We performed a systematic review to assess the effects of cigarette package with the graphic warnings on smoking cessation and summarized the studies which had been conducted. Methods: Using a prespecified search strategy, we identified the studies conducted in Korea from five Korean databases (DBPIA, KISS, KMBASE, NDSL, RISS) and three non-korean databases (EMBASE, MEDLINE, PUBMED). This review included published articles until August 2017, which focused on the effect on smoking cessation as the outcome. We considered all types of study designs. Results: Nine studies met our inclusion criteria, involving 3,522 participants in total. According to the studies, the graphic warnings on tobacco packaging significantly reduced the prevalence of smoking. Confidence in all findings was limited, due to the nature of the evidence available, and was therefore rated VERY LOW for eight studies and LOW for one study by GRADE standards. There were two studies which did not include exact effect size. All studies were cross-sectional study which didn t estimate the long term effect. Three studies analyzed the intermediate effect. All studies had limitations due to relatively small sample size, their study designs and conducting with self-administered questionnaire without objective measure. Five studies were published in Advertising & Marketing Journal. Conclusion: As graphic warning on tobacco packaging has already been implemented, well designed research need to be initiated to capture the long-term and objective effects on tobacco cessation for public health. 2 정소화외 : 국내담뱃갑경고그림의금연효과에관한체계적문헌고찰 서론 1. 연구의필요성흡연이건강전반에악영향을미친다는사실은여러국내외연구를통해알려져있다.(1,2) 흡연은폐암, 식도암, 구강암등각종암의원인으로알려져있으며, 이외에도심혈관질환, 호흡기질환은물론백내장, 위궤양, 치주염, 감염성질환에도영향을주어건강전반에영향을미친다.(3) 매년전세계의 700만명정도가담배로인해사망한다.(4) 2012년기준 30년이상조사한결과에따르면모든사인의사망자 26 만명중담배로인한사망자수는 5만 8천여명으로추정되었다. 성인남성에서는 4만 9천여명으로전체성인남성사망의 34.7% 를차지한다. 