Elafin Expression in Oral Lichen Planus Sang Shin Lee and Suk Keun Lee Department of Oral Pathology, College of Dentistry, Kangnung National University, Gangneung, Korea Background : Elafin is a potent anti-elastase in human saliva, and is supposed to play a role in preventing oral ulceration. The expression of elafin was observed in the oral lichen planus (OLP), one of the most common noninfectious oral mucosal diseases, which frequently manifests as extensive ulceration on the involved oral mucosa. Methods : 50 OLP, 10 oral leukoplakia, 3 inflammatory oral ulcers, and 3 normal oral mucosa cases were fixed with 10% buffered formalin, and immunohistochemically stained with monoclonal elafin antibody. Representative specimens were fixed with 4% paraformaldehyde, and RNA in situ hybridization, with an elafin RNA probe, was performed. Results : With both the immunohistochemistry and RNA in situ hybridization the expression of elafin was more decreased in the OLP compared to the normal mucosa, while in the hyperplastic epithelium of the leukoplakia and inflammatory ulcers the expressions of elafin was more intense. In the thin epithelia of the reticular and atrophic OLPs the expressions of elafin were reduced compared to the normal mucosa, and became almost negative in the epithelium of the erosive OLP. Conclusions : These data suggested that the extensive ulceration of the OLP was closely relevant to the reduced expression of elafin in the involved epithelium. Key Words : Elafin-Lichen Planus-Immunohistochemistry-In situ hybridization 구강편평태선 (oral lichen planus) 은구강점막에서비교적흔하게비감염성궤양을일으키지만아직도그원인이분명하게밝혀지지않은질환이다. 1-6 이질환은구강점막궤양이생기지않는그물망형태 (reticular type) 가가장많이나타나고증상이심해지면통증과저작장애등이일어나며궤양을수반하는위축성형태 (atrophic type) 나침식성형태 (erosive type) 로나타나기도한다. 4,7,8 구강편평태선은피부와점막에있는항원에대한세포매개면역적반응을보이는것으로보고되어있는데, 9,10 치료법으로는 retinoic acid, griseofulvin이나 cyclosporine 등과같은화학요법과 8-methoxypsoralen이나장파의자외선을이용하는광화학요법 11 이사용되고있으며최근에는병소의표면에스테로이드를도포하여증상을완화시키는방법들이보고되었다. 7 또한이질환이유전성질환은아니지만, 만성적이며세포매개면역과밀접한관련이있고질환의진행과정에서다수의림프구가출현하며, 4,12 스테로이드치료에민감하게반응하여심한증상이완화되는경향을보인다. 7 구강편평태선을자가면역질환의일종으로생각하기도하지만, 3,8 아직도편평태선에관련되는특이한항원또는항체가밝혀지지않고있다. 