Three isotypes of the B. fragilis toxin (BFT) gene have been identified: bft-1, bft-2, and bft-3. We investigated the presence of bft isotypes in clinical B. fragilis isolates and the antimicrobial resistance of BFT-negative and BFT-positive isolates. Overall, 537 B. fragilis isolates were collected from extraintestinal specimens over 8 years ( ) from a university hospital in Korea. Samples were analyzed by multiplex PCR to identify the bft gene isotypes. Additionally, the antimicrobial susceptibility of 107 B. fragilis isolates (74 BFT-negative and 33 BFT-positive) was examined by the CLSI agar dilution method. PCR revealed a total bft gene detection rate of 30%, while 33% and 29% of blood and other extraintestinal isolates contained the gene, respectively. Among ETBF isolates, the most common isotype was bft-1 gene, followed by bft-2 and bft-3 (bft-1 77%, bft-2 14%, bft-3 9%). Resistance rates (%) for BFT-negative and positive isolates differed in response to various antimicrobial agents, with 3%, 5%, 1% and 38% of BFT-negative isolates and 3%, 6%, 3% an 42% of BFT-positive isolates being resistant to piperacillin-tazobactam, cefoxitin, imipenem, and clindamycin, respectively. Interestingly, neither BFT-negative nor positive isolates showed antimicrobial resistance to chloramphenicol and metronidazole. Overall, the proportion of ETBF from blood was similar to that of other extraintestinal sites and the bft-1 gene was the predominant isotype. Higher antimicrobial resistance rates were found in BFT-positive isolates than BFT-negative isolates, but these differences were not statistically significant. Bacteroides 균속에는 20종이상의균종이있고, 이중에서 B. fragilis, B. thetaiotaomicron 및 B. ovatus는사람에서주로감염을일으킨다 [1,2]. B. fragilis 중에는장독소 (Bacteroides fragilis enterotoxin, BFT) 를생성하는세균 (enterotoxigenic Bacteroides fragilis, ETBF) 이있는데, 이들은 Myers 등 [3] 에의하여설사질환환자의변에서처음분리되었다. ETBF는염증성대장질환 (inflammatory bowel disease, IBD) 환자의변에서정상인보다검출빈도가높고 [4], 장암환자의변에서는더높은빈도로검출됨이보고되었다 [5]. 또한 Wu 등 [6] 은동물실험모델에서 ETBF가대장염을일으키고만성염증이지속되면결국대장암으로발전할수있다는가능성을제시한바있다. 