2013년기준으로, 우리나라에서흡연으로인해사회ㆍ경제적으로소모되는비용은 7조원이상이다.(5) 담배로인한피해가증가함에따라, 세계보건기구에서 2005 년에발효된담배규제기본협약 (Framework Convention on Tobacco Control, FCTC) 을바탕으로금연정책은더강력하고지속적인방향으로변하는추세이다. 전세계적으로금연을촉구하는움직임이다양한방식 ( 금연클리닉, 웹기반금연서비스, 금연상담전화, 대중매체캠페인등 ) 으로진행되고있다.(4) 우리나라는 1995년국민건강증진법제정후공공시설금연구역지정, 담뱃값인상등금연정책을지속적으로시행하였다. 더불어, 담뱃갑경고그림도입을 2002년부터 11차례시도하였다. 2016년 12월 23일흡연의위험성을알리는담뱃갑경고그림삽입의무화정책을시행하였다.(6) 세계보건기구에서는강력한이미지를포함한담배포장의건강경고는담배가건강에미치는악영향을전달하는효율적수단이라고밝히며담뱃갑경고그림부착을권고한바있다.(7) 2001년캐나다의담뱃갑경고그림부착정책을시작으로현재 100여개이상의나라가이정책을채택하고있다.(8) 2012 년 12월호주에서는담뱃갑패키지표준화 (plain packaging) 라는더욱강력한정책을시행하였다. 2017년프랑스, 영국에서도담뱃갑패키지표준화를채택하였으며뉴질랜드를포함한다른많은나라에서도담뱃갑패키지표준화정책을시행준비중이다.(9) 담뱃갑경고그림이도입된이후담배판매량이 2월까지는지속해서감소 (12 월 : 2억 9천만갑, 1월 : 2억 8천만갑, 2월 : 2억 3천 8백만갑 ) 하였으나 3월 (2억 8천 2백만갑 ), 4월 (3억 5백만갑 ) 에는소폭증가하였다. 정부의금연정책 ( 담뱃값인상및담뱃갑경고그림의무화시행등 ) 시행전의동월과비교했을때 20.8% 감소한결과이다.(10,11) 그러나담뱃갑경고그림으로인한금연효과인지담뱃값인상또는금연구역확대등의타정책의효과인지알기힘들다는비판이있다.(12) 담뱃갑경고그림이실제로금연에효과가있는지에대한꾸준한연구가있었으나이를종합적으로제시한연구가없다. 담뱃갑경고그림금연효과에대한연구현황분석이필요하다. 본연구에서는국내담뱃갑경고그림의연구현황을파악하고효과를종합하는체계적문헌고찰을수행하고자한다. 본론 1. 연구방법 1) 문헌검색전략코크란연합의중재법에대한체계적문헌고찰핸드북 (Cochrane Handbook for Systematic Reviews of Interventions) 에서제시한방법에따라수행하였다.(13) (1) 핵심질문 연구대상 : 한국인흡연자 중재 : 경고그림이있는담뱃갑에노출 ( 자체적으로제작한담뱃갑경고그림또는해외에서시행되고있는담뱃갑경고그림등 ) 비교중재 : 경고그림이없는담뱃갑에노출 중재결과 : 금연효과 ( 금연의도, 흡연기피도등 ) 연구유형 : RCT (Randomized Controlled Trial/Randomized Clinical Trial), 관찰연구 (2) 문헌검색데이터베이스인터넷기반전자데이터베이스 (Electronic Database, DB) 를활용하여연구를진행하였다. 한국인을대상으로한연구를모두포함하기위하여국내, 국외 DB 를모두활용하였다. 국내 DB 로는 DBPIA, KISS, KMBASE, NDSL, RISS 를활용하였으며국외 DB 로는 EMBASE, MEDLINE, PUBMED 를활용하였다. 논문의출판시점은발표연도에제한없이검색시점 ( 국내 , 국외 ) 까지 5

(3) 검색식국외 DB 에서는 Cochrane 의 흡연감소목적의담뱃갑디자인에대한체계적문헌고찰 (9) 검색식을수정하여검색하였다 (Appendix 2). 연구대상을한국인으로한정하였으므로검색식에모두 korea* 를추가하였다. 국내 DB 에서는 흡연 금연 담배 니코틴 & 담뱃갑 경고 * 그림 문구 pack visual 공포 * 의기본검색식을이용하여각 DB 의검색방식에맞게검색하였다 (Appendix 3). (4) 문헌선정및배제기준연령과성별등의제한없이한국인흡연자를대상으로수행한연구를모두포함하였다. 영어또는한국어로작성된문헌을대상으로하였다. 