Methods: All clinical cultures obtained within 48 hours of hospitalization from elderly patients of at least 55 years of age arriving at a 623-bed, public teaching hospital in Seoul, Republic of Korea from LTCFs between April 1, 2011 and April 1, 2012 were collected retrospectively. Results: During this period, 365 elderly persons from 13 LTCFs were hospitalized. This study enrolled 135 patients who had cultures performed. In this group, 27.4% harbored MDRGNB at hospitalization. The presence of MDRGNB during prior hospitalization was the only risk factor that predicted harboring it (p = 0.043, odds ratio = 5.00, confidence interval = ). Combinations of antibiotics or carbapenems were used initially in 35.6% of the patients, and this did not affect the mortality rate in this population. Conclusions: Hospitalized elderly patients from non-hospital LTCFs need more attention. Judicious antibiotic selection is needed according to the risk factor of harboring MDRGNB for antibiotics stewardship. 2 - The Korean Journal of Medicine: Vol. 94, No. 1, 비병원장기요양기관으로부터의다제내성그람음성균유입상황을모르는상태에서무분별하게광범위항생제를사용한다면항생제내성증가로이어져악순환이이루어질것이다. 이에저자들은일개공공의료기관에비병원장기요양기관에서감염이의심되어입원하는노인환자의입원시임상검체다제내성그람음성균보유율을확인하고그위험인자와이들에대한항생제사용의적절성에관하여연구하였다. 대상및방법 2011년 4월부터 2012년 4월까지서울의료원에비병원장기요양기관로부터입원한 55세이상환자 365명중감염성질환이의심되어입원후 48시간이내배양이시행되었던 135명환자를후향적으로조사하였다. 서울의료원은서울시중랑구소재 623병상규모의공공의료기관이다. 대상군의성별, 연령, 감염성질환, 기저동반질환, 최근 1년간입원력, 이전항생제사용력, 지난입원시의다제내성그람음성균의보유, 입원시침습적기구보유여부, 입원시중증도, 초기치료항생제사용, 초기항생제의적절성, 임상검체배양검사, 반코마이신내성장알균감시배양, 72시간내사망률, 원내사망률을평가하였다. 환자군을다제내성그람음성균군과비다제내성그람음성균군으로나누어다제내성그람음성균보유의위험인자를분석하였다. 장기요양기관은일상생활을하기위하여도움이필요한환자들에게재활또는지속되는숙련된간호보조를제공하는기관등으로노인요양원, 재활기관, 재활병원, 요양병원, 보훈병원이해당되나본연구에서는병원이아닌장기요양기관으로한정하였다. 다제내성그람음성균은다음중 3가지종류이상의항생제군에내성을보이는그람음성세균으로 1) ampicillin-sulbactam/piperacillin-tazobactam, 2) carbapenem, 3) ceftazidime or ceftriaxone, 4) quinolone, 5) gentamicin 또는 amikacin 로기존연구의방법을따라정의하였다 [2,8,9]. 부적절한항생제사용은원인균에감수성인항생제로 48시간이내치료하지못한경우로정의하였고, 초기병합요법은다제내성그람음성균을원인균으로의심하여두가지항생제를 48시간이내사용한것으로정의하였다. 장기요양기관을통한입원시임상검체다제내성그람음성균보유율장기요양기관으로부터입원하여 48시간이내에배양이시행되었던총환자중다제내성그람음성균이동정된환자수로정의하였다. 이전항생제사용은지난 3개월이내 14일이상의항생제를사용한경우로정의하였다. 동반질환은 Charlson 등 [10] 의동반질환점수를사용하여정량화하였다. 임상적감염의중등도는전신염증반응증후군, 패혈증, 중증패혈증, 패혈성쇼크로구분하였고임상적감염증이확인되지않은곳에서검출된균은집락균으로간주하였다 [11]. MDRGNB of the elderly from long-term care facility - 통계처리및분석 결 과 동정된균에대한항생제감수성검사는 Microscan WalkAway 96 plus system (Siemens Healthcare Diagnostics Inc., West Sacramento, CA, USA) 이사용되었다. 