In this study, the genetic variability and epidemiological relationships of clinical and environmental isolates of C. neoformans from Busan, Korea, 2000~2005 were investigated. A total of 12 strains of C. neoformans, 7 clinical and 5 environmental isolates were analyzed by random amplified polymorphic DNA (RAPD) using three different primers and PCR-fingerprinting with a minisatellite-specific core sequence of phage M13. All strains belonged to C. neoformans serotype A and mating type MATa. Two different RAPD profiles (I and II) and a single pattern by M13 PCRfingerprinting were identified. The major RAPD profile was pattern I (8 of 12 strains) and pattern II was identified from 2 clinical and 2 environmental strains, which clearly distinguished among isolates. Clinical strains with pattern II were isolated from the patients with HIV positive. Taken together, molecular patterns provide a good characterization of strains of C. neoformans as a heterogeneous group and epidemiological relationships in clinical and environmental strains. Key Words: Cryptocococcus neoformans, Random amplified polymorphic DNA, PCR-fingerprinting 서론 Cryptococcus neoformans는협막을보유한 basidiomycetes 효모양진균으로사람과동물의폐, 중추신경계및전신에침범하여생명을위협하는 cryptococcosis를일으킨다 (1). 최근연구에의하면 C. neoformans는다당체협막성분의항원성과생태학적, 분자생물학적특성차이에의하여 C. neoformans ( 혈청형 A, D 및 AD) 와 C. gattii ( 혈청형 B와 C) 의두종 (species) 으로재분류되었으며, C. neoformans는두변이종 var. grubii ( 혈청형 A) 와 var. neoformans ( 혈청형 D) 그리고 hybrid 형 AD로나누어진다 (2, 3). AIDS 환자를비롯하여면역기능에장애가있는환자에서주로기회감염의원인균이며, 인체감염의대부분은 C. neoformans var. grubii ( 혈청형 A) 이다. 이에비교하여 C. gattii ( 혈청형 B 와 C) 는열대와아열대지역에서만제한적으로분리되며, 자연서식처는오스트레일리아원산 Eucalyptus 나무로알려져있으며 (6~8), 일부열대지역을제외하고 AIDS 환자에서 C. gattii에의한감염증은거의없는것으로보고되고있다 (9, 10). Cryptococcus species의인체감염은자연환경에서 basidiospore를흡입함으로써감염되는것으로알려져있으나, 생태학적, 역학적관련성은여전히의문으로남아있다 (11). C. neoformans는 heterothallic basidiomycetes로두종류의 mating type MATa와 MATa가존재한다. 일반적으로환경이나임상검체에서분리되는 C. neoformans mating type은 MATa가대부분이며, 드물게 MATa/a 인 diploid 균주도분리된다 (1, 12). 91

