Objectives: The purpose of the current study was to examine the effects and generalization effects of Semantic Feature Analysis (SFA) treatment using a familiarity of nouns scale with individuals who have early dementia of the Alzheimer s type (DAT). Methods: Six individuals with early DAT participated in the study. Three individuals with DAT were treated with low familiarity items first then moved to high, whereas the other three began with high familiarity items first. The SFA treatment protocol consisted of 5 stages: 1) confrontation naming of 32 stimuli (16-treated, 16-untreated), 2) category sorting, 3) semantic feature selection, 4) yes/no questions, and 5) naming. Results: There were significant improvements on treated items after SFA treatment; There were limited generalization effects on untreated items and other naming tasks. In terms of noun familiarity, the High-to-Low group seemed to have greater benefits than the Low-to-High group. Conclusion: Individuals with DAT demonstrated significant increase in naming performance on treated items after SFA treatment using a familiarity of nouns scale. However, the treatment effects were not generalized to untreated noun exemplars and other linguistic-cognitive tasks. Furthermore, they showed significantly decreased performance on the K-MMSE, indicating that their disease had progressed. Despite their clear progress with DAT, it is interesting to note that they showed treatment effects on the treated items. Further studies are required to develop more diverse approaches to language treatments for individuals with neurodegenerative disease. Keywords: Semantic feature analysis (SFA) treatment, Dementia of the Alzheimer type (DAT), Familiarity of nouns 현재우리나라는 70세이상의고령노인증가로인해치매노인의수가늘어나는추세이며 (Korea National Statistical Office, 2006), 미국등고령사회로진입한국가의경우 75세노인의 5%, 80세노인의 9%, 85세노인의 16%, 90세노인의 30% 가치매인것으로추정된다 (Brookmeyer, Gray, & Kawas, 1998). 치매는지속적으로기억, 언어, 시공간, 개념화, 판단력중 3가지이상의인지영역손상이후 천적으로나타나는증후군으로이중알츠하이머성치매 (dementia of the Alzheimer s type, DAT) 가치매의절반을차지한다 (Cummings & Benson, 1992; Terry & Katzman, 1992). 