경고그림도입시기가약 8개월로얼마되지않아현경고그림에대한연구의수가제한적이기때문에, 경고그림에는자체제작한그림과외국에서시행하고있는경고그림등을모두포함하였다. 중재결과로는금연의도, 금연유도효과, 흡연태도, 흡연기피도와같이금연효과를파악한연구를모두포함했다. 중재결과가공포심유발과같이금연효과가아닌경우는배제하였다. 사례연구나면담에의한서술연구들은효과크기를파악할수없으므로질적연구는제외하였다. 국내외학술지에출판되지않 2) 문헌선택과정 2명의연구자가자료수집과선별과정등분석에포함된모든연구를독립적으로검토하였고, 평가결과가일치하는지확인하였다. 평가결과가불일치할경우자료선정또는제외기준에따라해당문헌을함께검토하여합의하였다. 국내외 DB에서총 1,284 편의연구를검색했다. 중복된 712 편의연구를제외하였다. 중복을제외한 572 편을대상으로자료선정및제외기준에따라 2명의연구자가초록을검토하였다. 제외된논문은총 552 편이었다. 초록만으로문헌선정이어려운경우전문을찾아확인하여결정하였다. 그결과 11편이제외되었고최종적으로총 9편의문헌을선정하였다 ( 그림 1). 3) 문헌의질평가선정한논문을대상으로 GRADE 평가틀기반의질적평가를실시하였다 ( 표 1).(14,15) 효과크기를제대로확인할수없는문헌이 2편존재하였다. 효과크기를확인할수있는문헌도 GRADE 평가기준에서원하는형식과일치하지않아질평가에효과크기를반영하지는못하였다. 그외의모든영역 (8개분류 ) 은각논문의결과값을반영하여각문헌별질평가의타당성을확보하였다 (Appendix 1). 6 정소화외 : 국내담뱃갑경고그림의금연효과에관한체계적문헌고찰 4) 자료분석평가에필요한모든정확히추출하기위해근거표기본서식을만들어작성하였다. 근거표에는연구형태, 각연구의연구대상과대상수, 중재방법과내용그리고중재결과를포함하였다. 두명의연구자가각각추출과정을독립적으로실시하였으며, 평가결과에대한평가자간의일치도를비교하였다. 각문헌별정의된용어의차이 ( 담뱃갑경고그림, 금연등 ) 를확인하여일관성을높였다. 5) 데이터종합각연구별디자인, 변수, 대상자그외의특징을모두고려하였을때, 연구간이질성이컸다. 연구간이질성이큰경우각연구결과를통계적으로올바르게결합할수없기때문에메타분석과정을수행할수없었다. 연구의특징에따라효과크기를포함한결과를서술하고표로작성하였다. 2. 연구결과 1) 최종문헌의일반적특성및질평가평가한 9편의문헌은모두단면연구였다. 문헌들의총대상자수는 3,522 명이었다. 문헌대부분이 명이내의대상자수로연구하였으며, 1000명이상을대상으로수행한연구는 1편이었다. 8편은흡연자를대상으로연구하였으며이중, 비흡연자까지포함한문헌은 6편이었다. 1편은흡연여부를파악할수없었다. 9편모두 20 대이상의성인을대상으로하였고 4편은 10대도포함하고있었다. 중재는크게경고그림만의금연효과를확인하는방법과경고문구와비교하여경고그림의금연효과를확인하는방법으로나뉜다. 경고문구와비교하여결과를나타낸연구는 4편이었다. 경고문구와비교없이경고그림의효과만을파악한문헌은 5편이었다. 경고그림의이미지강도에따른효과를보는연구가 1편, 경고그림으로인해유발된공포, 혐오감등이금연에미치는효과를보는연구가 2편, 성별과문제대처양식이경고그림효과에어떻게영향을미치는지를확인한연구가 1편그리고낙관적편견과제 3자효과에따른경고그림효과를확인한연구가 1편이었다. 모든연구의중재효과를평가한방법은자기기재방법설문이었다. 금연효과에관한정의는연구마다달랐다. 금연효과로금연유도효과 ( 금연에얼마나도움을줄것같은지주관적으 로판단 ) 를파악한문헌이 3편이었고금연의도 ( 향후실제로금연을행할지에대한주관적인의향 ) 를파악한문헌이 6편이었다. 금연의도를금연효과로측정한연구중, 흡연태도 ( 흡연을어떻게생각하는지에대해주관적으로판단 ) 나금연태도 ( 금연을어떻게생각하는지에대해주관적으로판단 ), 금연욕구 ( 향후실제로금연을행할지에대한여부와상관없이단순히금연하고싶어하는마음 ) 그리고흡연기피도 ( 비흡연자가흡연하지않을것이라는주관적인의향 ) 를함께확인한연구가 4편이었다. 금연효과에관한정의는모두다르나응답자의주관적인판단으로효과를측정했다는점은같다. 