C. jejuni infection in Korea has been reporeted in 1981, but the epidemiological information was laking in the report. This study was undertaken to determine the Campylobacter carrier rate of the chickens in Chullabuk - Do. To determine monthly the Campylobacter carrier rate of chickens, cultures were made from 313 samples taken in Iksan city, Chullabuk-Do during January 1996 to August Also, to find out susceptibility of 13 antimicrobial agents against 30 isolates of C. jejuni/ coli from chickens were determined by the disk diffusion and the agar dilution technique. The following results were obtained. First, C. jejuni/ coli was isolated from 64 % of chicken samples, and the frequency of isolation was highest in August, while the lowest 11 % in April. The seasonal vari ation of the rate was not remarkable. Second, all of the is 이 ates from chickens were susceptible to amikacin, gentamicin, imipenem and tobramycin. To erythromycin 12 %, nalidixic acid 22%, ciprofloxacin 70% and tetracycline 59% of is 이 ates showed resistance. Imipenem, gentamicin, tobramycin and amikacin were the low antibiotic agents with MIC 90 of 0.25, 1, 2, and 2 1- 땅 /ml, respectively. Imipenem was the most antimicrobial agent tested, and MIC range of that was 땅 /ml and MIC so and MIC 90 were 0.12,ug/ml and 0.25,ug/ml, respectively. Third, when the suceptibility test by the disk diffusin and by the agar dilution method were tested, resistance to ampicillin and tetracycline were 21 % and 40% respectively, but no difference were noted against other antibiotics. 1. 서론 Campylobacter 는그람음성의만곡또는나선형 의간균으로길이는 0.5... 8μn, 폭 0.2... 0.5αn 이 며, 호기성상태에서는발육하지않으나미호기 성의특수한환경에서증식하는세균이다. Campylobacter 라는속명은 curved rod 를뜻하는그리 스어로부터유래되었다.

2 1. 서론 Campylobacter 는그람음성의만곡또는나선형 의간균으로길이는 μn, 폭 αn 이 며, 호기성상태에서는발육하지않으나미호기 성의특수한환경에서증식하는세균이다. Campylobacter 라는속명은 curved rod 를뜻하는그리 스어로부터유래되었다. 최초로보고된것은 1909 년 Campylobacter 의감염이 소에서였으며, 처 음분리는 1913 년 McFadyean 과 Stockman 에 의해서유산된양의자궁삼출액에서였으며 l), 1919 년이균종에대해서 Smith 와 Taylor 는 Vibrio jetus 라고명명하였다 2) 이균속이사람의 장염과관계있는것으로보고한것은 1946 년 Levy 3) 이었고, 이어서 King( 1957) 4), Dekeyser (1972)5) 및 Butzler(1973 )6) 도보고하였다. King (1 957) 은사람의감염에서분리된균주 중에는 V. jetus 와는좀다른성상을가진세 균이 있음을보고하여이를 related vibrio" 로 