The aim of this study is to investigate the effects of temperament and character on specialty interests in medical students. MethodsZZA total 132 third-year medical students initially participated in this study. Among these participants, 128 students had filled out both demographic and Temperament and Character Inventory (TCI) questionnaires. Socio-demographic data, specialty interests and Cloninger s TCI scores were obtained. The specialty interests were categorized into surgical, medical and other departments. ResultsZZAmong 128 students, 88 (68.7%), 29 (22.7%) and 22 (8.6%) students chose medical, surgical and other departments, respectively. Students choosing surgery departments showed lower Harm Avoidance scores (F = 4.39, df = 1, p = 0.04) and higher Self-Directedness scores (F = 4.30, df = 1, p = 0.04) than those who chose medical departments. There was no significant differences in Novelty Seeking, Reward dependence, Persistence, Cooperativeness, Self-Transcendence scores between groups. ConclusionsZZTo the best of our knowledge, this is the first study that has demonstrated the relationship between TCI scores and specialty interests of medical students in Korea. The current study suggests that temperament and character influence specialty interests in medical students. These findings could have implications for medical education research and career counseling. Lee, MD Department of Psychiatry, Seoul National University College of Medicine, 101 Daehak-ro, Jongno-gu, Seoul , Korea Tel: , Fax: , junghyunlee@snu.ac.kr 의과대학학생들은졸업후자신의전공분야를선택하게되는데, 이과정에서개인적, 사회적, 문화적요인들을고려하여결정하게된다. 따라서전공과목선택에영향을줄수있는요인들을파악하는것은의과대학교육에서중요한과제일것이다. 현재까지다양한요인들이의과대학학생들의전공선택에영향을 미치는것으로알려져있다. 1-7) 이전연구에의해제안된대표적인변인으로는나이와성별, 경제적인자립정도등의인구사회학적변인, 의과대학다니기전의혹은다니는중의개인적인경험, 각전공의특징, 가족들의바람, 그리고학생들의흥미와적성, 의과대학졸업성적, 성격적인특징등이있었다. 4-6) 그러나의과대학학생들의전공선택에영향을준요인들을조사한선행연구들의대부분은특정전공분야에초점을맞추어져있었고, 여러 Copyright 2012 Korean Society of Biological Psychiatry 199