13 그리고자가면역질환에서흔하게나타나는다른형태의자가면역질환과의전이성이뚜렷하 게관찰되지않기때문에확실하게자가면역질환이라고구분하기도어렵다. 최근스트레스등과같은이유로구강편평태선의발병률이점차로증가하고있는추세이며, 폐경기이후의여성및중년기이후의남성에서높은발병분포를보이며대부분볼점막과치은에서의발생이보고되었다. 4 이와같이구강편평태선은퇴행성질환의형태로나타나기도하지만, 이물질, 구강내의보철물, 아말감, 금관등의금속과의과민반응으로나타나는경우도있어 현대문명의환경오염과도밀접한관련이있고, 당뇨병이나 C형간염, 18 간질환, 구강암과같은다른의학적상태에도연관되어있다고보고되었다. 4 Elafin 유전자는사람의염색체 20q 에위치하고있는유전자로, 크기는약 1.7 kb이고 3개의 exon으로나뉘어있다. 여기서생성되는 preproelafin은 25개의 signal peptide과 34개의 pro-sequence, 그리고 C-terminal에 57개의아미노산등으로모두 117개의아미노산으로구성된단백질이다. 19 분자량은약 9.9 kd인 epidermal serine proteinase inhibitor로 20,21 면역화학적방법으로염색하였을경우, 정상구강점막의각화된상피에서는약한발현양상을보이고가시세포층과과립세포층에서는더욱강 15

2 16 이상신 이석근 하게발현되며 22 타액선에서다량분비되므로구강내타액에서도풍부하게발견된다. 23 Elafin은건선이나창상치유, 세포배양과같이상피세포의증식이과도하게자극되는경우에도유도될수있으며 24 주로 suprabasal keratinocytes에서풍부하게발현된다. 25 이러한 elafin은사람의피부의각질층에서도분리할수있는데 leukocyte elastase와 pancreatic elastase에대하여강한억제제로작용하며, 26 호중성백혈구의 elastin을분해하는 proteinase 3에도억제작용을한다. 27 그러나 plasmin, 섬유소용해효소, trypsin, -chymotrypsin, cathepsin G에는억제작용이없다. 26 그리고기저세포암종이나편평세포암종과같은종양에서 elafin의발현양상을관찰함으로써편평상피세포의이상을조기에관찰할수있었다. 24 본연구는구강편평태선질병의진행양상가운데구강점막상피세포의변형과정을관찰하던중에, 구강점막상피를보호하는기능을하는 elafin의발현이현저하게감소되는것을발견하고구강편평태선의점막궤양과 elafin의관련성을알기위하여면역조직화학적및 RNA in situ hybridization 연구를수행하였다. 재료와방법병리학적소견으로구강편평태선으로진단되고임상적으로치료를받고계속관리되고있는강릉대학교치과병원에의뢰된 50예의조직을재료로하였다. 구강편평태선중에서 22예는남성으로평균연령은 56.9세였으며, 28예의여성평균연령은 51.8 세였다. 그리고다른양성점막질환들과비교하기위하여 10예의구강백반증, 3예의염증성, 3예의정상점막상피를함께이용하였다. 이들은모두통법에의하여 10% 중성포르말린에고정하여면역조직화학적염색을실행하였고, 4% 중성파라포름알데하이드에고정한후에 RNase 방지된과정으로파라핀블록을만들었다. 그리고 4 m 두께의파라핀절편을만들어 RNA in situ hybridization을실행하였다. 면역조직화학적염색면역조직화학염색은 DAKO LSABR kit (DAKO, Denmark) 를이용하여 streptoavidin-peroxidase 방법을사용하였다. 파라핀절편슬라이드를크실렌에서 10분씩 4회로파라핀을제거하고 100% 에탄올에서부터 90, 80, 70 및 60% 까지 3분씩수화시켰다. 그다음흐르는물에서에탄올이제거되면 PBS 용액 (137 mm NaCl, 2.7 mm KCl, 4.3 mm Na2HPO4 7H2O, 1.4 mm KH2PO4, ph 7.4) 으로수세한후, 메탄올로희석한 3% 과산화수소용액으로 6분간처리하여조직에내재되어있는과산화수소를불활성화시키고, 차단혈청을이용하여 30분간반응시켜비특이적항원-항체반응을극소화시켰다. 여분의혈청을 제거하고 elafin 1차항체 (Peptide Institute Inc. Osaka, Japan) 를 1:100으로 PBS에희석하여조직에반응시키고상온에서 1시간정도반응시켰다. 이때음성대조군에는 1차항원을가하지않고같은조건으로반응시켜대조군으로이용하였다. 바이오틴이결합된 2차항체로 30분간상온에서반응시킨후 PBS로 5분씩 3번수세하였다. 