칭하였다 7). Sevald 와 Veron( 1963) 은이세균이 V. cholerae 및 Vibrio 속에함유된균종과는생 리생화학적으로도또는 DNA 의 GC% 에서도다 르기때문에이들균종과명확히구분하기위해 서 Campylobacter 균속으로다시명명하였다 1, 8) 한편 Skirrow 8 ) 에의해서선택배지가개발되어 급속도로연구가진전되었고, Salmoη ella 와같이 사람에게주요장염의원인균으로인정되었다. Campylobacter 균속에는 18 종이보고되고있으 며, 이중사람에게식중독으로장염과관계있는균종은 C. jejuni subsp. jejuni 와 C. coli( 이하는 C. jejuni/ coli) 가대부분이며, 이외에도드물게 C. upsaliensis, C. lari, C. hyointestinalis, C. cryoaerophila 등이 있으며, 기존질환을가진환자나 면역부전환자에게패혈증을일으키는 C. jetus subsp. jetus 도드물게보고되고있다 9) C. jejuni 는사람, 닭, 칠면조, 돼지, 개, 고양이, 양, 소등여러가지동물의위장관이나생식기 관, 물및식품등의생태계에존재하며 10~20), 람의장염은동물유래의 C. jejuni 가오염된육 류나우유를날것으로먹음으로써또는동물 사 접촉이나동물의변이오염된물을먹음으로써정상인에게일으키는것으로생각되고있으나, 우리나라동물에대한보균율조사는드물다. 이세균은세계각지의장염환자에게서분리되고있고그분리율은다른장염세균의분리율보다높다 10) 특히열대지방에서높고 l3), 아프리카에서는 30% 이상의분리율이보고된 바있다 14) 일본에서는성인에게서 4.6%, 소아 에서 9.0% 로보고된바있다 10) C. jejuni에의한장염은 erythromycin이치료제로권장되고있다 15) 혼히우리나라에서분리되는세균은항생제내성균이많은것으로보고되고있다. 특히가축에게는항생제가첨가된사료를먹이는일이많으므로우리나라의닭에서분리되는 C. jejuni는내성이많을수도있을것으로생각된다. 본연구에서는닭으로부터의 C. jejuni의월별분리율과분리된균주에대한한천희석법과디스크확산법을비교하여항균제감수성을규명하고자한다. 11. 실험재료및방법 1. 시험검체닭에서 C. jejuni/ coli 분리를위해사용한검체는 1996년 1 월부터 1996년 8월사이에전북익산시황등면과오산면에서시판되는닭의장내용물을오염되지않게사용하였으며, 이검체에서분리된 C. jejuni/ coli는사용할때까지는 30 % glycerin이포함된 thioglycollate broth 1ml 에균을진하게풀어서냉동보존하였다. 2. 시험항균제및배지 항생제로써는 amikacin 30 /-땅, ampicillin 10/-횡, cephalothin 30 μg, chloramphenicol 30 μg, Clprofloxacin 5/-용, erythromycin 15 /-땅, gentamicin 10 l땅, kanamycin 7 /-땅, tetracycline 30 /-횡, tobramycin 10 μg, trimethomprim 1.25 μg/ sulf amethoxazole 23.75,ug이함유된디스크 (BBL) 를사용하였으며, 표준품분말항생제로써는 amikacin - 21-

3 (national committe for clinical laboratory stan- 깅(Sigma chemical, 80%), ampicillin(sigma chemical, 736 않 ), cefoxitin( 일본萬有製藥洙式會社, 923,l 땅 ), cephalothin (Sigma chemical, 910,l 땅 ), chloramphenicol(sigma chemical, 80 %), ciprofloxacin(miles Pharmaceuticals Vast Haven, 852,LLg), erythromycin(sigma chemical, 98%), gentamicin(sigma chemical, 654,l 땅 ), lmlpenem (Merk Sharp & Dohme, 930,LJg), kanamycin (Sigma chemical, 784,l 횡 ), nalidixic acid (Sigma chemical), tetracycline(sigma chemical, 985,l 땅 ) 및 tobramycin(sigma chemical, 710,l 횡 ) 을한천 희석법에사용하였다. 시험배지는 Campylobacter selective medium (Butzler, oxoid) 을사용하였다. 