A 58-year-old male was admitted to hospital due to multiple pulmonary nodules. In addition, the patient presented with multiple arthritis, and dyspnea on exertion. Rheumatoid arthritis had been diagnosed 35 years ago. The patient had worked as a stonemason for 20 years. Computed Tomography (CT) revealed numerous well-defined tiny nodules scattered in both lungs, which was suspicious of miliary tuberculosis or malignancy. The patient was started on antituberculous medications and referred to our hospital. First, a transbronchial lung biopsy was performed, which showed no evidence of granuloma. It was our opinion that the biopsy was insufficient, and a follow-up video-associated thoracoscopy was performed. The pathological report determined necrotizing granulomatous inflammation and silicosis on background. According to imaging studies, pathologic reports, and clinical symptoms, we concluded that the patient had Caplan's syndrome. We controlled his rheumatic medications, and instructed him to avoid exposure to hazardous dust. Caplan's syndrome은진폐증 (pneumoconiosis) 과함께혈청류마토이드인자 (rheumatoid factor) 가양성인류마티스관절염환자에서진단될수있으며, 결핵및악성종양은배제되어야한다. 1959년 Caplan 2 은이와같은증후군이탄광광부들이외에도석면이나규소등다양한진폐증환자에서도나타날수있음을보고하였다. 아직까지 Caplan's syndrome 의명확한병인은밝혀지지않았으나, 일부동물실험에서규소를흡입시켰을때면역글로불린의생산을촉진했다는연구결과도있다 3. Caplan 1 이최초보고하였을때 14,000 명의진폐증환자중류마티스관절염이있는경우는 0.4% 였고, 이들중 90% 가종괴성섬유화를보여 Caplan's syndrome 으로진단되었으며, 다른연구결과들에서도발생빈도는높지않은증후군으로국내에서도단 2개의증례만보고되었다. 본저자들은다발성폐결절을주소로내원한 58세남자 150

40년간하루에 2갑정도의흡연력이있었으며 2010년 2월에끊었다고했다. 술은소주를한주에 2병정도로마셨다고했다. 현병력 : 환자는 5년전부터간간히호흡곤란증상이있었으나특별한검사를하지않고지내던중 20여일전부터증상이심해져지역병원에내원하여시행한흉부단순촬영에서, 다발성폐결절이발견되었고, 속립성폐결핵또는폐암이의심되어응급실을통해본원으로전원되었다.

촉진으로만져지는종괴는없었으나, 양측슬관절, 수지및족관절, 무릎관절 에부종을동반한압통소견을보였다. 검사실소견 : 입원당시의말초혈액검사상혈색소 13.1 g/dl, 적혈구용적치 40.3%, 백혈구수 15,070/μL, 혈소판수 500,000/μL, 적혈구침강속도 98 mm/hr, 요검사소견은정상이었다. 간기능, 신기능및심전도소견은모두정상범위내였다.

No. 2, Feb 환자의증례를통하여, 류마티스관절염을동반한다발성폐결절환자에서드물지만원인질환의하나로 Caplan's syndrome도고려해야함을환기하려고한다. 증례환자 : 58세, 남자주소 : 호흡곤란, 관절통및다발성폐결절직업력및과거력 : 환자는 1978 년부터 1998 년까지 20년간석재가공업에종사하였으며, 이후로는어부로생활하는중이다. 35년전지역병원에서류마티스관절염진단을받고 1년간약물치료를하였으나, 이후증상이호전되어중단하고경과관찰하던중 2년전부터관절염증상이다시나타나지역병원에서약물및주사치료를간간히받았었다. 40년간하루에 2갑정도의흡연력이있었으며 2010년 2월에끊었다고했다. 술은소주를한주에 2병정도로마셨다고했다. 현병력 : 환자는 5년전부터간간히호흡곤란증상이있었으나특별한검사를하지않고지내던중 20여일전부터증상이심해져지역병원에내원하여시행한흉부단순촬영에서, 다발성폐결절이발견되었고, 속립성폐결핵또는폐암이의심되어응급실을통해본원으로전원되었다. 문진 : 20일전부터운동시호흡곤란및양쪽어깨, 팔꿈치, 손목, 손가락, 무릎등에다발성관절통증세를호소하였다. 이학적소견 : 환자의응급실내원시혈압은 137/92 mm Hg, 맥박은 110회 / 분, 호흡수 18회 / 분, 체온 36.5 o C 였다. 의식은명료하였으며, 전신상태는만성병색을보였다. 청진결과호흡음은양측폐하엽에서다소거칠게들렸으며, 심잡음은들리지않았다. 촉진으로만져지는종괴는없었으나, 양측슬관절, 수지및족관절, 무릎관절 에부종을동반한압통소견을보였다. 검사실소견 : 입원당시의말초혈액검사상혈색소 13.1 g/dl, 적혈구용적치 40.3%, 백혈구수 15,070/μL, 혈소판수 500,000/μL, 적혈구침강속도 98 mm/hr, 요검사소견은정상이었다. 간기능, 신기능및심전도소견은모두정상범위내였다. 류마티스관절염에관한혈청면역화학검사에서, 류마티스인자는 IU/mL, anti-cyclic citrullinated peptide antibody >100 U/mL, C- 반응단백질 157 mg/l로크게증가해있었고, anti-nuclear antibody, Immunofluorescence assay (ANA IFA) 는음성이었다. protoplasmic-staining antineutrophil cytoplasmic antibodies (P-ANCA) 는양성, classical antineutrophil cytoplasmic antibodies (C-ANCA) 는음성이었다. 임상경과 : 환자는외부병원의단순흉부촬영에서속립성폐결핵과, 폐암이의심되는상태로, isoniazid, rifampin, ethambutol, pyrazinamide 를포함한항결핵화학요법을시작하고본원에내원하였다. 본원에서시행한결핵균특이항원자극인터페론검사는음성이었고, 기관지내시경세척액에서시행한항상균도말및배양검사는음성이었다.

