MEN1, or Wermer's syndrome, is inherited as an autosomal dominant trait. This syndrome is characterized by neoplasia of the parathyroid glands, enteropancreatic tumors, anterior pituitary adenomas, and other neuroendocrine tumors with variable penetrance. Inherited medullary thyroid carcinoma (MTC) consists of MEN2A, MEN2B, and familial medullary thyroid cancer (FMTC). The identification of hereditary MTC has been facilitated in recent years by direct analysis of germline RET proto-oncogene mutation. Key Words: Multiple endocrine neoplasia, Familial medullary thyroid carcinoma 서론 다발내분비종양 (multiple endocrine neoplasia, MEN) 은한가계의여러구성원에서 2가지이상의내분비종양들이동반되어나타나는질환으로임상양상에따라 MEN1과 MEN2A와 MEN2B로분류된다. MEN1은기능성또는비기능성의증식증이나종양이부갑상선, 췌장과십이지장, 뇌하수체등에서주로발생한다. MEN2A는갑상선수질암, 갈색세포종, 부갑상선증식증 ( 선종 ) 이나타나고, MEN2B는갑상선수질암, 갈색세포종, Marfan형체형, 점막신경종, 두터운입술등의소견을보이나부갑상선증식증은동반되지않는다. 1) 갑상선수질암은전체갑상선암의약 5-10% 정도를차지하는드문종양으로, 70-80% 는산발적 (sporadic) 으로발생하며, 20-30% 에서는유전성 ( 가족성 ) 으로발생 한다. 2) 유전성갑상선수질암은가계의구성원들에서갑상선수질암과다른내분비종양이동반되는 MEN2A, MEN2B와갑상선수질암만발생하고다른내분비종양은동반되지않는 familial medullary thyroid carcinoma (FMTC) 로분류한다. 1) 다발내분비종양은국내에서도많은예가보고되어있으며, 특히유전성갑상선수질암의선별검사에가장유용한 RET 유전자검사가이전에는일부시설을갖춘실험실에서만가능하였으나, 현재는상업적으로검사해주는곳이보편화되어있어, 부갑상선기능항진증이나갈색세포종이있는환자에서동반된유전성갑상선수질암의선별검사가용이해졌을뿐아니라유전성갑상선수질암환자의가족들에대해서도 RET 유전자검사를통하여갑상선수질암의조기발견및치료가가능하게되었다. 본종설에서는 MEN1과유전성갑상선수질암의관 Received March 7, 2012 / Revised May 18, 2012 / Accepted May 21, 2012 Correspondence: Young Sik Choi, MD, Department of Internal Medicine, Kosin University College of Medicine, 34 Amnam-dong, Seo-gu, Busan , Korea Tel: , Fax: , yschoi@kosinmed.or.kr Copyright c 2012, the Korean Thyroid Association. Data were adapted from Brandi et al. 1) 후에발생한다. 주된종양발생부위인부갑상선, 췌장및십이지장, 뇌하수체중 2곳이상에서종양이동반될경우 MEN1으로정의하며, 지표환자의가족중에서종양이 3곳중 1곳에서발생될경우가족성 MEN1 (familial MEN1) 이라고한다. 병인 MEN1은종양억제유전자인 MEN1 유전자의돌연변이에의해발생되며, MEN1 유전자는염색체 11q13에위치한다. 3) MEN1 유전자는 61개의아미노산으로구성된 menin이라는핵단백질을번역한다. 4) Menin은세포의성장과주기를조절하는데이 menin에불활성화돌연변이가생길경우다발성으로종양이발생한다. MEN1의선별검사 MEN1의선별검사에는생화학적검사와유전자검사가있다. 대부분의 MEN1은임상적으로진단이되므로생화학적검사가중요하다. MEN1은여러장기에서동시에발생하지않고, 시간간격을두고발생하므로어린나이에진단된부갑상선기능항진증의경우 MEN1 의가능성을놓치기쉽다. 