Despite the advantage of reduced mortality and morbidity, the mid-to long-term prognosis and possible complications associated with the procedure are a source of concern. Among the many possible complications, retrograde type A dissection after stent grafting for type B dissection is considered rare; however, this life-threatening event appears to be underrecognized. Here, we report a case that converted to open surgery due to a retrograde type A dissection after endovascular stent graft placement for a Stanford type B dissection. This is the first report of a retrograde type A dissection after endovascular stent graft placement in Korea. (Korean J Med 79: , 2010) Key Words: Aortic disease; Dissection; Stents 서론전통적으로합병증없는 Stanford B형대동맥박리증은비침습적으로치료하는질환이지만심한고혈압의경우, 대동맥박리가계속진행되는경우, 가강 (false lumen) 의크기가계속진행되어대동맥파열이우려되는경우, 또는지속적인박리나통증으로말초장기손상이의심되는경우등에는불가피하게수술적치료가필요하다. 그러나이러한수술에관 련된합병증및사망률은상당히높은것으로알려져있어 1) 그에대한대안으로스텐트그라프트 (stent graft) 삽입치료가소개되었으며, 최근이러한스텐트그라프트삽입치료의안전성과효용성이입증되면서부터이를통한치료사례가지속적으로증가하고있다 2,3). 하지만낮은사망률과이환율에도불구하고스텐트그라프트삽입치료의중장기적인치료성적은아직까지논란이되고있으며대동맥박리증에대한스텐트그라프트삽입치료후의합병증이최근까지지속적 Received: Revised: Accepted: Correspondence to Donghoon Choi, M.D., Department of Internal Medicine, Yonsei University College of Medicine, Shinchondong, Seodaemun-gu, Seoul , Korea cdhlyj@yuhs.ac

01 ng/ml이었다. 요검사에서 이상 소견은 없었다. 단순 흉부 X-선 촬영: 심비대, 폐문부 울혈이나 종격동의 합병증이 최근에 여러 연구 결과에서 보고되고 있다. 그러나 국내에서는 아직 이러한 역행성 대동맥 박리증의 확장 소견은 보이지 않았다.

치료 및 경과: 급성 대동맥 박리증 의심하에 응급실에서 증 시행한 대동맥 컴퓨터단층촬영(CT) 상 Stanford B형 대동맥 례 박리증을 진단하였으며(그림 1), 약물 치료로 혈압과 맥박수 조절을 위하여 Propranolol 120 mg, Felodipine 10 mg, 환 자: 양, 남자, 48세 주 소: 명치

흡연력 및 음주력은 없음. 가족력: 특이사항 없음. 이학적 소견: 내원시 생체징후는 혈압 180/100 mmhg, 맥 박수 74회/분, 호흡수 15회/분, 체온은 36.8 C였다. 의식은 명 료하였다. 심음은 규칙적이었고, 심잡음은 청진되지 않았으 며, 호흡음은 정상 소견이었다.

