1 경도알츠하이머형치매환자의범주유창성과제에서의범주특정적손상특징 최현주 나사렛대학교언어치료학과 교신저자최현주나사렛대학교언어치료학과교수충청남도천안시쌍용동 456번지 tel.: 배경및목적 : 알츠하이머형치매환자의경우이름대기과제에서무생물범주에비해생물범주의손상이두드러지는범주특정적 (category specific) 손상이나타난다고보고되고있다. 그러나이러한연구에주로사용되는그림이름대기과제의방법론적인문제들이거론되고있다. 본연구는경도알츠하이머형치매환자를대상으로범주유창성 (category fluency) 과제를통해생물과무생물범주간의범주특정적손상이나타나는지알아보는것을목적으로하였다. 방법 : 경도알츠하이머형치매환자 15 명과정상고령자 15 명을대상으로생물범주 ( 동물, 과일 ) 와무생물범주 ( 탈것, 의류 ) 의범주유창성과제를시행하여집단간, 집단내차이를알아보았다. 또한, 알츠하이머형치매환자에게서나타난생물과무생물범주의오류수에차이가있는지를알아보았다. 결과 : 첫째, 알츠하이머형치매환자집단은정상고령자집단에비해생물범주와무생물범주모두에서유의하게수행이낮았다. 둘째, 정상고령자집단의경우무생물범주의수행에비해생물범주의수행이유의하게높은반면, 알츠하이머형치매환자집단에서는범주간의수행차이가통계적으로유의하지않았다. 셋째, 알츠하이머형치매환자의경우생물범주에서의오류수가무생물범주의오류수에비해유의하게높았다. 논의및결론 : 본연구에서사용된과제의영향으로정상고령자집단의경우생물에서의수행이무생물에서의수행보다월등히높은데반해, 알츠하이머형치매환자집단에서는이러한수행차이가나타나지않았다. 이러한결과는알츠하이머형치매환자들이상대적으로생물범주에선택적인손상을보임을시사한다. 언어청각장애연구, 2010;15; 핵심어 : 알츠하이머형치매, 범주특정적손상, 범주유창성과제 Ⅰ. 서론 알츠하이머형치매 (dementia of Alzheimer s type) 환자의이름대기장애 (anomia) 는질환의초기부터나타나는대표적인언어적결함으로이는의미기억 (semantic memory) 장애와관련이있다 (Balyes, Tomoeda & Trosset, 1990; Chertkow & Bub, 1990). 이러한의미기억장애는알츠하이머형치매의가장초기의증상중하나이며, 이는임상적으로알츠하이머형치매의전단계인경도인지장애 (mild cognitive impairment) 시기부터나타난다 (Adlam et al., 2006). 알츠하이머형치매환자의이름대기장애와관련된연구에서범주특정적 (category specific) 인손상, 특히생물범주와무생물범주에서의손상차이에대한연구들이종종이루어져왔다. 이들연구중대부분은무생물에비해생물범주의손상이두드러진다고보고하고있다 (Silveri et al., 1991; Whatmough et al., 2000; Zannino et al., 2002). 이러한알츠하이머형치매환자의생물범주에서의특정적손상을설명하는가장일반적인가설은의미기억에서의 감각- * 본연구는나사렛대학교의연구비지원에의하여수행되었음 (2010). 게재신청일 : 2010 년 10 월 20 일 최종수정일 : 2010 년 11 월 25 일 게재확정일 : 2010 년 11 월 28 일 c 2010 한국언어청각임상학회 572

2 최현주 / 경도알츠하이머형치매환자의범주유창성과제에서의범주특정적손상특징 기능이론 (sensory-functional theory) 이다. 이이론은생물과무생물아이템의의미적표상 (representation) 의차이로인해두범주의처리과정에차이가나타난다고설명한다. 즉, 생물아이템의의미기억은 감각적 ( 특히, 시각 ) 표상 에더의존적이며, 이는측두-변연구조 (temporo-limbic structure) 에서담당한다. 반면, 무생물아이템의의미기억은 기능적표상 에더의존적이며, 이는전두-두정피질영역 (fronto-parietal cortical area) 에서담당한다 (Farah & McClland, 1991). 이러한가설은알츠하이머형치매의전형적인신경병리인측두엽의두드러진손상이생물범주에서의선택적인손상으로나타난다고설명하고있다 (Daum et al., 1996; Silveri et al., 1991). 그러나알츠하이머형치매환자의생물과무생물범주에서의손상에차이가없다는연구도존재한다 (Laws et al., 2005). 또한, 이러한범주특정적수행차이는생물과무생물범주사이의본래의차이에기인한것이라는견해도있는데, Tippett, Grossman & Farah (1996) 는생물범주의어휘는그림이름대기와같은대면이름대기에영향을주는요인 ( 친숙함, 빈도, 시각적인복잡성등 ) 들이무생물범주보다더불리한특성이있다고주장하였다. 