통계적분석에불연속변수는 χ 2 -test, 연속변수에대해서는 t-test 를사용하였고 p-value < 0.05를유의한것으로하였으며, 다변량분석은다중로지스틱회귀분석을시행하였다. 모든분석은 SPSS software, version 12.0 (SPSS, Inc, Chicago, IL, USA) 을이용하였다. 연구대상자의특성연구기간내 13개의장기요양기관을통하여 365명의노인이입원하였고 48시간이내 135명에서배양이시행되어연구에포함되었다 (Fig. 1). 상기기간동안임상배양검체에서동정된다제내성그람음성균은 37건으로비병원장기요양기관을통한입원시임상검체다제내성그람음성균보유율은 27.4% 였다. 환자들의평균연령은 74.2 ± 14.2세, 남성 69명 (51%), 평균 Charson 동반질환점수는 1.7 ± 1.6였다. 4 - 대한내과학회지 : 제 94 권제 1 호통권제 686 호 다제내성그람음성균보유의위험인자분석다제내성그람음성균군과비다제내성그람음성균군간의단변량분석에서다제내성그람음성균보유의위험인자는나이, 만성신부전, 최근 1년간의입원력, 최근 3개월간의항생제사용, 지난입원시다제내성균의보유, 입원당시의중증도가유의하였다 (Table 1). 다변량분석결과지난입원시의다제내성그람음성균의보유만이다제내성그람음성균보유의독립적위험인자였다 (p = 0.043, odds ratio [OR] = 5.00, confidence interval [CI] = ). 균주별항생제내성의분포질병별로동정된 multidrug resistant, gram-negative bacteria (MDRGNB) 의분포는그림 2와같다. 분리된다제내성그람음성균은 8가지균종으로빈도는 Klebsiella pneumoniae (K.pneumoniae), Proteus mirabilis (P.mirabilis), Escherichia coli (E.coli), Pseudomonas aeruginosa (P.aeruginosa) 의순이 Figure 2. 5 - 김민성외 4 인. 요양기관에서입원하는노인과약제내성 - 었으며, 각각의항생제내성양상은표 2와같았다. 동정된균들중 Extended spectrum β-lactamase (ESBL) 분비균주는 16건으로 K.pneumoniae 6건 (16%), E.coli 는 5건 (13.5%), P.mirabilis 5건 (13.5%) 이었고 carbapenem 내성 P.aeruginosa 3건 (8%), carbapenem 내성 A.baumanii는 2건 (5%) 이었다. 항생제치료와예후환자들의임상적감염증빈도는폐렴 43.7%, 요로감염 17%, 복강내감염 6.7% 순이었고, 이들의원내사망률은 6.7% 였다. 초기항생제치료로병합요법또는카바페넴이사용된군은 35.6% 였고, 각질병별로폐렴에서 piperacillin/tazobactam + quinolone 34명, 3rd cephalosporin + ampicillin/sulbactam 3명, 3rd cephalosporin + quinolone 1명, piperacillin/tazobactam + aminoglycoside 1명, carbapenem 2명, 요로감염증에서 carbapenem 5명, 피부연조직감염증에서 piperacillin/tazobactam + aminoglycoside 1명으로나타났다. 초기치료로병합요법이나카바페넴의선택여부는 72시간사망률 (p = 0.157, 95% CI = ) 및원내사망률 (p = 0.157, 95% CI = ) 에영향을주지않았다. 고찰본연구에서비병원장기요양기관에서입원하는노인들의입원시임상검체다제내성그람음성균보유율은 27.4% 였으며, 이들환자군에서다제내성그람음성균보유의독립적위험인자는지난입원시의다제내성균의보유뿐이었다. 이들에대한병합또는카바페넴초기사용률은 35.6% 로높은경향을보였으며이는사망률에영향을주지않았다. 현재다제내성그람음성균에대한표준화된정의는없는상태로일반적으로 3가지종류이상의항생제군에내성으로보이는그람음성세균으로정의하나 cefazolin 또는 trimethoprim-sulfamethoxazole 등일차적으로사용하지않는항생제를포함하여다제내성여부를평가하기도하고 Pseudomonas 나 Acinetobacter 등의비발효군을배제하기도한다 [6,9]. 본연구에서는초기항생제치료로항생제를선택하는임상적상황을고려하여 cefazolin, trimethoprim-sulfamethoxazole 내성은배제하고, 비발효균을포함하는정의를택하였다 [8]. 선행연구들은다제내성그람음성균에의한임상적감염발생률과집락률로구분되어진행된다. 미국의 530병상 3차병원에서진행된다른연구에서는 8년 6개월의연구기간동 안입원한노인환자에서다제내성그람음성균에의한혈류감염이 1% 에서 16% 로 16배증가하였다 [2]. 한국에서진행된국립보건원의연구에따르면장기요양기관에서의분리된균주에서 E. coli 의 45.3%, K. pneumoniae의 42.7% 가 ESBL 분비균주로나타났으며, K. pneumoniae의 39.3%, E.coli 의 3.1% 가플라스미드매개 AmpC β-lactamase 를보유하고있는것으로 3차병원과같은높은양성률을제시하였다 [7]. 