지금까지 BFT는 bft-1와 bft-2의두개의유전자아형이보고되었으나, Chung 등 [7] 이혈액배양에서분리한 ETBF에서새로운유전자아형 (Korean BFT, bft-k) 을처음보고한이후 Kato 등 [8] 은일본환자변에서분리한 ETBF에서동일한유전자아형을보고하여 bft-3으로명명하였다. BFT 유전자아형중에서 bft-1은성인의변검체에서분리한 ETBF 중 65% 로가장흔하였고, bft-2는소아의변에서 bft-1 만큼흔하게분리된다고알려진바있다 [9]. bft-3 은국내임상검체에서분리된 ETBF 중 32% 를차지하는것으로알려져있다 [7]. BFT의아형은전세계적으로분포되어있지만, bft-3 아형을가지고있는 ETBF는주로한국, 일본및베트남환자에서보고되었다 [10]. 본연구에서는국내대학병원의임상검체에서분리된 B. fragilis 균주를대상으로 multiplex PCR법을이용하여임상검체에따른 bft 유전자검출, 아형의분포와 BFT 생성유무에따른항균제내성의차이를알아보고자하였다. 2. Multiplex PCR을이용한 bft 유전자아형검출 -70 o C에냉동보관된 B. fragilis 균주를혈액한천에무산소성계대배양하여순수집락을얻었다. 집락 2 3 개를증류수 100 L에넣고잘혼합하여 10분간끓인후, 13,000 rpm 5분간원심분리하여 DNA를추출하였다. Multiplex PCR은 Avila-Campos 등 [11] 이고안한 primer를사용하였다. Common forward primer sequence는 5 -GAA CCT AAA ACG GTA TAT GT-3 ), reverse primer는 bft-1: 5 -ATT GAA CCA GGA CAT CCC T-3, bft-2: 5 -CGC TCG GGC AAC TAT-3, bft-3: 5 -CGT GTT CCA TAA CCC CA-3 이었다. PCR 반응액은 PreMix-Top (Bioneer, Daejun, Korea) 에 DNA 추출액 1 L, 각각의 primer 1 L (2O pmol) 씩, 증류수 15 L를넣어제조하였다. Multiplex PCR은 94 o C에서 5분반응후 94 o C에서 5분, 52 o C에서 30초, 72 o C에서 30초반응을 30회반복, 72 o C에서 5분간최종연장반응을시행하였다. 증폭된 PCR 산물 10 L에 loading dye 7 L를넣고혼합한다음 ethidium bromide를포함한 1% agarose gel에서 20분간전기영동하여 bft-1 (310 bp), bft-2 (170 bp), 및 bft-3 (400 bp) 밴드를확인하였다 (Fig. 1). 3. 항균제감수성시험 B. fragilis의항균제감수성은 CLSI 한천희석법 [12,13] 으로시험하였다. 배지는 5 g/ml hemin, 1 g/ml vitamin K 1 과 5% laked sheep blood를첨가한 Brucella agar (Becton Dickinson, Cockeysville, MD, USA) 를사용하였고, 시험항균제로는 piperacillin 및 tazobactam (Yuhan, Seoul, Korea), cefoxitin 재료및방법 1. 대상균주 2006년 2013 년에국내의한대학병원환자의임상검체즉, 혈액, 체액및농양검체에서분리된 B. fragilis를대상으로하였다. 세균동정은전통적방법을비롯해 ATB 32A 키트 (biomérieux, Marcy I Etoile, France), ANC 카드 (biomérieux, Hazelwood, MO, USA), VITEK II 자동미생물동정기기 (biomérieux, Hazelwood, MO, USA) 또는 VITEK MS 자동미생물동정기기 (biomérieux) 를이용하였다. 3 84 Myungsook Kim, et al. bft Gene Isotypes of Enterotoxigenic Bacteroides fragilis (Merk Sharp & Dohme, West Point, PA, USA), cefotetan (Daiichi Pharmaceutical, Tokyo, Japan), clindamycin (Korea Upjohn, Seoul, Korea), imipenem 및 metronidazole (Choongwae, Seoul, Korea), chloramphenicol (Chongkundang, Seoul, Korea), meropenem (Sumitomo, Tokyo, Japan), moxifloxacin (Bayer Korea, seoul, Korea), 그리고 tigecycline (Wyeth Research, Pearl River, NY, USA) 을사용하였다. Piperacillin-tazobactam 합성은 piperacillin 각각의농도에 tazobactam 4 g/ml을동일하게첨가하였다. 