This viewpoint is similar to the new mental health paradigm initiated by the Ministry for Health & Welfare Affairs. Despite the existence of numerous screening tools for depressive disorders, the research group concluded that there is an urgent need to develop a new, valid depressive disorders screening tool that concisely and reliably measures key sub-domains of depressive disorders suicidal risks, mania, anxiety, and somatic symptoms. As a preliminary step to pursuing this goal, we first conducted a systematic review of existing screening tools for depressive disorders, thoroughly analyzed the strengths and limitations of each tool, and summarized the theoretical foundations for the development of the new Korean screening tool for depressive disorders (K-DEP). Second, the research group introduced procedures for developing preliminary item pools for the K-DEP (a total of 383 items), administered it to 153 non-psychiatric patients, and summarized the results. Finally, we discussed the directions for the next steps in developing the K-DEP. Keywords: major depressive disorder, screening tool, development of scale, item response theory, evidence-based assessment 주요우울장애는우울한기분혹은일상생활에대한흥미저하를 주요증상으로하는정신질환으로, 최소 2 주간지속되는정동, 인 지, 생장기능의변화가개인의기능수행능력에영향을줄때진단 내릴수있다 (American Psychiatric Association [APA], 2013). 우 울증은조기에발견된다면약물및심리치료의효과가높지만치 료개입이없을경우대개 6 개월이상지속되며, 우울증발생후 1 년 이지난시점에서도주요우울장애의진단기준을충족할환자의비 율이 40% 이상을기록한다 (Keller, 1994). 또한암이나심혈관계질 Correspondence to Kee-Hong Choi, Department of Psychology, Korea University, 145 Anam-ro, Seongbuk-gu, Seoul, Korea; kchoi1@korea. ac.kr Received Jul 1, 2017; Revised Nov 4, 2017; Accepted Nov 4, 2017 This study was supported by a grant of the Korea Mental Health Technology R&D Project, Ministry for Health & Welfare Affairs, Korea (HM15C1169). 환의발생및경과에서우울증은중요한심리적요인이며 (Musselman, Evans, & Nemeroff, 1998; Suls & Bunde, 2005) 급증하는자살률의주요원인중상당한비율을차지하는등 (Cho et al., 2009), 우울증은재발이잦고만성화되기쉬우며, 신체적질환으로이환될가능성이높고, 개인의생명유지에도부정적인영향을미치는정신질환으로보고되고있다. 세계보건기구 (World Health Organization) 에따르면전세계적으로약 3억 5천만명이우울증을앓고있으며, 17개국가를대상으로실시한정신건강조사결과, 20명중 1명이우울삽화를경험한다고한다 (World Health Organization [WHO], 2012). 2030년에는우울증이세계질병부담률 1위를기록할것으로 WHO는예측하고있다 (WHO, 2008) 년에실시한국내의건강실태조사에따르면, 기분장애의평생유병률은 7.5% 이며, 해당년도추정된기분장애환자수는약 130만명에육박한다고보고되고있으며주요우울장애의경우 2001년, 2006년조사결과와비교하였을때평생유병률이 4.0%, 5.6%, 6.7% 로꾸준히증가하는양상을보이고있다 642