그러나임상검체양성률이환자들의다제내성균보유또는그로인한유입을전부반영할수없다는전제하에집락률연구들이진행되고있다. 이스라엘 550병상규모의병원에서진행된직장배양보유율검사에서 ESBL 양성 Enterobacter는입원시 8% 로나타났다 [12]. 집락연구는선별배지를이용하여위장관집락형성유무를전수에서조사한다는장점이있어임상검체양성률보다무증상보균자들을포함할수있다는장점은있으나객담, 소변등다른검체의보유를배제할수없고임상적감염증을반영하지못하며일상적인적용이불가하다는단점이있다. 본연구자들은입원 48시간이내의임상검체에서의다제내성그람음성균보유율을통하여병원으로의유입을보고자하였다. 선행연구들의장기요양기관은다양한크기의노인요양병원, 보훈병원등의장기요양병원뿐만아니라노인요양원, 보호시설등비병원요양기관에이르기까지다양한종류의기관을포함하고있다. 이들기관들은노령이나만성질환, 반복적인입원등의다양한위험인자를공유하고있지만, 비병원장기요양기관은장기요양병원에비하여관혈적시술과광범위정주항생제의사용이용이하지않기때문에항생제내성이낮을것으로예측된다. 이에대한구분없이장기요양기관에입원했다는이유로초기병합항생제를사용하는것은내성증가의악순환을일으킬수있다 [5] 년미국 4개병원에서진행된연구에서는중증장기요양기관들 ( 인공호흡기를구비한전문요양원 [skilled nursing facility with ventilator unit] 와급성장기요양병원 [long term acute care hospital]) 에서입원한환자에서 carbapenemase 생성 K.pneumoniae 장내집락이지역사회에서입원한환자보다많았다. 집락균의형성은장기요양기관의종류에따라서도차이가있었는데중증장기요양기관의경우다른장기요양기관보다 7배가량높게발생하였다. 중증장기요양기관이아니경우지역사회와집락률이유의한차이를보이지않았다 [13]. 위연구는장기요양기관을인공호흡기사용유무에따라분류한것으로, 요양기관의종류에따라집락률의차이

6 - The Korean Journal of Medicine: Vol. 94, No. 1, 가있음을보여주었다. 본연구는요양기관중관헐적인시술이나항생제사용이일반적이지않은비병원장기요양기관에서유입되는노인환자를대상으로진행되었다. 본연구가수행된서울의료원은급성기치료가이루어지는공공의료기관으로 13개의다양한장기요양기관에서입원한 365명중 135명의많은수의대상이포함될수있었다. 이들에서의입원시다제내성그람음성균임상검체보유율은 27.4% 였고, 고전적인다제내성균보유의위험인자들을분석하였을때이전다제내성그람음성균의보유만이 5배높은유일한위험인자임을확인하였다. 추가적으로시행한항생제사용의적절성및사망률분석에서초기항생제치료로 35.6% 의환자에서병합요법또는카바페넴이사용되어검체의다제내성그람음성균보유율보다높은경향을보였고, 이는초기원내사망률에영향을주지않았다. 기존 pilot 연구에서다제내성그람음성균의보유유무가초기사망률에영향을주는것으로보고하였으나 [8], 본연구에서기간과환자수를늘려관찰하였을때초기사망률및원내사망률에영향을주지않았다. 본연구의제한점은다음과같다. 첫번째, 단일기관후향적연구로포함된환자수가적고미지의교란변수가존재할수있다. 본환자군에대한다기관전향적연구가필요하다. 두번째, 다제내성그람음성균의유무를집락균이아닌유증상자의초기임상적배양을통하여진행하였으므로무증상보유자를포함하여다제내성그람음성균의보유율이과소평가되었을것이다. 세번째, 대상군의사망률이낮아항생제사용과의통계적유의성이밝혀지지않았을수있겠다. 네번째, 유전자형에대한검사가시행되지않아다제내성그람음성균의기관내, 기관간역학적연관성, 각내성의종류를유전형으로확인하지못하고표현형으로보았다. 병원이아닌장기요양기관에서입원하는노인환자들을위험인자에따라다제내성균보균가능성을평가하고, 적절한항생제를주의깊게선택하는것이항생제내성관리에중요하겠다. 또한이들환자군에대한전향적다기관연구가요구된다. 요약목적 : 장기요양기관에서의입원하는노인환자들은다제내성그람음성균의고위험군으로여겨지나병원이아닌장기요양기관은차이가있을것으로예상된다. 이에저자들은 비병원장기요양기관에서입원하는노인환자의입원시임상검체다제내성그람음성균보유율을확인하고그위험인자와이들에대한항생제사용의적절성을연구하였다. 방법 : 2011년 4월부터 2012년 4월까지서울소재 623병상규모의공공의료기관에병원이아닌장기요양기관으로부터입원한 55세이상환자 365명중감염성질환이의심되어입원후 48시간이내배양이시행되었던 135명을후향적으로의무기록조사하였다. 결과 : 상기기간동안비병원장기요양기관에서입원한노인들의입원시임상검체다제내성그람음성균보유율 27.4% 였다. 다변량분석결과지난입원시의다제내성균의보유만이다제내성그람음성균보유의독립적위험인자였다 (p = 0.043, OR = 5.00, CI = ). 항생제의초기치료로병합요법과카바페넴사용은 35.6% 의환자에서이루어졌으며이는사망률에영향을미치지않았다. 결론 : 비병원장기요양기관에서입원하는노인들에대하여보유위험인자에따른적절한항생제의사용이필요하겠다. 중심단어 : 장기요양 ; 노인 ; 다제내성균 REFERENCES 1.