65세이상부터 5년을기준으로 DAT 발병률이두배씩증가하며 (Hofman et al., 1991), 노인의평균수명이 80.1세로 (Korea National Statistical Office, 2006) 늘어남에따라조기진단과중재서비스를통해 DAT의잔존능력 34 Copyright 2015 Korean Academy of Speech-Language Pathology and Audiology This is an open-access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( by-nc/3.0) which permits unrestricted noncommercial use, distribution, and reproduction in any medium, provided the original work is properly cited.

DAT 는새로운정보를등록 학습하기어렵고 (Hodges & Patterson, 1995; Nestor, Fryer, Smielewski, & Hodges, 2003), 병변이진행될수록점진적인의미기억 (semantic memory) 의손실 (loss) 이진행된다 (Giffard et al., 2002; Grossman et al., 2003). 의미기억은개인적인경험인일화기억 (episodic memory) 과다르게문화적공유 (culturally shared) 와지속적으로학습 (overlearned) 되며 (Garrard, Perry, & Hodes, 1997), 조직화된지식이다 (Wheeler, 2000). 의미기억의기본이되는요소들은범주 (category) 와개념 (concept) 으로이름대기, 분류 (sorting) 와 (Bayles, Tomoeda, & Trosset, 1992; Bentham, Jones, & Hodges, 1997; Hodges, Salmon, & Butters, 1991; Martin & Fedio, 1983) 의미유창성에영향을미친다 (Butters, Granholm, Salmon, Grant, & Wolfe, 1987; Hodges, Salmon, & Butters, 1992; Martin & Fedio, 1983). 점진적인 DAT의의미기억손상은초기보다중기와말기에의미기억손상으로인한영향을많이받는다 (Patterson, Nestor, & Rogers, 2007). 이에병변이진행될수록이름대기와그림설명하기, 명사와그림매칭하기과제, 의미유창성점수가떨어진다 (Howard & Patterson, 1992). 의미유창성은보유 저장과회상 인출이효과적으로이루어져야하며, 병소부위에따라영향을미친다 (Coslett, Bowers, Verfaellie, & Heilman, 1991; Martin, Loring, Meador, & Lee, 1990). Randolph, Braun, Goldberg와 Chase (1993) 의연구에따르면 DAT의의미유창성은이름대기능력과유의미한상관관계를갖고있다. 정상노년층과비교했을때 DAT 환자는초기부터이름대기저하가관찰된다 (Barr, Benedict, Tune, & Brandt, 1992; Locascio, Growdon, & Corkin, 1995; Morris et al., 1991; Powell, Cummings, Hill, & Benson, 1988). 이에 DAT는스스로이름대기어려움을인지하고사물의용도나의미를풀어서설명하거나착어 (paraphasia), 빈번한대용어사용이관찰된다 (Parks, Haxby, & Grady, 1993). DAT 중증도가심화될수록명사이름대기오류가더욱빈번해지며 (Henry, Crawford, & Phillips, 2004; Karrasch et al., 2010; Lee, 2011) 언어산출의양과질의유의미성이저하되고, 언어능력의저하를겪으며말수가점차줄어들게되어결국은말을하지못하는상태로악화된다 (Bayles, 1982; Huff, Corkins, & Growdon, 1986; Lee, 2002). DAT를대상으로한이름대기과제에서오류유형을분석해보면의미오류 (semantic error) 가빈도높게나타난다 (Martin & Fedio, 1983). 