2 Development of the K-DEP (Seoul National University College of Medicine & Ministry for Health & Welfare Affairs, 2011). 그러나이에비해우울증환자의상담및치료경험은적은것으로나타나는데, 국내우울증환자중 18.2% 만이상담또는치료경험을보고하였다 (National Health Insurance Service, 2014). 또한우울증은일반의료장면에서가장흔히나타나는정신건강이슈 (Kroenke, Spitzer, Williams, Monahan, & Lo we, 2007) 임에도불구하고, 일차진료기관에방문한우울증환자중 35 50% 는적절한진단을받지못하는것이현실이다 (Schurman, Kramer, & Mitchell, 1985; Simon & VonKorff, 1995; Von Korff, Shapiro et al., 1987; Whooley et al., 1997). 임상현장에서정확한진단, 효과적인치료, 추적이가능한체계를통해우울증환자를판별하고적절한치료기회를제공하기위한첫단계로일차의료장면에서부터의우울증선별이권장되고있다 (Pignone et al., 2002). 더불어지역사회의정신건강증진센터등일상에근접한다양한장면에서우울증환자에대한조기진단및선별을효과적으로시행한다면, 잠재적우울환자의치료서비스접근성이높아져우울장애로인한막대한사회-경제적비용과의학적위험성을예방할수있을것으로기대된다. Louise, Russel과 Milbank Memorial Fund(1994) 는효과적인선별도구는높은정확도로환자를감별해낼수있어야하고, 비용효율적이며, 단시간안에전문성이부족한사람이실시하더라도적용할수있어야한다고제언하였다. 우울증의정서, 인지, 신체적특성은그양상이개인마다다르므로선별평가도구의경우다양한영역에서나타나는증상을얼마나광범위하게측정할수있는지의여부가중요하다. 기존의우울증평가도구가주요우울장애의핵심증상을측정하지만, 그진단기준을모두포함하면서진단적타당도가확보된경우는소수에그친다 (Jung et al., 2017). 실제로주요우울장애환자가 DSM-5의주요우울장애의핵심진단기준 9개를모두충족하는경우는 3% 에불과하고, 대부분은진단기준의다양한세부조합으로분포되어있다 (Park et al., 2017). 주요우울장애의진단이관련증상의조합으로이루어진다는점을고려하면, 우울증환자의집단내이질성을극복하기위해서주요우울장애의핵심진단기준을전체적으로평가할수있는선별도구가필요한실정이다. 일반적으로평가도구를개발하고표준화하는과정에서타당도와신뢰도의입증은검사실시후수집한데이터를분석하여얻은준거타당도, 내적신뢰도등의지표로검증되는, 사후검증적접근법을취한다. 이에더하여검사개발과정자체를타당도의준거로보아야한다는움직임이측정이론학계에서논의되었고, 문항을개발하는과정에서부터타당하고표준화된절차를거쳤는지에대한근거를바탕으로하는절차검증적접근법에대한필요성이대 두되었다. 근거중심의평가도구설계체계 (evidence centered assessment design, ECD) 는절차의타당성을확인하기위한준거로활용되는프레임이다. ECD는평가도구의개발과정을측정영역의분석 (domain analysis), 측정영역의구조화 (domain modeling), 평가도구의개념적구조화 (conceptual assessment framework), 평가도구실시및적용 (assessment implementation), 평가도구의배급 (assessment delivery) 이라는 5개의단계가유기적으로되먹이 (feedback) 하며이루어지는과정으로정의한다 (Mislevy & Riconscente, 2005). 다양한주제의측정도구개발에적용할수있다는점이 ECD의장점이며, 해외교육학관련측정도구개발과정에서활발하게사용되고있고심리측정개발분야에서도 ECD의활용도가높을것으로기대되는반면, 국내우울증평가도구의개발장면에서는적용한선례가없다. 이에본연구진은한국형우울장애선별도구 (Korean Screening Tool for Depressive disorders, K-DEP) 개발의필요성을인지하고, 개발과정에근거중심의평가도구설계체계를적용하고자한다. 우선측정영역의분석단계에서국내에서주로사용되는우울증평가도구에대한체계적인리뷰를실시하였다 (Jung et al., 2017). 측정영역의구조화단계에서는 Toulmin 다이어그램 (Toulmin, 1958) 의적용으로우울장애환자이지만 K-DEP 에서낮은점수를기록하게끔하는가외변인을고려한결과조증삽화와불안장애의공병여부를판단할수있는문항이선별과정에서부터포함되는것이합당하다고판단하였다 (Figure 1). 그결과 K-DEP 는 DSM-5 의주요우울장애주요진단기준을포괄하며, 우울장애환자군이자주호소하는불안및신체증상관련측정영역을보충하고, 양극성장애와의감별을보다용이하게하기위한조증선별문항을제시하고자한다. 또한 K-DEP 의선별도구로서의정확성을제고하기위해측정이론중하나인문항반응이론 (item response theory, IRT) 을통한문항의특성을반영하고자한다. 검사개발과정에서문항반응이론을활용할경우, 수검자의우울증수준에따라서각각의문항에어떻게반응을할것인지를추정함으로써문항단위의난이도및변별도프로파일을구할수있으며, 이러한문항단위정보는특정한난이도를목표로하여검사를재구성하거나소검사를구축하는데활용될수있다. 국외에서는교육분야의검사에서 IRT 기법이활발하게활용되고있다. 정신건강관련검사에는우울, 불안, 스트레스등의정신질환평가를컴퓨터적응평가 (computerized adaptive testing, CAT) 를활용하여개발한사례가있는데, 이들검사의검사-재검사신뢰도가기존검사와비슷하거나더높았다는연구결과가보고되고있다 (Devine et al., 2016). 643