다시말해, 생물범주의이름대기항목들은무생물범주에비해덜친숙하고, 어휘의사용빈도가더낮으며, 시각적으로더복잡하기때문에뇌손상을가진치매환자의경우대면이름대기과제에서생물범주를더어려워한다는것이다. 이들연구자들은이러한변수들을통제했을때알츠하이머형치매환자들에게서나타나는범주특정적수행의효과는사라진다고보고한다. 그러나알츠하이머형치매환자 500명과정상성인 500명이참여한 21개의연구를종합한최근의메타분석의결과, 대개의연구에서알츠하이머형치매환자집단이생물범주에서의선택적손상을보였다 (Laws et al., 2007). 지금까지이루어진범주특정적손상을다룬연구의대부분에서는그림이름대기 (picture naming) 과제가사용되었다 (Laws et al., 2007). 그러나이러한과제는방법론적으로여러가지문제를가진다. 첫째, 앞에서언급했듯이친숙함, 빈도, 시각적인복잡성등의이름대기에영향을주는요인에있어서무생물과제가더유리한특성이있다 (Funnell & Sheridan, 1992; Tippett, Grossman & Farah, 1996). 둘째, 그림이름대기과제에서는정상집단의수행에서의천정효과 (ceiling effect) 가나타나는경우가많다 (Laws, 2005). 이러한천정효과로인해정상집단의범주에따른수행차이는논의되기어렵다. 마지막으로, 그림이름대기과제는생물과무생물의하위범주에서평가범위가제한적이다 (Aronoff et al., 2006). 예를들어, 많은그림이름대기과제는흑백의선그림을사용하는데생물범주의경우식물그림이름대기는동물그림이름대기에비해그림으로표현할수있는항목이제한적이어서동물에편향된과제가많이사용되어왔다. 그림이름대기과제의이러한문제점을해결하기위한대안으로최근의연구에서는범주유창성 (category fluency) 과제의사용이제안되고있다 (Moreno-Martínez, Laws & Schulz, 2008). 주어진시간동안해당범주의이름을말하는범주유창성과제는친숙함이나빈도에영향을받지않고, 정상집단에서도천정효과가나타나지않으며, 보다광범위한범주의항목들을평가할수있다 (Laws, 2004). 또한범주유창성과제는의미적손상에민감하기때문에그림이름대기과제에비해알츠하이머형치매환자의손상을민감하게검출해내기에효과적이다 (Herny, Crawford & Phillips, 2004). 범주에따른선택적손상을보고한국내의연구를살펴보면, 윤정미 김영태 (2002) 는브로카실어증환자의동사이해결함에서의범주특정적특성을, 강연재 (2009) 는브로카실어증환자의생물범주에서의선택적손상에대해보고하고있다. 그러나연구대상이실어증환자에국한되어있고뇌손상에의한다양한범주의손상을다루지못한점등제한점이많다. 본연구는알츠하이머형치매환자를대상으로범주유창성과제를통해생물과무생물범주간의범주특정적손상의특징을정상고령자와의비교를통해알아보았다. 또한, 알츠하이머형치매환자에게서나타난생물과무생물범주의오류수에차이가있는지를알아보았다. Ⅱ. 연구방법 1. 연구대상본연구의대상은경기도및충청도지역의병원여섯군데에입원해있는환자들로, DSM-IV (American Psychiatric Association, 1994) 의진단기준을적용하여신경과전문의로부터진단받은알츠하이머형치 573

3 언어청각장애연구 2010;15; 매환자 15 명과, 같은지역에거주하는정상고령자 15명으로총 30명이었다. 본연구의대상이된알츠하이머형치매환자의선정기준은다음과같다 : 1) 한국판간이정신상태검사 (Korean Mini-Mental State Examination: K-MMSE, 이하 K-MMSE) ( 강연욱 나덕렬 한승혜, 1997) 에서 17점이상으로, 2) CDR (Clinical Dementia Rating) (Morris, 1993) 0.5 혹은 1의경도치매이며, 3) 치매를제외한신경학적, 정신적질병이없고, 4) 과제를수행하고지시를따르는데필요한시각및청각능력을가지고있으며, 5) 뇌손상으로인한마비말장애및말실행증등의말운동장애를동반하지않아야한다. 또한, 본연구에참여한정상고령자의선정기준은다음과같다 : 1) K-MMSE 에서 24점이상으로, 대상자본인이나보호자의보고에의해 2) 인지기능에영향을미치는신경학적, 정신적질병이없고, 3) 과제를수행하고지시를따르는데필요한시각및청각능력을가지고있어야한다. 집단별대상자의성비및연령, 교육년수, K-MMSE 점수의평균을 < 표 -1> 에제시하였다. 