2 Korean J Biol Psychiatry 2012;19: 변인의상호작용에대해서연구한경우는더욱적었다. 2)4)6) 기존연구들중개인의기질 (temperament) 및성격 (character) 특성이전공선택에미치는영향을보고한몇개의연구들이있었다. 1)5)7-19) Yufit 등 18) 은친밀감 (intimacy) 이높은학생들은다양한전공을선택하였으나고립감 (isolation) 이높은학생들은주로기술위주전공 (technique-oriented specialty) 을선택하는경향이있었다고보고하였다. 의과대학입학시기에 Myers-Briggs Type Inventory( 이하 MBTI) 에서외향형 (extraverted type), 사고형 (thinking type) 경향을보인학생들은외과계열을, 내향형 (introverted type), 감정형 (feeling type) 경향을보인학생들은일차진료계열을더많이선택하였다. 15) 의과대학에입학직후학생들을대상으로한또다른연구에서 Zuckerman-Kuhlman Personality Questionnaire-short form 검사에서외과계전공에흥미를보이는학생들은충동적감각추구 (impulsive sensation seeking) 와공격성 -적개심 (aggression-hostility) 의경향이높고, 신경증-불안 (neuroticism-anxiety), 사회성 (sociability) 척도점수가낮은경향을보였다. 11) 의과대학 3학년및 4학년학생들을대상으로한다른연구에서는관심전공과목에따라기질및성격척도 (Temperament and Character Inventory, 이하 TCI) 에서기질척도점수의차이가있었다. 16) 이연구에서내과를선택한학생들은외과와응급의학과를선택한학생들에비해서위험회피성경향이높았으며, 응급의학과를선택한학생들은가정의학과와내과를선택한학생들에비해서자극추구경향이높았다. 16) 이러한기존연구들은개인의기질및성격특성이의과대학학생들의내과계또는외과계의전공선택할때에영향을줄수있음을시사한다. 그러나기존연구들은북미및유럽의의과대학학생들을대상으로하였기때문에국내의과대학학생들에게적용하기에여러제한점이있었다. 따라서국내의과대학학생들을대상으로하여개인의기질및성격특성이전공선택에어떤영향을미치는지에대한연구가필요할것으로생각된다. Cloninger 등 20) 이개발한 TCI는기질및성격에영향을주는생물사회학적요인이모두고려된자가보고형검사로신뢰도와타당도가확립되어있어, 20-22) 설문연구에서개인의기질및성격특성을평가하기위해적합한척도이다. 따라서본연구는한국판 TCI 23)24) 를이용하여, 국내의과대학학생들의기질및성격특성이내과계또는외과계전공선택에미치는영향을알아보고자하였다. 대상 본연구는서울대학교의과대학에재학중인본과 3 학년학 생들을대상으로 2012 년 2월 13일부터 2012 년 2월 15일까지수행되었다. 참여학생들은모두임상실습을시작하지않은시점에설문연구에참여하였다. 연구동의서를배부받은 154명의학생중, 연구의목적과방법에대한설명을듣고, 동의서에서명한 132명의학생들이최종적으로참여하였다. 설문지는인구사회학적정보, 관심전공분야그리고 TCI 설문지로구분하였고설문지는참여자의이니셜이없는고유번호만을부여하여설문내용의익명성을보장하였다. 본연구는서울대학교병원의학연구윤리심의위원회 (Institutional Review Board) 의승인을받아시행되었다. 평가도구및내용인구사회학적정보기본적인인구사회학적정보로나이, 성별, 결혼, 종교, 군복무여부, 가구소득을평가하였다. 그외전공선택에영향을줄수있는변수로서가족중의료인과의사여부및전공분야를평가하였다. 또한과거정신과치료력, 한국판 Beck의우울척도 2판 (Beck Depression Inventory II, 이하 BDI-II) 23) 과한국판 Beck의불안척도 (Beck Anxiety Inventory, 이하 BAI) 25) 를평가하였다. BDI-II 는 Beck 등 26) 에의해개발된자가보고형우울증상평가도구로신뢰도와타당도가광벙위하게검증된척도이다. 