많은연구에서이러한이름대기원인으로 DAT의의미체계손상을보고하고있다 (Bayles, Tomeda, Kaszniak, & Trosset, 1991; Chertkow & Bub, 1990; Hodges, Salmon, et al., 1992; Kim, Kim, & Na, 1997; Snyder, Holland, & Forbes, 1996). 의미체계의손상은어휘와음운산출체계에영향을미쳐명사산출을방해한다 (Bowles, Obler, & Albert, 1987). 반면 DAT의음운및문법적요소와관련된언어처리영역은상대적으로잘보존되는것으로보고되고있다 (Lee, 2002; Schwartz, Martin, & Saffran, 1979). 의미자질 (Semantic Feature Analysis, SFA) 중재는의미체계손상과관련된가장대표적인언어중재방법이다. SFA 중재는목표명사산출시어휘체계 (lexical system) 에서명사의의미자질을비교및대조하여의미자질을강화하고, 의미적네트워크를활성화시키는중재방법으로의미적산출과정을포함하는중재이다 (Boyle & Coelho, 1995). 즉, SFA 중재는목표명사의어휘체계를활성화시켜명사산출을도움으로서이름대기능력을향상시킨다 (Coelho, McHugh, & Boyle, 2000). 이러한 SFA 중재는브로카, 베르니케, 명칭실어증환자의이름대기능력향상과유지에효과적이다 (Boyle & Coelho, 1995; Coelho et al., 2000; Conley & Coelho, 2003; Law, Wong, Sung, & Hon, 2006). DAT의이름대기오류는의미체계손상으로기인한것으로보고되고있음에도 (Bayles et al., 1991; Cherkow & Bub, 1990; Daum, Riesch, Sartori, & Birbaumer, 1996; Hodges, Salmon, & Butters, 1992; Martin & Fedio, 1983) DAT 환자를대상으로한연구는주로중증도에따른명사이름대기특징및오류를밝히는연구로진행되고있으며, SFA 중재는지금까지실어증환자를대상으로만연구가진행되어왔다. DAT 환자를대상으로한 SFA 중재연구는전무한실정이다. 친숙도, 범주, 시각적복잡성등명사의속성은이름대기에영향을미친다 (Boyle, 2004; Kim & Kang, 2005). 명사친숙도란 개인적인경험에의하여명사가일상생활에서얼마나자주쓰이는가, 얼마나친숙한가 이다. 즉, 명사친숙도는그명사를개인이일상생활에얼마나자주사용하는가와관련된주관적개념이며, 주관적빈도라고도한다 (Yi, 2005). Snodgrass 와 Vanderwart (1980) 은이러한명사친숙도를표준점수로산출하여친숙도연구에사용하고있다. DAT 환자를대상으로한이름대기과제에서높은친숙도명사가낮은친숙도명사와비교하여 2배정도높은정반응을보였다 (Gainotti, Di Betta, & Silveri, 1996). 실어증환자를대상으로한연구에서도낮은친숙도명사가높은친숙도명사에비해산출에어려움이있다고보고하고있다 (Badecker, Miozzo, & Zanuttini, 1995; Duffy & Coelho, 2001; Thompson, 1994). 친숙도는의미기억을조직 (organization) 하는데중요한역할을하는요소로서이름대기중재에서반드시고려해야할요소중하나이다 (Hirsh & Funnell, 1995)

3 Kyeong Ok Mo, et al. Semantic Feature Analysis Treatment in Early DAT 최근이러한명사친숙도특징을기반으로실어증환자를대상으로 SFA 중재연구가실시되었다. Conley와 Coelho (2003) 의연구에서는비유창성실어증환자 1명을대상으로스스로높은친숙도와낮은친숙도명사를분류하도록하였다. 한회기에높은친숙도명사와낮은친숙도명사를동시에 SFA 중재를실시하였다. 사전 사후중재명사와비중재명사의정반응률과친숙도별로중재명사이름대기정반응률을비교하였다. SFA 중재결과중재명사와비중재명사모두사전보다사후명사이름대기정반응률이증가하였으며, 사후중재명사이름대기정반응률이비중재명사보다높았다. 또한사후높은친숙도와낮은친숙도중재명사모두정반응률이 80% 로상승하였으며, 증가폭을보면높은친숙도중재명사는 50%, 낮은친숙도중재명사는 66.7% 로낮은친숙도중재명사의증가폭이더컸다. Law 등 (2006) 의연구에서는 3명의실어증환자를대상으로 SFA 중재가이루어졌으며, 명사친숙도는정상노년층을대상으로조사하여선정하였다. 중재순서는높은친숙도명사 SFA 중재후이름대기정반응률이 80% 이상이면, 낮은친숙도명사중재로전환하였다. 