Czermak 등, Lopera 등, Kato 등 은 용적률 39.4, 백혈구 10,870/μL, 혈소판 264,000/μL이었고, 전 이식편주위누출(perigraft leakage), 이식편의 이동(migration), 해질 검사상 Na/K/Cl/totalCO2 143/3.4/105/26 meq/l, 혈청 생 박리의 진행, 심부정맥 혈전증, 뇌색전증, 스텐트 끝부분에 화학 검사상 혈당 109 mg/dl, BUN/Creatinine 19.0/1.65 mg/dl, 새로이 발생하는 대동맥류 및 동맥류 파열 등 다양한 합병 알부민 4.4 g/dl, 총 콜레스테롤 168 mg/dl, NT-proBNP 증을 보고해 왔다. 여러 합병증 중에서 역행성 A형 대동맥 pg/ml이었다. 심근효소 검사상 CK 76 IU/L, CK-MB 1.98 ng/ml, 박리증은 매우 드문 것으로 생각되어 왔으나 이 치명적인 troponin T 0.01 ng/ml이었다. 요검사에서 이상 소견은 없었다. 단순 흉부 X-선 촬영: 심비대, 폐문부 울혈이나 종격동의 합병증이 최근에 여러 연구 결과에서 보고되고 있다. 그러나 국내에서는 아직 이러한 역행성 대동맥 박리증의 확장 소견은 보이지 않았다. 표면 심전도 검사: 정상 동율동이었으며 ST 분절의 변화나 발생에 대한 보고가 없어 저자 등이 경험한 증례를 관련 문 다른 심전도상의 변화는 보이지 않았다. 헌고찰과 함께 보고하는 바이다. 치료 및 경과: 급성 대동맥 박리증 의심하에 응급실에서 증 시행한 대동맥 컴퓨터단층촬영(CT) 상 Stanford B형 대동맥 례 박리증을 진단하였으며(그림 1), 약물 치료로 혈압과 맥박수 조절을 위하여 Propranolol 120 mg, Felodipine 10 mg, 환 자: 양, 남자, 48세 주 소: 명치 부위 및 등의 통증 현병력: 과거력상 3년 전 다낭성 신질환(Polycystic kidney disease) 및 고혈압을 진단받고 약물 치료 중이던 환자로 내 원 1일 전부터 명치 부위 및 등의 통증을 주소로 응급실에 내원하였다. 개인력: 3년 전 다낭성 신질환 및 고혈압을 진단받고 약물 치료 중임. 흡연력 및 음주력은 없음. 가족력: 특이사항 없음. 이학적 소견: 내원시 생체징후는 혈압 180/100 mmhg, 맥 박수 74회/분, 호흡수 15회/분, 체온은 36.8 C였다. 의식은 명 료하였다. 심음은 규칙적이었고, 심잡음은 청진되지 않았으 며, 호흡음은 정상 소견이었다. 복부 진찰상 잡음은 청진되 지 않았고 사지의 맥박은 정상적으로 촉지되었다. 혈액 검사 소견: 말초혈액검사상 혈색소 14.1 g/dl, 적혈구 Figure 1. 대동맥조영술을위해 18G angiocath 천자침으로경피적으로양측대퇴동맥을천자하여이를통해 0.035 inch 안내철선 (Radiofocus, Terumo, Tokyo, Japan) 을대동맥까지전진시켰다.

좌측대퇴동맥에는 8 Fr 혈관용피포를삽입한후 5 Fr pig-tail 도관 (Cook, Bloomington, IN, USA) 을병변보다근위부대동맥에위치시켰다.

035 inch Lunderquist 안내철선 (Cook, Bloomington, IN, USA) 으로교체하였으며이후이안내철선을따라 S&G seal thoracic stent graft (S&G Biotech INC, Seongnam-si, Korea) 2개 ( 직경 36 mm

Losartan 100 mg을투여하였고, 환자의혈압과맥박수는수축기혈압 110 mmhg 이하, 맥박수 60회 / 분이하로유지되었으나, 환자가지속적인복통및점차심해지는양측다리의통증을호소하여내원 15일째스텐트그라프트삽입치료를진행하였다 ( 그림 2). 대동맥조영술을위해 18G angiocath 천자침으로경피적으로양측대퇴동맥을천자하여이를통해 inch 안내철선 (Radiofocus, Terumo, Tokyo, Japan) 을대동맥까지전진시켰다. 천자침을제거하고안내철선을따라우측대퇴동맥에 8 Fr (French) 혈관용피포 (sheath) 를삽입한후 16 Fr 혈관용피포로교체하였다. 좌측대퇴동맥에는 8 Fr 혈관용피포를삽입한후 5 Fr pig-tail 도관 (Cook, Bloomington, IN, USA) 을병변보다근위부대동맥에위치시켰다. 대동맥조영술로파열입구와파열출구의위치를확인한후우측대퇴동맥피포에삽입된 5 Fr multipurpose 도관을통해안내철선을 inch Lunderquist 안내철선 (Cook, Bloomington, IN, USA) 으로교체하였으며이후이안내철선을따라 S&G seal thoracic stent graft (S&G Biotech INC, Seongnam-si, Korea) 2개 ( 직경 36 mm 길이 150 mm, 직경 36 mm 길이 100 mm) 를병변부위에삽입하였다. 스텐트그라프트삽입치료시행이후약물치료를지속하면서혈압과맥박수는수축기혈압 110 mmhg 이하, 맥박수 60회 / 분이하로지속적으로안정적으로유지되었다. 환자는내원당시에호소하던찢어지는듯한느낌의명치및등의 Figure 4. Image collected during open surgery, showing a bare spring wire protruding out of an intimal tear in the wall of the aorta. 통증은호전되었으나하루에두세차례, 한시간가량지속되는둔한느낌의경미한흉통을호소하였다. 스텐트그라프트삽입치료 7일째인내원 22일째시행한추적대동맥컴퓨터단층촬영상스텐트그라프트근위부의순용수철 (bare spring) 부위가대동맥벽을뚫고대동맥바깥으로돌출되어있는소견을관찰할수있었으며, 그로인해스텐트그라프트근위부에서부터시작되는역행성 Stanford A형대동맥박리증이발견되어 ( 그림 3) 1일후인내원 23일째개흉술로전환하여상행대동맥에 Graft replacement 수술을시행받았다 ( 그림 4). 환자는내원 34일째퇴원하였으며현재까지별다른증상없이외래추적관찰중이다. 고 스텐트그라프트삽입술을시행받은 B형대동맥박리증환자에서시술후발생하는역행성 A형대동맥박리증은과거생각되던것보다빈번한합병증으로여겨지고있다. Dong 등 7) 은 B형대동맥박리증에대하여스텐트그라프트삽입술을시행한환자의 2.5% 에서역행성 A형대동맥박리증이발생함을보고하였고, 이는가장흔한합병증중하나인이식편의이동과견줄만한발생율이라할수있다. 또한 Neuhauser 등 8) 은스텐트그라프트삽입치료를받은 28명의환자중 4 명의환자에서결과적으로개흉수술로의전환이필요했으며이중 1명은지속되는이식편주위누출 (perigraft leakage), 3명은역행성 A형대동맥박리증의발생에의한것으로보고하 찰