27)28) 한국판 BDI-II 은 Sung 등 23) 에의해 Cronbach alpha 0.83으로신뢰도가높으며, 우울증상평가에대한타당도가확립되어있다. BAI은 Beck 등 29) 에의해개발된자가보고형불안증상평가도구로 Kim과 Yook 25) 에의해타당도와신뢰도가확립되었다. 기질및성격척도 (Temperament and Character Inventory, TCI) TCI는총 240개의예 / 아니오문항으로구성된자기보고식설문지이며, 각각의기질과성격에해당하는항목점수를계산하도록되어있다. 20) 한국판 TCI는 Sung 등 24) 에의해번안되었으며, 신뢰도와타당도가검증되어있다. 기질은감정적인자극에대한개인의자동적인반응의차이를일컬으며, 공포와평안, 흥분과분노, 혐오와애착과같은기본적인감정반응양상을포함한다. 30) 기질은독립적으로유전이되고, 생애초기에발현이되며, 시간이지나도변하지않는특징이있다. 30) TCI에서는자극추구성 (novelty seeking), 위험회피성 (harm avoidance), 보상의존성 (reward dependence), 완고성 (persistence) 으로네가지기질차원을평가한다. 22) 자극추구성은새로운자극에호기심을보이고탐색하려고하며, 좌절을능동적으로회피하려는경향을말한다. 20) 이러한자극추 200

3 Temperament and Character for Specialty Interests Kim IH, et al 구경향이높은사람들은새로운상황에대한호기심이많고탐색적이며충동적인경향이있다. 20) 위험회피성은처벌, 새로운자극이나무보상이주어지는상황에서활동을억제하는경향을말하며, 20) 위험회피성이높은사람들은새로운상황을두려워하고회피하는경향이있다. 20) 보상의존성은긍정적인강화로이어지는행동을하고유지하려는경향을말한다. 20) 보상의존성이높은사람들은주로다른사람들의인정을통해행동을추구하는경향을보인다. 20) 완고성은좌절과피로에도불구하고행동을지속하려는경향을말하며, 완고성이높은사람들은목표의식이뚜렷하고완벽주의적인경향이있다. 20) 성격은우리자신과다른사람들, 다른대상들에대한개념에의해결정이되는자신의목표와가치의개인적인차이를일컫는다. 20) 성격은유전적인요인보다사회적인학습에의해결정이되고, 전생애에걸쳐서변화하는특징을가지고있다. 20)21) TCI는자기중심성 (self-directedness), 협조성 (cooperativeness), 자기초월성 (self-transcendence) 로 3가지성격차원을평가한다. 20) 이중자기중심성은자신이선택한목표와가치에맞게끔상황에맞추어서자신의행동을조절하고적응시키는경향을말한다. 20)21) 자기중심성이높은사람들은책임감이강하며목표의식이강하다. 20) 협조성은다른사람의반응에의해강화를받고다른사람을수용할수있는경향을의미하며공동체의일원으로서자신을지각하는것을말한다. 20) 협조성이높은사람들은공감적이며동정적인경향을보인다. 20) 자기초월성은자신과다른사물을우주의일부로서동일시하는경향을의미하며, 20) 자기초월성이높은사람들은철학적이고영적이며이상주의경향이있다. 20)21)30) 관심전공분야참여학생들은총 24개의전공중에서관심전공을선택하도록하였다. 선택한전공은대한병원협회의구분방법에따라다음과같이내과계와외과계로분류하였다 : 내과계 ( 내과, 소아과, 정신과, 신경과, 가정의학과, 피부과, 방사선종양학과, 핵의학과 ), 외과계 ( 외과, 산부인과, 안과, 이비인후과, 흉부외과, 신경외과, 정형외과, 비뇨기과, 성형외과 ). 지원계로분류되는응급의학과, 재활의학과, 마취통증의학과, 영상의학과, 병리학과, 진단검사의학과, 기초학문, 기타를선택한학생들을 TCI 항목점수차이분석에포함하지않았다. 통계분석인구사회학적인정보는범주형범수에대해서는 Fisher 의직접확률법 (Fisher s exact test), 연속형변수에대해서는 Kruskal-Wallis test 를이용하여비교하였다. 내과계와외과계의 TCI 항목점수의차이는공분산분석 (anlaysis of covariace) 을이 용하여비교하였다. 