즉냉동보관된 작은병에분말로들어있는 Campylocacter selective medium 의내용물을 50% 알코올 3 ml ( 배지 500ml 용임 ) 에용해시켜배지에첨가하 였으며, 최종농도는배지 ml 당 bacitracin 25 unit, cycloeximide 50μg, colistin sulphate 10 unit, cephazolin sodium 15,LJg novobiocin 5,l 땅 이었다. Tryptose blood agar base(difco) 33 g 을증류수 1,000 ml 에녹여멸균후 50 0 C 로식 힌후수혈용혈액 5% 와 Campylobacter growth supplement 10 ml ( 증류수 10 ml 에 sodium pyruvate 0.25 g, sodium metasulfite 0.25 g, ferrous sulfite 0.25 g 이들어가도록수용액을 만틀고여과멸균하였다 ) 및 selective supple ment 6 ml 를첨가한후 Petri 접시에두께가 4mm 되도록 25 ml 씩분주하였다. cephalothin 디스크에감수성이면 C. jejuni/ coli 로동정하였다. 필요에따라서 catalase 시험, 포도당산화시험, 운동성시험, HzS 생성시험, 질산염환원시험, 25 0 C 및 1 % bile, 1 % gly cine, 3.5 % NaCl 첨가 fluid thioglycollate 배지 에서증식시험을하여 Table 1 과같은성상을 보이는것을 C. jejuni 로최종동정하였다. C. jejuni/ c 이 i 분라 닭의장내용물에서 C. jejuni/ coli 를분리하기 위해서 Butzler 의선택배지를사용하였다. 즉 이배지에면봉으로닭의장내용물의검체를 평판배지에고루접종한후양초단지에넣고 42 0 C 에서 2 일간배양하였다. 집락이형성되었 으변호기성배양에서증식하지않은집락을 선택하여 oxidase 양성인그람음성의만곡된간 균임을확인하였다. 분리된균주가 30 명의 Kirby - Bauer 디스크확산법으로하되배지 는혈액한천배지를사용하였다. 순수배양된 집락으로면봉을사용해서배지표면에고루바 른다음디스크를올려놓고양초통단지에넣 어 35 0 C 에서 16..., 18 시간배양후에결과를판 독하였다. 판독이애매한것은 24 시간후에다 시확인하였다. 한천희석법 (agar dilution method) 은 NCCLS

4 dards) 표준방법을따랐으며, 규정에따라녹인 항균제를멸균된 MHM 배지에혈액을첨가하 여평판배지를만들었다. 실험균주는 회계 대배양된집락을이용하여 TSB 에접종한후 McFarland 0.5 관에식염수로탁도를맞추고접 종에사용하였다. Replicator 를써서접종하고 35 0 C 에서 48 시간 6% CO 2 에배양하여 1 개이 상자란집락의배지의항균제농도를최소억제 농도 (minimum inhibitor concentration, MICs) 로판정하였다. m. 결과 1. 닭에서월벌 C. je juml co/is. 걱분리율 총 313 예의닭내용물검체중에서 190(61 %) 예의 C. jeujuni/ coli 가분리되었으며, 8 월이 87% 로가장높고이어서 3 월이 84% 로다음순이었 고, 4 월이 11% 로가장낮았다 (Table 2). 2. 디스크확산법에의한항균제감수성 닭에서분리된 c. jejuni/ coli 70 주의디스크 확산법에의한항균제감수성결과는모두가 cephalothin 에내성이었고 amikacin 과 gentamicm 에모두감수성이었다. Co - trimoxazole 과 tetracycline 및 xciprofloxacin 에대해서는각각 96%, 89% 및 69% 의내성을보였다 (Table 3). 3. 한천희석법에의한항균제감수성 Table 2. MIC 90 8% 는역시 lmlpenem 0.25 탱 /ml 로가장낮았 고 gentamicin 은 11 땅 /ml 이었다. Breakpoint 를 적용하여각항균제를해석할때모든균주가 cephalothin 에내성이었으나 ciprofloxacin 은 %, chloramphenicol 과 kanamycin 은각각 4% 내성이었고, amikacin, gentamicin, imipenem 및 tobramycin 011 대해서는모두감수성으로해석 되었다 (Table 4). 4. 디스크확산법과한천희석법의비교 %, tetracycline 은 59 %, ampicillin 은 4cefoxitin C. jejuni/ coli 에대한항균제감수성시험의 은 37 %, nalidixic acid 는 22 %, erythromycin 은디스크법과한천희석법에서 amikacin 과 genta