2 92 SM Hwang 전세계적으로 C. neoformans에관한분자역학적인연구방법으로 M13 primer 및 (GACA) 4 반복서열에의한 PCR fingerprint 분석법 (13, 14), amplified fragment length polymorphisms (15), rdna intergenic space regions I과 II 분석 (16), multilocus gene seqeunce typing (MLST) 등이 (17, 18) 이용되고있으며, 이들분석법에의하여 8가지의주요유전자형이알려져있다. 우리나라에서는 Cryptococcus 균종의분리율이저조하며, 이균종에관한몇몇의학적인보고만있을뿐, 생태적특성을비롯하여역학연구는거의없는실정이다 (19, 20). 본연구에서는 2000년에서 2005년사이에부산지역임상검체와환경에서분리된환경균주, 특히비둘기분변에서분리된 C. neoformans 균주를이용하여, 이들의혈청형, mating type, random amplified polymorphic DNA (RAPD) 와 M13 PCR fingerprint 법에의한분자생물학적특성을분석하여비교함으로써지역에서분리된 C. neoformans의생태학적, 역학적기초자료를얻고자하였다. 재료및방법균주의분리및동정부산지역종합병원 4곳에서 2000년에서 2005년사이분리된임상균주 7주와 2002년비둘기분변에서분리된환경균주 5주를포함한총 12균주를사용하였다 (Table 1). 임상검체에서분리된 7주는각병원미생물검사실에서 C. neoformans로동정된균주로써, 묵즙염색에의한협막관찰, API 20C AUX system (biomerieux, Marcy-l'Etoile, France) 을이용한당동화시험과 Christensen's urea broth (Difco, Detroit, MI, USA) 를이용한 urease 시험양성, nitrate 환원시험음성, 37 성장능양성인지를재확인하였고, bird seed media (BSM) 에서 phenol oxidase 생성에의한갈색집락을확인하여동정하였다 (19). 환경균주는 Oh 등 (20) 에의하여 2002년부산시내비둘기서식처 12곳의비둘기분변으로부터분리된 C. neoformans 균주를사용하였으며, 균주분리방법을간단히소개하면다음과같다. 매우건조된비둘기둥지에서얻은약 2.0 g의비둘기분변에 10 ml의멸균식염수를가하고, 용액이균질해질수있도록 20분간진탕한후 1500 g에서 5분간원침하였다. 상층액 100 μl를균일하게 BSM에접종하고, 30 에서 2~10일간배양한후에, 균주의 phenol oxidase 활성의결과로나타나는멜라닌색소생성, 즉갈색집락만을취하여 Sabouraud dextrose agar (SDA) 로옮긴다음다시재배양한후에임상분리균주와동일한방법에의하여최종동정하였다. 혈청형과변종확인시험 C. neoformans 변종의확인은협막성다당체항혈청을이용한혈청형분석과두가지생화학적반응시험을실 Table 1. 3 Molecular Characterization of C.neoformans 93 시하여결정하였다. 혈청형의감별은협막성다당체항원을확인하는 slide agglutination test (Crypto Check Iatron RM 304-K kit; Iatron Laboratories, Tokyo, Japan) 를이용하였다. 분리균주들은 30 에서 yeast extract malt extract agar (YMA, Difco) 에배양하였고, 48시간후에 McFarland scale pattern 2 ( 약 CFU/ml) 에맞춰멸균식염수에부유하였다. 각항혈청 5종 (F1, F5, F6, F7, F8) 한방울씩을 agglutination glass slide에먼저떨어뜨리고, 각항혈청당 50 μl의 C. neoformans 부유액을떨어뜨려혼합하였다. 혼합액이균질화될수있도록 2분간교반하면서슬라이드에서작은응집이생기는것을직접관찰하여다음과같이판정하였다.: 혈청형 A는 F1과 F7에응집, 혈청형 B 는 F1과 F5에응집, 혈청형 C는 F1과 F6에응집, 혈청형 D는 F1과 F8에응집그리고혈청형 AD는 F1, F7, F8 에응집을확인하여결정하였다. 