K-DEP개발과정에대한이해를돕기위해 3년계획플로우를 Figure 2에정리하였다. 그중 1년차에진행된예비연구를살펴보면다음과같다. 첫째, 국내우울증및조증평가도구에대한선행연구를살펴보고, 이들의타당도와관련된증거를수집및정리하였다 (Jung et al., 2017). 둘째, 선행연구분석과더불어연구진의내부전문가회의, 외부전문가회의, 주요우울장애환자들을대상으로진행한포커스그룹인터뷰를바탕으로주요우울장애및기분장애의각영역을대표하는문항을도출하였다. 셋째, 최종적으로선정한 383개의예비문항과기존우울증및불안평가도구로예비검사를실시하였으며, 그결과를통계적으로분석하였다. 이후 2년차에는문항반응이론을적용하여문제은행 (item bank) 을구축할예정이다. 문제은행의검증을위한검사는이중맹검법으로진행되며전문가진단면접인 M.I.N.I. 와함께자기보고식우울장애평가도구인벡우울검사 2판 (Beck Depression Inventory-II, BDI-II), 역학연구센터우울척도 (Center For Epidemiological Studies-Depression Scale, CES-D), 그리고우울증선별도구 (Patient Health Questionnaire-9, PHQ-9) 를실시할예정이다. 이를바탕으로한국형우울장애선별도구의최종문항을선정하여 3년차에는 K-DEP 을표준화하고타당도와신뢰도를보고할예정이며, K-DEP의 CAT을위한알고리즘을구축하는것이한국형우울장애선별도구개발의마지막단계이다. 방법한국형우울장애선별도구예비문항개발과정예비문항수집 기존우울증및조증척도수집, 분석및번안국내외임상및연구장면에서자주사용되는우울증및조증평가도구, 그리고자살관련평가도구를선정및수집하였다. 우울장애진단면접도구로는 Hamilton Depression Rating Scale (HDRS), Montgomery-Åsberg Depression Rating Scale (MADRS) 를선정하였으며, 자기보고식우울증평가도구로는 Beck Depression Inventory (BDI), BDI-II, Major Depression Inventory (MDI), MMPI Depression Scale (MMPI-D), Quick Inventory of Depressive Symptomatology-Self Report (QIDS-SR), Zung Self-Rating Depression Scale (SDS), CES-D, PHQ-9, Geriatric Depression Scale (GDS), Edinburgh Postnatal Depression Scale (EPDS), Korean Depression Scale (KDS), Masculine Depression Scale (MDS) 를선정하였다. 우울장애와공병률이높은불안장애증상을측정하는문항을포함하기위해자기보고식우울및불안평가도구인 Hospital Anxiety and Depression Scale (HAD), New Anxiety-Depression Scale (NADS), Depression and Somatic Symptoms Scale (DSSS), Mood and Anxiety Symptom Questionnaire (MASQ) 를수집하였 644

Developmental process of K-DEP. 다. 또한우울장애와양극성장애를감별하기위해조증증상을측정하는자기보고식기분장애증상평가도구인 Bipolar Spectrum Diagnostic Scale (BSDS), Hypomania/mania symptom checklist (HCL-32), Mood Disorder Questionnaire (MDQ) 을포함시켰으며, 우울장애환자의높은자살위험성을변별하기위해자기보고식자살사고및행동평가도구인 Reasons for Living Inventory (RFL), Suicide Behaviors Questionnaire-Revised (SBQ-R), Beck Scale for Suicide Ideation (BSI), Columbia-Suicide Severity Rating Scale (C-SSRS) 을수집하였다. 수집된각척도의측정영역, 측정기간, 문항기술형태, 역문항여부, 앵커링형태, 타당도, 민감도, 특이도에대하여문헌을고찰하였고, 이를통해한국형우울장애선별도구의문항개발을위한기초작업을시작하였다 (Jung et al., 2017). 고찰의대상이된준거척도중국내번안혹은타당화가진행되지않은 MDI와 QIDS-SR의경우영어로된원문항을본연구진이번역하여문항내용에초점을맞추어참조하였으며, 나머지준거척도는기존에국내연구진에의해번안혹은표준화과정을거쳐논문에기재한국문문항을활용하였다. 이렇게최종선정된 하위차원을기준으로한국형우울장애선별도구와기존우울증및조증평가도구의측정영역을비교한결과 (Table 1), 한국형우울장애선별도구만이주요우울장애핵심증상기준과불안장애증상, 그리고조증증상까지감별에필요한구성개념을아울러측정할수있으리라기대되었다. 포커스그룹인터뷰를통한추가문항수집기존평가도구에서상대적으로문항수량이부족했던사회, 직업, 그외삶의중요한영역에서의기능손상영역 (DSM-5의주요우울장애진단기준 B) 과화병 ( 火病 ) 영역을아울러전반적인우울증상에대한서술문을수집하여문항을보충하기위해포커스그룹인터뷰를진행하였다. 그첫단계로주진행자인임상심리전문가 1 인과보조진행자인임상및상담심리전공박사 1명, 석사 1명이소비자심리학대학원박사 1인에게포커스그룹인터뷰진행에대한사전교육을받았다. 인터뷰그룹은일산에위치한 B 대학병원에서정신건강의학과전문의에의해 M.I.N.I. 를토대로하여주요우울장애로진단된환자 7명을대상으로하였다. 집단의연령은