사전 사후중재명사와비중재명사이름대기정반응률을비교하였으며, 친숙도별로이름대기정반응률을비교하였다. 참여자모두사후중재명사이름대기정반응률이사전보다높았으며, 사전 사후비중재명사정반응률은참여자별로달랐다. 즉, 3명중 1명은사후비중재명사정반응률이사전보다높았으나나머지 2명은거의차이가없었다. 현재까지친숙도를활용한 SFA 중재연구는실어증환자를대상으로만진행되었으며, DAT 환자를대상으로한 SFA 중재연구는제한적이다. 따라서, 본연구에서는친숙도의중재순서를역균형화하여초기 DAT 환자들을대상으로 SFA 중재효과를알아보고자하였다. 구체적인연구질문은아래와같다. 첫째, SFA 중재전 후에따라중재명사이름대기수행력에유의한차이가있는가? 둘째, SFA 중재전 후에따라비중재명사이름대기수행력에유의한차이가있는가? 셋째, SFA 중재전 후에따라생성이름대기과제수행력에유의한차이가있는가? 넷째, SFA 중재전 후에따라표준화된대면이름대기검사에서유의한차이가있는가? 다섯째, SFA 중재전 후에따라표준화된언어검사에서유의한차이가있는가? 여섯째, SFA 중재전 후에따라인지검사에서유의한차이가있는가? 연구방법 연구대상 본연구의참여자는서울지역의주간보호센터를이용하는 DAT 환자중에서스스로명사이름대기에어려움을호소하거나가족이 나기관의담당자의판단으로명사이름대기에어려움을보이지만 읽기가가능한초기 DAT 6 명을선정하였다. 선정기준은 (1) 신경과 전문의에의해 DAT 로진단받고, (2) Seoul-Instrumental Activities of Daily Living (S-IADL; Ku et al., 2004) 에따른 Global Deterioration Scale (GDS; Reisberg, Ferris, de Leon, & Crook, 1982) 에서치매 로인한손상이행동상의이상이나심한정신과적인증상으로나타 나지않는 GDS 5 로 Ku 등 (2004) 에서제시한 Appendix 1 과같이 초기치매인지장애로판단되는환자 이다. SFA 중재자극 SFA 중재를위한자극은 DAT 환자의일상생활에필요한범주 를위주로 Sung, Mo, Lee 와 Sim (2014) 의논문을참조하여구성하 였다. 이중우선순위가높으면서 DAT 의이름대기에영향을적게 받는무생물범주 (Garrard et al., 2001) 중 생활용품 범주를선정 하였다. Sung 등 (2014) 에서친숙도의 Z-score 가.40 이상을높은친 숙도로 -.40 이하를낮은친숙도로분류하여중재자극명사를선 정하였다. 중재에사용된명사는 Appendix 3 에제시하였다. 실험 자극명사는 Figure 1 과같이컬러실물카드로제작하여정상성 인 20 명을대상으로실시한이름대기에서일치율이 90% 이상인실 물사진만을자극으로사용하였다. 중재에포함된명사는친숙도 별로 8 개씩총 16 개이며, 비중재명사또한친숙도별로 8 개로구성 하여 16 개로이루어졌다. 의미자질제공에사용되는자극은글자카드로제시하였으며, Knife Kettle Figure 1. 4 명사친숙도를활용한의미자질중재가초기알츠하이머성치매노인의이름대기능력에미치는효과 모경옥외 의미자질은표준국어대사전, 두산백과사전, 위키백과사전, Rhee (1991) 의연구에서조사된공통된의미자질중외형적 (physical) 특징과기능적 (functional) 특징을중심으로분류하였다. 중재에사용된명사자극의외형적및기능적특징별로옳은의미자질카드를 1개씩준비하고, 틀린의미자질카드또한특징별로 1개씩선정하였다. 틀린의미자질은방해자극 (distractor) 으로중재에사용하였다. 각중재명사에대한의미자질은 Appendix 4에제시하였다. Na, 1997) 의홀수문항, (4) PK-WAB-R (Kim & Na, 2012), (5) Seoul Neuropsychological Screening Battery (SNSB; Kang & Na, 2003) 의 Seoul Verbal Learning Test (SVLT), Rey Complex Figure Test (RCFT), Korean-Color Word Stroop Test (K-CWST), K-MMSE, (6) Korean-Wechsler Adult Intelligence Scale (K-WAIS; Oh, Yum, Park, Kim, & Lee, 1992) 의숫자폭과제와 Sung (2011) 의단어폭과제를실시하였다. 