1 ISSN (print) ISSN (online) The Qualification of Dentist for Sedation : BLS and ACLS Jongbin Kim, Seunghoon Yoo, Jongsoo Kim Department of Pediatric Dentistry, School of Dentistry, Dankook University Abstract Dentists who want to perform sedation must have abilities to control any emergent situation that may occur during treatment procedures. In the 2010 guideline for the use of sedation by dentists, he/she must has valid certifications for basic life support (BLS) and for advanced cardiovascular life support (ACLS). The Korean Association of Cardiopulmonary Resuscitation (KACPR) has made a contract with the American Heart Association for education and certification. From 2004, they have held many courses for BLS and ACLS providers and instructors. The author of this research participated in the above mentioned courses and qualified as a BLS provider, BLS instructor and ACLS provider. The present paper was intended as an introduction to and arrangement of BLS and ACLS courses for health care providers through a year of experience. However, the previous course of the ACLS provider was focused more toward medical doctors. It is necessary to develop a new advanced course for dentists who employ sedation in their medical practices. Key words : Sedation, Basic Life Support, Advanced Cardiovascular Life Support, Guideline Ⅰ. 서론생애처음으로치과진료를받기위해치과를찾는영, 유아들은본능적으로공포에휩싸이게된다. 아이들의행동을적절하게조절하기란어려운일이며, 의사소통이어려운어린나이일수록어려움은더커진다. 깊은진정은행동조절이어려운아이들에게자주행해지는술식이다. 그러나깊은진정은불안조절의연속적인스펙트럼선상에서전신마취와의경계가불분명하고기도유지가필요하다는측면에서, 보다더위험하고어려운술식이다. 따라서깊은진정을고려하는술자는반드시기도유지, 무호흡, 기도폐쇄, 저산소증등과같은부작용에대비하여기도관리, 환자감시그리고응급상황에대한대처능력을갖추어야한다 1). 진정법을수행하는치과의사뿐만아니라보조인력도기본생 명구조술을시행할수있도록교육받고정기적으로이수증을갱신하여야한다 2). 대한치의학회에서 2010년발간한치과진정법가이드라인에서는중등도진정을시술하는치과의사는반드시최신의기본생명구조술 (Basic Life Support, BLS) 과전문심장구조술 (Advanced Cardiovascular Life Support, ACLS) 을이수했음을서류로증명하거나, 적절한치과진정 / 마취응급관리과정을이수하여야함을강조하였다 3). 2014년에실시한소아치과의사의진정법사용에대한실태조사에서정기적인응급처치교육을받고있는응답자는약 20% 였으며 4), 이것은 2005년안등 5) 이조사했던결과인약 56% 보다오히려 26% 포인트나감소한것이다. 진정법을시행하고있는치과의사들에게응급상황에대한대처방법과능력을강조하면서도, 이러한교육과정에대한구체적인정보가부족한것또한현실이다. 2 대한심폐소생협회는 2004년부터미국심장협회 (American Heart Association, AHA) 와 International Training Organization(ITO) 계약을체결하고교육시스템과자격관리를책임지고있다. 이에저자는대한심폐소생협회에서주관하는 BLS HCP (Health Care Provider) 교육과정과 BLS Monitor, BLS Instructor 그리고 ACLS Provider 과정을이수하기위해필요한절차와준비과정에관한내용을공유하고자한다. Ⅱ. 본론 1. BLS HCP (Basic Life Support Health Care Provider) BLS HCP과정은일반인과의료인으로나뉘어시행된다. 