경고그림의이미지강도에따른금연효과를본연구가 3편있었으며, 그중 1편은이미지강도를고강도인신체적위협, 저강도인관계적위협으로정의하였다. 경고그림과경고문구의금연효과비교를포함한연구가 5편이었다. 경고그림과경고문구의비교없이, 함께작용했을때의효과를보는상호작용을본연구가 1편이었다. GRADE 기준질평가결과, 9편의연구중 8편의연구등급은 매우낮음 이었다. 나머지 1편의등급은 낮음 이었다 (Appendix 1). 광고ㆍ홍보분야 4편, 과학예술분야 1편, 건강소통 1편, 의료분야 3편이학술지에발표되었다. 2) 문헌별주요특징요약담뱃갑경고그림부착이의무화된 2016년 12월 23일이전에연구를수행한문헌이 9편중 8편이었다. 현재시행되고있는담뱃갑경고그림에관한효과를본연구는 1편이었다. 8편중자체제작한그림을이용한문헌이 3편이었으며 1편은보건복지부에서경고그림도입사전조사에사용하였던그림이었다. 두편은외국에서실제로담뱃갑에사용하고있는그림을이용하였고, 2편은담배규제기본협약데이터베이스사진을사용하였다. 전체중 4편은대상자를편의추출하였다. 2편은마케팅조사회사소속온라인패널에서흡연자를연구대상자로선정하였다. 나머지 3편은성별, 지역, 연령별비례할당추출로대상자를선정하여설문을진행하였다. 그중 1편은국제담배규제평가조사의 4개국조사기준에따라모집하였다.(16) 최종선정된모든연구가단면연구였지만, 경고그림과금연효과사이의매개효과를파악한연구가 3편이었다. 9

8 정소화외 : 국내담뱃갑경고그림의금연효과에관한체계적문헌고찰 선정된문헌의이해관계를확인했을때, 교내지원을받은연구 3편, 식품의약품안전처의지원을받은연구 2편, 이해관계표기가없는연구 4편이었다. 3) 문헌별효과크기산출방법모든문헌에서담뱃갑경고그림은금연에유의한효과가있었다. 표 3은각문헌의독립변수 ( 담뱃갑경고그림관련 ) 와종속변수 ( 금연관련효과 ) 를분석방법과효과크기별로정리한표이다. 매개분석을한경우, 매개분석에관한사항도제시하였다. 비흡연자를포함하고있는문헌 6편중비흡연자의흡연예방에관한결과까지분석한문헌은 4편이었다. 본연구는흡연자의 금연 을종속변수로분석하였기때문에비흡연자의흡연예방결과는결과제시에서제외하였다. 담뱃갑경고그림의금연관련효과분석을위해총 9편의문헌중 5편의문헌이회귀분석을수행하였다. 그중 1편의문헌은위계적회귀분석과더불어단순기울기분석접근법을이용하였다. 남은 4편의문헌은각각카이제곱검정, 윌콕슨부호순위검정, 3원공분산분석, 상관분석을수행하였다. 회귀분석을수행한 5편의연구에서경고그림의금연효과에대한직접효과 (direct effect) 는 B( 표준화또는비표준화계수 ) 와 SE( 표준오차 ) 로제시하였고, 매개효과 (mediating effect) 는 B( 표준화또는비표준화계수 ) 와 CI( 신뢰구간 ) 또는 SD( 표준편차 ) 로효과크기를제시하였다. 2편의문헌 ( 카이제곱검정, 윌콕슨부호순위검정을사용한문헌 ) 은빈도표를제시하였다. 그외 2편의문헌 (3원공분산분석, 상관분석을사용한문헌 ) 은평균과표준편차를사용하여효과크기를제시하였다. 최유진 (2014) 은회귀분석으로효과크기를산출하였다.(17) 담뱃갑경고그림으로유발된공포, 혐오감, 분노로인한흡연태도 ( 흡연을어떻게생각하지는지에대한주관적판단 ) 에대한효과크기는각각 β=-.152 (P-value<.05, SE=1, R 2 =.224), β=-.015 (P-value>.10, SE=1, R 2 =.224), β=.124 (P-value<.10, SE=1, R 2 =.224) 였다. 담뱃갑경고그림으로유발된공포, 혐오감, 분노로인한금연효과 ( 금연의도, 향후실제로금연을행할주관적의향 ) 크기는각각 β=.078 (P-value>.10, SE=1, R 2 =.207), β=.025 (P-valu e>.10, SE=1, R 2 =.224), β=-.006 (P-value>.10, SE=1, R 2 =.207) 였다. 담뱃갑경고그림으로유발된공포가흡연태 도에유의하게부정적인영향이있었다. 박준우등 은회귀분석으로효과크기를산출하였다.