그리고 1:100으로희석된 streptavidin-peroxidase (DAKO, K681, Carpinteria, CA, USA) 로 30분간실온에서반응시키고 PBS로 5분씩 3번수세하였다. 그다음 DAKO LSABR kit (DAKO, Denmark) 로 3분간발색하고흐르는물에서수세한뒤 Mayer hematoxylin 용액으로 1분간배경염색을하고수세하여 70% 에탄올용액에서부터 80, 90 그리고 100% 의에탄올에서탈수하여크실렌을거쳐발삼으로봉입하였다. RNA in situ hybridization 신선한구강편평태선조직을 4% 중성파라포름알데하이드에고정시킨후 RNase 방지된방법으로파라핀블록을제작한다음 4 m 두께의현미경절편을 silanized 유리슬라이드에부착하여보관하였다. RNA in situ hybridization에사용되는 RNA probe를제작하기위하여 RNA in vitro transcription 반응을시켰다. 그리고 DNA template (6 L), 10 buffer (2 L), RNase inhibitor (1 L), NTP (DIG-UTP) mixture (1 L), T3 또는 T7 RNA polymerase (2 L) 와 DEPC H2O로 20 L 의용량을만들어혼합하여 37 에서 2시간동안반응시킨후포름아마이드용액 20 L를첨가하여 -20 에보관하였다. RNA probe의생산을확인하기위하여보관된반응물중에서 2 L를 1% 아가로우즈겔에전기영동하여띠를조사하였다. 파라핀절편슬라이드를크실렌에 20분간넣어파라핀을제거하였고 100% 에탄올에서부터 90, 80, 70% 에탄올까지 2분씩수화시킨후 DEPC H2O로혼합한 PBS 용액으로 5분간수세하였다. 단백질완충액 (1M tris-hcl 20 L, 0.5 M EDTA 4 L) 에서 proteinase K 용액 (0.01 g/ L) 을넣어실온에서 15분간반응시킨후, 4% 파라포름알데하이드용액으로실온에서 10분간재고정을시킨후 0.2N HCl 용액으로실온에서 10분동안반응시켜내재성 alkaline phosphatase를불활성화시켰다. DEPC H2O로처리한 PBS 용액으로 5분씩 2회수세한후 0.1 M triethanolamine (TEA) 용액으로 10분간반응시키고, 0.25% acetic acid 가포함된 0.1 M TEA 용액으로실온에서 10분간반응시켰으며, DEPC H2O로혼합한 PBS 로 5분씩 2회수세하였다. 50 로예열된 100 L의 hybridization 용액 (50% formamide, 10 mm tris-hcl ph 7.6, 200 g/ L trna, 1 Denhardt's solution, 10% Dextran sulfate, 600 mm NaCl, 0.25% SDS, 1 mm EDTA, DEPC H2O) 에 5 L의 RNA probe를첨가한후조직위에기포가생기지않도록 hybrislip으로봉입시켜 55 chamber에서 12시간동안 hybridization시켰다. 50 로예열된 2 SSC (sodium chloride sodium citrate)-50% 포름아마이드

3 구강편평태선에서 Elafin 의단백질의발현 17 용액에서 30분간반응시킨후 hybrislip을제거하고다시 50 로예열된 2 SSC-50% 포름아마이드용액에서 30분간반응시켰다. DIG 1 (0.1 M Tris, 0.15 M NaCl) 용액으로 37 에서 30분간반응시킨후 blocking reagent가들어있는 DIG 2 (0.1 M Tris, 0.15 M NaCl, 2% blocking) 용액으로실온에서 2시간동안반응시켰다. 여분의용액을제거하고 anti-dig antibody (Boehringer Mannheim GmbH, Germany) 를 DIG 1 용액에 1:1,000으로희석하여실온에서 2시간반응시켰다. DIG 1 용액으로 30분씩 4회수세하고 BCIP-NBT (Kirkegaard&Perry Lab, SF17, MA, USA) 로실온에서 20분간발색시켰다. 발색이끝난후 DIG 4 (10 mm Tris, 1 mm EDTA) 용액으로 10분간수세하여발색을중지시킨후, glycerol gel (DAKO, 106-1, CA, USA) 로봉입하였다. 결과 50예의구강편평태선증례중에서그물망형태의구강편평태선은모두 20예였다. 여성이 8예고평균연령은 51.5세였으며남성은 12예고평균연령은 54.4세로나타났다. 