의료인과정에는의료인 ( 의사, 간호사, 치과의사, 한의사, 조산사 ) 과해부학과생리학을이수한해당학부생, 약사와해부학과생리학을이수한해당학부생, 응급구조사 (1급) 와해부학과생리학을이수한해당학부생, 임상병리사, 방사선사, 작업치료사, 물리치료사, 운동처방사, 치위생사와해부학과생리학을이수한해당학부생 ( 단, 운동처방사의경우운동처방학과학부생또는졸업생만 BLS HCP 가능함 ) 그리고 2급응급구조사가참여할수있다 6). 이두과정은 BLS 시행시맥박확인과정의유무로써구분된다. 의학적지식이없는일반인의경우, 정확한맥박확인이어려울수있으므로, 이과정을생략함으로써가슴압박을보다신속하게시행하도록하기위함이다. 교육과정에참여하려면 AHA에서만든 BLS for Health Care Providers용교재와 pocket mask 그리고 pretest에서 90점이상을획득한결과지가필요하다. Pretest는인터넷에 접속하여수행할수있으며, 수회반복응시할수있다. 이는예습을통해교육을보다효율적으로진행하고자하는협회의노력이라고사료된다. BLS HCP과정을신청하기위해서는다음의과정이필요하다 상단메뉴의 소식 / 교육 / 행사 교육일정 선택한후, 나오는달력에서해당교육을선택하고등록한다. 이때, 검색방법 1에서지역, 구분, 코스등을선택하면보다쉽게원하는날짜의교육을찾을수있다. 신청안내메일과 SMS 발송받았다고반드시교육선정된것은아니므로확인이필요하다 (Fig. 1). 3 교육대상자로선정되면선정안내메일과 SMS가발송되며최종적으로신청이완료된다. 모든교육과정은실습위주로진행되며, 어떠한응급상황에처하더라도즉시 BLS를시행할수있도록하는것이교육목표이다. 총교육시간은 5-7시간이며, 강사 1명당최대 6명의교육생을넘지않도록규정하고있다. 교육이모두끝나면, 1인및 2인구조자성인기본소생술및 AED(Automated External Defibrillator) 술기평가를시행한다. 술기평가를통과하고필기평가를치르게되며필기평가에서 20문제중 17문제이상을맞출경우이수증을받을수있다. 만약, 강사 (Instructor) 과정을등록하고싶은교육생은필기평가에서 18문제이상을맞춰야신청자격이주어진다. 그런데, 15문제또는 16문제를맞춘경우다른유형의시험지로재시험을시행하며, 이때도 17문제이상을맞춰야통과된다. 다만, 재시험을치른경우에는강사과정을등록하기위해서는다시한번교육을이수하여야한다. 이수증은 2년마다갱신하는것이원칙이다. 교육과정은다음과같다. BLS HCP (Basic Life Support Health Care Provider), KACPR (Korean Association of Cardiopulmonary resuscitation) 1 성인심폐소생술 (8세이상 ); 1인및 2인구조자 2 소아심폐소생술 (8세미만 ); 1인및 2인구조자 3 영아심폐소생술 (1세미만 ); 1인및 2인구조자 4 구조호흡 (Rescue Breathing) 5 기도폐쇄관리 (Chocking) 이모든과정을통과하면 AHA에서발급하는이수증을받게된다 (Fig. 2). 2. BLS HCP 참관 BLS HCP과정을이수한후강사가되고자한다면, BLS HCP과정중필기시험을 20문제중 18문제이상을맞춰야 BLS Instructor과정을시작하게된다 7). 먼저 BLS Instructor Essential 과정을 에접속하여이수하고평가를받은뒤에지역의 BLS Training Site(TS) 에서교육을참관하게된다. 온라인교육과참관수업을통과하면, 참관한곳의강사로부터추천서를받을수있으며다음의강사과정에등록할수있게된다. BLS Instructor가되기위한전반적인과정은다음과같다 (Fig. 3). Fig. 3. Flow Chart for BLS Instructor (data form the BLS Instructor과정에등록하기위해필요한서류는다음과같다. 1 BLS Provider Certification Card 2 BLS Provider과정에서진행을보조하면서끝까지참관한후교육에참여한 BLS Instructor에게서명을받은사전참관확인서 3 BLS Instructor로부터성인 1인및 2인 CPR 및 AED 사용그리고영아 1인및 2인 CPR 술기평가를받은술기평가지 4 강사추천서 5 소속예정 Training Site Director 확인서 6 BLS Instructor Essential (IE) Certification 이때, 강사추천서와참관확인서는 BLS 교육일을기준하여 6개월이내의것이어야한다. 3. BLS Instructor BLS Instructor 과정은약 1개월에한번정도개설된다. 교육및평가내용은 BLS HCP와유사하며, 강사로서갖추어야할요건과교육생의교육방법, 평가방법등이추가된다. 필기평가는 20문제중 18점이상을맞아야이수증을신청할수있다. 이수증은아래와같다 (Fig. 4). 82