(18) 흡연자가담뱃갑경고그림으로유발되는정서로인한금연효과 ( 금연의도 ) 크기는불확실했다. 본문에서는 β=.045 (P-value=.48) 라고표기하였지만, 정리된표에서는 β=-.87 로표기하였다. 또한, 흡연자에대한경고그림의금연효과매개경로를분석하였고그크기는 β=.21 (95% CI=[.0032,.0776]) 이었다. 흡연자에대한경고그림의매개경로를통한금연효과가유의하게나타남을알수있었다. 김봉철, 최영준 (2017) 은회귀분석으로효과크기를산출하였다.(19) 담뱃갑경고그림을보았을때제 3자효과나낙관적편견으로인해유발되는금연의도효과를보았다. 유발된금연의도효과크기는각각 β=.396 (P-value=.000, SE=.043, R 2 =.191), β=-028 (P-value=.560, SE=.043, R 2 =.001) 이었다. 담뱃갑경고그림의영향력이자신보다다른사람에게더크게미칠것이라고지각하는것과금연의도는유의미하게긍정적인관계가있음을알수있었다. Chun S 등 (2017) 은회귀분석으로효과크기를산출하였다.(20) 흡연자가담뱃갑경고그림을본후유발되는금연의도 (Intention) 와금연에대한열망 (Desire) 의효과크기를알아보았다. 유발된효과크기는각각 β=.53 (SE=.24, P-value=.027), β=.78 (SE=.19, P-value<.001) 이었다. 또한, 담뱃갑경고그림의금연효과 ( 금연의도, 금연에대한열망 ) 에대한매개분석을시행하였고그크기는각각 β=.12 (SE=.06, 95% CI=[.0253,.2893]), β=.06 (SE=.10, 95% CI=[.0597,.462]) 였다. 흡연자담뱃갑경고그림의금연효과 ( 금연의도와금연에대한열망 ) 와매개효과가유의함을알수있었다. 전승우, 박준우 는위계적회귀분석및단순기울기분석접근법을이용하여효과크기를산출하였다.(21) 문제중심대처에서담뱃갑경고그림의금연효과 ( 금연태도 ) 와사회적지원대처에서의담뱃갑경고그림의금연효과 ( 금연태도 ) 의효과크기는각각 β=.292 (SE=.143, P-value=.042, R 2 =.066), β=.263 (SE=.145, P-value=.07, R 2 =.042) 이다. 또한, 담뱃갑경고그림과성별, 대처양식 ( 문제중심대처, 사회적지원대처 ) 에따라금연효과 ( 금연태도 ) 크기를 PROCESS 매크로 (Model 4) 를사용하여산출하였다. 금연효과의크기는각각 β=-.291 (SD=1.69, 95% CI=[-.680, -.026]), β=.119 (SD=.070, 95% CI=[.014,.283]) 였다. 금연 11

11 JKSRNT Vol. 9, No. 1, Jun 효과에대한문제중심대처에서담뱃갑경고그림의주효과와성별과대처양식별담뱃갑경고그림의매개효과가있음을확인할수있었다. 조경숙, 신윤정 (2004) 은카이제곱검정을사용하여흡연여부와담뱃갑경고그림에따른금연효과와담배구매억제효과를보았다.(22) 각각의 P-value 는 1.0,.397 이었다. 또한경고그림강도별금연효과와담배구매억제효과를보았으며각각의 P-value 는모두 <0.001 이었다. 경고그림강도에따라금연효과와담배구매유의한억제효과가있었다. 김현숙등 (2004) 는윌콕슨부호순위검정을이용하여담뱃갑경고그림과경고문구의금연효과차이를보았다.(23) 담뱃갑경고그림과경고문구의금연효과차이에대한 P-value 는 <0.001 이였다. 담뱃갑경고그림과경고문구의금연효과가유의한차이가있었으며담뱃갑경고그림의금연효과가더컸다. 박종미등 은 3원공분산분석을이용하여담뱃갑경고그림의종류에따른금연효과 ( 금연의도 ) 와담뱃갑경고그림과경고문구의조합에따른금연효과 ( 금연의도 ) 차이를보았다.(24) 신체위협의경고그림을보여주었을때금연의도를묻는경우 7점만점 ( 금연할의향이있다.) 중평균 5.17 점 (SD=.13) 을얻었고사회관계위협의경고그림을보여주었을때는평균 4.70 점 (SD=.14) 을얻었다. 두담뱃갑경고그림종류의금연효과차이에대한 P-value 는 <.05 였다. 