상피위축성구강편평태선은여성에서만 2예로나타났으며평균연령은 67.5 세였다. 상피침식성의구강편평태선은모두 28예였는데여성이 18예였고평균연령은 50.2세였으며남성은 10예로평균연령은 Table 1. Immunohistochemical observation of elafin in different types of oral lichen planus, compared to the oral leukoplakia, inflammatory ulcer, and normal mucosa 68 세로나타났다 (Table 1). 면역조직화학적염색 Image Pro-4.0 (Media cybernetics, Des Moines, USA) 를이용하여면역염색된표본에서전체세포의수에대한염색된세포의수를백분율로측정하였다. 정상점막상피의전층에서 elafin 항체가미만성으로양성반응 (5-10%) 을보였는데, 가시세포층과과립세포층에서중등도의양성반응 (10-20%) 을보였으며각화세포층에서는국소적으로양성반응을보였다 (Table 2, Fig. 1-1a, b). 염증성점막궤양병소에서는염증성상피증식으로상피가두꺼워지고망상돌출이증식되었는데, 상피세포전층에서정상적인점막조직보다매우강한 elafin 양성반응 (20% 이상 ) 이관찰되었다 (Fig. 1-3). 구강백반증병소에서는각화상피층을제외한모든상피세포층에서 elafin의강한양성반응 (10-20%) 이관찰되었는데염증성점막병소에비하여다소감소된양성반응을보였다 (Fig. 1-2a, b). 구강편평태선병소중그물망형태의구강편평태선에서는점막상피의과립세포층에서중등도의 elafin 양성반응 (10-20%) 이나타났으며, 세포층이얇게위축되는곳에서는양성반응이 5% 이하로현저히감소하였다 (Fig. 1-5a, b). 상피위축성과침식성구강편평태선은그물망형태보다 elafin의양성반응이전반적으로감소하였다. 상피위축성구강편평태선에서점막의궤양이진행되는얇은세포층에서는 elafin 양성반응이 3% 이하로거의관찰되지않았다 (Fig. 1-6a-c). 상피침식성구강편평태선의점막상피세포에서는 elafin 양성반응이비교적감소되어 (5-10%) 일부의과립세포들에서제한적인양성반응이관찰되었다 (Fig. 1-7a-c). 6 20 이상신 이석근 mrna 양성반응이관찰되었는데특히가시세포층과과립세포층에서중등도의양성반응 (10-20%) 을보였다. 상피세포내 elafin mrna의발현이상피세포의바깥쪽세포질에치중되어나타났으며, 각화층으로이행될수록점차 elafin mrna의발현이현저하게감소되어거의나타나지않았다 (Table 3, Fig. 2-1a-c). 염증성병소의점막상피에서는기저층의증식및상피층의비대가보이는상피전층에서 elafin mrna의강한발현 (20% 이상 ) 이관찰되었는데, 비대하게증식되는망상돌출부위에서도경등도의양성발현 (5-10%) 을보였다 (Fig. 2-4a, b). 구강백반증병소의점막상피에서는기저층과가시세포층에서 elafin mrna의발현이증가되어있으나 (10-20%), 과립세포층과각화층에서는 elafin mrna의발현이감소되었다 (5-10%) (Fig. 2-3a-c). 구강편평태선병소중에서그물망형태의구강편평태선에서는점막상피의과립세포층에서 elafin mrna의발현이증가 (10-20%) 되었으나, 상피의위축이일어나는부분에서는발현이감소하였다 (3% 이하 )(Fig. 2-5). 상피위축성구강편평태선에서는점막상피궤양이진행되는얇은상피세포층에서 elafin mrna 의발현이거의관찰되지않았으며 (Fig. 2-6), 상피침식성구강편평태선점막상피의과립세포층에서는 elafin mrna의발현이국한되어나타났다 (5-10%)(Fig. 2-7). 고찰구강편평태선에는상피층하방의결체조직에심한림프구침윤층을보이고점막상피의괴사가나타나는데, 결국기저막의파괴로상피의기저층세포들이점차로위축되거나괴사되는현상이연속적으로나타난다. 28 이러한상피의괴사는단순히림프구의침윤과여러종류의 cytokine들이분비되어야기되는상피세포외부적요인에의한괴사뿐만이아니라이들외부적요인들에충분히대응하지못하여상피세포의비대, 각화등이생기고점막상피의망상돌출이위축되어톱니모양의상피구조를이루기때문인것으로생각된다. 따라서비록림프구가침윤하여점막상피의자극이야기되지만점막상피자체는비교적나쁜조건에서도활발하게반응하는대상성기능이많은것으로알려져있다. 29 이때임상적으로점막상피의각화및궤양이주요증상으로나타난다. 