4 - 김태훈외 6 인. 스텐트그라프트관련역행성 A 형대동맥박리증 - 였다. 또한이중 1명의역행성대동맥박리증발생환자는사망하였다. 그러나아직까지이합병증이과연순수하게시술에의해발생하는합병증인지, 또는질병그자체의자연진행에의해큰영향을받는것으로봐야하는지에대한부분은아직까지불분명하다. 게다가스텐트그라프트삽입치료를하지않고약물치료를진행한 B형대동맥박리증의경우에서도대동맥벽의허약함 (fragility) 및기존대동맥박리증의진행에의해서 A형대동맥박리증이발생한증례가다수보고되고있다 9,10). 그러나본증례및그외보고되고있는 case들 7) 의대부분에서는스텐트그라프트삽입치료와관련된부분이많다. 그중에서도가장중요하게생각되는것은스텐트그라프트원위부의순용수철 (bare spring) 부위이다. 이는 polyester graft에둘러쌓이지않은부위이며, 이부위에의해서원위부의고정을위한강한반지름방향의힘 (radial force) 을얻을수있게된다. 이러한반지름방향의힘이강해질수록대동맥벽에손상을받을위험성은더커진다. 실제로현재까지보고된역행성 A형대동맥박리증환자에있어삽입된스텐트그라프트는대부분이러한순용수철부위를가지고있는것으로확인되고있다 7,11-14). 또한이합병증이발생한대부분의증례에서대동맥궁 (aortic arch) 에스텐트그라프트가위치하게되는경우가많았는데, 이는스텐트그라프트가초기의일직선상태로돌아가려는경향이있으므로이힘으로인해대동맥궁이손상을받게될것이라고생각되고있으며더경직된형태의스텐트그라프트일수록손상의정도가더클것으로추측된다. 그외에시술중의 guidewire 또는도관 (catheter) 으로인해직접적인혈관손상이발생하는경우도있다. 이러한여러가지사항을고려할때환자에따른장비및장치 (device) 의선택, stent graft 설계의발전그리고혈관내시술의표준화및안전화를통해역행성 A형대동맥박리증의예방을위해노력하여야할것이다. 요약대동맥박리증에대한스텐트그라프트삽입치료후의합병증중역행성 A형대동맥박리증은매우드문것으로생각되어왔으나이치명적인합병증이최근에다수보고되고있다. 국내에서는아직이러한역행성대동맥박리증의발생에대한보고가없으나실제로는상당수발생하고있을것으로생각되며그예방을위해환자에따른장비및장치 (device) 의선택, 스텐트그라프트설계의발전그리고혈관내시술의표준화및안전화가반드시필요하다. 중심단어 : 대동맥질환 ; 박리 ; 스텐트 REFERENCES 1) Chieffo A, Park SJ, Valgimigli M, Kim YH, Daemen J, Sheiban I, Truffa A, Montorfano M, Airoldi F, Sangiorgi G, Carlino M, Michev I, Lee CW, Hong MK, Park SW, Moretti C, Bonizzoni E, Rogacka R, Serruys PW, Colombo A.