RDoC의차원중기존주요우울장애와가장연관이높은 negative valence system의 Loss 구성개념에서명시한행동및자기보고영역에서 K-DEP 에활용할수있는구성개념이있는지확인하였다. 이전단계에서수집된모든문항을 DSM-5 와 RDoC 기준으로각각어느영역에해당하는지코딩한결과, RDoC 에존재하였고우울장애에중요한구성개념으로판단된 무 하위차원구성예비 K-DEP 척도의하위차원은 2단계로구성과정을거쳤다. 첫번째로, K-DEP 가주요우울장애의선별을일차적목표로하기때문에, DSM-5의진단기준을측정영역으로하여 K-DEP 의 9개하위차원으로구성하였다. 두번째로, 우울증의측정을필요로하는연구현장에서 K-DEP 의활용성을높이기위해미국국립정신건강 망감 영역을연구진논의결과 K-DEP 의하위차원으로포함시켰다. 마지막으로 DSM-5에서명시되지않았으나, 다수의기존우울증평가도구에서측정의대상으로하였던 신체증상관련, 불안및공포관련 영역을하위차원으로포함하였으며, 기존기분장애평가도구를참조하여조증선별을위한문항을추가하고 조증증상 영역으로명명하였다. 646

( 지난 2 주간 ) 내장래는희망적이지않다고생각했다. 최종적으로한국형우울장애선별도구의하위차원은 우울기분, 흥미저하, 체중및식욕변화, 불면혹은과다수면, 정신운동초조혹은지연, 피로및활력상실, 무가치감혹은죄책감, 집중력감소, 자살사고및시도, 사회 / 직업 / 일상기능저하, 신체증상관련, 불안및공포관련, 조증증상, 무망감 의총 14개하위차원으로구성되었다. 예비문항의선정상기과정을거쳐총 14개의영역을기준으로 383개예비문항이최종선정되었다. 영역별로분류된문항의수는우울기분 = 44개, 흥미저하 = 39개, 체중및식욕변화 =19개, 불면혹은과다수면 = 26개, 정신운동초조혹은지연 = 21개, 피로및활력상실 = 27개, 무가치감혹은죄책감 = 51개, 집중력감소 =17개, 자살사고및시도 = 46개, 사회 / 직업 / 일상기능저하 =19개, 신체증상관련 =19개, 불안및공포관련 = 21개, 조증증상 = 31개, 무망감 = 3개이다. 분류된문항은연구진의토의를거쳐한국형우울장애선별도구개발이라는목적에맞게수정및제작되었다. 선정된예비문항의예시는 Table 2에서확인할수있다. 문항의난이도설정및문항속성결정최종선정된예비문항을대상으로문항의이론적난이도를설정하여측정영역별로다양한수준으로예비문항을구성하고자하였다. 심리측정전문가 1명, 임상심리전문가 3명, 정신과전문의 1명이논의하여난이도측정기준을우울증이심각한환자가응답할것같은문항은 상, 그보다덜심각한환자가응답할것같은문항은 중, 우울증이경미한수준의환자라면응답할것같은문항을 하 로합의하고측정하였다. 전문가집단이예비문항의난이도를사전에설정하는이유는, 예를들어 우울기분 영역은난이도가 647

The committee recommends to set the main goal of the K-DEP as the primary screening tool, which gives latent depression patients a chance to get proper psychiatric services. Time/Cost efficiency: The K-DEP needs to be short and low cost(or free) enough to simply rate the depression in the primary medical setting or local community setting for screening. It would be critical to retain sufficient sample size to be survived as a useful screening tool in Korean clinical and public health settings within the first several years from the distribution of K-DEP. It is needed to consider both the universal criteria to capture diagnostic depression and unique properties of Korean depression. The item-driven scoring pattern will give us the information that the Korean depression looks like. Domains: To check whether the examinee has had any manic episode, it would be recommended to add items rating manic symptoms. Anchoring: The united anchoring system would be recommended, as the people with depression tends to be under the diminished ability to concentrate. Item numbers: Questions of K-DEP driven by the item response theory will assess the