의미자질 (semantic feature analysis) 중재 SFA 중재는 Kiran과 Thompson (2003) 이사용한프로토콜을보완하여다음과같이총 5단계로구성하였다. 1단계 ( 이름대기 ): 32개 ( 중재명사 16개, 비중재명사 16개 ) 의명사자극에대한이름대기를실시하였다. 오반응시피드백을제공하지않고 2단계로진행한다. 2단계 ( 범주분류 ): 책상위에 생활용품 글자카드와방해범주 교통수단 글자카드를배치한다. 중재명사그림카드 8개와방해범주그림카드 8개를무작위로책상위에배열한다. 이후, 그림카드를각각의범주에맞게배열하도록한다. 오반응시, 치료사가수정해준다. 3단계 ( 의미자질분류 ): 중재명사그림카드 1개만남겨두고, 책상위에모든그림카드는제거한다. 중재명사에해당하는옳은의미자질 2개와틀린의미자질 2개를무작위로책상위에배열한다. O/ X 카드를제시한후, 중재명사에해당하는의미자질은 O 카드아래, 해당하지않는의미자질은 X 카드아래에분류하도록지시한다. 오반응시, 치료사가수정해준다. 4단계 ( 예 / 아니요질문 ): 3단계에서사용된의미자질에대해서 예 / 아니요 로질문한다. 예를들어, 이것은딱딱합니까? 라고의미자질카드를질문형태로수정하여실시한다. 오반응시수정해준다. 이중, 옳은의미자질에대한카드만소리내어읽는다. 5단계 ( 이름대기 ): 중재명사의그림카드를가리키면서이름대기를실시한다. 정반응시다음중재명사로넘어가고, 오반응시중재명사이름을 1음절씩음절단서를제시한다. 단서후에도오반응시, 치료사는이름을알려주고따라말하도록한후다음중재명사로넘어간다. 평가과제 SFA 중재사전 ( 중재시작 1주일이내 ) 및사후 ( 중재종료후 1주일이내 ) 에따라다음과같은과제를실시하였다 : (1) 중재포함및불포함명사자극대면이름대기, (2) 1분동안 Controlled Oral Word Association Test (COWAT; Kang, Chin, Na, Lee, & Park, 2000) 의 동물 과 슈퍼마켓 범주의생성이름대기실시, (3) K-BNT (Kim & 실험설계및통계분석본연구는사전-사후비교설계 (pretest-posttest design) 하였다. 의미유창성점수는 COWAT (Kang et al., 2000) 과제로 1분동안 동물 과 슈퍼마켓 범주에포한된명사 1개를 1점씩산출하여사전 사후비교하였다. 이에본연구에서는의미유창성을생성이름대기점수로통일하여사용하였다. 중재에참가한 6명의대상자는친숙도에따른중재순서를역균형화 (counterbalance) 하여두집단으로구성하였다. 첫번째집단은친숙도가높은것에서낮은순서로중재한집단 ( 고 저집단 ) 이고, 두번째집단은친숙도가낮은것에서높은순서로중재한집단 ( 저 고집단 ) 이다. 참여자들은두집단으로유사무작위 (peudo-randomization) 로배정하는것을원칙으로하였다. 단, 높은친숙도이름대기평균정반응값이 Table 1과같이 8점중 5.5점이상인참여자 1, 2, 3은천장효과 (ceiling effect) 를고려하여친숙도가낮은명사부터중재를시작하도록 저 고집단 으로구성하였고, 참여자 4, 5, 6은친숙도가높은명사부터중재를시작하도록 고 저집단 으로구성하였다. 각친숙도 ( 고 / 저 ) 별로 8회기씩구성하여, 중재는 1시간씩, 주 2회로써총 16회기로구성하였다. 사전평가는중재시작 1주일내에총 2회에걸쳐진행하였으며, 사후평가또한중재가끝난후 1주일내에동일한방식으로진행하였다. 통계분석은비모수통계로써윌콕슨부호순위검정 (Wilcoxon signed-ranks test) 을실시하였다. 친숙도중재순서에따른집단간비교 ( 저 고집단, 고 저집단 ) 는대상자수가제한적인점을고려하여중재효과와직접적인관련이있는중재명사및비중재명사에대한정반응률에한에서기술통계를활용한평균비교로자료분석을실시하였다. Semantic Feature Analysis Treatment in Early DAT 연구결과중재명사의사전 사후이름대기비교및친숙도중재순서에따른집단간비교총 6명의참여자의중재사전-사후중재명사이름대기수행력변화를살펴보기위해윌콕슨부호순위검정 (Wilcoxon signed-ranks test) 을실시하였다. 그결과, 중재후평균정반응률은 61.5% (SD = 2.04) 로서중재전 52.13% (SD =1.83) 에비해유의하게증가한것으로나타났다 (Z= -2.06, p<.05). 친숙도중재순서에따른 고 저집단 과 저 고집단 간사전 사후중재명사이름대기평균정반응률은 Table 2와같다. 이름대기평균정반응률차이를살펴보았을때 고 저집단 이중재후에 12.5% 증가율을보인반면, 저 고집단 은 6.25% 의증가율을보였다. 