그러므로젊은나이에진단된부갑상선기능항진증의경우는후에 MEN1을고려하여추적관리가요구된다. MEN1 유전자검사는유전자의 sequence가길고복잡하고, 검사에시간이소비되고, 가격이비싸므로일상적인진단과정에이용하기에는제약이있다. 5) 또한 MEN1 유전자는 RET 유전자와달리유전형과표현형사이에는연관이없고, 임상적소견이나타나기전 MEN1 유전자검사를하는것이환자의이환율과사망률을예방하는데효과가없다고한다. 그러므로유전자검사의대상은개인적상황에따라고려해볼수있다. 3 Young Sik Choi 보인자에서각질환에대한검사시작시기와검사방법은 Table 2에기술하였다. MEN1의임상양상 MEN1의임상양상중본란에서는가장흔한부갑상선기능항진증만기술하기로한다. 부갑상선기능항진증은 MEN1에서가장흔하게나타나는호르몬이상이며, 연령이 40-40세가되면 90% 이상에서발생한다. 6) 원발성부갑상선기능항진증환자중에서는 2-4% 에서만 MEN1과관련이있다고한다. 7) MEN1에동반된부갑상선기능항진증은산발적부갑상선기능항진증보다조기에발병하며, 다발성 (multiple gland) 으로발생하고, 남녀비에차이가없고, 재발이흔하다. 산발성인경우는여성에서호발하고, 80-85% 는단일종양 (single adenoma) 이흔하다. 임상증상은산발성부갑상선기능항진증과차이가없으며, 악성종양으로진행되지는않는다고한다. 8) MEN1의진단부갑상선기능항진증은혈청 Ca의증가와 P의감소및부갑상선호르몬 (parathyroid hormone, PTH) 의증가로진단할수있다. 뇌하수체와췌장의종양은종양에서분비하는호르몬검사와컴퓨터단층촬영 (computed tomography, CT) 이나자기공명영상 (magnetic resonance imaging, MRI) 으로진단이가능하다. MEN1의예후 MEN1의치료는수술및약물치료로복잡하며, 호르몬분비정도와악성화정도에따라예후가결정된다. MEN1의사망의 20-30% 는 MEN1과동반된암과관련이있다. MEN1에동반된암은주로췌장이나위장관의종양이흔하다. 남자는평균 55세, 여자는 47세에사망하는것으로보고되어있다. 9,10) MEN2 와유전성갑상선수질암 1961년 Sipple 11) 은갑상선암에동반된갈색세포종 1 예를보고하였고, 1968년 Steiner 등 12) 은갈색세포종, 갑상선수질암, 부갑상선기능항진증및 Cushing병을가진소아 1예를보고하면서 MEN2라고명명하였다. MEN2 는 1984년 MEN2A와 MEN2B로분류되었으며, MEN2 와 FMTC의임상양상은 Table 3과같다. 13) 갑상선수질암은 1906년 Jaquet 14) 에의해 아밀로이드를가진악성갑상선종 으로처음기술되었으며, 1959 년 Hazard에의해조직학적특징이기술되었고, 그후 Williams 15) 에의해 calcitonin을분비하는갑상선의 parafollicular C세포에서발생하는종양으로밝혀졌다. 갑상선수질암은 MEN2의주된질환으로 95% 이상에서발생되며, 수술만으로완치가가능하므로치료목표는위험을가진 (at-risk) 환자를조기에발견하여예방적수술을해주는것이다. 최근에는 MEN2 환자가족들에대한선별검사가시행됨에따라진행된갑상선수질암보다초기단계인 C세포과증식이더흔하게진단되는추세이다. 갈색세포종은 MEN2A에서는 10-60%, MEN2B에서는 50% 이상에서발생되고 1/3은양측성으로발생한다. 16) 부갑상선기능항진증은 MEN2A에만발생하며 MEN2B에서는발생하지않는다. 부갑상선기능항진증의유병률은약 15-25% 정도이며, 부갑상선증식증과다발성선종에의해생긴다. 점막신경종, Marfan형체형등은 MEN2B에서동반되어나타난다 (Fig. 1). 17) FMTC는위에서언급하였듯이여러가계에서갑상선수질암만발생하고다른내분비종양은동반되지않는경우인데, FMTC를엄격하게 (rigid) 정의할경우 10 명이상의소아에서유전자돌연변이를가지고있고, 50 세이상의가족에서도수질암만진단될경우, 다세대에걸쳐서갑상선수질암은발생하나, 갈색세포종이나부갑상선기능항진증이동반되지않은경우이며, 1) 덜 Table 3. 유전성갑상선수질암의유전적이상 1987년에유전성갑상선수질암에대한유전자가연관분석에서 10번염색체의중심절부위 (centromeric) 에있다는것이알려졌다. 18) 그후 1993년 MEN2형의유전자가염색체 10번의장완에위치하며, RET protooncogene이질환의후보유전자로인식되었다.