depression with the minimum number of items. Sensitivity/Specificity: The K-DEP is needed to be improve the diagnostic efficiency better than existing screening tools. The K-DEP will be expected as the successful replacement of the existing depression rating scales, which have a royalty issue, as the K-DEP will be a free test for public health settings. Based on the bi-factor model, the compulsory items, which function as a linking items between onlineoffline test, will be included in both on-line and off-line version of K-DEP. Note. The table was cited from A systematic review of validation studies on depression rating scales in Korea, with a focus on diagnostic validity information: Preliminary study for development of Korean Screening Tool for Depressive Disorders by Jung, Kim, Park, Jaekal, Lee, Choi, Choi, & Lee, Anxiety and Mood, 13(2). 높은문항만구성이되고 정신운동초조혹은지연 영역은난이도가낮은문항만구성이되는등, 측정영역간의난이도균형이맞지않는상태에서수검자에게예비검사를실시하는경우를방지하기위함이다. 최종문항의난이도는검사이후수검자의문항응답패턴을통해난이도를추정하는사후추정방법을원칙으로하지만, 사전에예비문항구성단계에서도전문가집단의사전난이도추정과정을더하여검사개발과정전반의타당도를확보하고자하였다. 최종적으로분류된난이도별문항수는다음과같다 ; 상 =117개, 중 =189개, 하 = 77개. 예비문항의기술형태, 시제, 측정기간및앵커링등의문항속성은앞서리뷰한기존의자기보고식우울증평가도구를참조하여전문가집단의논의끝에다음과같이결정하였다. 문항의기술형태는평서문이고시제는과거형으로결정하였으며, 측정기간은 DSM-5 의주요우울장애진단기준에따라지난 2주간을제시하였다. 각문항의앵커링은목표하는내용을측정하기에적절하다고판단되는유형 ( 강도, 빈도, 혹은예 / 아니오 ) 으로임상심리전문가 3 인, 심리측정전문가 1인, 그리고정신과전문의 1인의논의를통해부여되었다. 강도앵커링 = 전혀그렇지않다-그렇지않다-중간정도이다-그렇다 -매우그렇다 로, 빈도앵커링 = 결코그렇지않다- 가끔그렇다 -때때로그렇다 -자주그렇다 -항상그렇다, 이분유형 = 예-아니오 로기술하였다. 예를들어 우울기분 영역에서난이도가가장높은문항중 ( 지난 2주간 ) 비참하고울고싶었다 는강도앵커링으로, ( 지난 2주간 ) 이유없이눈물이줄줄났다 는빈도앵커링으로, 그리고 ( 지난 2주간 ) 나는많이울었다 는예 / 아니오의앵커링으로묻는등, 비슷한내용의문항을다양한앵커링으로제시하였다. K-DEP 의최종문항에적합한앵커링은예비검사와이후개발과정에서진행될문제은행구축과정에서앵커링에따른수검자의응답패턴을바탕으로선정될예정이다. 전문가견해수집임상심리전문가 3명, 정신과전문의 1명, 심리측정전문가 1명으로구성된본연구진은외부자문단 ( 정신과전문의 4명, 임상심리전문가 2명, 심리측정전문가 2명 ) 을초청하여외부전문가자문회의를개최하였다. 한국형우울장애선별도구개발의의미및지향점, 문항의기간단서, 앵커링, 수량, 채점방식, 추가해야할측정영역, 온라인 / 오프라인검사의호환성등의최종평가도구를위한구체적결정사항을중점으로토의가이루어졌으며, 자세한내용은 Table 3에정리하였다. 외부전문가자문단회의에서쟁점으로논의된 648

8 Development of the K-DEP 사항은 한국형 평가도구의정의와지향점이다. 기존의 한국형 평가도구라고하면증상에대한한국인고유의사회문화적의미를구성개념화하여이를측정하고자하는데중점을두었는데, 이과정에서자칫 한국형 증상에지나치게집중하면결국진단의기준이되는 DSM-5 나 ICD-10 등의진단체계와괴리가생겨주요우울장애의선별이라는목적에부합하지않을것이라는의견이제시되었다. 이에 한국형 의의미는번역체로서술된문항이주를이루는기존의번안타당화된자기보고식우울증평가도구와달리자연스러운한국어표현으로기술된문항을뜻하며, 또한문항에반응한한국인의고유한응답특성을바탕으로구축할한국형채점체계 (scoring system) 로정의하는것이합당하다고의견을모았다. 국문맞춤법확인및검사지제작표준화된예비검사를제작하기위해, 본연구진이최종제작한예비문항을대상으로한국어맞춤법및문법검사를실시하였다. 부산대학교와 나라인포테크가개발한한국어맞춤법및문법검사프로그램을사용하였으며, 예비문항의검사가종료된후최종선정된본검사문항은국립국어원에검토를의뢰할예정이다. 예비검사지의문항은모든문항을앵커링 ( 강도, 빈도, 예 / 아니오 ) 을기준으로분류한뒤문항에난수를부여하여무선정렬하였다. 강도와빈도앵커링의경우 5점리커트척도로측정하도록제시하였다. 