The genus Nocardia contains more than 70 species. Nocardia takedensis has been recently reported as a new species of the genus Nocardia. In this study, we describes the first clinical isolate of N. takedensis from the skin lesion in Busan, Korea. For the identification of clinical isolate to the species level as N. takedensis, classical methods (colony morphology, biochemical characteristics, and antimicrobial susceptibility), molecular method (16S rrna gene sequencing), and MS (mass spectrometry) analysis were conducted. Clinical isolates grew slowly on the culture media (5% sheep blood agar and chocolate agar) under 5% CO 2 condition. Especially, carotene pigmentation was detected well on the media. Using mass spectrometry, Nocardia isolate was not identified to the species level. However, molecular method based on 16S rrna sequencing confirmed the isolate as N. takedensis correctly. N. takedensis isolate was partial positive for acid-fast bacilli on the Ziehl-Neelsen method. And it was observed to be resistance to amoxicillin/clavulanic acid and ciprofloxacin. Our results provide useful information to develop optimal identification protocol of N. takedensis in clinical diagnostic laboratories. Key Words: Colony morphology, White aerial mycelium, Filament shape, Partial AFB positive INTRODUCTION Nocardia 균종은물과토양등환경에주로존재하는그람양성산소성방선균으로전세계적으로현재 88종이알려져있고, 그중 46종이사람으로부터분리되며, 나머지는거의동물병원성이다 (Versalovic et al., 2011). 특히, 면역력이저하된환자나폐질환을가지고있는환자의폐와피부를통해감염되어노카르디아증 (Nocardiosis) 을일으킨다 (Lerner, 1996; Brown-Elliott et al., 2006). 주된균종으로 Nocardia asteroides, N. brasiliensis, N. otitidiscaviarum 등이있으며, 임상적으로가장흔하게분리되는균종은 N. asteroides이고, 일차성피부감염의가장흔한균주는 N. brasiliensis로알려져있다 (Kiska et al., 2002; Roth et al., 2003). 노카르디아증의진단은일차적으로균주의동정을통해이루어지고, 분자유전학적기법인염기서열분석을통해확진될수있다 (Wellinghausen et al., 2002; Nolte and Caliendo, 2007). Nocardia takedensis( 이하 N. takedensis) 는 2005년일본에서최초로분리동정된이후, 스페인에서도 2009년에분리되었다 (Betran et al., 2009). 2 락성상및생화학적특징을참조하여동정하는것이일반적이다. 최근에는분자생물학적방법 ( 염기서열분석법등 ) 을이용하여종수준까지확진하고있다 (Kiska et al., 2002; Saubolle and Sussland, 2003). Nocardia 균종들은느리게발육하는균이므로상처나농양, 호흡기분비물에서채취한검체를배양하는경우통상적인배양시간인 48~72시간내에서는발육이좋지못하다. 따라서, 노카르디아증이의심될경우에는좀더오랜시간동안배지를관찰할필요가있다 (Saubolle and Sussland, 2003; Brown-Elliott et al., 2006). 