MethodsZZEighty four inpatients with schizophrenia and 81 control subjects were recruited. Height and weight were measured for calculation of body mass index (BMI). Symptom severity scales [Positive and Negative Syndrome Scale (PANSS), Brief Psychiatric Rating Scale] and Global Assessment of Functioning Scale (GAF) were conducted in all patients. Comorbid medical conditions, years of education, smoking status, age of onset, duration of illness, and medication history were collected. Among patients, obese patients scored lower on symptom severity scales and better on functional assessment than not obese patients. Multiple regression analysis revealed an association of late onset and being obese with lower PANSS total score and better GAF score in patients. Late onset and being female were inversely related with negative symptom score. The type and dose of antipsychotics showed no association with obesity. ConclusionZZThe results of this study indicate that patients with schizophrenia are likely to be more obese. Some characteristics of patients with less severe psychopathology, such as late onset, being obese, were coincident with the result of previous studies. In addition, the result showing that the heavier the weight of patients, the lower severity of symptoms was repeatedly reported in previous studies. The type and dose of antipsychotics were not associated with obesity. These results prompt further investigation of the relationship among schizophrenia, antipsychotics, and weight gain. J Korean Neuropsychiatr Assoc 2015;54(2): KEY WORDSZZ Schizophrenia Obesity Psychopathology Severity. 서론 조현병 환자들은 일반 인구 집단에 비해 비만도가 높은 것으로 알려져 있으며 비만은 정도에 따라 복부비만 고혈압 이상지질혈증이나 내당능장애 등과 같은 대사증후군으로 발전하게 된다 대사증후군은 치료 경과에 따라 비만의 정도가 심해지고 인슐린 저항 등의 위험이 공존하게 되면 관상동맥질환 및 당뇨병 등의 유병 위험이 높아지고 더 나아가 높은 사망률과 연관이 된다 정신질환 환자들에서 보고된 대사증후군 유병률은 일반 인구집단에 비해 배 높게 나타나며 조현병 이나 양극성장애 우울장애 환자들에서 유사한 유병률이 보고되고 있다 이러한 비만과 대사증후군의 발병은 항정신병약물을 위시한 향정신성 약물의 효과 정신질환의 유발요인으로 간주되는 유전이나 체질과 같은 생물학적 요소 정신증상의 심각도와 그에 따른 건강하지 못한 식생활과 생활습관 등과 연관되는 것으로 보고되고 있다 조현병 환자에서 항정신병약물인 등의 비정형항정신병약물들이 체중증가 당과 지질대사 이상에 뚜렷한 요인으로 작용을 하는 것으로 보고되고 있다 또한 등 은 비만과 연관된 유전자 다형성 과 조현병의 증상발현 항정신병약물 치료반응이 연관되어 있다는 연구 결과를 발표하였다 또 항우울제 기분조절제 등도 대부분 체중과 대사증후군의 발병에 영향을 주는 것으로 알려져 있다 한편 172 Copyright 2015 Korean Neuropsychiatric Association