Figures adapted from Kim et al. 17) 엄격한 (less rigid) 정의는가족구성원중 4명에서유전성수질암만나타나는경우이다. 그러나 MEN2A가진단되기전갑상선수질암만가족들에서발견될경우 FMTC로진단할수있고, FMTC로생각한가계에서 MEN2A, 2B의임상양상들이발견되어최근에는 FMTC 를 MEN2A의변종으로생각하기도한다. FMTC는 MEN2A, 2B보다임상경과가양호하다. 유전성갑상선수질암의유전적이상 1987년에유전성갑상선수질암에대한유전자가연관분석에서 10번염색체의중심절부위 (centromeric) 에있다는것이알려졌다. 18) 그후 1993년 MEN2형의유전자가염색체 10번의장완에위치하며, RET protooncogene이질환의후보유전자로인식되었다. 그후 MEN2A, MEN2B와 FMTC 환자에서 RET protooncogene의배선돌연변이가밝혀졌다. 19) RET 유전자는 21개의 exon gene으로구성되어있으며, 세포형질막에부착된티로신키나제수용체인 RET 단백질을기호화하는데, RET 단백질은시스테인아미노산이풍부한세포외분획과카르복실기말단으로구성된세포내분획으로구성되어있다 (Fig. 2). 20) RET 티로신키나제를구성하는아미노산중한개의아미노산을변환시키는유전성돌연변이에의하여티로신키나제가활성화되어 MEN2가유발되며, DNA 염기순서의분석을이용하면 MEN2 환자의거의 에서 RET proto-oncogene의배선돌연변이를확인할수있다. RET 유전자의유전형과표현형사이에는연관성이높다. MEN2A는 exon 11의 codon 634, exon 10의 codon 609, 611, 618, 620에서시스테인아미노산이풍부한세포외분획에서시스테인이다른아미노산으로치환되어발생하는데이중 codon 634가가장흔하다. MEN2B 의 95% 에서 exon 16의 codon 918에서 methionine이 threonine으로치환되며, 드물게 exon 15에서 codon 883 에서도발견된다. FMTC는주로 exon 10의 codon 618, 620에서발생하고, 드물게 exon 11의 codon 630, 631에서발견되고, 그외나머지반수의 FMTC 소아에서 exon 13의 codon 768, 790, 791과 exon 14의 codon 804, 844와 exon 15의 codon 891에서발견된다 (Fig. 1, Table 4). 20,21) 병리학적소견갑상선수질암은 C세포과증식증에서 C세포의결절성과증식증과미세갑상선수질암으로이행하고최종적으로갑상선수질암으로진행하게된다. 이러한진행은다중심성 (multicentric) 으로일어나므로, 갑상선의여러부위에서서로이행정도가다른조직학적형태로발견된다. 한단계에서다음단계로의이행하는데걸리는시간은알려져있지않으나, MEN2A에서는 3세, MEN2B에서는생후첫수개월내에악성으로변화된것이보고되어있다. 종양이 1 cm 이상일경우 80% 이 127 J Korean Thyroid Assoc

Genotype and phenotype correlation in MEN2 Domain Exon Codon Phenotype Extracelluar Intracellular , 611, 618, , , 790, , 상에서인접림프선으로전이가동반된다. 21) 갑상선수질암의임상적특성 MEN2A, FMTC FMTC, MEN2A FMTC FMTC, MEN2B MEN2B 유전성수질암의경우산발성갑상선수질암과달리다중심성, 양측성으로발생한다. 4가지형태의수질암의임상적특성은 Table 5와같다. 22) 선별검사 1) Calcitonin: 1968년부터임상에서갑상선수질암의유용한 marker로이용되어왔다. 정상인에서는 10 ng/l 이하를보인다. 미세또는거대갑상선수질암에서는 calcitonin이증가되어있으나, C세포증식증의경우에는정상치를보일수있어, MEN2A, 2B에서초기병리소견인 C세포증식증을보인경우에는음성으로나타날수있다. 2) Pentagastrin stimulation test: C세포과증식또는미세갑상선수질암의조기진단에유용하나현재는유럽이나미국에서도사용하지않는검사이다. 