본연구에서는국내최초로환자의피부병소로부터 N. takedensis를분리, 동정한경험을보고함으로써, 임상미생물검사실에서 N. takedensis를분리동정하는데필요한실질적인기초자료를제시하고자한다. MATERIALS AND METHODS 검체및실험균주부산지역모병원에입원중인환자의팔주위피부병소에서, 3일간격으로 1, 2회차배양의뢰된각검체로부터균을분리하였고, 16S ribosomal RNA( 이하 rrna) gene 을이용하여 N. takedensis로최종동정된균주 (Genbank of the National Center for Biotechnology Information strain WE30 : 99.56% 일치 ) 를본연구의실험균주로사용하였다. 배양배지의선택과집락의관찰실험에는 5% sheep blood agar plate( 이하 5% SBAP), chocolate agar, thioglycollate broth, 3% Ogawa 배지, Mac- Conkey agar 배지를사용하였고, 35, 5% CO 2 조건하에서배양한후, 시간에따라최대 5주간발육하는집락의형태학적성상을관찰하였다. 염색및현미경적관찰염색시약은 Gram stain kit( 영동제약, Korea) 와 Acid-fast bacilli (AFB) stain kit(ziehl-neelsen, 영동제약, Korea) 를사용하였다. 모든염색은제공된시약업체가추천하는표준법에의거해수행하였다. 현미경검경을통해균의염색성과형태학적특징을관찰하였으며, 배지에서자란집락과 thioglycollate broth에서자란균을도말하여, 각각의형태학적특징들을비교관찰하였다. 동정을위한생화학적시험생화학적시험을위한 Casein, L-Tyrosin (Sigma, Germany), Xanthine (2.6-Dhydroxypurine, Sigma, Germany), Citrate 시험은직접배지를제조하여검사를시행하였고, Urease 검사는 API 20E kit (biomerieux, France) 를사용하였다. 또한, 카탈라아제시험 (Catalase test) 과 MIO (Motility, indole, ornithine) 배지를이용한운동성시험, 옥시다아제시험 (Oxidase test) 등을시행하였다. 동정을위한분자생물학적시험 Norcardia spp. 를종 (species) 수준까지확진하기위해 16S rrna gene을이용하여, 염기서열분석 (sequencing) 을실시하였다. 5% SBAP, chocolate agar에서배양, 분리된순수한집락으로부터추출한 genomic DNA를 template DNA 로사용하였으며, 16S rrna gene을타겟으로하여중합효소연쇄반응 (polymerase chain reaction, PCR) 을통해얻어진증폭산물을염기서열분석을하였다. 동정을위한질량분석 (Mass spectrometry) 시험 VITEK MS (biomerieux, France: Matrix Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry) system 을이용하였다. 방법의일반적인검사원리는다음과같다. 유기화합물 (matrix) 과분석하고자하는균주의혼합용액 1 μl를 sample plate에서말려서 co-crystal을만든후에탈착 / 이온화시킨다. 이이온들을전기장으로가속시킨후진공상태에서 1 m 거리의 flight tube를비행하여검출기에도달하도록한다. 후에각각의이온들이검출기에도달하는데걸린시간을분자량으로환산하여분석하고자하는균주를동정해낸다. 이번실험에서는 VITEK MS - Mycobacteria / Nocardia spp. kit를이용하여제조사의지침에준하여검사를수행하였다. 항균제감수성시험항균제감수성검사는 Broth Microdilution Method Kit (Thermo Scientific Sensititre Rapid and Slow growing mycobacterium MIC plates, USA) 와디스크확산법 (disk diffusion test) 을사용하여결과를비교하였다. Broth microdilution method를활용한 MIC (minimum inhibitory concentration) 의측정방법은아래와같이수행하였다. 5% SBAP에서 5일정도배양한 N. takedensis 균주를멸균증류수에풀어 0.5 MacFarland 농도를맞춘후, 잘혼합하여 1~2분방치한다

그리고 Sensitire panel를 이용 Broth microdilution 덮어 버리는 현상도 확인할 수 있었다(Fig. 1).

