We report a catheter-related bacteremia of T. pulmonis. A 39 yr-old male with ALL was hospitalized to receive bone marrow transplantation (BMT). Although the patient developed a high fever at the 7th hospital day (HD), it subsided with vancomycin treatment, and he received BMT at 9th HD. Fever resurged at 16th HD despite sustained treatment with vancomycin, meropenem, and amphotericin B, but subsided with removal of Hickman catheter (HC) at 19th HD. Three sets of blood cultures comprising one from the HC and two from venipunctures were taken at 7th, 16th, and 19th HD, and the distal tip of the HC was also cultured. The aerobic vials of all 3 HC-withdrawn blood cultures and one peripheral blood culture taken at 19HD and the HC tip culture grew long, straight, thin gram-positive rods that were positive on modified Kinyoun stain. This organism showed tiny, rough, grey colonies after 3-day incubation and grew to large flat colonies when incubation was extended. It was catalase-positive, ureasepositive, and alkaline-slant/alkaline-deep on triple sugar iron agar, and hydrolyzed hypoxanthine. The sequence of 1,296 base pairs of 16S rrna of this organism showed a.0% homology with the published sequence of T. pulmonis T. To our knowledge, this is the first report of T. pulmonis bacteremia in Korea. Tsukamurella 속은약한항산성을띄며운동성이없고, 아포와균사를형성하지않는그람양성막대균이다. 절대호기성인이균종은토양이나절지동물에서발견되며, 면역억제상태나결핵과같은만성폐감염시병원소로작용할수있다 [2]. Tsukamurella 속에의한인체감염은드물게보고되는데만성폐감염, 피하농양과괴사성힘줄윤활막염 [3], 뇌수막염 [4], 복막염, 균혈증, 중심정맥관관련균혈증 [5, 6] 등이있다. 국내에서 1997년 Tsukamurella inchonensis 균혈증이유일하게보고된바있다 [7]. 저자는골수이식을받은급성림프성백혈병환자에서 Tsukamurella 41

2 42 심효은 성흥섭 백승미 외 4인 pulmonis에 의해 중심정맥관 관련 혈행성 감염을 일으킨 증례 Ciprofloxacin, fluconazole, acyclovir 등으로 예방적 항생제 를 문헌고찰과 함께 보고하고자 한다. 치료를 받고 있던 중 입원 7일째 열이 발생하여 vancomycin을 추가한 후 증상이 호전되어 9일째 골수이식을 받았다. 입원 16일 증 례 째 고열이 다시 발생하여 vancomycin, meropenem과 amphotericin B를 3일간 투여하였으나 발열이 계속되어 입원 19일째 39세 급성림프성 백혈병 환자가 골수이식을 위해 입원하였다. 3 Tsukamurella pulmonis Bacteremia 43 은없었고 14일동안 vancomycin 치료후 34 일째퇴원하였다. 입원 7일, 16일, 17 일과 19 일째말초정맥과카테터에서채취한 2세트와 1세트의혈액배양은각각 Bactec Plus Aerobic/F 병과 Lytic/10, Anaerobic/F (Becton Dickinson, Sparks, MD, USA) 병에접종하여배양하였고, 입원 19일째제거한 Hickman 카테터의말단 5 cm을혈액한천배지에굴려서반정량배양하였다. 