집단별대상자의성비에차이가있는지알아보기위해교차분석을, 두집단의연령, 교육년수, K-MMSE 점수에차이가있는지를알아보기위해독립표본 t-검정을실시하였다. 그결과, 성비, 연령및교육년수에서는집단간유의한차이가없었으며, K-MMSE에서는알츠하이머형치매환자의점수가유의하게낮았다. < 표 - 1> 대상자의성비및연령, 교육년수, K-MMSE 점수평균 성비 ( 남 : 녀 ) 연령평균 ( 표준편차 ) 교육년수평균 ( 표준편차 ) K-MMSE 점수평균 ( 표준편차 ) DAT a) 6 : (4.3) 2.2 (3.4) 18.6 (2.0) NC b) 7 : (7.8) 2.6 (4.5) 25.7 (1.4) 통계치 =.536 t = t =-.315 t = ** a) DAT: Dementia of Alzheimer s Type, b) NC: Normal Control ** p < 연구과제및절차 본연구에서는생물과무생물범주의범주유창성 (category fluency) 과제를실시하였다. 범주유창성과제의범주는생물과무생물로나누어생물범주는동물과과일, 무생물범주는탈것과의류로총 4개의범주를사용하였다. 대상자의과제에대한이해를돕 기위해연구과제이외의범주중기존의연구에서생물과무생물의범주특성을모두가진 신체부위 범주를예를들어설명하고 (Gainotti & Silveri, 1996), 과제에대한이해가가능하다고판단된후에본과제를제시하였다. 본과제에서는대상자에게각범주에해당되는어휘를 1분간가능한많이열거하도록지시하고, 열거한단어의수를점수로하였다. 해당범주에속하는어휘를정확히산출했을경우만정답으로처리하였으며, 반복되거나불명료한어휘, 해당범주와무관한어휘의경우는 0점으로처리하였다. 치매환자의인지기능장애를고려하여동물범주는물고기나조류, 곤충을포함한동물전반을정답으로처리하였다 (Abe et al., 2004). 또한동물, 과일, 탈것과의류의검사순서는대상자마다무작위로제시하였다. 검사는방해를받지않는조용한장소에서일대일로이루어졌으며, 모든반응은 EDIROL R-09 WAVE/ MP3 Recorder를사용하여녹음한후전사하였다. 3. 자료분석통계적분석을위해 PASW 18.0 프로그램을사용하였다. 알츠하이머형치매환자와정상고령자집단에따른생물과무생물의범주유창성수행차이를알아보기위해범주와집단간반복측정다변량분석을실시하였다. 또한, 알츠하이머형치매환자에게서나타난오류수가생물, 무생물범주에따라차이가있는지를알아보기위해종속표본 t-검정을실시하였다. 유의수준은.05로하였다. Ⅲ. 연구결과 1. 알츠하이머형치매환자와정상고령자집단의범주별수행차이생물범주 ( 동물, 과일 ) 와무생물범주 ( 탈것, 의류 ) 의범주유창성과제의수행에있어서알츠하이머형치매환자와정상고령자의집단의기술통계치를 < 표 -2> 에제시하였다. 생물범주와무생물범주에서알츠하이머형치매환자집단의수행은동물, 의류, 탈것, 과일의순으로높았으며, 정상고령자집단의경우는동물, 과일, 의류, 탈것의순으로높았다. 두집단모두에서동물범주의수행이가장높았다. 그러나정상고령자집단의경우생물범주인과일범주가동물 574

4 최현주 / 경도알츠하이머형치매환자의범주유창성과제에서의범주특정적손상특징 다음으로높은수행을보인반면, 알츠하이머형치매환자집단은과일에서가장낮은수행을보였다. 무생물범주에서는두집단모두의류범주의수행이탈것범주의수행에비해높았다. 실시하였다. 그결과, 생물범주에서의오류수가무생물범주의오류수에비해유의하게높게나타났다. 범주에따른오류수의차이에대한결과를 < 표 -4> 에제시하였다. < 표 - 2> 집단에따른범주별범주유창성과제의수행에대한기술통계결과 생물범주평균무생물범주평균총점동물과일합계탈것의류합계 DAT a) (2.2) (2.3) (3.9) (1.6) (2.5) (3.4) (6.5) NC b) (2.3) (1.8) (3.7) (2.4) (2.8) (4.5) (6.8) a) DAT: Dementia of Alzheimer s Type, b) NC: Normal Control 다음으로알츠하이머형치매환자와정상고령자집단에따른생물과무생물의범주유창성수행에서의차이를알아보기위해범주와집단간반복측정다변량분석을실시하였다. 그결과, 집단과범주의주효과및집단과범주의상호작용효과모두에서유의미하였다. 이러한결과를통해생물과무생물범주모두에서알츠하이머형치매환자의수행이정상고령자집단의수행에비해유의하게낮았으며, 정상집단은생물범주의수행이무생물범주에비해유의하게높은데반해, 알츠하이머형치매환자집단은생물과무생물범주사이의수행차이가유의하지않음을알수있다. 