Colonial characteristics of N. takedensis according to the culture media. *5% sheep blood agar plate, N. takedensisa: clinical isolates 그 후, 균액을 CAMHBT (Sensititre cation adjusted Mueller- 고 4~5주 후에는 집락 가장자리에 하얀색의 공기균사들 Hinton broth with TES buffer)에 50 μl를 분주하여 가볍게 이 집락 주변을 감싸며 뻗어나가고, 둥근 모양의 집락을 혼합한다. 그리고 Sensitire panel를 이용 Broth microdilution 덮어 버리는 현상도 확인할 수 있었다(Fig. 1). method Kit에 각각 100 μl씩 분주한 후, kit well을 전용 필 Thioglycollate broth에서는 카로틴 색소의 아주 작은 모 름으로 덮고 35, 산소환경에서 3일간 배양 후, 결과를 양의 구슬알갱이 균체가 액체배지 상층에 떠 있는 것과 확인하였다. 디스크 확산법은 5일간 배양한 N. takedensis 기벽에 붙어서 배양되는 것을 볼 수 있었다. 배양 시간을 균주를 멸균 증류수에 균을 풀어 0.5 MacFarland 농도를 연장하면 선명한 카로틴 색소의 많은 균체들이 기벽에 붙 맞춘 후, 5% SBAP와 Muller Hinton agar 배지에 접종하여 어서 자라는 것을 볼 수 있었다. 3% Ogawa 배지에서는 디스크를 놓고 35, 산소환경에서 3일, 5일 배양 후, 결 균 계대 배양 3일이 지난 후, 집락 형태가 관찰되었고, 과를 확인하였다. 1주일 추가 배양 시, 둥근 모양의 짙은 카로틴 색의 집락 들을 확인할 수 있었다. 이러한 집락 특징들은 카로틴 색 RESULTS 소를 띄는 NTM (Non-tuberculous mycobacteria) 균종들과 유사한 형태를 보였다(Fig. 2). Chloramphenicol를 첨가한 배지에서 집락의 형태와 성상 SDA (Sabouraud dextrose agar) 배지에서는 발육이 거의 되 5% SBAP, chocolate agar, thioglycollate broth에 균을 접종 지 않았다. 그러나 chloramphenicol을 첨가하지 않은 배지 하고 35, CO2 조건하에서 3~4일 배양한 결과, 0.2~0.3 에서는 노란색의 집락이 느리게 발육하는 것을 확인할 수 mm 정도의 아주 작은 집락을 확인하였으나, 색소는 육안 있었고, MacConkey agar에서는 발육이 확인되지 않았다. 으로 관찰할 수 없었다. 5일 추가 배양 후에는 카로틴 색 소를 띈(Fig. 3E) 둥근 모양의 작은 집락을 확인할 수 있 염색을 통한 N. takedensis 의 형태학적 특징 그람염색과 AFB 염색은 5% SBAP에서 배양된 순수 집 었다. 배양 2주 후에는 배지를 파고드는 듯한 건조한 느 낌의 둥근 집락과 함께 그 주변으로 약하게 생성된 공기 락을 선택 도말한 슬라이드와 thioglycollate broth에서 배양 균사를 확인할 수 있었고, 배양 3주 후에는 집락 주변으 된 균주를 도말한 슬라이드를 염색한 후, 현미경으로 검 로 뻗어나가는 하얀색의 공기균사를 볼 수 있었다. 그리 경하여 비교 관찰하였다. 그 결과, 모두 긴 형태의 그람양

4), Ziehl-Neelsen 운동성의 유무와 인돌 생성능을 확인한 검사에서는 모두 법을 이용한 AFB 염색상에서는 부분적으로(partially) acid- 음성 결과가 도출되었다(Fig. 3A, 3B, and 3C).