6 m lcm 은모두감수성인것과, cephalothin 100 %, chloramphenicol 4 % 내성인것은동일하였으나, 디스 크확산법에서 tetracycline 과 ampicillin 의내성이각 각 89%, 27% 이었으나한천희석법에서는내성이각각 59%, 48% 로현저하게차이가있었고, 이외에 ciprofloxacin, erythromycin 및 tobramycin 에대한 내성은동일하거나비슷하였다 (Table 3, 4, Fig. 1). N. 고찰 Campylobacter 균속은 catalase 생성의유무에 따라크게두가지군별로나눈다. Catalase 음 성의세균은 C. sputorum 으로분류하고, catalase 양성인군은다시 43 0 C 에서자라는균군과 자라지않는균군으로구분하는데특히 43 0 C 에서자라는균군을 themophilic Campylobacter ( 고온성 Campylobacter) " 라고통칭하여분류하 는데식중독과관계있는균종은여기에속한 다. 고온성 Campylobacter 에는 C. ]e]uni 와 C. coli 의 2 균종이있는데생화학적감별시험으로 마뇨산가수분해시험 (hippurate hydrolysis) 뿐 이기때문에대개는검사실에서이세균을보 고할때 C. 꾀 ;unil coli 라고보고하는경우가많 다 17) 본연구에서도이 2 균종을감별하지않 았기때문에 C. jejunil coli 라고하였다. 우리나 라와일본및다른나라에서도 Campylobacter 장 염의원인균으로보고되는대부분은 C. jejuni 이며 C. coli는드문것으로알려져있으며, 캐 나다와영국의보고에서는 C. coli 가 3--5% 를 차지하는것으로보고되고있다 18) 이세균은 높은위도로부터열대에이르는여러지역의 장염환자에게서분리되고있고, 그분리율은 다른장염세균보다높다. 특히아열대지방에 서분리율이높아요한네스버그에서는 2 세이 하장염환자에게서 35%14), 방글라데시에서는 12.3% 의보고가있다 13) 한편미국, 영국, 북유 럽, 일본등에서는 5% 내외의보고 10) 가있으 에 Amikacin Ampicillin :f Cefoxitin 톨 Cephalothin 까 Chloramphenicol Ciprofloxacinít Erythromycin 꽁 Gentamicin 빼 Imipenem Kanamycin p.. 나우리나라에서는정 19) 의보고에의하면 Salmonella다음인 13.6% 의분리율이보고되어있다. 열대지방에서는건강보균율이높기때문에장염환자에게서이세균이분리된경우에도그감염이이세균에의한것인지해석이어렵다. 우리나라에서건강한사람의보균율 20) 은 1.6%, 요한네스버그에서는건강인 14) 의 35%, 스웨멘에서는건강인의 0.25% 가보고되고있다.

7 나우리나라에서는정 19) 의보고에의하면 Salmonella다음인 13.6% 의분리율이보고되어있다. 열대지방에서는건강보균율이높기때문에장염환자에게서이세균이분리된경우에도그감염이이세균에의한것인지해석이어렵다. 우리나라에서건강한사람의보균율 20) 은 1.6%, 요한네스버그에서는건강인 14) 의 35%, 스웨멘에서는건강인의 0.25% 가보고되고있다. 우리나라장염환자로부터이세균이분리되었을때는장염의원인균으로해석하여도툴림이없을것으로생각된다. 이세균은닭, 칠면조, 개, 고양이등에높은비율로보균함이보고되어있고, 본연구에서닭장내용물의검체로부터분리율은 61% 로김등 20) 의 50.3%, 정등 21 ) 의 45% 보다보다다소높게보고되었는데, 검체채취시장내용물을즉시배양했을때는보균율은더높을것으로생각된다. 왜냐하면 4월에 11% 로낮은것은계절적인영향보다도검체를냉장고에보존후분리하였기때문이다. 이세균은감염동물이나환자에의해오염된음식물을통해경구적으로감염되는것으로알려져있다. 닭에게보균된 C. jejuni가사람에게감염을일으키는것이라면우리나라에도이세균감염에의한식중독이있을것으로생각된다. 식중독은대개산발적인경우와대규모적으로발생하는경우가있는데음식물에의한감염은통상산발적인경우가많다. 우리나라에서 Campylobacter에 의한식중독예는보고된바없지만일본에서보고된것에의하면세번째로많은것으로알려져있다 17) 이세균의감염원으로주로닭이나개등의동물과육류등의식품이주목되고있다. 사람에게서분리되는세균이동물에게서분리되는것과같은것인지보기위해서는혈청형, 생물형, 항생제감수성등이비교되고있다 22) 그러나 Campylobacter의항균제감수성시험에는한천희석법과 E test가권장되고있으나표준화되어있지는않다. 검사실에서흔히사용하는디스크확산법은그감수성결과가한천희석법 에의한결과와다른경우가많아서바랍직하지않은것으로보고되고있다. 