비중재명사의사전 사후이름대기비교및친숙도중재순서에따른집단간비교총 6명의참여자의중재사전-사후비중재명사이름대기수행력변화를살펴보기위해윌콕슨부호순위검정 (Wilcoxon signed-ranks test) 을실시하였다. 그결과, 중재후평균정반응률은 55.25% (SD =1.88) 로써중재전 52.63% (SD =1.71) 에비해수치는증가하였지만, 통계적으로는유의하지않은것으로나타났다 (Z=-.315, p=.752). 친숙도중재순서에따른 고 저집단 과 저 고집단 간비중재명사사전 사후이름대기평균정반응률은 Table 3과같다. 고 저집단 은사후비중재이름대기평균정반응률이 41.63% 로 저 고집단 의이름대기평균정반응률인 63.5% 에비해낮은것으로나타났지만, 사전 사후이름대기평균정반응률차이를살펴보았을때 고 저집단 이사후 4.25% 증가하였다. 반면, 저 고집단 의경우사후 1.13% 의근소한증가율을보이는것으로나타났다. Table 4 에제시한바와같이유의수준.05 에서통계적으로 유의미하지않은것으로나타났다. 사전 사후표준화된이름대기검사 (K-BNT) 수행력비교 총 6 명의참여자들의사전 사후표준화된이름대기검사 (K-BNT) 수행력을비교하기위해윌콕슨부호순위검정 (Wilcoxon signedrank test) 을실시하였다. Table 5 에제시한바와같이유의수준.05 에서통계적으로유의미하지않은것으로나타났다. 사전 사후표준화된언어검사 (PK-WAB-R) 에서의수행력변화 비교 총 6 명의참여자들의사전 사후표준화된언어검사 (PK-WAB-R) 수행력을비교하기위해윌콕슨부호순위검정 (Wilcoxon signedrank test) 을실시하였다. Table 6 에제시한바와같이표준화된언 어검사에서 AQ 및 AQ 하위영역모두에서통계적으로유의한수행 력변화는관찰되지않았다. 사전 사후전반적인지능력변화비교 총 6 명의참여자들의사전 사후전반적인지능력변화비교를 위해윌콕슨부호순위검정 (Wilcoxon signed-rank test) 을실시하 Table 4. Table 7 에제시한바와같이 K-MMSE 평균점수에서통계적 으로유의한차이가관찰되었다 (Z= -2.02, p<.05). 사전에비해사 후에 K-MMSE 점수가유의하게낮아진것으로나타났다. 나머지 항목에서는유의수준.05 에서통계적으로유의미하지않은것으로 나타났다. 논의및결론 Mean SD Z p-value 본연구의목적은초기 DAT 환자를대상으로친숙도를활용한 SFA 중재후 (1) 중재명사및비중재명사수행력에유의한차이가 있는지, (2) 이름대기과제 ( 생성이름대기과제및표준화된대면이름 대기검사 ) 에서수행력변화가나타나는지, (3) 이외에도표준화된 언어검사및인지검사에서수행력차이가나타나는지를알아보는 것이다. 총 6 명의초기 DAT 환자를대상으로 SFA 중재를실시한결 과, 중재후중재명사의평균이름대기정반응률이중재전보다통 계적으로유의하게증가하였다. 반면비중재명사의경우, 사후평 균이름대기정반응률은증가하였으나통계적으로유의한차이는 없는것으로나타났다. 즉, DAT 환자를대상으로한 SFA 중재가중 재명사의경우에는수행력증가에도움을주는것으로나타났으나 비중재명사로의일반화는제한적임을알수있다 PK-WAB-R= Paradise Korean version of the Western Aphasia Battery-revised; AQ= Aphasia Quotient. Beeson 등 (2011) 에따르면 PPA 환자를대상으로생성이름대기와 SFA 를활용한이 름대기중재를실시하였을때, 중재를실시한범주에서의이름대기 수행력과중재를실시하지않았던범주의이름대기수행력모두증 가하였다. 이러한수행력은중재가끝난후 3 주및 6 개월후에도이 름대기수행력이유지되는것으로나타났다. PPA 의경우언어손상 이주호소로시작되는치매의한종류로서언어중재에대한효과 를살펴보는연구가최근다양하게진행되고있지만, DAT 환자를