2 조현병환자의비만도와정신병리 MS Song, et al. 등 은 저체중인 환자들이 정신병리가 더 심하다는 연구 결과를 보고하였고 항정신병약물 투약시 체중 증가가 큰 환자들이 정신병리의 호전 폭이 더 크다는 연구 결과들이 있다 이러한 앞서의 연구들을 근간으로 저자들은 입원 중인 조현병 환자들을 대상으로 첫째 조현병 환자들이 일반 인구에 비해 비만인 비율이 높은지를 살펴보았으며 둘째 비만의 정도에 따라 환자의 정신병리적 특성과 중증도가 양성 증상 음성 증상 및 개별 증상 측면에서 차이가 있는지를 알아보기 위해 본 연구를 시행하였다 방법 대상본 연구는 년 월부터 월까지 계요병원에 입원해 있는 환자 중 정신질환 진단 및 통계편람 제 판 에 근거하여 명 이상의 정신건강의학과 의사가 조현병으로 진단한 환자를 대상으로 이루어졌다 조현병으로 진단 받은 환자 중에서 기질성 정신장애나 신경과적 장애 정신 지체 경련성 뇌 질환이나 뇌 손상 병력 알콜 및 물질 중독의 병력이 있는 환자는 배제하였다 대상 환자들에게 연구의 목적과 방법에 대해 사전에 충분히 설명하고 연구 참여에 관한 동의서를 작성한 후 연구를 시행하였으며 최종적으로 연구에 참여한 환자는 명이었다 대조군으로는 년 월부터 월까지 계요병원에 근무 중인 세 자원자를 사용하였으며 기질성 정신장애나 신경과적 장애 정신 지체 경련성 뇌 질환이나 뇌 손상 병력 알콜 및 물질 중독의 병력이 있는 경우와 본인 및 직계 가족 중 정신질환의 과거력이 있는 경우는 배제하였다 본 연구는 계요병원 임상시험심사위원회의 심사 후 허가를 받아 진행되었다 평가내용및도구비만도를 측정하기 위해 체질량 지수 이하 를 사용하였다 는 환자의 체중 을 신장 의 제곱으로 나누는 것으로 비만 판정의 기준인 체지방량과의 상관계수가 로 체지방량을 잘 반영하므로 성인기 비만 판정에 유용한 것으로 알려져 있다 대한비만학회에서는 가 미만인 경우 저체중 인 경우 정상 체중 인 경우 과체중 이상인 경우 비만으로 정의하고 있다 본 연구에서는 저체중 정상 비만의 가지군으로 나누었으나 측정 결과 저체중에 해당하는 인원이 환자군 전체 명 중 명 대조군 전체 명 중 명에 불과해 비만이 아닌 경우와 비만인 가지 군으로 나누어 분석했다 환자군 대조군 모두 동일한 디지털 체중계와 간이 신장 측정계를 사용하여 숙련된 간호사 인이 가벼운 겉옷 차림을 한 피험자들의 키 체중 을 측정하였다 대조군은 학력 흡연 여부 정신질환의 과거력 가족력 배제 질환 병력에 대한 설문지를 자가 작성하였다 환자군의 경우 명 이상의 정신건강의학과 의사가 환자와의 면담과 의무기록을 참조하여 학력 흡연 여부 당뇨와 고혈압 유병 여부 배제 질환 병력에 대한 자료를 수집하였고 임상적 증상의 심각도에 대해서는 양성 및 음성 증후군 척도 이하 와 단순 정신과적 평가 척도 이하 전반적인 기능 평가 척도 이하 평가를 시행하였다 환자군이 투약 중인 약물과 공복 혈당 총콜레스테롤 수치는 의무기록을 통해 수집하였다 투약 중인 약물에 따른 비만도의 차이가 있는지 보기 위해 비정형항정신병약물 중에서 체중 증가가 큰 것으로 알려진 투약군과 그렇지 않은 군으로 분류하였다 항정신병약물의 용량은 등가 용량으로 환산하였다 통계분석통계분석에는 이하 을 사용하였다 나이 성별 교육 기간 등의 인구사회학적 특성을 비교하기 위해 연속변수에 대해서는 범주형 변수에 대해서는 를 시행하였다 집단 간 체중 분포 비만이 아닌 사람과 비만인 사람의 비율 비교에는 각각 와 를 시행하였고 나이 교육 기간 등을 보정하기 위해 이하 와 을 사용하였다 이 임상적 특성과 비만 여부가 정신병리의 중증도를 예측할 수 있는지 보기 위해 사용되었다 환자군에서 비만 여부에 따라 각종 척도의 점수 차이가 있는지 보기 위해 를 사용하였다 모든 분석은 양측성 확률로 시행했으며 유의수준 은 로 하였다 결과 임상적특성환자군과 정상 대조군 사이의 인구학적 및 임상적 특성은 표 과 같다 연구에 참여한 환자들은 총 명 평균 나이가 ± 세 남 ± 여 ± 이며 남자가 명 173