범주와집단간반복측정다변량분석결과는 < 표 -3> 과같다. < 표 - 3> 범주와집단간반복측정다변량분석결과 변량원 제곱합 자유도 F 집단 ** 범주 ** 집단 범주 ** 오차 합계 ** p < 알츠하이머형치매환자의범주별오류수의차이 생물범주 ( 동물, 과일 ) 와무생물범주 ( 탈것, 의류 ) 의범주에따른알츠하이머형치매환자의오류수에차이가있는지를알아보기위하여종속표본 t-검정을 < 표 - 4> 알츠하이머형치매환자의범주별오류수에대한종속표본 t- 검정결과 평균표준편차 t 생물범주 * 무생물범주 * p <.05 Ⅳ. 논의및결론 본연구는알츠하이머형치매환자를대상으로범주유창성과제를통해생물과무생물범주간범주특정적손상의특징을정상고령자와의비교를통해알아보는것을목적으로하였다. 우선, 생물범주 ( 동물, 과일 ) 와무생물범주 ( 탈것, 의류 ) 의범주유창성과제의수행에있어서알츠하이머형치매환자와정상고령자의집단간차이를알아본결과, 알츠하이머형치매환자집단은정상고령자집단에비해생물범주와무생물범주모두에서유의하게낮은수행을보였다. 이러한결과는범주유창성과제에서알츠하이머형치매환자의손상을설명하는기존의연구들의결과와일치한다 (Albert et al., 2001; Perry, Watson & Hodge, 2000). 범주유창성과제에서의전반적인손상은알츠하이머형치매환자의의미기억장애의초기의증상으로여겨진다. 본연구에서는과제수행에미치는전반적인인지기능의영향을줄이기위해경도알츠하이머형치매환자만을대상으로하였다. 그럼에도불구하고범주유창성과제에서정상고령자와의수행차이는현저히드러났다. 특히, 동물과과일의생물범주에서는정상고령자의 40% 정도의수행을보여그차이가더현격하였다. 범주유창성과제는정해진범주의어휘를정해진시간안에산출하게하는과제로언어성의미기억뿐아니라주의력이나전반적인모니터링능력, 작업기억, 시각적이미지의재생능력과같은여러가지인지능력을요구하는복잡한과제이다 ( 오혜경 최현주 김수진, 2010; Chertkow & Bub, 1990; Salmon, Heindel & Lange, 1999). 이렇듯, 범주유창성과제는통합적인인지및언어능력을요구 575

5 언어청각장애연구 2010;15; 하기때문에질환의아주초기의환자에게서도손상이나타나며, 이러한이유로범주유창성과제는일상적인대화나대면이름대기에손상이나타나기이전의단계의환자를검출할수있는유용한검사로여겨진다 ( 최현주, 2008; Adlam et al., 2006). 다음으로, 본연구에서는생물범주 ( 동물, 과일 ) 와무생물범주 ( 탈것, 의류 ) 의범주유창성과제의수행에있어서알츠하이머형치매환자와정상고령자의집단내차이를알아보았다. 그결과, 정상고령자의경우무생물범주에비해생물범주에서의수행이유의하게높은데반해, 알츠하이머형치매환자의경우에는범주간차이가나타나지않았다. 이러한결과는알츠하이머형치매환자들이상대적으로생물범주에서선택적인손상을보임을시사한다. 또한이는알츠하이머형치매환자들의생물범주의범주특정적손상과관련된지금까지의대부분의연구결과와일치하는결과이다 (Daum et al., 1996; Laws et al., 2007; Silveri et al., 1991). 그러나이러한범주특정적손상과관련된대부분의연구에서그림이름대기과제를사용한반면, 본연구에서는범주유창성과제를사용하여기존의연구방법의문제점을보완하였다. 범주유창성과제는앞에서언급했듯이의미적손상에민감하기때문에그림이름대기과제에비해알츠하이머형치매환자의손상을검출해내기에효과적이다 (Henry, Crawford & Phillips, 2004). 또한, 주어진시간동안해당범주의이름을말하는범주유창성과제는그림이름대기과제와는달리어휘의친숙함이나빈도에영향을받지않고, 정상집단에서도천정효과가나타나지않으며, 보다광범위한범주의항목들을평가할수있다는장점이있다 (Laws, 2004). 본연구의결과에서나타났듯이정상고령자의경우 동물 범주의수행이 탈것 범주수행의두배정도높은것으로나타나는등정상고령자의수행에서도천정효과없이각범주에따른수행차이가드러났다. 범주유창성과제로연령과성별의수행차이를살펴본 Moreno-Martínez, Laws & Schulz (2008) 의연구에서는정상성인의동물범주의수행이본연구의결과처럼다른범주에비해월등히높지않았는데, 이는 동물 (animal) 을곤충이나조류, 어류를제외한일반적인네발동물로국한시킨결과가영향을미쳤을것으로여겨진다. 일반적인범주유창성과제에서사용되는범주의수준 (level) 을고려할때동물을보다하위범주 ( 예 : 곤충, 어류, 조류 ) 로분류하는것이 그밖의생물범주 ( 예 : 채소, 과일, 나무등 ) 나무생물범주 ( 예 : 도구, 악기, 가구등 ) 의수준과동일한수준으로맞추는데적합하다 (Moreno & Cañamón, 2005, Moreno-Martínez, Laws & Schulz (2008) 에서재인용 ). 