TSI (Triple fast bacilli를 확인할 수 있었다(Fig. 4). sugar iron)와 citrate 검사에서는 결과가 모두 음성, urease 검사에서는 양성 결과가 도출되었다(Fig. 3C and 3D). 생화학적 시험 Casein, tyrosine, xanthine 가수분해능을 확인한 실험에 5% SBAP에서 자란 순수 집락으로 카탈라아제 시험과 서는 모두 음성 결과를 보였다. 대조군으로 Streptomyces 옥시다아제 시험을 수행하였고, 그 결과 카탈라아제 양성, spp.(임상 균주)와 N. farcinica (The Korean Association of 옥시다아제 음성의 결과를 보였다. 또한, MIO 배지에서 Quality assurance for Clinical Laboratory sample: MI113)을 사

6 용하였다 (Fig. 5(A)-a, (B)-a, and (C)-a), (Table 1). 질량분석기 (Mass Spectrometry) 를이용한균동정 VITEK MS (biomerieux, France: Matrix Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry) system 을이용하였다. (MS1-3 T ) Catalase reaction - a - Oxidase reaction - - Casein hydrolysis - - Tyrosine hydrolysis - - Xanthine hydrolysis - - Urease product + b + Motility - - Citrate utilization a : negative, + b : positive 지는명확히동정되지않았으며, 이는아마도 VITEK MS - system data base에 N. takedensis에대한데이터가없기때문으로추정된다. 분자생물학적방법을이용한균동정 Norcardia spp. 를종 (species) 수준까지확진하기위해 16S rrna gene을이용하여, 염기서열분석 (sequencing) 을실시하였다. 염기서열을 GenBank sqeuence database의 BLAST Similarity searching program으로비교하였을때 GenBank accession number: WE30 N. takedensis와 688 bp 부분이 99.56% 가일치하여, N. takedensis 로최종동정되었다. 항균제감수성검사 CLSI (Clinical and Laboratory Standard lnstitute) guideline (2007) 에따라서항균제감수성결과를확인한결과, broth microdilution method에서는 amoxicillin/clavulanic acid, ciprofloxacin은내성, clarithromycin, imipenem은중간내성을보였고, 나머지약제에대해서는모두감수성결과를보였다. 디스크확산검사는 5% SBAP와 Mueller Hinton agar 에서함께실험을수행하였으나, Mueller Hinton agar에서는균의발육이불량하여결과를최종판독하지못하였다 (Table 2). 7 DISCUSSION N. takedensis는 Yamamura 등에의해일본 Yamanashi 현의 Takeda 신사 (Shrine) 의성곽둘레호 (moat surrounding) 에서 2003년처음으로분리된이후로, 2005년에 N. takedensis 로명명되었다. 이후스페인에서도 2009년에분리되었다 (Yamamura et al., 2005; Betran et al., 2009). N. takedensis는일반적으로산소성방선균으로운동성이없고, 카탈라제반응에서양성이며, AFB 염색에서부분적으로항산성을보이는등의특성이있는그람양성막대균으로알려져있다 (Yamamura et al., 2005). 본연구에서는한국에서처음으로환자의팔주변병소의검체를배양하여 N. takedensis를분리하였고, 16S rrna sequencing 기법을이용, 최종동정하였다. 본증례를통해 N. takedensis 균의형태, 집락의성상, 생화학적특성, 항균제감수성패턴등을보고하고, 향후임상미생물검사실에서 N. takedensis 균을분리동정할때, 도움이될수있는자료들을제시하고자하였다. N. takedensis는다른 Nocardia 균종들처럼아주느리게성장하기때문에환자의검체에서직접배양시에는최소 3~5일정도배양해야육안적으로집락의형태를볼수있었다. 집락은 N. brasiliensis와비슷하나카로틴색소 ( 주황색 ) 가더선명하고크기가 N. brasiliensis에비해매우작고, 배지를파고드는듯한거친과립상의특징을보였다. 