따라서상피세포의괴사과정을구강편평태선의진행과정중에살펴보는것은매우중요하다. 일반적으로구강점막상피는단단한탄력섬유 (elastic fiber) 에의하여피개되어있어서기계적또는화학적손상에대하여강한내성이있으며, 특히구강내세균의침투를억제하는두터운상피구조를갖고있다. 그런데구강내세균중에는구강점막상피세포내의탄력섬유를파괴시킬수있는효소를분비함으로써쉽게구강점막궤양을일으키기도하는것이있다. 따라서구강점막상피는이러한세균성또는내인성 elastase에대항하기위 한성분을발달시켜왔는데, 그중하나가 elafin이라는 anti-elastase이다. 이 elafin은주로타액내에풍부히존재하면서구강점막을보호하는데, 일반적인상피세포에서도 elafin을다량분비하는기능이있어서마찬가지로상피를보호하는역할을담당하고있다. 23 Elafin은하나의분자가단백분해효소의위치에결합하여그효소의촉매중심부위인 serine을방해함으로써효과를나타낸다. 30 그리고 elafin은비교적작은크기의단백질로서점액질내에서쉽게분해되지않고 C-말단부에 transglutaminase binding motif 와 anchoring domain을갖고있어서, 단백분해효소나다른세포외기질에부착되어장기간항단백분해효소의기능을할것으로여겨진다. 따라서 elafin은구강내점막상피에서강력한항단백분해효소의기능을효과적으로유지할것으로추측된다. 본연구에서는면역조직화학적방법과 RNA in situ hybridization 방법들을동시에사용하여 elafin의발현을조사하였다. 특히임상적으로편평태선으로간주되는병소는절제한후바로 4% 글루타르알데하이드에고정하여 RNase 방지된상태에서파라핀절편을제작하여 RNA in situ hybridization을하였는데, 생검시통법에의하여 10% 중성포르말린에고정된파라핀 block을 RNase 방지된상태에서제작된파라핀절편을이용한 RNA in situ hybridization의결과에서도 4% 글루타르알데하이드에고정된경우와유사한 elafin 발현양상이관찰되었다. 결과적으로정상점막상피, 구강백반증이나염증성궤양상피에비하여구강편평태선의점막상피에서 elafin mrna 발현에큰변화가있는것이확인되었다. 대체로정상구강점막상피보다는비후된구강백반증이나염증성궤양을일으킨병소에서 elafin의발현이증가하였다 (Table 2, 3). 이것은정상구강점막상피보다증가된림프구등으로인해분비된 elastase을 elafin의작용으로억제시키기위한대상적반응이라고생각한다. 구강편평태선은병소의진행이나재발양상등에따라대체로세가지형태로나타나게되는데, 망상형편평태선에서과립세포층은 elafin 발현이비교적뚜렷하게나타났으나상피층이점차위축되어얇아지는부위에서는 elafin이거의발현되지않았다. 이것으로미루어볼때 elafin으로인한 elastase의억제작용이한도가넘어서는경우상피층의손상이생길것으로추측된다. 망상형편평태선보다예후가나쁜것으로알려진위축성이나침식성편평태선에서는병소의주변부위의두터운상피에서 elafin에대한양성반응이약하게관찰되지만, 병소의중심부위에서이형성된상피에서는 elafin의양성반응이점차소실되었다. 이것은분화가잘되어성숙된상피세포에서는외부의단백분해효소작용으로부터자신의세포를보호하기위하여 elafin을생산할수있으므로아직잘분화된상피세포로이루어진병소주변부위에서는 elafin의생산이비교적뚜렷하게관찰되지만, 편평태선병소의중앙부위에서는면역세포에의한지속적인염증반응으로구강점막상피의성숙이이루어지지않아서 elafin 생산이점차적으로감소되어염증반응에의한구강

7 구강편평태선에서 Elafin 의단백질의발현 21 점막의파괴가증가된다는사실을보여주는것이다. 본연구에서관찰한구강내편평태선은심한면역세포침윤과함께점막의궤양을수반하는고질적인병소이다. 구강편평태선에서보이는구강점막상피의궤양현상에서는비대화된상피가점차로그물형또는위축성점막상피로이행되면서상피의망상돌출이소실되고결국상피가궤양으로되는현상이나타난다. 이때 elafin의분비가현저하게감소된다는것에는중요한의미가있다. 본연구에서는편평태선의종류와병소의위치에따라면역조직화학적및 RNA in situ hybridization 방법에의하여 elafin의발현을관찰하였는데, elafin의발현이상이편평태선발생의직접적인원인은아니지만편평태선이진행될수록점막상피에서 elafin의생산이현저하게감소됨으로써편평태선의병소를악화시켜서광범위한궤양성질환을일으킨다는것을알수있었다. 아울러이들에대한심도있는연구를계속하면 elafin 단백질을편평태선의치료제로사용할수있는방법이개발될수있을것이라사료된다. 참고문헌 1.