변종확인생화학적검사법으로, 첫번째시험은 canavanine-glycine-bromthymol blue (CGB) 배지에서 glycine 을탄소원으로이용할수있는 glycine decarboxylase 의생성유무에성장능을관찰하는것으로, 최종동정된균주들을 CGB 배지에접종하고 30 에서 24~48시간배양한다음, CGB 배지의색깔이원래배지의색깔인녹황색에서청색으로색깔변화를보이면양성으로판정하였다 (21). 두번째시험은 Kwon 등 (22) 의 EDTA 를이용한 urease 억제시험법으로, 최종동정된균주들을 yeast extractglucose-peptone agar (YEPG) 에 100 μm EDTA 를첨가한배지인 YEPGE에접종하고 30 에서 48시간배양한후, 자란집락을멸균증류수 2 ml에 spectrophotometer (Jasco Co., Ishiawa-cho, Japan) 를이용하여흡광도 (A 600 ) 0.8~1.0 이되도록맞춘다음 ( 균주수, 약 ~ / ml), 진탕혼합한뒤혼합액 1 ml를취해 rapid urea broth (RUH broth, Difco) 를 2배되도록분주한뒤 ice bath에서적당시간방치하였다. 다시이 RUH broth를 37 shaking water bath에넣고매시간마다자홍색 (magenta red color) 이나타나면음성, 그리고황색 (yellow color) 이나타나면양성으로판정하였다. 위두가지시험모두에양성이면 C. gattii ( 혈청형 B 또는 C) 이며, 모두음성이면 C. neoformans var. grubii 또는 var. neoformans ( 혈청형 A, D 또는 AD hybrid) 로변종을확인하였다. DNA 분리및 mating typing 균주의 genomic DNA는 Yamamoto (23) 방법을일부변형하여분리하였다. Brain heart infusion broth (BHI) 에서각실험균주를 30, 48시간배양한후원심분리하여약 100 μl pellet를수집하여 10 mm TE buffer (ph 8.0, 1 mm EDTA) 로두번세척하였다. 각검체에 250 μl 100 mm TE (ph 9.0, 40 mm EDTA), 50 μl 10% sodium dodecyl sulfate 와 200 μl benzyl chloride를가하여혼합한후 50 에서 30분동안가볍게진탕하면서방치한다음, rpm에서 10분간원심분리한후상층액을분리하였다. 상층액에 3 M sodium acetate 50 μl를가한후 0 에서 10분간방치하였다. DNA 침전물은 250 μl isopropanol를가하여 -70 냉동고에서 1시간방치하여얻었으며, 침전된 DNA를 70% ethanol로세척하여건조한후 40 μl TE로용해하여사용하였다. Mating type은 Chaturvedi 법 (12) 에따라 C. neoformans 혈청형 A의특이적 primer, MATa (5'-CTTCACTGCC- ATCTTCACCA-3' 과 5'-GACACAAAGGGTCATGCCA-3'), MATa (5'-CGCCTTCACTGCTACCTTCT-3' 과 5'-AACGC- AAGAGTAAGTCGGGC-3') 를이용하여 PCR법을실시하였다. PCR 반응혼합액은 AccuPower PCR premix kit (Bioneer Co. Ltd, Daejeon, Korea) 를사용하여, DNA 시료 (75 ng) 1 μl, primer (10 pmol) 를각각 1 μl 넣고증류수로총 20 μl로최종부피를맞추었다. PCR 반응조건은 95 에서 3분간초기반응, 94 1분, 분, 72 1분의과정으로 30회반복하였고, 마지막으로 72 에서 7분연장으로 PCR을종결하였다. 증폭된 DNA 시료는 2% agarose gel을통하여전기영동을실시하였고, 증폭산물의크기에따라 mating type, MATa (101 bp) 와 MATa (117 bp) 를확인하였다. RAPD 및 PCR-fingerprinting RAPD 분석은 3종의 primer, OPH-02 (5'-TCGGACGTGA -3'), OPH-12 (5'-ACGCGCATGT-3'), OPH-20 (5'-GGGAG- ACATC-3') 를사용하여실시하였다. PCR 반응혼합액은 AccuPower PCR premix kit를사용하여, DNA 시료 (75 ng) 1 μl, 한종류의 primer (10 pmol) 를 1 μl 넣고증류수로총 20 μl로최종부피를맞추었다. PCR 반응조건은 94 에서 4분간초기반응, 92 30초, 34 1분, 72 1분의과정으로 35회반복하였고, 마지막으로 72 에서 5분연장으