그러나알츠하이머형치매환자의경우초기부터의미기억의손상이두드러지고이러한의미기억의손상과관련하여범주분류능력의제한이보고되고있다 (Koenig et al., 2007). 이러한의미기억손상이범주유창성과제수행시모호한범주에서의어려움을가중시키는영향을배제하기위해치매환자를포함한고령자의경우동물범주의구분을최소화할것을권고하고있다 (Abe et al., 2004). 본연구를통해일반적으로사용되는그림이름대기과제가아닌범주유창성과제에서의생물범주의선택적손상이알츠하이머병환자에게나타남을알수있다. 이는과제와관계없이생물범주에서의범주특정적손상즉, 생물범주의의미지식이선택적으로손상되었음을의미한다. 이는알츠하이머형치매에서특징적으로나타나는신경병리인내측측두엽 (medial temporal lobe) 의위축과관련지을수있다. 이부위는시각을포함한감각적표상을담당하는곳으로생물범주의지식과관련성이높다 (Daum et al., 1996; Silveri et al., 1991). 알츠하이머형치매환자의내측측두엽의위축은치매의초기부터의전형적인증상으로, 본연구의대상이되었던경도치매환자들의경우에도이러한손상이나타났다. 또한, 알츠하이머형치매가진행되어전반적인뇌의위축이나타나게되면이러한생물범주에서의범주특정적손상의효과는사라진다고알려져있다 (Gonnerman et al., 1997). 이와관련하여, 전두-두정피질영역 (fronto-temporal cortical area) 및실비안구주변 (perisylvian) 영역의국소적인손상은무생물범주에서의범주특정적손상을초래한다 (Gainotti, 2000; 2005). 이부위는기능적인표상을담당하는곳으로무생물범주는생물범주에비해기능적인측면에서의의미지식이중요시되기때문에, 이부위의손상은인공물 ( 비생물 ) 의선택적손상으로이어진다. 따라서, 실비안구주변의언어영역에손상을입은일부실어증환자의경우알츠하이머형치매환자와는반대로무생물범주에서의범주특정적손상이나타난다 ( 강연재, 2009; Warrinton & McCarthy, 1983). 이러한결과들로부터생물과무생물범주의범주특정적손상은뇌손상의병소에따른이중해리 (double dissociation) 로설명될수있다 (Hills 576

Since picture naming tests are frequently affected by methodological problems, the present study used category fluency tasks to investigate performance differences between living and non-living objects in patients with dementia of Alzheimer s type (DAT). Methods: Fifteen patients with DAT and fifteen healthy elderly adults participated in the present study. The category fluency tasks included four semantic categories, two living domains (animals and fruits) and two non-living domains (vehicles and clothing). Results: The performance of the control group was better than the DAT group in all categories. However, the control group s performance was higher in the living categories than in the non-living, while the DAT group showed no significant differences between the two domains. Additionally, in the performance of the DAT group, the number of errors was greater in the living categories than in the non-living. Discussion & Conclusion: The results of the present study suggest that patients with DAT have some selective impairment in their knowledge of living objects.