그리고문항의제시순서효과를배제하기위한조치를취하였는데, 이는문항의제시순서에따라설문에대한주의집중과성실성이달라져결과적으로응답의신뢰성을떨어뜨릴수있는가능성을통제하기위함이다. 결과적으로본연구진은두종류의설문지 ( 유형A: 강도- 빈도- 예 / 아니오앵커링문항의순서로제시, 유형 B: 빈도-강도-예 / 아니오앵커링문항의순서로제시 ) 를제작하여수검자에게무선으로제시하였다. 한국형우울장애선별도구의예비검사방법연구대상본연구를위해모집된대상은총 159명의일반인으로, 2016년 5월부터 7월까지아르바이트구인사이트를통해 20대에서 60대에걸쳐모집된인원중연구참여에동의한사람들을대상으로실시하였다. 검사실시에는한시간에서한시간반정도소요되었으며, 검사에참여한수검자에게는소정의사례비를지급하였다. 검사과정에서정신과질환관련병력을묻는설문지를통해일반인을선별하였고, 이후분석과정에서정신과치료및상담을받고있거나과거병력이있는 5명과 K-DEP 의예비문항을실시하지않은 1명을 제외한 153명을최종연구대상으로하였다. 본연구는고려대학교기관윤리심의위원회의승인을받아진행하였다연구도구및방법모집된일반인을대상으로한국형우울장애선별도구 (K-DEP) 와함께두개의추가적인평가도구를직접작성하도록하였다. 두개의검사는우울증평가도구인 BDI-II와불안평가도구인 Beck Anxiety Inventory (BAI) 로구성하였다. BAI는 1988년 Beck, Epstein, Brown과 Steer가개발한자기보고식불안평가도구로, 지난일주일간수검자가경험할만한불안관련증상 ( 인지, 정서, 신체적영역 ) 에대해서 21개의문항을 0-3점의리커트척도로측정하고있다. 국내에서는 Yook과 Kim(1997) 에의해표준화된바있다. BDI- II와 BAI는한국심리주식회사에서출판된한글판 K-BDI-II와 K- BAI 검사지를사용하였다. 통계분석본연구에서는 K-DEP 의다른변수를통한관련성에따른타당도증거 (evidence based on relationship with other variables) 를수집하기위해서예비검사연구집단을대상으로 BDI-II, BAI 점수와의상관을분석하고 K-DEP 의하위차원과 BDI-II와 BDI-II의하위요인인신체-정서요인과인지요인간의상관분석을실시하였다. 다음으로고전검사이론의신뢰도관점에서의신뢰도를알아보기위해서내적일치도를계산하기위한 Cronbach s α값을구하였다. K-DEP 의채점은다음과같이이루어졌다. 5점리커트척도인강도와빈도문항의경우 1 5점으로지정하였으며, 이분문항의경우예는 1, 아니오는 0점으로지정하여채점하였다. 한편 K-DEP 의총점은조증측정문항을제외한나머지 352개문항의점수를합산한것으로하였다. 이는조증선별문항과나머지우울및불안관련영역의질적차이를반영하기위함이다. 통계분석에는 Statistical package for social science (SPSS) 20.0 for Windows 가사용되었으며, 각분석에서의유의수준은.05로적용하였다. 결과연구대상자의일반적특성전체연구대상자는정신과병력이없다고응답한일반인 153명중남자는 63명 (41.2%), 여자는 90명 (58.8%) 이다. 수검자의평균나이는 (SD =12.99) 세이며평균교육연한은 (SD = 2.50) 년이다 (Table 4). 인구통계학적특성과 K-DEP 의총점에영향을미치는지확인하였는데, 독립표본 t검정분석결과성별과총점은통계적으로 649

9 Jung, et al. 유의하지않아 (p<.05) K-DEP 의총점은성별에영향을받지않는것 으로확인된다. 나이와학력에따라총점의차이가있는지상관분석 을통해확인한결과, 나이의경우 의근소한부적상관 (p<.05) 이나타났다. 학력의경우유의미한상관이나타나지않았다. 기술통계결과 K-DEP 에포함된 14 개영역을강도와빈도, 예 / 아니오앵커로구분 하여각각의최소값, 최대값, 평균, 표준편차, 왜도를분석하였다. K-DEP 의문항전체점수의평균은 1.496점 (SD = 0.545) 이었다. 그러나 K-DEP 에서조증을선별하는문항은그외우울관련증상영역과질적으로다르며, 조증여부의측정목적이우울장애와의감별을위해서포함된것이므로, 이는따로채점하는것이합리적이라고판단되어이후타당도및신뢰도, 상관검증에서 K-DEP 의총점은 조증증상 영역을제외한 352개문항을합산하였다. 조증증상 영역을제외한 K-DEP의평균은 1.591점 (SD = 0.582) 이다. 한편수검자의 BDI-II 평균점수는 점 (SD = 8.45) 으로, 정상범위인 13점이하에속한다. 이는예비검사문항에대한응답이대부분증상이없거나경미하다고응답한것과맥락상일치한다. 10 Development of the K-DEP 다른변수를통한관련성에따른타당도증거 K-DEP 의다른변수를통한관련성에따른타당도증거를수집하기 위해서 조증증상 영역을제외한 K-DEP 의문항총점과 BDI-II, BAI 의총점간의상관을구하고자하였다. 우선두검사결과의정규 성을검증하기위해왜도를분석한결과, K-DEP 의우울관련문항 총점과 BDI-II, BAI 총점모두 수준의왜도를보고하였다. 이 에각총점에대해제곱근을계산하여최종적으로는 수준의 왜도로조정한총점으로상관을구하였다. 그결과 K-DEP 는 BDI-II 와.803 (p<.005), BAI 와.771 (p<.005) 의유의한상관을보였고, BDI-II 와 BAI 는.759 (p<.005) 로유의미한상관을보였다 (Table 6). 