Minisatellite 특이 M13 primer (5'-GAGGGTG- GCGGTTCT-3') 를이용한 PCR fingerprint 분석은 Meyer 법 (14) 을응용하여실시하였다. PCR 반응혼합액은 AccuPower PCR premix kit를사용하여, DNA 시료 (25 ng) 1 μl, primer (10 pmol) 를 1 μl 넣고증류수로총 20 μl로최종부피를맞추었다. PCR 반응조건은 94 에서 5분간초기반응, 93 20초, 50 1분, 72 20초의과정으로 40회 반복하였고, 마지막으로 72 에서 5분연장으로 PCR을종결하였다. 증폭된 DNA 시료는 1.5% agarose gel에서전기영동을실시하여, 증폭산물의크기를비교확인하였다. 혈청형및 mating type 결과 Primer OPH-20 I 1 a A Strain No. C21 C22 C28 C36 C40 C23 C26 C27 II 2 b B C41 C58 C24 C25 부산지역 4곳의종합병원에서 2000년에서 2005년동안분리된임상균주 7주와 2002년비둘기분변에서분리된환경균주 5주를포함한총 12균주의 C. neoformans의협막다당체항원성분은모두혈청형 A이었으며, 변종확인생화학적검사결과에서 CGB 배지에서음성, EDTA 억제 urease 시험음성으로 C. neoformans var. grubii ( 혈청형 A) 로확인되었다. C. neoformans 혈청형 A 균주의 mating type을 MATa와 MATa 두종류의특이적 primer를이용하여 PCR법을실시한결과모두 101 bp가검출되어 MATa임을확인하였다 (Fig. 1). RAPD 및 PCR-fingerprinting Random primer 3종류를이용하여 RAPD 분석결과는 Figure 2와같다. OPH-02 primer를이용한결과에서 (Fig. 2-A) 임상균주 2주 (C41와 C58) 및환경균주 2주 (C24 와 C25) 는 700 bp에해당하는 band에서나머지 8균주와차이가있음을확인하여 profile 1과 2로구분되었으며, OPH-12 primer에서는 850 bp, OPH-20 primer에서는 280 bp band에서각각 profile a와 b, A와 B로구분되어져, 균주간의현저한차이를나타내었다. 3종류 primer에의한 RAPD profile을분석한결과, pattern I과 II의두종류로분류되었으며, 임상균주 7주중에서 5주, 환경균주 5주중 3주는 pattern I이었고, 임상균주 2주와환경균주 2주는 pattern II를나타내었다 (Table 2). Minisatellite M13에의한 PCR-fingerprinting 결과에서 12균주모두동일한양상을나타내어, 균주간의차이점이없음을확인하였다 (Fig. 3). 고찰 Cryptococcus 감염증은주위자연환경에서이균종의 basidiospore를흡입함으로써주로감염되는것으로알려져있으며, 특히 AIDS 환자를비롯하여면역기능이현저히떨어진사람에게서기회감염을일으킴으로사회적으로문제가되고있다. C. neoformans와그변종들로인해야기되는임상질환과지역적인분포사이에는유의한상관관계가있는것으로보고되고있으며, 전세계적으로많은연구가진행되고있다. AIDS 환자에서 Cryptococcus 감염의윈인균은 C. neoformans var. grubii, 혈청형 A으로

6 96 SM Hwang 알려져있으나, 브라질에서는 C. neoformans var. grubii와 C. gattii 모두그원인균으로보고되었다 (24). 우리나라의경우에 1986년 Kim 등 (25) 이한국의임상에서분리된 10균주모두가 C. neoformans 혈청형 A로보고되었으며, Hwang 등 (26) 은 1993년에서 2005년사이분리된임상균주 51주와환경균주 7주의혈청형을분석한결과혈청형 A형이 55주, B형 2주, D형 1주이었으며, 환경균주는모두 A형으로보고하였다. 일본의경우에 Ikeda 등 (27) 은임상에서분리된 62주모두가 C. neoformans 혈청형 A임을보고하였다. 