofloxacin 400 mg을하루 3 회로항생제를변경하여 9일간투여한후증상및혈액검사소견이호전되어퇴원했다. 경기도보건환경연구원에혈청형확인을의뢰했는데원인균은 Salmonella enteritidis로확인되었다. Erythrocyte sedimentation rate (ESR), CRP 는첫입원시 76 mm/hr, mg/l, 재입원시 75 mm/hr, mg/l였고퇴원시 CRP는 mg/l로감소했다. 퇴원후경구 ciprofloxacin 750 mg을하루 2회투여했으며초음파추적검사를퇴원 2주, 6주, 12주경에실시했고퇴원 13주혈액검사에서백혈구 6,600/mm 3 ( 호중구 35.0%), ESR 9 mm/hr, CRP 3.01 mg/l로염증지표가안정된소견을보여항생제치료를 종료했다. 골수염및농양이호전되기까지총 112일간고용량 fluoroquinolone 치료를했으며치료종료후 1년이상외래추적시재발이없었다. 고찰살모넬라는장내세균과 (family) 살모넬라속 (genus) 에속하는그람음성막대균으로사람과동물에다양한질병을일으킨다. 비장티푸스성살모넬라균은감염경로에다양한동물이병원소로존재하며동물들의가공품이나부산물로써오염된식품들에의해전파된다. 농장에서의오염방지노력, 냉장시스템사용등으로상당히감소했으나선진국의경우식품가공의집중화와광범위한식품집배로오히려증가하고있으며국내의경우 1999년 2,000예까지증가했다가최근에는매년 500예이내로보고되고있다 [2]. 주로급성위장염을일으키며대부분항생제치료없이호전되나균혈증및국소감염이있는경우항생제치료가필요하다. 국소감염중골수염은살모넬라감염의 1% 이하에서발생하는드문합병증으로겸상적혈구빈혈증, 남성, 결합조직질환, 면역저하환자에서잘발생하며대퇴골, 경골, 상완골, 요추에호발한다고알려져있다. 영상의학적검사와배양검사로진단하고 4주이상의항생제치료와필요시수술적치료도이루어진다 [1]. 본증례의환자는갑상선기능항진증외에기저질환은없었으며병력에서살모넬라균혈증이발생할만한원인은찾을수없었다. 균혈증발생후대퇴부의인공삽입물및대퇴직근의찢김이국소감염의유발요인으로작용했을것으로추정된다

5 - The Korean Journal of Medicine: Vol. 88, No. 3, 비장티푸스성살모넬라감염의치료는 1990년대초다약제내성균이발생하면서 3세대 cephalosporin 또는 fluoroquinolone을 1차치료제로사용하고있다. 그러나낼리딕산내성, fluoroquinolones에대한감수성저하를보이는균이전세계적으로증가하고있으며낼리딕산내성균주는검사상 fluoroquinolones에감수성임에도불구하고임상적으로치료반응이저하되거나치료실패가발생할수있어주의를요한다 [1]. 우리나라에서도비장티푸스성살모넬라균의낼리딕산내성률이 년 1.8% 에서 년에는 21.8% 로증가했고 [2] 2013년질병관리본부보고에따르면전체살모넬라균을포함한검체는 33.8% 에서내성을보였고가장많이분리되는균주인 S.enteritidis는 85.9% 에서내성을보여낼리딕산내성균주가빠르게증가하고있음을알수있다 [3]. 낼리딕산내성살모넬라균은 3세대 cephalosporins, azithromycin 또는고용량 fluoroquinolone으로치료하며 [1] cephalosporin 과 fluoroquinolone 병합치료도보고되고있다 [4,5]. 낼리딕산내성비장티푸스성살모넬라감염은국외에서 1990년부터총 7예가보고되었는데 [4-6] 화농성관절염 2예, 균혈증, 상처감염, 골수염, 요로감염, 둔부농양각 1예였다. 원인균은 Salmonella typhimurium이 4예로가장많았고 S.enteritidis, Salmonella virchow, Salmonella choleraesuis 가각 1 예였다. 