담뱃갑경고그림과경고문구조합에따른금연효과를보았을때, P-value 와 F값외에는효과크기를알수없었다. P-value 는 >.05 였다. 신체위협의담뱃갑경고그림이사회관계위협담뱃갑경고그림보다효과적으로금연을유도하였으며담뱃갑에경고그림 ( 신체위협의경고그림 ) 과경고문구 ( 사실적경고문구 ) 를함께제시하였을때가장효과적이었다. 서영성등 (2007) 에서는상관분석을이용하여담뱃갑경고그림의이미지강도별금연유도효과를보았다.(25) 이미지강도의평균값은 3.3(SD=1.12) 이고금연유도효과의평균값은 3.3(SD=1.01) 이었다. 상관계수는.670 이었으며 P-value 는.00 이었다. 이미지의강도가강력할수록금연효과가컸다. 1편의문헌에서는본문에서제시한효과크기와정리된표에서제시한효과크기가달라정확한효과크기를알수없었다. 다른 1편의문헌에서는유의하지않은일부결과에 대해평균과표준편차를표기하지않아효과크기를평가할수없었다. 결론본연구는한국인을대상으로담뱃갑경고그림의금연효과에대한선행연구를체계적으로고찰하였다. 최종선정된 9편문헌에대하여질평가를시행하였고각문헌의일반적결과및주요특징을정리하였다. 개별문헌의분석방법과효과크기를비교하여제시하였다. 체계적문헌고찰을통해경고그림의금연효과를종합하고연구현황을파악하여향후연구방향을제안하고자하였다. 연구설계측면에서고찰한결과, 최종선정된모든연구는단면연구였다. 단면연구의경우, 담뱃갑경고그림이금연효과에미치는지속적인영향을파악하기어렵고인과관계추론에도제한이있다. 경고그림이금연효과에영향을줄때매개가될수있는요인 ( 정서, 인지등 ) 의효과를평가하기어렵다. 최종선정된 9편의문헌중 3편의문헌에서매개분석결과를포함하였다. 본연구와같은주제인코크란국외체계적문헌고찰 (9) 에서는다양한방법의연구설계형태를볼수있었다. 총 51편의연구중중재전후비교연구가 7편, 코호트연구가 5편, 실험연구가 27편, 패널연구가 15 편이었다. 국내에서도경고그림효과에대한종단적연구설계를고려할필요가있다. 종단적연구설계에서매개분석이가능할지라도, 매개변수의측정만으로실제흡연율이나금연시도, 재발률의변화를보장할수는없다. 질평가결과 8편의문헌은 매우낮음 이었고 1편의문헌만 낮음 이었다. 각문헌에서담뱃갑경고그림의금연효과가유의했으나질평가결과가낮아금연효과를신뢰할수없다는한계가있다. 8편의문헌중심지어금연효과의크기를정확히확인할수없는 2편의문헌은본문서술이나표에서효과크기를다르게표시하거나유의한결과만효과크기를제시하였다. 국외체계적문헌고찰 (9) 에서는 51편의문헌중 1편만이선택적으로효과크기를제시하였다. 9편의문헌중 6편이대상자를편의추출하였다. 국외체계적문헌고찰 (9) 의경우, 임의추출문헌의비율이약 55% 였다. 대상자선정에있어서도좀더체계적인추출방법이고안되어야할것이다. 작은대상자수역시연구의검정력을감소시키는원인중 14

12 정소화외 : 국내담뱃갑경고그림의금연효과에관한체계적문헌고찰 하나이다. 8편의문헌이 100명에서 500명이내의대상자수로연구하였고 1000 명이상의대상자수로연구한문헌은단 1편이었다. 비흡연자를포함하지않은연구가많았고 1편의연구는흡연여부조차파악할수없었다. 효과크기를미리제시하여연구에필요한대상자수를계산한문헌은 1편도없었다. 국외체계적문헌고찰 (9) 에따르면, 호주는국가차원의대규모연구를수행해많게는 70 만명을대상으로결과를산출했다. 그외대부분의연구에서는대상자가적어도 1,000명이상이었다. 9편의모든연구가금연효과를설문조사와같은주관적응답 ( 금연에대한태도, 금연동기부여등 ) 으로측정하여객관성이떨어진다는문제가있었다. 국외체계적문헌고찰 (9) 에서주관적응답결과는부수적으로제시하고있다. 실제흡연율이나흡연자담배소비량변화등객관적결과를주로제시하고객관적인측정이가능한시표추적검사 (eye-tracking test) 를이용하기도한다. 국외체계적문헌고찰 (9) 에서객관적측정도구를이용한문헌은 18편이었다. 국외체계적문헌고찰 (9) 에서는최종선정된 51편의해외문헌중 40여편의문헌이건강관련학술지문헌이었다. 우리나라는선정된 9편의문헌중 4편의문헌만건강관련학술지에발표된연구였다. 