검사방법은 pentagastrin을 0.5 ug/kg 정맥주사후 0분, 2분, 5 분, 10분후에 calcitonin을측정하면된다. 약제를빨리주사하면오심, 구토, substernal tightness, flushing, tingling 등이나타날수있다. Figure adapted from Eng et al. 24) 상을보일경우양성으로진단한다. 3) Calcium stimulation test: 2.5 mg/kg의 calcium gluconate를 30초이상에걸쳐 IV 한후 0, 1분, 2분, 5분에 calcitonin을측정하고, calcitonin이 100 ng/l 이상을보일경우양성으로진단한다. Pentagastrin 보다덜 sensitive 한단점이있으나, pentagastrin과같은부작용은없는장점이있어, 현재에는 pentagastrin 검사를이용하지않고 calcium 자극검사를이용하고있다. 4) RET 유전자배선돌연변이검사 : 배선 RET 유전자검사는산발성갑상선수질암과유전성갑상선수질암을구분하는데이용된다. RET 유전자돌연변이는 MEN2A에서는 95%, FMTC 85% 에서동정된다고한다. 2009년미국갑상선학회 (American Thyroid Association, ATA) 의갑상선수질암에대한진료지침에서는 C세포증식증, MTC, MEN2 병력을가진모든환자에서배선 RET 유전자검사를시행할것을권고하고있다 ( 권고수준 A). 또한 MEN2와 FMTC의가족력이있는경우에배선 RET 유전자검사를시행할것을권고하고있으며, MEN2B일경우에는출생후즉시에시행하고, MEN2A 와 FMTC인경우에는 5세전에시행할것을권고하고있다 ( 권고수준 A). 23) Calcium 자극검사는위양성과위음성의가능성이있으나유전자검사는이러한단점이없고, MEN1과달리 RET 유전자는유전형과표현형이관련이있고, 임 129 J Korean Thyroid Assoc

RET 유전자를이용한선별검사는 Fig. 3과같다. 21) 가족성수질암의위험수준 RET 유전자배선돌연변이의유전형과갑상선수질암의발생시기와악성도 ( 표현형 ) 가연관이있다고알려져있으며, 이전에는 3가지의위험수준 (risk level) 으로분류되었으나, 최근 ATA 지침에서는 4가지로분류되었다. ATA risk level-d (ATA-D) 는가족성갑상선수질암에서가장높은 (highest) 위험군으로 codon 883, 918을가지며, 아주어린나이에발병하고전이와사망률이높다. ATA risk level-c는다음단계의위험도 (a lower, still high risk of aggressive) 를가지며, codon 634 돌연변이를보인다. ATA risk level-b는다음단계 (lower risk for aggressive) 로 codon 609, 611, 618, 620, 630을가진다. ATA risk level-a는가장덜위험한 (least high risk) 군이며 codon 768, 790, 791, 804, 891을가진다. 23) ATA-B와 C 수준이이전에는중간위험군이었다. 가족성수질암의치료갑상선전절제술이필수적이다. C세포이상은항상양측성 (bilaterally) 으로발생하며, 수술당시에 C세포에서악성이발견되지않았더라도나중에악성으로진행될수있다. 2009년 ATA의갑상선수질암에대한진료지침에서는수술전림프절전이가발견되지않을경우갑상선전절제술과예방적중심림프절절제술을권고 하고있고, 수술전외측림프절전이가발견될경우에만외측림프절절제술을하는것으로권고하고있다. 또한갑상선수질암이발견되지않았더라도 RET 유전자의돌연변이가있으면 ATA 위험수준에따라예방적수술을시행한다 (Table 6). 23) 결론 본란을통하여 MEN1과가족성수질암을기준으로 MEN2에대해서간략하게살펴보았다. 이들질환은국내에서도흔히발견되는질환이므로, 향후더많은관심이필요할것으로생각된다. 또한최근에는국내에서도이들질환에대한유전자검사를하는기관이있으므로, 의심되는환자와가족들에게유전자검사를시행하면조기에유전성갑상선수질암을진단할수있을뿐아니라치료를통해이환율과사망률을줄일수있을것으로생각한다. 중심단어 : 다발성내분비선종, 가족성수질암.