3 J Korean Neuropsychiatr Assoc 2015;54(2): 여자가 명 으로 교육받은 기간이 년 남 여 처음 조현병을 진단 받은 나이는 평균 세 남 여 유병 기간은 평균 년 남 여 이었다 환자군 중 현재 흡연자는 명 남 명 여 명 과거 흡연자는 명 남 명 여 명 평생 비흡연자는 명 남 명 여 명 이었다 대조군은 총 명으로 평균 나이가 ± 세 남 ± 여 ± 이며 남자가 명 여자가 명 으로 교육받은 기간은 년 남 여 이었다 대조군 중 현재 흡연자는 명 남 명 여 명 과거 흡연자는 명 남 명 여 명 평생 비흡연자는 명 남 명 여 명 이었다 정상 대조군과 환자군 사이에 성별의 비율은 차이가 없었으나 환자군이 연령과 현재 흡연자 비율이 더 높았고 교육 연한은 더 짧았다 각각 < < 대조군에서는 연령 성별 흡연 여부 교육 기간 모두 비만과 연관성이 나오지 않았다 를 통해 나이와 교육 기간을 보정하여 환자군과 대조군의 를 비교한 결과 환자군의 평균은 ± 남 ± 여 ± 대조군의 평균은 ± 남 ± 여 ± 으로 환자군의 가 더 높았다 비만인 비율도 환자군에서는 명 남 여 으로 대조군 명 남 여 에 비해 높았다 대조군에서는 여자가 남자보다 평균이 낮은 것으로 나왔으며 환자군 내에서는 성별에 따른 평균 차이는 없었다 표 환자군의 혈중 총콜레스테롤 농도는 ± 남 ± 여 ± 였고 공복 혈당은 ± 남 ± 여 ± 였다 혈중 콜레스테롤치의 경우 여자가 남자보다 높은 것으로 나왔다 환자군 중 당뇨를 진단 받아 치료 중인 사람은 명 남 여 고혈압 약물 복용 중인 사람은 명 남 여 이었다 표 환자군 내에서 비만인 군과 그렇지 않은 군의 특성을 비교했을 때 비만인 군이 처음 조현병을 진단 받은 시기가 그렇지 않은 군에 비해 더 빨랐고 비만 ± 비만이 아닌 군 ± 그 외 다른 특성들은 차이가 없었다 표 Table 1. 5 J Korean Neuropsychiatr Assoc 2015;54(2): 자로 나누어 각각 집단 내에서 비만 여부에 따라 정신병리의 심각도가 달라지는지 살펴본 결과 남자에서는 양성 척도에서는 의심 피해감 음성 척도에서 감정적 위축 빈약한 신뢰감 수동적 무감동적 사회적 위축 추상적 사고의 장애 가 비만인 군이 더 낮았다 일반병리항목에서는 신체적 관심 죄책감 은 비만인 군이 더 높았고 긴장 몰두행동 능동적인 사회적 회피 는 비만인 군이 더 낮았다 음성 증상 척도 총점은 비만인 군이 더 낮았고 는 비만인 군이 더 높았으며 총점과 총점은 비만인 군이 더 낮았다 그림 여자에서는 비만 여부에 따라 유의한 차이가 발견된 항목이 없었다 결과 조현병을 진단 받은 나이와 비만 여부가 총점 변이의 를 예측할 수 있었다 비만과 β 조현병을 처음 진단 받은 나이가 β 총점에 음 의 영향을 주는 것으로 나타났다 또 마찬가지로 조현병을 진단 받은 나이와 비만 여부가 변이의 를 설명할 수 있었으며 비만과 β 조현병을 처음 진단 받은 나이가 β 에 양 의 영향을 주는 것으로 나타났다 또 비만과 성별이 음성 증상의 변이의 를 설명할 수 있었고 여자인 경우 β 비만인 경우가 β 음성 증상에 음 의 영향을 미치는 것으로 나타났다 표 환자군 모두 - - 등의 비정형항정신병약물을 한 가지 이상 투약하고 있었으며 등의 정형항정신병약물 중 한 종류나 비정형항정신병약물을 추가로 병용하고 있는 경우가 명 이었다 표 - 투약 여부에 따른 비만율의 차이는 없었다 표 비정형항정신병약물 중 체중 증가가 없거나 적은 것으로 알려진 만 단독으로 투약한 경우는 명에 불과해 통계적 차이를 보기는 어려웠다 비만인 군과 비만이 아닌 군이 투여한 항정신병약물의 용량 - 등가 용량 도 차이가 없었다 표 고찰 본 연구 결과 환자군의 평균 가 정상 대조군보다 높은 것으로 나타났다 이는 이전 연구들 과 일치하는 결과로 비만 유병률도 정상 대조군의 보다 높은 로 나타났다 이는 년 건강검진통계연보 에 수록된 대한민국 120 Obese 100 Not obese P6 N2 N3 N4 N5 G1 G3 G4 G15 G16 Negative PANSS GAF BPRS scale Fig. 2. 