3 SK Lee et al: Caplan's syndrome presenting as multiple pulmonary nodules 로정상범위였다. 동맥혈가스검사는 ph, 7.425; PaO 2, 75.1 mm Hg; PaCO 2, 32.0 mm Hg; HCO-3, 21.2 meq/l 였다. 방사선학적소견 : 단순흉부 X-선촬영상전폐야에걸쳐작은결절들이관찰되었다 (Figure 1). 흉부전산화단층촬영에서도전폐야에작은결절들과양쪽유문부와종격동에임파선종대가관찰되었고 (Figure 2), 외부병원에서시행한양전자방출단층촬영결과에서폐실질과, 흉막양측그리고양쪽유문부와종격동림프절에 SUV Figure 3. (standard uptake value) 가증가된소견이관찰되었다 (Figure 3). 폐병변의조직학적진단을위하여기관지내시경과경기관지폐생검을시행하였다. 기관지내시경검사소견 : 우중엽으로외부에서압박하는소견을보였으며오른쪽중축중기관지 (RB5) 는좁아져있었으나, 기관지내병변은보이지않았다. 좌하엽에서경기관지폐생검을시행하였다. 임상경과 : 환자는외부병원의단순흉부촬영에서속립성폐결핵과, 폐암이의심되는상태로, isoniazid, rifampin, ethambutol, pyrazinamide 를포함한항결핵화학요법을시작하고본원에내원하였다. 본원에서시행한결핵균특이항원자극인터페론검사는음성이었고, 기관지내시경세척액에서시행한항상균도말및배양검사는음성이었다. 경기관지폐생검을통한조직검사결과국소적림프구의침윤과폐포벽의증대를포함한만성염증소견외에특이적진단을얻지못하였으며, 조직에서시행한항산균염색검사도음성이었다. 경기관지폐생검을통한조직검사결과가충분하지못하여, 추가로비디오내시경을이용한생검술 (Video assisted thoracoscopic surgery) 을통한폐조직검사를시행하였다. 폐조직검사결과규소입자가배경에있으며, 괴사성육아종을동반한염증 (necrotizing granulomatous inflammation) 과규소입자를탐식한대식세포가보였고 (Figure 4), 혈관염의증거는없었으며추가로시행한항산균염색및, 결핵균특이 PCR 검사는음성이었다. 류마티스관절염의과거력및환자의임상증상등을종합하여, Caplan's syndrome 으로진단하였다. 환자는류마티스관절염에대하여약물을 Figure 4. No. 2, Feb 조절하였고, 호흡기계증상에대해선대증요법시행후임상증상이호전되어퇴원하였다. 현재환자는호흡기내과와류마티스내과외래에서추적관찰중이다. 고찰폐결절은경계가좋거나나쁜직경 3 cm 이하의원형또는불규칙한국소음영으로, 다발성폐결절이보일수있는질환으로는사르코이드증, 규폐증 / 탄광부진폐증, 활석증, 폐전이, 폐암, 림프종, 림프세포증식질환, 기질화폐렴, 베게너육아종증, Churg-Strauss 증후군, 아밀로이드증, 속립성폐결핵, 폐감염, 원형무기폐와드물게 Caplan's syndrome이있다. 1953년영국흉곽외과학회에서 Caplan's syndrome 이발표된이후 1, 이를입증하는여러연구와사례가보고되었다. Caplan 1 은이질환이사진에서진행성종괴성섬유화 (Progressive massive fibrosis) 와유사해보이지만, 둥글고, 모양이규칙적이며, 균일한밀도를가지는결절성음영이 cm 크기로, 폐주변부에주로위치하고, 짧은기간동안나타나공동을잘형성하는점들이다르다고하였다. 또한 Caplan's syndrome 은결절수가다발성인경우가많다고하였다 년에 Caplan 4 은류마티스관절염환자에서, 석탄과규소의비직업적인노출이폐에서의류마토이드결절을일으킬수있다고하였다. 