기존연구에서전공분야선택에영향을주는것으로알려진나이, 성별, 종교, 가족내의료인여부, 가족내의사의전공계열, 가구소득, 과거정신과치료력, BDI-II, BAI 점수가공변량으로사용되었다. 본연구는 Stata SE, version 11.0(Stata, College Station, TX, USA) 을사용하여통계분석을시행하였고, 통계분석의유의수준은 0.05 미만으로하였다. 총 154명중 132명의서울대학교의과대학본과 3학년학생들이설문연구에참여하였고전체응답률은 85.7% 였다. 이중인구사회학적정보설문지와 TCI 설문지중하나만작성한 4 명은본연구의분석에서제외하였다. 연구에참여한학생들의인구사회학적정보및설문지응답내용은 Table 1에제시하였다. 총 128명의학생들중남자는 80 명 (62.5%), 여자는 48명 (37.5%) 이었으며, 평균연령은 24.6 ± 2.8세였다. 종교는무교가 63명 (49.2%) 으로가장많았으며, 기독교 34명 (26.6%), 천주교 22명 (17.2%), 불교 9명 (7.0%) 이었다. 가구소득은 200만원미만 9명 (7.0%), 200~400만원 20명 (15.6%), 400~600만원 38(29.7%), 600만원이상 55명 (43.0%), 무응답 6명 (4.7%) 이었다. 또한가족중에의료인이있다고응답한학생은 52명 (40.6%) 이었다. 이중가족이의사인경우는총 37명 (28.9%) 이었다. 가족의전공분야는내과계 25명 (19.5%), 외과계 8명 (6.3 %), 기타 4명 (3.1%) 이었다. 설문연구에참여한학생중에서과거정신과치료를받은학생들은총 7명 (5.5%) 였다. 우울증상을평가하는 BDI-II 총점은 7.0 ± 8.4점, 불안증상을평가하는 BAI 총점은 3.7 ± 5.2점이었다. 전체응답자중내과계를선택한학생은 88명 (68.8%), 외과계를선택한학생은 29명 (22.7%), 지원계를선택한학생은 11명 (8.4%) 이었다. 내과계를선택한학생들은내과 48명 (37.5%), 정신건강의학과 19명 (14.8%), 신경과 6명 (4.7%), 가정의학과 5명 (3.9%), 피부과 6명 (4.7%) 이각각선택하였다. 외과계를선택한학생들은외과 6명 (4,7%), 산부인과 4명 (3.1%), 안과 4명 (3.1%), 이비인후과 1명 (0.8%), 흉부외과 1명 (0.8%), 신경외과 5명 (3.9%), 정형외과 9명 (7.0%), 성형외과 3명 (2.3%) 이각각선택하였다. 지원계의경우, 응급의학과 2명 (1.6%), 재활의학과 4명 (3.1%), 영상의학과 3명 (2.3%), 병리학과 1명 (0.8%), 기초학문 1명 (0.8%) 이각각선택하였다. 인구사회학적정보는관심전공분야 ( 내과계, 외과계, 지원계 ) 에따른차이가없었다. 내과계와외과계를선택한학생들간의 TCI 항목점수의차이는 Table 2에제시하였다. 외과계를선택한학생들은내과계를선택한학생들에비해위험회피성점수가유의하게낮았으 journal.biolpsychiatry.or.kr 201

Age, sex, Income of household, specialty of doctors within family, past psychiatric treatment, BDI-II and BAI total scores used as covariate, : p < TCI : Temperament and Character Inventory, SD : standard deviation, BDI-II : Beck Depression Inventory II, BA : Beck Anxiety Inventory 며 (F = 4.39 ; df = 1, 71 ; p = 0.04), 자기중심성점수가유의하게높았다 (F = 4.30 ; df = 1, 71 ; p = 0.04). 자극추구성, 보상의존성, 완고성, 협조성, 자기초월점수에서는두집단간의유의한차이가없었다. 본연구는한국판 TCI를사용하여의과대학학생들의기질및성격특성과관심전공분야의관련성을분석하였다. 내과 계와외과계를선택한학생들은위험회피성점수와자기중심성점수에서유의한차이가있었다. 내과계를선택한학생들은위험회피성경향이높고자기중심성경향은낮았다. 반면외과계를선택한학생들은위험회피성경향이낮고, 자기중심성경향이높았다. 위험회피성경향이높은사람은새로운상황을두려워하고불안감이높으며, 이러한기질로인해꼼꼼하고위험을높게평가한다. 20) 반대로위험회피성경향이낮은사람들은자신감이있고외향적이며, 새로운상황을두려워하지않는경향을보인 202