7 J Korean Neuropsychiatr Assoc 2015;54(2): 성인 비만율 와 비교해도 높은 수치이며 아시아인들을 대상으로 한 이전 연구들 보다 높은 수치이다 본 연구에서 비만인 경우 총점 음성 증상 점수가 비만이 아닌 환자들보다 낮은 경향을 보였고 는 높은 경향을 보였다 개별 척도 항목별로 살펴 보면 비만인 군이 그렇지 않은 군보다 의심 피해감 수동적 무감동적 사회적 위축 이 낮게 나온 결과는 - 등 과 일치한다 비만일수록 정신병리가 심하지 않은 것으로 나온 결과는 이전 연구 결과 에서도 나타나듯이 항정신병약물에 대한 반응이 체중 증가와 연관되어 있을 가능성으로 설명할 수 있다 을 투약한 경우에서 체중의 증가가 정신병리가 더 크게 호전되는 것과 연관이 있다는 연구들 이 있으며 유사한 연구 결과가 에 대한 연구에서도 보고되었다 등 이 개의 개별 연구들을 검토한 논문에서도 이러한 경향성은 유지되고 있다 이에 대한 정확한 기전은 아직 밝혀져 있지 않으나 여러 가설들이 제시되어 있다 조현병 환자에서 비만과 연관된 것으로 알려진 과 의 농도가 비정상적인 것이 발견되는데 이는 등의 약물 치료를 받은 환자들뿐만 아니라 치료 받지 않은 환자들에서도 발견되는 것으로 보고되었다 등 은 항정신병약물이 수용체에 작용할 뿐만 아니라 를 통해서도 작용을 한다는 모델을 제시했다 또 식욕을 조절하는 주요한 호르몬들인 이 조현병 환자에서 비정상적으로 조절되는 것이 체중 증가의 요인이 되며 항정신병약물 투약시 이들 호르몬의 농도가 변화한다는 연구 결과들이 있다 본 연구에서는 항정신병약물 용량이 비만과의 연관성이 없는 것으로 나타났으며 이는 이전 연구들과 일치하는 결과이다 항정신병약물의 종류도 비만도 차이에 영향을 주지 않는 것으로 나왔는데 이는 등의 비정형항정신병약물이 체중 증가가 더 크다는 이전 연구결과 와 다른 결과이다 장기 치료에서는 단기 치료와 달리 약물의 종류에 따른 체중 증가의 차이가 없다는 연구 결과 도 있어 치료 기간 차이의 영향이 있을 것으로 생각된다 본 연구의 환자군에서 남녀별로 각각 비만 여부에 따른 정신병리의 심각도를 살펴보았을 때 남자에서는 비만인 군이 전반적으로 그렇지 않은 군보다 심각도가 낮은 것으로 나왔으나 여자에서는 차이가 없었다 항정신병약물이 성별에 따라 약물역동학적 차이가 있어 그에 따른 체중 증가 등 부작용이 남녀 간 차이가 난다는 연구 들이 있으나 여성에서 항정신병약물에 의한 체중 증가의 크기에 대해서는 서로 상반되는 결과를 보고한 연구들이 있어 이에 대해서는 좀 더 연구 가 필요할 것으로 생각된다 또 본 연구에서 환자군을 남녀별로 나누었을 때 표본 수가 각각 명 명으로 각각의 집단 내에서 비만과 비만이 아닌 경우를 비교할 때 표본 수가 적은 것을 감안하여 결과를 해석해야 할 것으로 생각된다 본 연구의 제한점을 살펴보면 첫째 본 연구에서 비만을 측정하는 방법으로 만을 사용한 점이다 대사증후군은 높은 뿐만 아니라 증가된 혈당 혈중 지질 농도 등과도 연관이 되는데 본 연구 환자군의 평균 혈당은 정상 범위였고 케톤혈증 당뇨콩팥병증과 같은 고혈당으로 인한 합병증은 관찰되지 않았다 또 총콜레스테롤 수치의 경우도 임상적으로 의미 있는 고지혈증 범위에 속한 환자는 명뿐이어서 본 연구에서 혈당 혈중 지질 농도가 의미 있는 영향을 주지 않는 것으로 생각된다 두 번째 본 연구에서는 정상대조군 환자군 모두에서 식이나 신체 활동량에 대한 조사를 하지 않았다 또 횡단면적 연구 설계로 인해 항정신병약물 치료와 체중 증가 사이의 인과 관계를 명확히 볼 수 없는 한계가 있다 다만 연구 참여 환자들 중 명 이 개월 이상 입원 치료를 받고 있었으며 비만이 아닌 군이 오히려 음성 증상이 더 심한 것으로 나온 결과로 미루어 볼 때 활동량의 차이는 환자군의 체중 분포에 큰 영향을 주지 않았을 가능성이 있다 세 번째 본 연구 결과가 이전의 연구들과 유사하지만 본 연구에서는 입원 환자군만 대상으로 하고 외래 환자군이 포함되지 않아 일반화에는 한계가 있다 반면에 재원 일수가 일인 환자 명을 제외한 나머지는 일 이상 입원한 환자들로 충분한 기간 항정신병약물 투약 기간을 가졌으며 외래 환자군 대상 연구의 약점인 약물 순응도 문제를 배제할 수 있는 장점이 있겠다 결론 본 연구는 조현병 환자군과 정상 대조군 사이의 비만도 차이와 함께 비만도가 환자의 정신병리적 특성 및 중증도와 연관되어 있는지를 알아보고자 하였다 그 결과 조현병 환자군에서 정상 대조군보다 더 높은 와 비만율을 보였으며 조현병 환자군 내에서는 비만인 경우 정신병리가 덜한 것으로 나왔다 개별 정신병리 항목에서는 환각행동 의심 피해감 수동적 무감동적 사회적 위축 이 낮게 나왔다 약물의 종류나 용량은 비만 여부와 연관성이 없는 것으로 나타났다 환자군 내에서 와 정신병리의 심각도가 음의 상관 관계를 가졌는데 그 이유에 대해서는 여러 가설들이 있으나 추가적 연구가 필요할 것으로 생각된다 또 이러한 경향이 항정신병약물의 영향인지 아니면 약물과 무관한 환자의 특성인지에 대해서도 연구가 필요할 것으로 보인다 이전에도 국내 178