이와같이 C. neoformans 혈청형 A는세계각지에서가장많이분리되고있는반면에, 혈청형 B의경우에는대만, 베트남, 호주, 브라질그리고아프리카중부등의열대또는아열대지방에서분리율이높다고알려져있고, 혈청형 D 균주는유럽에서다른지역에비해더많이분리되는것으로알려져있다 (4, 6). 본연구에서분리된 12균주는모두 C. neoformans var. grubii 혈청형 A로혈청형 A 균주의높은분리율을확인하였다. C. neoformans의 mating type은이균종의생태학적, 병원성관련연구에중요하다. 임상균주나환경균주에서 MATa type은 MATa type 보다더많이존재하며, mating 에관계없이적절한환경에서균사를만들거나눈으로볼수있는 basidiospore를형성하는것으로알려져있다. Kwon-Chung 등 (28) 은 MATa type 균주가 MATa type보다병원성이더강한것으로보고하였다. Ohkusu 등 (24) 은임상균주 75주의 mating type 분석결과 73주는 MATa이었고 2주는 type이결정되지않았음을보고하였다. 본실험에서임상균주와환경균주모두 MATa type을나타냄으로써 MATa Cryptococcus 균종이우세함을재확인하였다. Cryptococcus 균종의분자유전학적, 역학적분석방법으로여러가지분석법이이용되고있는데, 이중에서 RAPD, M13 primer 및 (GACA) 4 반복서열에의한 PCRfingerprint법등이널리이용되고있다. 특정 primer를이용한 RAPD fingerprint법은 C. neoformans의역학적연구에보다쉽게응용할수있는방법으로감염경로등을추정하는데도움이되고있다 (29, 30). 본실험에서사용된 Cryptococcus 12주의 M13 primer에의한분석결과는모두동일한 PCR fingerprint pattern를나타냄으로서 C. neoformans var. grubii ( 혈청형 A) 의유전자형의차이는없었으나, 3종류의 primer 에의한 RAPD 분석결과에서는두종류의 pattern I과 II을나타냄으로서, 균주간의차이 가있음을확인하였다. Pattern I을나타낸균주는총 8주로임상균주 (5주) 와환경균주 (3주) 에서우세하였으며, 나머지임상균주 (2주) 와환경균주 (2주) 는 pattern II를나타내었다. 부산지역에국한되었고검체균주수가적었지만, 임상균주와환경균주사이에상관성이있음을확인할수있었다. 특히 pattern II를나타낸임상균주는모두 HIV 양성환자에서분리된균주로서흥미로운결과를얻었다. Yamamoto 등 (23) 은나가사키지역에서분리된총 21주의 Cryptococcus를이용하여 RAPD 분석을한결과 4종류의 pattern의특성을보고한바있다. 균종의유전적변이는숙주의면역반응에중요한역할을하며, 유전자기능변화는병원성에중요한영향을미치게된다. 또한미생물의진화, 군집의구조변화를인식하는데변이균주의유전적분석은필수적이라하겠다. Cryptococcus 균종은자연환경인조류의분변이나썩은나무등에서식하면서인체와많은접촉을할기회가많으며, 노령인구의증가, 면역기능저하환자의증가등에인한기회감염균으로중요시하고있는진균이다. 그러므로전세계적으로 Cryptococcus 균종에관련하여다양한연구가이루어지고있는반면에, 우리나라에서는이균종에관하여분자역학적인연구가거의없는실정이다. 이분야의연구가저조한이유로서는여러가지요인이있을것으로사려되나, 의진균성감염에관심과임상에서진균의분리율이저조한까닭이아닐까생각한다. 부산지역임상과환경검체에서분리된 Cryptococcus 종은모두혈청형 A형인 C. neoformans var. grubii이었으며, M13 primer에의한유전자형도동일하였다. 그러나 RAPD 분석에의한유전자형은두 pattern으로구분되었으며, 임상균주와환경균주모두에서분리됨을확인하였다. 비록지역과균주수가제한적이었지만, 비둘기분변에서식하는 Cryptococcus 환경균주는환자에서분리된균주와무관하지않으며, 역학적상관성이있는것으로추정된다. 그러므로우리나라전지역에서분리되는 C. neoformans 변이균주의생태적특성, 병원성및분자역학분야에지속적인연구가활발히이루어져, 글로벌균주와의상관성을규명해야될것으로사려된다. 참고문헌 1) Matsumoto MT, Fusco-Almeida AM, Baeza LC, Melhem Mde S, Medes-Giannini MJ.