5 Temperament and Character for Specialty Interests Kim IH, et al 다. 12)22) 따라서본연구결과는내향적이며위험에대해높이평가하여걱정및주의를기울이는기질의학생들이내과계를더많이선호하며, 외향적이고새로운상황에적극적으로대처하는기질을가진학생들이수술및응급상황이많은외과계를더많이선택하는경향이있음을시사한다. 이결과는기존연구결과들과일관된소견이었으며, 22) TCI를사용한외국연구에서도내과를선택한학생들이외과나응급의학과를선택한학생들에비해위험회피성경향이높은것으로보고된바있다. 16) 자기중심성경향은책임감, 목적의식이뚜렷하며자신의행동을통해결정을하는자기결정적인성격을의미한다. 12)20) 본연구에서외과계를선택한학생들에서자기중심성점수가높았는데, 기존연구에서도외과의사들이내적통제위 (internal locus of control) 경향을보였다. 14)31) 또한국내의과대학생들의방어기제연구에서도외과를선택한학생들이강인한마음 (highly aggressive and though-minded, migh masculinity-feminity) 을가지고있고, 높은자아확신감을보여, 일관된경향을나타내었다. 32) 이는외적요인보다자신의행동을근거로하여의사결정을하는성격을가진학생들이외과계를더선호할가능성을시사한다. 이러한기질의차이는현재의성호르몬이나, 태내에서의남성호르몬의노출정도등의주산기환경등과같은생물학적요소가미치는영향이크다. 33)34) 일례로제2수지와제4수지길이 (2D : 4D) 는태내성호르몬의노출정도와예민성을반영하는지표로써, 이러한 2D : 4D 비율이 TCI의위험회피성과의연관성이유의미하게나타난바있다. 35) 이는의과대학생들의전공선택이환경적인영향은물론, 생물학적특성에의해영향을받을수있음을시사한다. 특히본연구에서는임상실습의경험이없는학생들을대상으로하였기때문에, 임상경험과관련된개인적인경험의영향을배제할수있어, 보다학생들의생물학적인측면을반영하였다고생각된다. 기존의국내연구들을살펴보면, 의과대학생들의심리상태나 36) 방어기제에대한연구 32)37) 가주를이루는데, 이러한결과가의과대학을졸업하고전공의시기를보내는과정에서심리적적응기간을거치면서정서적불안정성및미성숙한방어기제가감소하는변화양상등이보고된바가있다. 37) 이러한연구들은의과대학생의정서, 진로선택등에대한생물학적영향에대한결과를제시하고있지는않아, 본연구결과가보완적인역할을할수있을것으로생각된다. 본연구는국내의과대학학생들을대상으로기질및성격특성이전공선택에미치는영향을처음으로보고하였다는점에서의의가있다. 임상현장경험전에학생들이기질및성격특성이진로를결정할때에미치는영향은의과대학학생의진로교육에유용한참고자료가될수있을것이다. 그러나본연구는설문연구로서참여자가응답을과장하거 나거짓응답또는무성의한응답을했을가능성을배제할수 없었다. 또한관심전공선택에서지원계를선택한학생이매우 적었고, 외과계보다내과계를선택한학생의수가 2 배이상많 아결과해석의한계가있었다. 본연구는한곳의의과대학학 생만을대상으로하였기때문에국내전체의과대학학생을대 표하지못할가능성이있다. 또한본과 3 학년학생만을대상으 로하였기때문에, 졸업후실제전공선택시관심전공이바뀌 게될가능성이높다. 따라서추후여러의과대학학생들을대 상으로하여, 관심전공및실제전공선택에서기질및성격특 성이미치는영향을조사하는후속연구가요구된다. 또한전공 분류방법에서국외연구의분류방식은국내연구적용에한 계점이있어, 대한병원협회에서통용되는국내기준을사용하 였는데, 이기준에서제시하는내과계, 외과계, 지원계가각각의 집단의특성을대표하는지에대한추가적요인분석연구가필 요할것이다. 중심단어 : 기질및성격척도 내과전공 외과전공 의과대 학생 기질 성격.