Semantic Feature Analysis Treatment in Early DAT 대상으로한언어치료연구는매우드물다. 이에본연구에서나타난이름대기중재효과는이러한점에서임상적의미가클것으로기대한다. 본연구에서는 DAT 환자들의손상이의미기억과관련된손상으로부터시작된다는점을착안하여, 의미기억을강화하는원리가내재된중재방법으로써실어증환자에게중재효과가다양하게증명되어온 SFA 중재방법을선택하였다. 다양한촉진단계로구성된 SFA 중재를친숙도에맞추어중재를구성하였다. 실어증환자들을대상으로한 SFA 중재는최근복잡성이론 (complexity theory) (Kiran & Thompson, 2003) 에근거하여복잡한의미자극부터치료하는것이보다효과적이라는연구가발표되었다. 하지만 DAT 환자에게는이러한복잡성이론이적용되기에는어려움이있다. 즉, PPA와실어증환자들은친숙도가낮은단어의이해와이름대기어려움은초기단계부터진행되며이러한원인은의미기억저하 (deficit) 로인해발생하며 (Adlam et al., 2006; Hodges, Patterson, Oxbury, & Funnell, 1992; Luzzi et al., 2007) DAT 의이름대기오류는의미체계와 (Bayles et al., 1991; Chertkow & Bub, 1990; Hodges, Salmon, & Butters, 1992; Kim et al., 1997; Snyder et al., 1996) 의미기억의손실 (loss) 로인한실패 (breakdown) 로발생하기때문이다 (Cher kow & Bub, 1990; Chertkow, Bub, & Caplan, 1992; Hodges et al., 1991). 이에 DAT 환자의경우, 복잡한자극을통해오류를통한학습체계 (trials and errors) 보다는오류배제훈련 (errorless learning) 이보다적합할수있다 (Crowe & Gabriel, 2013). 따라서, 본연구에서는일상생활에가장필요하다고생각되는의미범주에해당하는사물들을선택하여 DAT 환자를위한 SFA 중재를구성하였고, 명사이름대기에영향을미칠수있는다양한요인중에서무엇보다도친숙도에집중하여중재를실시하였다. 친숙도를고 / 저로나누어중재를실시하는순서를역균형화하였다. 제한된대상자수로인해, 중재순서에따른효과를통계적으로살펴볼수없었다는점은본논문의분명한한계점이다. 하지만이러한중재효과를기술통계방식으로라도살펴보는것은임상적으로매우의미있는분석이라고생각되어, 중재순서에따른평균을비교해보았다. 그결과 고 저 집단의사후이름대기정반응률의증가폭이 저 고 집단에비해약 2배높았다. 즉, 친숙도를활용한 SFA 중재에서중재순서를 고 저 순서로했을때효율성이증가될수있음을시사한다. 중재의일반화효과로살펴본생성이름대기과제및표준화된언어및인지검사전반에는통계적으로유의한차이를보이는과제는없었다. 참여자수의제한으로일반화하기는어렵지만본연구에서는 K-MMSE 점수만중재후유의하게낮아지는것으로나타 났다. Appendix 5과같이대상자 6명중 1명만사후 K-MMSE 점수가유지되었고, 5명은감소한것으로나타났다. DAT는퇴행성질환으로써점진적으로진행하는것이그특징임을고려할때, 전반적인인지및언어능력이시간이지남에따라수행력이저하되는것을예상하는것이일반적이다. 이러한특징때문에언어치료에대한필요성이다른신경언어장애군에비해부각되지않은것이현실이다. 하지만본연구결과를살펴보면, 중재명사에서는대상자 6명이모두수행력이증가하는것을보였으며, 이러한결과는통계적으로유의하였다. 즉, 퇴행성질환환자들이지만중재한자극에대한수행력증가는분명하게관찰되었다. 이러한결과는 DAT와같은퇴행성질환환자들에대한언어중재필요성을임상적으로제고할필요성있음을시사한다. 하지만본연구의대상자수가적고참여자들의사전이름대기능력과 K-MMSE 점수를비슷한수준으로통제하지못한점에서제한적이며, 중재에대한일반화효과도분명치않아보다다양한방법으로중재를시행하는후속연구가반드시필요하다. 친숙도를활용한 SFA 중재가 DAT 대상자에게효과를보였다는본연구의결과를기초로이러한중재방법을보다많은대상자에게적용하여결과를재확인할필요가있다. 또한 DAT 환자에게필요한최적의언어중재방법에대해서도더많은연구가필요하다. Semantic Feature Analysis Treatment in Early DAT 국문초록 명사친숙도를활용한의미자질중재가초기알츠하이머성치매노인의이름대기능력에미치는효과 모경옥 1,2 성지은 1 정지향 3 1 이화여자대학교언어병리학과, 2 베데스다병원재활센터, 3 이화여자대학교의과대학신경과 배경및목적 : 초기알츠하이머성치매 (dementia of the Alzheimer s type, DAT) 환자 6명을대상으로친숙도를활용한의미자질 (semantic feature analysis, SFA) 중재가중재및비중재명사이름대기, 기타이름대기, 표준화된언어및인지검사과제에서유의한차이가있는지살펴보았다. 방법 : 3명은높은친숙도중재후낮은친숙도중재를실시하였고, 나머지는낮은친숙도중재를먼저실시하였다.