1955년에 Gough 등 5 은이증후군의조직병리학적변화를기술하였는데, 결절에는다양한정도의분진침착이동반되며중심부에는괴사성교원질이있고, 주변부로는대식세포와임파구를포함하는염증과정이존재한다고보고하였다. 또한진폐증결절, 규폐성결핵증, 진행성종괴성섬유화등이공존할수있으며구별은쉽지않다고하였다. 초기에 Caplan 은이증후군을탄광부진폐증환자들에서주로기술하였지만, 1959 년발표에서규폐증및석면폐증환자에서도발생할수있으며 2, Zielonka 등 6 은본환자에서처럼주물업근로자에게서규소에노출된후 Caplan's syndrome 이발생한것을기술하였다. Caplan's syndrome은두가지형태로분류될수있는데, 고전적인 (caplan) 형태와규폐증형태가있다. 고전적형태는경미한진폐증으로나타나고, 균일한괴사조직을가지고있는큰결절들로특징지어진다. 이와달리, 규폐증형태는괴사조직안에규소입자들을포함하고있는작은결절들이특징이다 7. 본환자의경우는규폐증형태라고볼수있다. Caplan's syndrome 의병리생태학적기전 은명확히밝혀지진않았다. 일부연구결과에따르면 Caplan's syndrome 에서 HLA-DR4 항원이대조군보다더높게발견되었고 (61% vs. 21%) 8, Hahon 등 9 이발표한내용에따르면 C3, IgA, IgG가 Caplan's syndrome 환자에서증가한다고하였는데, 본환자에서도 C3는 mg/dl 로증가소견을보였다. 류마티스인자는 Caplan's syndrome 환자의약 70% 에서양성소견이관찰되었다는보고가있었고 10, 본증례에서도양성소견을보였다. Angiotensin convertase 와 b-glucuronidase 가증가한다는보고도있었는데, 이는규소분진에의해변형된폐포내피세포와, 자극된대식세포가지속적으로활성을가지기때문이라고하였다 11. 또한동물실험에서흡인된규소분진은류마토이드인자와유사한면역글로불린의생산을촉진한다는보고도있었다 3. Allison 등 12 은대식세포에탐식된규소입자에의해, 대식세포의기능과항원성이변형된다고하였다. 대식세포에탐식되었을때, 규소입자는세포막을공격하고활성산소와, 리소좀의효소를방출하여독성효과를발휘하게된다. 동물모델에서규소입자는단핵세포와대식세포를자극하여, 결과적으로인터루킨-1 (interleukin), 과립구-대식세포군락촉진인자 (granulocyte-macrophage colony-stimulating factor), 종양괴사인자 (Tumor necrosis factor-a), 혈소판유래성장인자 (Platelet-derived growth factor) 등의방출을촉진한다는보고도있다 13. 이로인해폐실질이파괴되고폐의비가역적인섬유화가일어난다. 일부국외문헌에따르면진폐증환자의 0.4 4% 에서 Caplan's syndrome 이발생한다고하나, 국내에서는규폐증환자에서발생한 Caplan's syndrome 이보고된예는단한개밖에없었다. Caplan's syndrome 은현재까지류마티스관절염에대한것이주치료로알려져있고, 진폐증에대해선입증된치료는없지만심각한호흡기계증상을일으키는경우는드물다고알려져있다 14. Honma 와 Vallyathan 15 은류마토이드결절이없는진폐증환자에서보다, Caplan's syndrome 의경우결핵발생률이증가한다는것을보고하기도하였다. Caplan's syndrome은매우드문질환이며, 규소분진에일어나는사례는더욱더드물다. 더욱이국내에선현재탄광이나석재가공업등의직업은상당부분감소추세에있으며, 직업력과동반한 Caplan's syndrome 은증례를찾아보기가힘들다. 하지만다발성폐결절이있고폐암등과의감별이꼭필요할때본환자처럼 Caplan's syndrome 의가능성도있으므로, 영상검사및조직검사등과함께환자의기저질환과과거 153