2주동안의추가배양후에는배지주변으로뻗어나는하얀색의공기균사를볼수가있었고, 3~4주동안의추가배양을수행한결과, 하얀색의공기균사가집락표면을덮어버리는것이특징으로나타났다. 일반적으로노카르디아증을유발하는주된균종으로는 N. asteroides, N. brasiliensis, N. otitidiscaviarum 등이며임상적으로가장자주분리되는균종은 N. asteroides이고, 일차성피부감염의가장흔한균주는 N. brasiliensis로알려져있다. 집락은균종에따라백색 (N. brasiliensis, N. elegans, N. iowenis 등 ), 회색, 황토색 (N. farcinica 등 ), 주황색등의다양한색소를생성하고, 공기균사 (aerial hyphae) 를만들기도하며집락표면이대체로건조하며과립상을보이기도한다 (Berd, 1973; Brown-Elliott et al., 2006). 또한, N. takedensis는기존에산소성이라고알려져있었지만 (Yamamura et al., 2005; Betran et al., 2009), 실험에사용된 N. takedensis는처음의뢰된검체부터 CO 2 환경에서배양되었으며, N. takedensis로최종동정후에여러실험을 통해저자가비교배양한결과, CO 2 환경하에서도무리없이발육이잘되었다. 특히, thioglycollate broth 표면에균들이뭉쳐서발육하고시간이흐르면서기벽으로공기균사가뻗어올라가는것을볼수있었고, 카로틴색소형성도잘나타났다. 그러나기존의연구들에서 N. takedensis 배양시 CO 2 환경하에서발육하는지에대한증례나연구보고가없어명확한검증은할수가없었다. 그람염색상에서는 5% SBAP 얻어진순수집락을도말한슬라이드보다 thioglycollate broth에서배양된균주를이용한슬라이드에서 N. takedensis의가지친긴형태의그람양성막대균모양이더선명하게확인되었다 (Fig. 4). AFB 염색에서는그람염색상에서보인가지친형태의그람양성막대균이아닌짧은막대균이뭉쳐있는것을확인할수있었고 (Fig. 4), 뿐만아니라기존논문에기록된 Nocardia 균종들과는달리 Ziehl-Neelsen 법을이용한 AFB 염색에서, 부분적으로 (partially) acid fast bacilli를확인할수있었다 (Fig. 4) (Brown-Elliott et al., 2006; Shin et al., 2009; Kang et al., 2011). Casein, tyrosine, xanthine 등의분해능을보기위한실험에서는상품화된배지 (Nocardia ID Quad plate) 의국내반입이쉽지않아, 직접배지를제조하여실험을진행하였다. 이러한기질들을이용하는균들은균주위집락주변에투명한후광 (halo) 이나타났으며 (Fig. 5A-c and C-c), melanin 생성은 tyrosine, xanthine 배지에서볼수있었고 (Fig. 5A-b and B-b). 분리된 N. takedensis 는 casein, tyrosine, xanthine 기질중어떤것도분해하지않았고, 멜라닌색소생성도확인할수없었다 (Fig. 5A-a, B-a and C-a). 항균제감수성검사는 Sensitire panel를이용한 broth microdilution method를이용하여검사를시행하였다. Clinical and Laboratory Standard lnstitute (CLSI) guideline 에따라감수성을확인한결과, amoxicillin/clavulanic acid, ciprofloxacin은내성 clarithromycin, imipenem은중간내성을보였고나머지약제에는모두감수성을보였다. Mueller-Hinton agar와 5% SBAP를사용하여디스크확산검사도함께수행해보았으나일부논문에나와있는 Nocardia 균종의약제감수성결과와는 (Park et al., 2008), 달리 N. takedensis 는 Mueller-Hilton agar에서 4일을배양하여도균발육이확인되지않았다. 5% SBAP에서는 48시간배양후에결과판독이가능하였지만, 디스크확산검사법의결과판독기준이없어서감수성결과를정확하게예측하여비교할수없었다. 다만항균제디스크 zone 크기와비교해보면 broth microdilution method와대체로유사한결과를

8 확인할수있었다 (Table 2). 본연구에서는한국에서임상환자로부터 N. takedensis의분리경험을통해배양배지에서집락특징과배양시간에따른배지에서의형태적특성, 배지에따른염색에서균의형태, 생화학적시험의특징, 약제감수성검사등임상미생물검사실에서이세균을분리동정할때도움이될수있는여러가지기초자료들을정리하였다. 이러한기초적인정보들은추후임상에서분리동정될수있는 N. takedensis 균주에대한진단및치료에있어많은도움이되리라기대한다.