혈액한천에계대배양된균주를 thioglycollate broth에풀어 McFarland 0.5관탁도에맞춘후 steers replicator (Craft Machine Inc., Woodline, PA, USA) 를사용하여 10 5 CFU가되도록배지에접종한후 anaerobic chamber (Forma Scientific, Marietta, OH, USA) 에넣어 36 o C에서 48시간배양하였다. 정도관리를위해서 B. fragilis ATCC 25285와 B. thetaiotaomicron ATCC 29741를동시에시험하였다. 최소억제농도는증식을억제한제일낮은농도로결정하였고, 여러개의정상집락이나육안으로매우엷게보이는증식은무시하였다. Tigecycline 에대한결과판독은미국 FDA 권장기준, 즉감수성은 4, 내성은 16 g/ml를적용하였다. 결과 1. ETBF의 bft 유전자아형들의분포 B. fragilis 537주중 162주 (30%) 에서 bft 유전자아형들이검출되었고, 이중혈액배양분리주에서는 48주 (33%), 그외장외검체분리주에서는 114주 (29%) 가검출되었다. bft 유전자아형들의분포는 bft-1이 125주 (77%) 로가장많았고, bft-2가 23주 (14%), bft-3가 14주 (9%) 순이었다 (Table 1). 연도별 BFT 양성은 2009년이 22% 로가장낮았고, 2012년이 35% 로가장높았다. 2006년부터 2013년사이 BFT 아형은 bft-1이 70 87% 로가장많았고, bft-2는 3 32%, bft-3은 4 13% 이었으며, bft-2는 2007년에, bft-3은 2009년에검출되지않았다. BFT 아형들의분포는 2007년과 2009년을제외한 년에는 bft-1 다음으로 bft-2, bft-3순이었으나, 2013년에는 bft-3, bft-2 순으로분포가바뀌었다. 연도별 BFT 양성율과 2007년과 2009년을제외한 bft 아형들의분포는통계학적으로유의한차이는없었다 (Table 2). 2. Sears 등 [10] 은 ETBF의분리가혈액분리주중에서는 23.4% 이었고, 장외검체에서는 15.3% 로통계학적으로유의한차이가있었다고보고하였다. 본연구에서는혈액분리주에서의 ETBF 검출율이장외검체분리주보다높았지만통계학적의미는없었다. BFT의유전자아형은 bft-1, bft-2 및 bft-3가있다. 이중 bft-1은임상검체에서분리된 ETBF에서가장흔한아형으로여러나라에서보고되었다 [9,11,16]. 즉, 터키에서는 91% (49/51) 로가장높았고, 그다음으로미국에서는 68%, 이탈리아에서는 66% 이었다. 본연구에서도 bft-1이 77% 로터키와는차이가있었지만다른나라와비슷하였고, 가장우세한아형은다른연구들과일치하였다. bft-3는한국환자에서처음분리되었고, 그비율도 32% 로높 았다 [7]. 한편일본에서는 12%[8], 베트남에서는 14%[17] 이었다. 흥미롭게도 Kato 등 [8] 은 bft-3가소의변에서분리된 ETBF의 12.5% 를차지한다고보고하였다. 반면에미국에서는 7%[11] 이었고, 이탈리아에서는 3%[9] 로매우낮았다. 따라서 bft-3은지역적으로동남아시아에서더흔하게분리되었다. 본연구에서 bft-3의분리율은 9% 로일본, 베트남, 한국에서의이전연구에비해서낮았고미국과이탈리아에비해서는약간높았다. Chung 등 [7] 의보고에의하면 1995년부터 1997년사이에국내한대학병원의임상검체에서분리된 B. fragilis 중 ETBF는 34% 이었고, bft-1은 41%, bft-2는 27%, bft-3은 32% 이었다. 본연구에서는 2006년부터 2013년사이에분리된 B. fragilis 중 ETBF 비율은 30% (22 35%) 이었고, bft-1은 77% (70 87%), bft-2는 14% (3 32%), bft-3은 9% (4 13%) 로연도에따라달랐다. ETBF 분리율은 년과비슷하였지만, bft-1은크게증가하였고, 반면에 bft-2와 bft-3는매우감소하였다. 특히 bft-3는 90년대에는 bft-2 보다훨씬많이검출되었지만본연구에서는가장적게검출되어서 bft 유전자아형들의분포는 Chung 등의보고와차이가있었다. BFT 아형들의분포는검출시기에따른차이는보고된바가없어서추후연도별추이와함께역학적연구가필요하다고사료된다. 임상검체에서분리된 B. fragilis에대한항균제감수성에관한연구는여러나라에서보고되었지만 [18-21], ETBF의항균제감수성에관한연구는매우적다 [22,23]. 이들연구는설사질환을가진환자에서분리된 ETBF가대상이었는데, 방글라데시에서는 [22] clindamycin 및 metronidazole 에대하여 10%, 7% 가내성인반면에인도에서는 [23] 48%, 41% 가내성으로지역적인차이가매우크게나타났다. 본연구에서 ETBF의 clindamycin에대한내성율은