4 Fig. 4. BLS Instructor Certification from AHA. 4. BLS Monitor BLS Monitor과정은 BLS Faculty가소속되어있는 BLS TS에서이수한다. Monitor과정은 Instructor과정에합격하고 3개월이내에받아야하며, 3개월이지난후에는 Instructor과정을처음부터다시이수해야한다. 이과정을통해교육생을직접교육하며 BLS HCP 전반적인과정을진행하는법을평가받게된다. 해당강사의추천이있으면비로소 BLS Instructor 의이수증을받을수있게된다. BLS Instructor 자격을유지하기위해서는 2년동안적어도 4번의교육을진행해야한다. 5. ACLS Provider (Advanced Cardiovascular Life Support) 전문심장구조술과정은응급상황에보다전문적으로대처할수있는능력을갖추도록교육하는것을목적으로개설되었다. 국내에서운영되고있는과정의지원지격은반드시기본생명구조술의이수증 ( 유효기간이내 ) 을유지하고있는사람중의사, 간호사, 응급구조사등이다. 그러나, 치과마취과학회의노력으로치과의사가대상에포함되어현재운영되고있으며, 최근에는 Instructor과정을통과한치과의사가나오기도하였다. 신청자격에대한정리는아래 Table 1에정리하였다. 신청방법은 BLS HCP와비슷하다. 다만, 등록시기본생명구조술 (BLS Certification) 이수증을첨부해야한다. 교육은 2일에걸쳐진행되며, ACLS 교육과정은다음을포함한다. 1 BLS 과정포함 2 고급기도유지기구의사용 3 정맥로및골내주사경로의확보 4 심전도분석 (EKG: Electrocardiography, Interpretation & Algorithm Review) 5 Team Approach (The Megacode and Resuscitation Team Concept) 6 약물의사용 (Pharmacology / Electrical Therapy) 7 Acute Coronary Syndrome A. ACLS Certification from AHA. 모든교육을완료하면, 보통 6명이팀을이루어팀의 leader 로서응급상황에대처하는능력을평가받게된다. 술기평가를통과한후에필기시험 50문제중 42점이상을획득하면이수증을신청하고발급받게된다. 이수증은아래와같다 (Fig. 5). 여기까지진정법을시행하는치과의사와보조인력에게필요한여러요건중기본생명구조술과전문심장구조술을이수하는과정에관해검토해보았다. 진정법을보다안전하게시행하기위해서는학부과정을통해습득한지식만으로생명구조술을익히는것보다는이와같이직접술기능력을검증받고정기적으로재평가받는과정이필요하리라사료된다. 2010년치의학회에서발간한진정법가이드라인에서는안전한진정법시행을위한치과의사의자세를다음과같이소개하고있다 3). 1 진정법을시행하는치과의사는반드시대한치의학회인정, 치과진정법가이드라인 을숙지해야한다. 2 진정법을수행하도록교육을받은치과의사는치과치료중환자의위험을최소화할책임을가지고있으며아래와같이시행하여야한다. 3 자신이적절히교육받은약제와기법만사용한다 4 진정법이요구되는환자에서만제한적으로진정법을시행한다. 5 진정법시행전각환자에서의과및치과적과거력을조사하고환자평가를시행하여필요한경우적절한임상검사및타과의뢰가이루어져야한다. 6 진정법시행중적절한환자감시가이루어져야한다. 7 적절한응급약제, 장비, 시설을가지고있어야하며, 항상언제든지사용이가능하도록유지되어야한다. 8 사용약제, 용량, 감시된활력징후, 부작용, 진정법으로부터회복그리고수행된응급치료행위에대한문서적기록이이루어져야한다. 9 진정법시보조인력도심폐소생술등의교육을받아야한다. 10 고위험환자는이를치료할수있는준비가갖추어진상태에서치료되어야한다. 진정법을시행하면서마주치게되는응급상황에적절히대처할수있도록시술자의역량을갖추고정기적으로검증하는것과진료실내에잘준비된시스템을갖추는것은더이상선택이아니다. 미국소아치과협회에서추천하는시술자가갖춰야할요건은기본생명구조술과소아전문생명구조술 (Pediatric Advanced Life Support, PALS) 이다 8). 국내가이드라인에서는기본생명구조술과전문심장구조술을강조하고있으나, 실제로소아치과의사가진정법을시행하는주요대상이 3~4세전후의아이들이므로전문심장구조술에서주로마주치게되는고령의심정지환자와는그대응법이달라야할것이다. 따라서, 미국소아치과협회에서권장하는 PALS에대해좀더알아볼필요가있으리라생각된다. 더나아가서치과의사라는직업의특수성에맞는최적화된전문구조술을개발하기위한노력이시작되야할것으로사료된다. Ⅲ. 결론치과진료는여전히불안과공포의대상이되고있다. 이러한상황을부드럽게넘겨줄수있는대안으로진정법은술자와환자에게꼭필요한술식일것이다. 그러나, 적절하게시행되지않은진정법에서는환자의생명을위협하게되는경우가발생할수있다. 따라서, 진정법을시행하는술자는반드시응급상황에적절히대처할수있는능력과환경을잘갖춰야할책임이있다. 진정법이국내에소개되고본격적으로시행된지도 30여년이지났고, 2004년전문의제도가시행된후로 10년이넘는시간이흘렀다. 더욱이 2014년부터는전문의표방이가능하도록제도가바뀌었다. 지난세월동안많은제도적인개선이이뤄졌지만응급처치에대해준비하고있는치과의사는오히려감소하는현실에서전문의로서스스로당당할수있도록노력하는자세가필요하다. 기본생명구조술과전문심장구조술을익히는것만으로모든 84
6 응급상황을해결할수는없으나, 진정법시행중에발생하는응급상황의대부분은빠른인식과신속하고적절한초기대응만으로도해결할수있는경우가많다. 그리고, 이러한준비과정에서최적화된진정법을위한개선점을발견하고수준높은진정법시행능력을발전시킬수있을것이다. References 1. Kim HJ : Sedation in Dental Treatment Part.