카테터를통해채혈된혈액배양의호기성병에서모두 시간후에그람양성막대균이검출되었으며, 말초혈액호기성병에서는 19일째채취한혈액배양의호기성병에서만 47.7시간후동일한균이검출되었다. 균은그람염색상 μm 의길고반듯한그람양성막대균이었고, Ziehl-Neelsen 염색에서음성, 변법 Kinyoun 염색에서양성이었다 (Fig. 1). 혈액한천배지에서 24시간배양후에는집락을관찰하기어려웠고, 3일간배양후 1 mm 직경, 비용혈성회색의거친집락을형성하였다. 배양을 6일까지연장했을때집락은 3-4 mm까지커지고회색의불규칙하고, 편평한변연부와노란색과립상의중앙부로구성된집락들이관찰되었다 (Fig. 2). 변연부를현미경으로관찰했을때영양팡이실 (vegetative hyphae) 로구성되어있었고, 공 기팡이실 (aerial hyphae) 은관찰되지않았다. 카테터끝배양에서, 혈액한천배지에서같은형태의집락이 개이상자랐다. Catalase 양성, 운동성음성, triple sugar iron 한천배지에접종시염기성사면 / 염기성고층의반응으로 Corynebacterium spp. 로동정하였다. 첫번째혈액배양은 2세트중카테터를통해채취한호기성 1병에서만 coryneform 그람양성막대균이자랐기때문에더이상의종동정을실시하지않고, Corynebacterium spp. 로보고되었다. 이후반복적으로동일한균이분리되어종동정을위해 16S rrna 유전자염기서열분석을실시하였다. 순수분리된집락에서 GenElute Bacterial Genomic DNA 키트 (Sigma, St. Louis, MO, USA) 로 DNA를추출한후 16S rrna의 8-806번째염기와 515-1,390번째염기부위를각각증폭하였다. 시발체의염기서열은 8FPL 5 -AGT- TTGATCCTGGCTCAG-3, 806R 5 -GGACTACCAGGGT- ATCTAAT-3, 515FPL 5 -TGCCAGCAGCCGCGGTAA-3, 13B5 -AGGCCCGGGAACGTATTCAC-3 이었으며, PCR 조건은기존문헌을따랐다 [8]. 증폭산물은 Power Gel Extraction kit (TaKaRa Bio Inc., Shiga, Japan) 로정제한후마크 66 Rhodococcus erythropolis Rhodococcus globerulus NCIMB12315T 74 Rhodococcus koreensis DNP505T 61 Rhodococcus fascians ATCC12974T 72 Rhodococcus wratislaviensis NCIMB13082T 95 Rhodococcus opacus 43206T Rhodococcus equi ATCC6939T Nocardia farcinica 43665T Gordonia bronchialis 3247 Rhodococcus rhodochrous ATCC271T 35 Streptomyces albus 40313T 56 Streptomyces griseus ATCC10137 Nocardiopsis dassonvillei 43111T 66 Actinomadura madurae 43067T Actinomadura latina 43382T 70 Dietzia maris ATCC35013T Tsukamurella paurometabola 20162T Tsukamurella spumae N1171T Tsukamurella inchonensis IMMIB D-771T 76 Tsukamurella strandjordae ATCCBAA-173T Tsukamurella pseudospumae JC85 Tsukamurella tyrosinosolvens Z44234T Tsukamurella pulmonis KUL50 Tsukamurella pulmonis 44142T 7A1960 Tsukamurela poriferae K362T 0.01 Fig. 3. 4 44 심효은 성흥섭 백승미외 4 인 로젠 (Seoul, Korea) 에염기서열분석을의뢰하였다. 16S rrna 염기서열 BLAST database ( blast/) 에서검색한결과 T. pulmonis T (GenBank accession no. X92981) 와 1,296 bp 전체가.0% 일치하였다. 