5 86 Myungsook Kim, et al. bft Gene Isotypes of Enterotoxigenic Bacteroides fragilis 42% 로인도와비슷하였지만, 방글라데시보다는매우높았는데이는 clindamycin 내성무산소성세균비율은지역에따라서차이가크다는 Boyanova 등 [24] 의보고와일치하였다. Metronidazole 또는 piperacillin-tazobactam은 B. fragilis을포함한 Bacteroides spp. 에의한감염치료를위한권장항균제이고, doripenem, ertapenem, imipenem, meropenem과 amoxicillin-clavulanate 는추가로권장되는항균제이다 [25]. 본연구에서 BFT 음성및양성인 B. fragilis는 metronidazole과 chloramphenicol에대하여모두감수성이었고, 권장항균제중 imipenem에대한내성율은 BFT 양성균주가음성균주보다다소높았지만 meropenem은반대로 BFT 음성균주가양성균주보다높았다. Piperacillin, cefoxitin, tigecycline 에대한내성율은양성균주가음성균주에비해높았지만 moxifloxacin 에대한내성율은낮았다. BFT 생성유무에따라항균제내성양상은달랐으나통계학적으로유의한차이는없었다. 다만, BFT 음성및양성 B. fragilis의항균제감수성을비교한보고는매우드물다는점에서의의를두겠다. 본연구에서는국내대학병원의임상검체에서분리된 B. fragilis에서의 BFT 생성및 bft 유전자아형들의분포를조사하였고, BFT 음성및양성균주의항균제내성양상을검토하였다. 분리된균주중 ETBF는 30% 이었고, 이중 bft-1이가장많았으며, BFT 양성균주의항균제내성율이다소높긴하였지만, 통계학적으로유의한차이는없었다. 향후임상검체에서분리된 B. fragilis의 BFT 검출및 bft 유전자아형에관한역학적연구는 B. fragilis 감염과 BFT의연관성을규명하는데유용한자료를제공하리라기대한다. 요약 Enterotoxigenic Bacteroides fragilis (ETBF) 는병독소인자로알려진장독소를생성하는균종이다. B. fragilis enterotoxin (BFT) 은 bft-1, bft-2 및 bft-3 세개의유전자아형들이밝혀졌다. 본연구에서는임상검체에서분리된 B. fragilis에서 bft 유전자의유무와 BFT 음성및양성균주의항균제내성을조사하였다. 국내의한대학병원에서 8년간 ( 년 ) 장외검체에서분리된 B. fragilis는총 537주이었다. 다중중합연쇄반응으로시험하여 bft 유전자아형을확인하였다. BFT 음성 74주와양성 33주를포함한 B. fragilis 107주의항균제감수성은 CLSI 한천희석법으로시험하였다. 임상검체에서분리된 B. fragilis의 bft 유전자검출율은 30% 이었고, 이중혈액과혈액외장외검체분리주에서는각각 33%, 29% 이었다. ETBF 중에서가장흔한아형은 bft-1 이었고, 그다음은 bft-2, bft-3 순이었다 (bft-1: 77%, bft-2: 14%, bft-3: 9%). BFT-음성과양성균주의내성률은일부항균제에대해서차이가있었다 (BFT-음성균주 : piperacillin-tazobactam 3%, cefoxitin 5%, imipenem 1%, clindamycin 38%; BFT-양성균주 : piperacillin-tazobactam 3%, cefoxitin 6%, imipenem 3%, clindamycin 42%). BFT-음성및 BFT-양성균주모두는 chloramphenicol 과 metronidazole 에대한내성은없었다. 결론적으로, 혈액분리주에서의 ETBF 검출율은혈액외장외검체분리주에서와비슷하였고, 가장흔한유전자아형은 bft-1 이었다. 항균제내성은 BFT 양성균주가음성균주보다대체로높았으나통계학적으로유의한차이는없었다.