본연구에서디스크확산법의결과는한천희석법과비교한바모두감수성인 amikacin, gentamicin, tobramycm과모두내성인 cephalothin과 4% 만내성을보인 chloramphenicol은같았으나, ampicillin과 tetracycline은 % 의내성차이가있었 고, erythromycin이나 ciprofloxacin은큰차이가없었다. Erythromycin이 Campylobacter감염에는유효 한것으로추천되고있다. 그러나내성균이있 음이알려지고있으며, Vanhoof2 2) 등은벨기에 에서 1978년에사람에서분리된 Campylobacter 균주중 8.4% 가, 1980년에는 2.3% 가, Walder 는스웨멘에서 8% 가, 캐나다에서 Karmali 등 15) 과, 영국에서 Brunton 등은 % 가 erythromycm 에내성이있음을보고하였다. 저자의 결과에서는 erythromycin은 MIC 90에서 8핑Iml 이었고 12% 의내성으로 Vanhoof 등 22) 의보고 보다모두높게보였다. 이 결과는닭의균주가 사료에포함된항생제에항상노출되고있기때 문인지는알수없다. 정등 21 ) 은사람에게서분 리한균주중내성균주를보고하였으나시험균 주중 gentamicin, tobramycin 은내성균주가 없었으며, kanamycin 역시 4% 만이내성을보 여 aminoglycoside 에대한닭균주의내성은드 옮을알수있었다. Nalidixic acid 는 C. jejuni/ coli 에감수성으로알려졌으나 Taylor 등 23) 의 보고에의하면 MIC 9 이 256멍Iml로 nalidixic acid내성이보고되고있다. 본연구에서도 MIC 90 이 32μg/ml 이었고내성은 22% 를보였으며, 특히 C. jejunil coli 동정시험에서 nalidixic acid 30명디스크를사용할때주의를해야하겠다. Ciprofloxacin은새로운퀴놀론계열의항균제로 C. jejunil coli에감수성 25) 으로알려졌으나본저자의결과에서는 MIC 90이 8명Iml이었고 70% 의높은내성을보였다. Cephalosporin제제등몇가지항생제는자연내성이다 Cefoxitin과 cephalothin의 MIC 90이각각 128핑Iml, ~ 128 이었고, 내성은 cefoxitin이 37%, cephalothin이 - 26-

8 모두내성을보였다. Ampicillin은김등 20) 의결과에서닭균주의 10% 가내성이있음을보 고하였으나본저자의결과는 MIC 9 이 64 멍 / ml 이었고, 내성은 48% 로높게나타났다. 이 세균중에는 tetracy line 내성인것이있고그 내성은 plasmid 에의해전달될수있음이보고 되었으며, Michel 등 24) 은 C. jejuni 에대하여 MIC 90 이 :2 90t 땅 Iml 이었고, 내성은 37.9% 로보 고하였으나저자의결과에서는 59% 로높았다. 가장낮은감수성을보인항균제는 ß-lactam 항생제의 carbapenem 계열인 lmlpenem 은 MIC 범위가 명 Iml, MIC 9 이 0.25 tjglml 이 었고, 내성균주는 Page 등삐의보고와같이, 본연구에서도없었고가장항균력이컸다. V. 결론 전라북도일부지역에서 C. jejuni/ coli 에대 한닭의보균율을조사하기위하여 1996 년 1 월부터 1996 년 8 월까지 313 예의검체를배양 하였고, 분리된균주의항균제감수성을파악 하기위하여 30 균주와 13 종항생제를시험하 여다음과같은결론을얻었다. 전라북도일부 지역에서 C. jejunil coli 의닭보균율은 61 % 이었 고, 월별로는 8 월이가장높았고, 4 월이 11% 로가장낮았으나계절에따른분리율은현저 한차이는없었다. 닭에서분리한균주모두가 amikacin, gentamicin, imipenem, tobramycin 에 는감수성이었다. Erythromycin 에대해서는 12 %, nalidixic acid 에서는 22% 가, ciprofloxacin 에서는 70% 가, tetracycline 에대해서는 59% 가내성을보였다. 가장낮은 MIC 를보인항균 제는 imipenem, gentamicin, tobramycin 및 amikacin 순서이었으며, MIC 90 은각각 0.25, 1, 2, 2 명 Iml 이었다. 가장낮은항균제인 lmlpenem 의 MIC 범위는 t 땅 Iml, MIC so 은 t 땅 / ml, MIC 90 은 0.25 핑 Iml 이었다. 항균제감수성시 험에서디스크법과한천희석법에의한항균제 감수성시험에서 ampicillin 과 tetracycline 을제 외하고는큰차이가없었다. 이상의결과로 C jejunul coli 감염치료에 aminoglycoside 계열과 lmlpenem 이유용하다는결론을얻었다.