다음으로는 Tsukamurella tyrosinosolvens T 와 99.7% 일치하였고, Tsukamurella strandjordae ATCCBAA- 173 T 와 99.5%, Tsukamurella pseudospumae JC85주와 99.5 %, T. inchonensis ATCC T 와 99.3% 의일치도를보여, Clinical Laboratory Standards Institute (CLSI) MM-18 [9] 의지침에따라판독하면 T. pulmonis. 염기서열의유사성때문에 T. tyrosinosolvens의가능성을배제하지못함 이었다. 유사한염기서열을갖는 24종의염기서열을대상으로 MEGA software (ver. 4.0) 프로그램을이용하여 neighbour-joining 방법으로계통발생학적분석을시행하였다 (Fig. 3). 이분리주는 24, 30, 37 에서자라고 45 에서는자라지않았다. 생화학적균종감별을위해 Christensen s urea agar 검사, API Coryne (BioMerieux Sa., Marcy-l Etoile, France), API 20C Aux (BioMerieux Sa.), RapID CB plus (Remel, Lenexa, KS, USA) 등을이용하여검사하였다. 각키트검사의 bionumber는 API 20C Aux에서 2일째판독시 , 3일째 로각각 Saccharomyces cerevisiae, Candida zeylanoides에해당하는생화학적성상을보였고, API Coryne 판독결과는 1일째 , 2일째 로 Rhodococcus spp. 로동정되었다. RapID CB plus는 4시간배양후판독시 로 Rhodococcus equi로동정되었다. 여러가지키트결과를종합하면 API 20C AUX에서 glucose, galactose, N- acetyl galactosamine, trehalose, sucrose, sorbitol을, API Coryne에서 esculin을, RapID CB plus에서 ribose를, API NE에서 mannitol과 gluconate를분해하고, arabinose, maltose, citrate, gelatin, inositol, rhamnose 등은분해하지않았다. Urease는 API Coryne, RapID CB plus 검사에서는음성이었지만, Christensen s urea agar에서는배양 5일째양성반응을보였으며, nitrate reductase 음성, ONPG, pyrazinamidase 양성, alkaline phosphatase 양성, β-glucosidase 양성, β-galactosidase 는음성이었다 (Table 1). Casein, tyrosine, xanthine 가수분해는음성이었으며, hypoxanthine 가수분해는양성이었다. 이상의반응결과는기존에보고된 T. pulmonis에일치하는결과였다 (Table 1). 이분리주를 Mueller-Hinton 한천에서 E-test strip (Biodisk, Solna, Sweden) 을사용하여항균제감수성검사를하였다. 최소억제농도는 amikacin 1.0 μg/ml, ceftriaxone 1.5 μg/ml, Table 1. 5 Tsukamurella pulmonis Bacteremia 45 검출시간이 2시간이상차이가난경우를권장하고있다. 본증례는입원 19일째쌍으로실시한말초혈액배양과카테터를통해채혈한혈액배양에서각각 47.7시간, 25.1시간만에 T. pulmonis 가검출되었고, 카테터말단부배양에서도 T. pulmonis가 개이상자라서 T. pulmonis에의한중심정맥관관련균혈증으로진단할수있었다. 지금까지보고된문헌에의하면 Tsukamurella spp. 는정상적인사람에서는비병원성이고, 면역억제상태의환자들에서병인성균으로작용할수있으나, 이경우도대부분집락화한상태이거나검사실오염균으로출현한다 [12]. 하지만, 혈액에서검출될때는대부분혈액배양에서단독으로검출되어균혈증의원인이될수있음이알려져있다 [13]. 본증례역시골수이식을받은급성림프성백혈병환자에서발생하여면역저하환자였으며, 중심정맥관을가지고있어서 Tsukamurella spp. 에의한균혈증의위험요인을갖추고있었다. 국내에서 Tsukamurella spp. 에의한감염은 1991년염산을마신환자에서발생한 T. inchonensis에의한균혈증이유일한보고이다 [7]. Tsukamurella spp. 