다음으로조증문항을제외한 K-DEP 의예비문항을 DSM-5 의 주요우울장애진단준거를참조하여각각 14 개의하위차원으로 분류한뒤, 각영역에해당하는문항들의총점을 BDI-II 와 BDI-II 의 2 요인 ( 신체 - 정서, 인지요인 ) 총점과비교하여상관분석을실시 하였다 (Table 7). BDI-II의하부요인구조는여러연구에서다양하게제시되어왔으나, 신체-정서요인 (somatic-affective factor) 과인지요인 (cognitive factor) 의 2요인구조가주요하다 (Beck, Steer, Ball, & Ranieri, 1996; Steer, Ball, & Ranieri, 1999). K-DEP 의모든영역과 BDI-II의 2개요인은유의미한상관을보였으며, 모든영역에서유의미한정적상관을기록하였다. BDI-II 2요인과의상관크기를세부적으로살펴보면, BDI-II의신체-정서요인과 K-DEP 의 체중혹은식욕의증감, 불면이나과다수면, 피로나활력의상실, 신체증상관련영역 과상관이 BDI-II 인지요인과해당영역과의상관보다더높았다. 한편 BDI-II의인지요인은 K-DEP 의 우울기분, 흥미저하, 무가치감혹은죄책감, 자살사고및시도, 사회 / 직업 / 일상기능저하, 불안및공포관련, 조증증상, 무망감 영역과비교적높은상관을보였다. K-DEP 의 정신운동초조혹은지연, 집중력감소 영역은 BDI-II의두요인간에상관의차이가거의나타나지않았다. 예비문항의내적신뢰도검증 K-DEP 총점에대한하위차원의기여도를알아보기위하여 K-DEP 의하위차원-총점간상관을분석한결과, 조증선별문항을제외한나머지영역과총점의상관도가 r = 의범위를보여안정된하위차원-총점간상관을보고하였다. 가장높은상관을보인영역은일상활동에대한흥미저하, 우울기분영역이며, 조증선별문항에서는총점과.436 수준의유의한정적상관을보였다. 또한조증문항을제외한나머지문항의 Cronbach s α 계수는.998로보고되었다. 11 Jung, et al. 논의전세계적으로주요우울장애의유병률은점차증가하고있지만우울증환자들중치료를받는환자는절반이채되지않아고통경감과생산적생활로의복귀기회를놓치고있는실정이다. 우울증의발병과치료를연결하는것이바로우울증의적절한평가단계임을고려하였을때, 효과적인우울증선별도구를개발하는과정은의미가있다. 본연구진이개발하고자하는한국형우울장애선별도구 (K-DEP) 는기존우울증선별도구에비해조증및불안증상의문항을추가하는등측정영역을확장하여감별에대한정보를수집하고자한다. 방법론에있어서도기존의심리검사와는달리, 최신측정이론인문항반응이론 (IRT) 을활용하여최소한의문항으로도최적의진단적타당도를확보하고자한다. K-DEP 의예비연구는체계적인척도개발절차를밟고있다. 우선예비문항의구성을위해기존의우울증, 기분장애, 자살사고및행동, 우울-불안장애측정도구 25종의문항을수집, 분석및번안하였다. 더불어주요우울장애진단을받은환자를대상으로하여포커스그룹인터뷰를통해사회, 직업, 가족, 집안일기능유지영역과화병 ( 火病 ), 우울장애에대한전반적인내용에대한내용을분석하여추가로문항을추출하였다. 다음단계로연구진은 DSM-5의주요우울장애진단기준과조증및불안및걱정, 신체증상영역에따라하위차원을구성하여수집된예비문항을분류하였다. 더불어미국 NIMH에서제시한새로운연구영역기준인 RDoC의 Loss 영역의하위구성개념에따라분류하였다. 상기과정을거쳐총 14개의영역을기준으로 383개예비문항이최종선정되었고, 전문가 5인에의해문항의난이도가매겨졌다. 동시에 K-DEP 의개발에조언을구하기위해외부전문가로구성된자문단회의를개최하여구체적이고실용적인자문을얻었다. 최종적으로예비검사는 5점리커트의강도, 빈도앵커와예 / 아니오의앵커를기준으로무선정렬하여문항제시의순서효과를배제한검사지로제작되었다. K-DEP 의예비검사는기존에우울증선별에주로사용되는우울증평가도구 BDI-II와불안평가도구 BAI와함께일반인 153명을대상으로실시되었다. 그결과 K-DEP 는기존에우울증선별에주로사용되는우울증평가도구인 BDI-II와유의미한상관을보여준거타당도를확보하였다. 한편 K-DEP 와 BDI-II 모두불안장애평가도구인 BAI와유의미한상관을보고하였다. Clark 와 Watson(1991) 은혼재성우울-불안증을설명하기위한 3자구조모델 (tripartite model) 을제시하면서, 불안장애및우울장애는일반적고통감 (general distress) 을공유하되각각과각성 (hyperarousal) 증상과 무쾌감증 (anhedonia) 으로변별할수있다고보고하였다. 본연구에서진행한우울및불안장애평가도구의상관이높은것은부정적정서와 ( 우울기분, 걱정등 ) 그에따른일상의불편함 ( 수면곤란, 소화불량등 ) 으로나타나는일반적인고통감이두드러진것에기반한것으로판단되지만, 여전히 50% 정도의분산이서로를설명하고있지못하므로, 우울과불안은서로관련되어있으나, 독특한특성을지닌요인으로보는것이타당하다. K-DEP 의하위차원은 BDI-II의총점과각각유의미한상관을보였으며, BDI-II의하위 2요인과각각유의미한상관을보였다. 신체-정서요인과인지요인은각각연관되는측정영역과비교적높은상관을보였으며, 그세부내용을살펴보면다음과같다. 신체-정동요인은인지요인과비교하였을때 K-DEP의 체중및식욕변화, 불면혹은과다수면, 피로및활력상실, 신체증상관련 과비교적높은상관을보였다. 한편인지요인은신체-정동요인에비해 K-DEP 의 우울기분, 흥미저하, 무가치감혹은죄책감, 자살사고및시도, 사회 / 직업 / 일상기능저하, 불안및공포관련, 조증증상, 무망감 영역과비교적높은상관을보였다. 