항생제치료는 3세대 cephalosporin과 fluoroquinolone 병합치료 2예, trimethoprim, aztreonam, 3세대 cephalosporin 단독치료각 1예이며 2예는치료법을확인할수없었다. 골수염증례는 3세대 cephalosporin과 fluoroquinolone 병합치료 3주, 3세대 cephalosporin 단독치료 5주로호전되었다 [4]. 우리나라의비장티푸스성살모넬라균에의한증례는 1980 년대부터 30예이상보고되었는데이중골수염은 6예였다 (Table 2) [7-10]. 남녀비는 2:1, 평균연령은 44세였고원인균은 Salmonella D군 3예 (S.enteritidis 1예포함 ), Salmonella C군 2예 (Salmonella othmarschen 1예포함 ), 혈청형이확인되지않은것이 1예였다. 항생제치료는 fluoroquinolone 단독치료 2예, 3세대 cephalosporin 단독치료, 3세대 cephalosporin과 fluoroquinolone 병합치료, 1세대 cephalosporin과 fluoroquinolone 병합치료, chloramphenicol 과 ampicillin 병합치료각 1예였다. 낼리딕산내성균주가문제로대두되고있으나국내증례중에낼리딕산내성을확인한예는없었다. 본증례에서는첫입원시균혈증에대해표준용량의 fluoroquinolone을 2주간투여했으나치료종료후 3주뒤대퇴 골골수염및골막농양이발생했다. 항생제와 NSAIDs 투여로일시적으로증상이호전되었으나완전히치료되지않은국소감염이골수염및골막농양으로진행된것으로생각된다. 재치료시초음파가이드하농양흡인천자를실시했으나충분한배농이이루어지지않아고용량 levofloxacin 치료에도호전이없었고수술적배농후농양이점차호전되었다. 초치료시실시하지않은낼리딕산감수성검사를추가로실시하여내성을확인했으며국내의높은낼리딕산내성률과 2주간의치료기간을고려할때치료과정중획득내성의가능성보다는기존낼리딕산내성균주에의한감염인것으로생각된다. 낼리딕산내성, 골조직투과력및경구흡수율을감안하여고용량 fluoroquinolone으로치료했으며골수염과골막농양이동반되어수술적배농과장기간의항생제치료가필요했다. 질병관리본부보고에따르면 S.enteritidis는 85.9% 의높은낼리딕산내성을보이나 S.typhimurium은 29.3%, 기타혈청형은 17.1% 로내성률이상대적으로낮다 [3]. 따라서내성률이높은 S.enteritidis가동정된경우에는추가적인낼리딕산감수성검사없이고용량 fluoroquinolone 또는 3세대 cephalosporin 치료를고려해볼수있겠다. 하지만이증례에서와같이 Salmonella균의혈청형이확인되지않는경우에는낼리딕산감수성검사를실시하거나혈청형을확인하여치료를결정하는것이과다한항생제사용을줄이고내성균의증가를막는바람직한방법으로생각된다. 식용동물과동물성사료에서항생제를광범위하게사용하면서비장티푸스성살모넬라균의항생제내성문제는앞으로더중요해질것이다 [1]. 낼리딕산내성균주가빠르게증가하고있어 fluoroquinolone에감수성을보이더라도혈청형또는낼리딕산내성을확인하여고용량 fluoroquinolone 및 3세대 cephalosporin 치료를고려하는것이치료반응저하또는치료실패를줄이는방법이될것으로생각된다. 요약비장티푸스성살모넬라균은주로급성위장염을일으키고자연호전되는경우가많지만균혈증이나전이성국소감염을일으킨경우항생제치료가필요하다. 3세대 cephalosporin, fluoroquinolone이 1차치료제로사용되나 1990년대부터낼리딕산내성균주에의한감염이보고되었고이경