는산에대한저항성이있기때문에염산에서균이살아남아서손상된점막을통해침입했을가능성을배제할수없지만, 쇄골하정맥관을가지고있던환자였기때문에카테터관련균혈증의가능성이더높다고하였다. 외국에서 1990 년대초보고된 T. paurometabola에의한균혈증 4예모두카테터와관련이있었고, 이중 3명은호중구감소가있는면역억제환자였으며 [6], 2002년 Tsukamurella spp. 에의한균혈증 6예를모아서보고한논문에서도모두중심정맥관을가지고있었고, 5명은악성종양과골수이식등으로면역억제된환자였다 [13]. 따라서중심정맥관, 면역억제상태등이 Tsukamurella spp. 균혈증의대표적인위험요인이며, 유치도자를가진면역억제환자의혈액에서그람양성간균이검출되었을때균혈증의원인으로감별해야할것이다. 호기성 actinomycetes를종수준까지감별해야할필요성은정립되어있지않다. 하지만, 본증례와같이임상적으로심각한감염을일으켰을때는적절한항균제치료를위해균종동정이필요하다. 본증례에서분리된균은 16S rrna 염기서열분석에서 T. pulmonis와 % 일치하여 T. pulmonis일가능성이가장높다고생각하고추가적인검사로확인동정하였다. 호기성 actinomycetes의동정에는염기서열분석과가스크로마토그래피를이용한단쇄지방산분석, 고속액체크로마토그래피법을이용한 mycolic acid 분석이전통적으로사용되었고최근에는 16S rrna 염기서열분석이가장우수한능력을보인다 [1]. 하지만, 이들방법도 Tsukamurella spp. 를종수준까지감별하는데한계가있다 [14]. 호기성 actionomycetes 균종감별에사 용하는 mycolic acid 분석또한 Tsukamurella wratislaviensis 를제외한모든 Tsukamurella spp. 가거의동일한양상을보여서종동정에유용하지않다 [15]. T. pulmonis는 T. paurometabola, T. inchonensis 와 16S rrna 염기서열상동성이 99% 이상으로매우높기때문에 16S rrna 염기서열분석만으로이들균종을확실히감별하기는어렵다 [9]. 특히 16S rrna 염기서열을 500 bp 정도만분석하는 MicroSeq (Perkin-Elmer Applied Biosystems Division, Foster City, CA, USA) database 를이용하면이들세균종은 % 동일한결과를얻는다 [15]. 따라서 16S rrna 염기서열분석을할때 500 bp만분석하기보다는전체길이를분석하고, GenBank 검색을하는것이필요하다. T. pulmonis는폐결핵환자의객담에서처음분리되어생화학적성상, mycolic acid의분석, 16S rrna 상동성분석결과 Tsukamurella 속으로분류되었다 [16]. Tsukamurella spp. 는그람양성막대균으로 catalase 양성, 절대호기성이기때문에본증례에서처럼 Corynebacterium spp. 로잘못동정되어, 오염균취급을받는경우가발생한다 [13]. Tsukamurella는항산성이있어서 Nocardia spp. 나신속성장미코박테리아종으로오인되기도하지만 [7], 미코박테리아와달리변법항산성에만양성이고 Nocardia spp. 와달리집락이자라면편평해지고 urease 반응이느린점등의차이가있다. Tsukamurella spp. 는 lysozyme 내성, 68 내열성 catalase, 5일째 Tween 80 가수분해, urease 양성, pyrazinamidase 양성, 철분섭취, 1주일째 5% NaCl 내성, 2주째 nitrate 환원능음성, 3, 14일째 arylsulfatase 음성등의특징으로다른호기성 actinomycetes인 Nocardia spp., Rhodococcus spp., Gordonia spp., 신속성장미코박테리아등과감별이되지만 [16], 임상검사실에서는일상적으로이루어지지않은검사들이고, 오랜시간이걸려서실제이용하기는어렵다. 상품화된여러가지동정키트를이용해서생화학적검사를시도한연구들이있는데본증례에서 Christensen s urea agar 검사, API Coryne, API 20C Aux, RapID CB plus 등이종동정에유용하였다. API 50CH이나 biotype- strip을이용하여더많은종류의당검사를하여종동정을시도한연구도있다 [15, 17]. T. pulmonis는 T. paurometabola와달리 hypoxanthine을가수분해하고, T. inchonensis 와비교했을때, maltose, cellobiose, melezitose, inositol과 citrate의탄소를이용하지못하는점과 45 에서자라지못하는점으로구별될수있다 [16]. 본증례에서분리된균은 β-galactosidase가음성인점을제외하고는 T. pulmonis T strain의생화학적성상과일치하였다. β-galactosidase는 Tsukamurella spp. 에일반적으로존재한다고하지만, 50% 에