즉, BDI-II의신체-정서요인은 K-DEP 의측정영역중 피로및활력상실 과, BDI- II의인지요인은 K-DEP 의 무가치감혹은죄책감 과가장상관이높은것으로보아예비문항의영역이타당하게구성되어있음을확인할수있다. 뿐만아니라 K-DEP 는기존우울증선별도구가측정하지않던조증증상의여부, 불안, 신체증상관련, 무망감, 사회 / 직업 / 일상생활의기능손상을측정함으로써수검자의우울증선별에보다높은정보가를제공하는검사도구가될것으로기대된다. 또한예비문항개발과정에서예비문항의특성을중간점검하기위한목적으로실시한 K-DEP 의하위차원과 K-DEP 의총점과의상관분석결과, 모든하위차원이유의미한정적상관을보였다. K-DEP가주요우울장애추후개발될 K-DEP의최종버전에서는측정영역의신뢰도검증은물론, 검사-재검사신뢰도를보고하는것이합당할것이다. 본연구의한계점은다음과같다. 첫째, 추가문항수집을위해진행된포커스그룹인터뷰의대상이된주요우울장애환자의사례수가적어인터뷰내용을한국우울장애환자의전반적인특성으로일반화하기어렵다. 포커스그룹인터뷰를시행한목적은기존우울증평가도구에서수집한문항에서수량적으로적다고판단한영역인 사회 / 직업 / 일상기능저하 과 화병 이라는구성개념에대한서술문을수집하여보충하기위한것이다. 즉, 포커스그룹인터뷰를통한문항수집을부수적과정으로간주하여진행하였다. 기존평가도구에수록된상당수의문항이번역체로기술되어어색했는데, 한국환자의인터뷰에서추출된문항은구어체에가깝고표 652

12 Development of the K-DEP 현이자연스럽다는점에서문항수록의의미가있다. 둘째, 양극성장애의감별을위해 K-DEP 예비문항에조증선별문항을포함하였으나검사과정에서이들문항의타당도를입증할기존의자기보고식조증증상평가도구를실시하지않았다는것이다. K-DEP 의문제은행과최종온라인 / 지필검사문항의타당도검증이각각 2년차와 3년차에실시될예정으로, 조증증상문항들의타당도를검증하기위한준거척도로기존의조증증상평가도구를포함하여실시할필요성이있다. 최종문항의표준화단계에서는수검자의진단결과및기존조증증상평가도구의점수를비교하여 K-DEP 의조증선별문항의타당도를검증할것이다. 본연구는한국형우울장애선별도구를개발하는과정의 1년차성과이다. 추후검사개발과정은다음과같다. K-DEP 의예비문항을주요우울장애환자를대상으로도실시하고문항반응이론을기반으로변별도가높은문항을추려문제은행을구축한다. 그다음단계로 K-DEP 의문제은행을기존의자기보고식우울장애평가도구및진단면접과함께실시하여각후보문항의난이도와변별도등의문항특성프로파일을구할예정이다. K-DEP 문제은행검사의실시는과학적으로엄정한연구설계를통해임상적정확성을확보하고자하는데, 구체적으로는 K-DEP 문항의실시및채점자와진단면접을실시하는전문가를다르게설정하는이중맹검법을도입하여기존의우울증선별및평가도구보다더욱합리적이고유용한검사임을입증하는방향으로타당화하고자한다. 이러한과정을통해선발된 K-DEP 의최종문항은온라인검사와지필검사의형식으로제작되어표준화될예정이다. K-DEP는보건복지부정신건강기술개발사업단의지원을받아개발되는평가도구로, 추후지역사회, 정신의학과현장, 연구장면등에서비용부담없이무료로사용될예정이다. K-DEP 의비용효율성은최근외국척도에대한저작권료로인해국내대규모국가 R&D 과제수행시외국에저작권료를지불해야하는불편함에대한대안이될것으로기대된다. 또한 K-DEP 는개발과정전반의타당도를확보하기위해체계적인척도개발과정을밟고있다는점, 한국인들을대상으로모든우울장애구성개념에대해방대한양의예비문항을실시, 그에대한문항반응에기초하여최종문항을선정한점, 그리고문항반응이론에근거하여한국인의문항반응특성을채점에통합하는최신의측정방법을채택하였다는점에서기존우울장애평가도구와차별점을지니고있다. 16 Development of the K-DEP 국문초록한국형우울장애선별도구의개발 : 예비연구정수연 1 김신향 1 박기호 1 제갈은주 1 이승환 2 최윤영 3 이원혜 4 최기홍 1 1 고려대학교심리학과, 2 인제대학교의과대학일산백병원정신건강의학과, 3 한양사이버대학교청소년상담학과, 4 국립정신건강센터심리건강과 우울장애를선별하는신뢰롭고타당한선별도구를개발하는것은잠재적인위험군이나발병초기단계있는사람에게최적의치료를제공하기위한가장첫번째과제이다. 이는현대정신건강정책이정신질환의예방과조기개입을강조하는흐름과도일치한다. 현재까지우울장애를선별하는다수의도구들이개발되어왔으나, 해외저작권지불의문제를해결하고, 우울의하위영역은물론자살위험과조증, 불안및신체증상을포괄하며, 국가연구프로젝트와병원및다양한임상현장에서활용할수있는간결한형태의한국형우울장애선별도구를개발하는것이과제로남아있다. 본연구에서는현재까지개발된우울장애선별도구를체계적으로개관하여각선별도구의강점과약점을분석하였으며이를바탕으로새롭게개발할한국형우울장애선별도구 (Korean Screening Tool for Depressive Disorders, K-DEP) 의이론적기초를요약하였다. 임상심리, 정신의학, 심리측정전문가그룹으로구성된다학제연구팀과전문가자문위원회와함께한국형우울장애선별도구를개발하는과정을요약하였으며, 이과정에서개발된총 383문항의예비문항을소개하고, 153명의참여자에게실시한예비분석결과를제시하였다. 마지막으로앞으로실시될한국형우울장애선별도구개발프로젝트의차기단계과정과고려사항들을논의하였다. 주요어 : 주요우울장애, 선별도구, 평가도구개발, 문항반응이론, 근거기반심리평가 657