5편의문헌은광고및예술분야의학술지에발표된연구로건강관련학술지와는분석방법과결과해석등에서차이를보이고있다. 건강관련학술지 4편중매개효과를분석하는연구가 1편이었으나광고및예술분야의연구 5편에서는 3편이매개효과를보는분석을수행하였다. 광고및예술분야의연구는주로경고그림의시각적구성에초점이맞추어져있다. 예를들면, 금연효과를극대화하는사진의종류혹은경고그림과경고문구의동시삽입의효과등이다. 효과적인경고그림을제작하는연구도중요하지만, 현시점에서는이미시판되고있는담뱃갑경고그림자체의금연효과를보는연구도필요하다. 광고및예술분야에서담뱃갑경고그림에대한연구가활발했던만큼, 의학및보건분야에서도공중보건학적관점으로담뱃갑경고그림의금연효과를분석하는연구를활발하게수행해야할것이다. 본연구는우리나라담뱃갑경고그림의금연효과평가를위해잘설계된연구가시급하다는현실을제시한데의의가있다. 국내데이터베이스뿐아니라국외데이터베이스도활 용하여한국인을대상으로수행한모든연구를포함시켰다. 다양한선행연구를참고하여검색식을구성해타당도를높였다. 한국인흡연자를대상으로담뱃갑경고그림의금연효과에대한선행연구들을체계적고찰한결과담뱃갑경고그림은금연에효과적이었다. 체계적고찰을통해드러난문제점과향후연구를통해문제점을해결할수있는방향은다음과같다. 첫째, 모든연구가단면연구로이루어져있어지속적인효과를파악하기어렵다. 또한, 단면연구에서추정할수없는매개효과를분석한연구가많았다. 둘째, 모든연구의질평가등급이낮아결과의신뢰성이떨어진다. 셋째, 연구대상자수가적고대상자선정방법에편향이존재한다. 넷째, 경고그림의종류가모두다르고금연효과를설문지를이용한주관적응답으로파악해객관적효과평가가어렵다. 다섯째, 효과크기를정확히추정할수없는연구가 2편 (22.2%) 이었다. 여섯째로, 예술혹은마케팅분야에서수행한연구가 5편 (55.6%) 이었다. 향후담뱃갑경고그림의장기적인금연효과평가를위한종단연구 (longitudinal study) 나개입의전후비교를위한무작위교차시험연구 (randomized crossover trial) 등증거수준이높은설계법을이용한연구가필수적이다. 시표추적검사나담배소비량변화와같은객관적인지표로금연효과를측정하는연구가더욱필요하다. 현재시행되고있는담뱃갑경고그림의이미지종류별금연효과를파악하는연구도필요하다. 2017년청소년건강행태온라인조사에서담뱃갑경고그림을본경험과담뱃갑경고그림을본후담배에대한인식을조사하였다. 앞으로전연령을대상으로하는연구도활발히수행되어야할것이다. 감사의글이연구는질병관리본부 ( 과제번호 ) 연구비로수행하였습니다. 요약연구배경 : 2016 년 12월, 흡연율감소를위해담뱃갑경고그림부착정책이시행되었다. 경고그림이부착된담뱃갑이금 15

13 JKSRNT Vol. 9, No. 1, Jun 연에미치는영향을평가하고결과들을종합해보고자체계적문헌고찰을수행하였다. 방법 : 사전에설정한검색전략을사용하여, 한국에서수행된연구를확인하고자한국데이터베이스 5개 (DBPIA, KISS, KMBASE, NDSL, RISS) 와 3개외국데이터베이스에서문헌을검색하였다. 연구에포함된문헌은 2017 년 8월까지출간된연구이며, 결과를금연에대한영향에대해파악한연구로한정하였다. 연구설계는제한을두지않았다. 결과 : 기준에부합하는연구는 9개였으며총연구참여자수는 3,522 명이었다. 최종문헌들에따르면, 담뱃갑경고그림은흡연율을감소에크게영향을주었다. 대상연구의증거수준이높지않아신뢰도가낮았다. GRADE 기준으로선정된문헌의질을평가하였을때 8개연구가 'Very low' 였고 1 개연구만이 'low' 로평가되었다. 2개연구는정확한효과크기를포함하지않았다. 모든연구가장기적인효과를추정하지않는단면적연구였고, 3개연구는매개분석을수행하였다. 모든연구는상대적으로작은표본크기와연구설계방법그리고객관성이떨어지는자기기입식설문지를사용하여연구를수행하였다는측면에서한계가있었다. 5개연구는광고및마케팅학술지에출판되었다. 결론 : 현재담뱃갑경고그림부착정책이시행되고있는만큼, 공중보건학적측면에서금연에대한장기적이고객관적인영향을파악하기위해잘설계된연구가수행되어야할것이다. 중심단어 : 담뱃갑경고그림 ; 금연 ; 체계적문헌고찰 References 1.