API 20C Aux와 RapID CB plus 를이용한동정결과를이전보고와비교했을때 [15] 3일째판독결과가 bionumber 으로동일한결과를보였고, RapID CB Plus 동정결과는 로서이전보고된 과는 ribose를분해한점만이달랐다. 따라서 Tsukamurella spp. 를종동정하기위해서는염기서열분석과함께 urease, 45 에서성장능, hypoxanthine 등아미노산분해능에대한수기검사가필요하고, API 20C Aux, API Coryne, Rapid ID CB Plus 등키트로생화학적반응을검사하는것이유용할것이다. Tsukamurella spp. 의병원성과항균제감수성에대한자료는부족한편이다. 기존문헌 [15] 에서 T. paurometabola는 imipenem MIC >32 μg/ml의고도내성을보이는반면다른종은 1 μg/ml 이하로감수성이있고, T. wratislaviensis는 cefoxitin 감수성이지만, 다른균들은고도내성이라고한다. T. pulmonis는다른tsukamurella spp. 와는달리 gentamicin 에중등도내성인균주가있다고보고된바있는데 [15], 당시연구는판독기준을 Staphylococcus spp., Enterobacteriaceae 에대한기준을준용했기때문에본증례의균주가 MIC 4 μg/ ml인것과유사한결과였을것으로추정된다, 따라서아직효과적인표준적인항균제치료지침이없고, 항균제감수성을예측할수없기때문에심각한감염의원인이라면항균제감수성검사가필요할것으로생각된다. 본증례는경험적으로 vancomycin, meropenem으로치료했지만, 수일간발열이지속되었다. 본증례에서분리된균의 MIC가 meropenem에 0.5 μg/ml, vancomycin에 4 μg/ml 로두가지항균제에감수성으로판단됨에도항균제치료에반응하지않았고카테터를제거한후열이조절되었다. 이전연구에서도 vancomycin으로 3일간치료해도발열이지속되었던환자가중심정맥관을제거하여치료되었고, 이연구에서다른 5 명의 Tsukamurella spp. 균혈증환자들모두항균제치료와중심정맥관제거를병용하여치료를하였다 [13]. 카테터를제거하지않았을때 1개월까지도균혈증이지속된예가있어서 [13]. Tsukamurella spp. 에의한중심정맥관관련균혈증이발생했을때는카테터를제거해야할것으로생각된다. 이증례는 T. pulmonis에의한균혈증의국내첫보고이다. 골수이식을받은백혈병환자에서중심정맥관관련균혈증으로발생하였으며, 중심정맥관을제거하여치유되었다. 면역저하환자에서중심정맥관관련그람양성막대균에의한균혈증이발생했을때 Tsukamurella spp. 도균종감별에고려해야할것이다. 요약 Tsukamurella pulmonis 는호기성 actinomycetes에속한다. 저자들은 T. pulmonis 균혈증 1예를경험하여보고하고자한다. 급성림프성백혈병인 39세남자가입원 9일째골수이식을받았다. 7일째고열이발생하여 vancomycin을추가한후열이해소되었으나 vancomycin, meropenem, amphotericin B 치료를계속하였는데도 16일째고열이재발하여 19일째 Hickman 카테터 (HC) 를제거하였다. 7, 16, 19일째각각 HC를통해채혈한혈액한쌍과말초혈관의혈액두쌍등 3쌍씩혈액배양을실시하였고, 제거했던 HC의말단부도배양하였다. HC로채취한혈액배양의모든호기성병과말초혈관에서얻은혈액한병, HC 말단부배양에서길고, 가는반듯한그람양성막대균이분리되었고, 변법 Kinyoun 염색에양성이었다. 배양 3일째에작고비용혈성의회색의거친집락이자랐고, 배양을연장하면집락이크고납작해졌다. 균은 catalase, urease 양성이었으며 triple sugar iron 한천배지에서알칼리사면 / 알칼리고층반응을보이고, hypoxanthine을분해하였다. 1,296 bp의 16S rrna 염기서열분석결과 T. pulmonis T 과 % 일치하였다. 본증례는 T. pulmonis에의한국내첫균혈증보고이다. 참고문헌 1.
