We evaluated the usefulness of MacConkey () and colistin-nalidixic acid blood agar () for the isolation of pathogens from these specimens. Methods: A total of 1,508 clinical specimens [144 peritoneal fluid, 241 body fluids (19 bile, 70 joint fluid, 6 pericardial fluid, 104 pleural fluid, and other fluids in 42 cases) and 1,123 wound/abscess] were inoculated onto basic media [Blood agar plate (BAP), chocolate agar or BAP with streaking of Staphylococcus aureus] and simultaneously inoculated onto and. The pathogens isolated by basic media and by additional use of and/or were compared. Results: With basic media, 885 isolates from 588 specimens were detected, and by additional use of and, an additional 27 isolates from 24 specimens and an additional 128 isolates from 112 specimens were isolated, respectively. Compared to the basic media, by adding, an additional 233.3%, 38.5% and 4.5% of gram-negative bacteria were isolated from peritoneal fluids, body fluid and wound/abscess, respectively, and by adding, an additional 106.7%, 45.0%, and 20.7% of gram-positive bacteria/yeast were isolated, respectively. The isolates detected by additional use of were mainly Enterobacteriaceae (77.0%), and those detected by were S. aureus (21.1%), Coagulase-negative Staphylococcus spp. (20.3%), Enterococcus spp. (16.4%), Streptococcus spp. (10.2%) and yeasts (16.4%). Conclusion: For peritoneal fluid and body fluid specimens, additional use of plus seems necessary for detection of pathogens. For wound/abscess, additional use of will be cost effective. (Ann Clin Microbiol 2015;18:1-6) Key Words: Body fluids, Colistin-nalidixic acid blood agar, McConkey agar, Peritoneal fluid, Wound/Abscess INTRODUCTION 뇌척수액, 복수, 복막투석액, 담즙, 관절강액, 심낭액, 흉수액등의체액감염시환자가빠른시간안에위중한상태에빠질수있기때문에신속하고예민하게감염균을검출하는것이중요하다 [1-3]. 검체에따라주로분리되는균이다른데, 복막액에서는 Enterobacteriaceae, Streptococcus pneumoniae, Streptococcus pyogenes, Enterococcus spp., Staphylococcus aureus 등이, 복막투석액에서는 S. aureus, Staphylococcus epidermidis, viridans group streptococci, Enterobacteriaceae, 비발효성포도당그람음성막대균, Corynebacterium spp., Candida spp. 를비롯한진균류등이분리된다. 관절강액에서는 S. aureus, S. pyogenes가, 흉수액에서는 S. pneumoniae, S. aureus, Hemophilus influenzae, Enterobacteriaceae, Pseudomonas aeruginosa, 무산소성세균, S. pneumoniae를제외한 Streptococcus spp., 결핵균, Actinomyces, Norcardia, 진균등이분리된다 [2,3]. 위와같은정상적인무균검체들은한개의집락만생겨도의의가있다고간주하고보고하는것이원칙이지만, 검체오염가능성때문에 Received 5 September, 2014, Revised 26 November, 2014, Accepted 15 December, 2014 Correspondence: Yeon-Joon Park, Department of Laboratory Medicine, Seoul St. Mary's Hospital, The Catholic University of Korea, Seoul , Korea. 2 2 Ann Clin Microbiol 2015;18(1):1-6 오염확률이높은균이소량 (1-2집락) 배양되었거나혼합감염이나우세균이없을경우에는오염균과감염균을구분하는데어려움이있다 [4,5]. 한편, 창상 / 농양부위검체에서는주로 S. aureus, P. aeruginosa, Enterobacteriaceae, β-hemolytic streptococci, 및다양한무산소성세균들이분리되며깊은부위감염에서는호기성균과무산소성균이같이분리될수있다. 그러나개방성병변일경우정착균이들어가기쉬워이런균들이분리된경우감염의원인균인지해석하기가어렵다 [4-6]. Clinical Microbiology Procedures Handbook에서는무균검체 ( 뇌척수액, 심장낭액, 흉수액, 관절강액, 및기타무균검체 ) 와창상 / 농양검체는기본접종배지가혈액평판배지 (BAP) 와초코렛배지이며그람염색결과를먼저확인한후기본배지이외의추가접종배지를결정하도록권고하고있다. 복막액, 고막천자액, 안구의초자양액 / 수양액검체등은 MacConkey배지 () 와같이 Colistin-nalidixic acid blood agar () 를추가접종하며, 그람염색에서다양한염색상이보이거나장관내미생물들에의한복강액의오염이의심된다면반드시 ( 혹은 EMB), ( 혹은 phenylethyl alcohol agar, PEA) 및혐기성세균배양을위한선택배지에접종하여그람양성균을정확하게분리하는것이중요하다고강조하고있다. 창상 / 농양검체또한그람염색결과를먼저확인하여추후미생물검사에대한지침을잡도록권유하고있는데, 표피에서채취한검체는혈액한천배지, 초콜릿배지, 에접종하고심부검체는 만접종할것을권고하고있다 [4]. 그러나, 국내에서는통상적으로 를사용하지않고, 검사실에서는검체의정확한채취부위에대한정보를얻기부족하며, 대부분그람염색용슬라이드제작과배지의접종을동시에하므로그람염색결과를확인한후에배지의종류를정하기도어려운실정이다. 이에본연구에서는 Clinical Microbiology Procedures Handbook에서권고한지침에따라복막액, 체액 ( 혈액, 뇌척수액, 소변제외 ) 및창상 / 농양에서기본배지와 와 를각각추가했을때의균동정결과를각각비교함으로써이들배지를추가로접종하는것에대한효용성을평가하고자하였다. MATERIALS AND METHODS 1. 대상검체 2012년 9월부터 2013년 3월까지 6개월간 2개대학병원 ( 가톨릭대학교서울성모병원, 가톨릭대학교대전성모병원 ) 및 1개임상검사센터 ( 삼광의료재단 ) 에배양검사가의뢰된복막액 144 검체 ( 복수 63검체, 복막투석액, paracentesis 및 pelvic cavity fluids 81검체 ), 체액 241 검체 ( 담즙액 19 검체, 관절강액 70 검체, 심장낭액 6 검체, 흉수액 104 검체, 및기타체액 42 검체 ) 및창상 / 농양부위 1,123 검체를대상으로하였다. 창상 / 농양검체는정확한채취부위에대한정보가부족하여표피성병변 과심부병변을구분하지않았다. 이연구는가톨릭대학교중앙의료원임상시험심사위원회심의 (XC12ECMI0087 KD) 를통과하였다. 2. 검사방법채취된검체는기본배지 (BAP, 초콜릿배지혹은 S. aureus가획선된 BAP) 와, (BBL, Becton-Dickinson, Cockeysville, MD, USA) 에동시에접종하고 35 o C, 5% CO 2 항온기에서 2일간호기성배양을시행하였다. 검체량이 1 ml 이하면검체전부를평판배지에직접접종하였고, 그이상이면 3,000 rpm에서 10분원심분리후침사를평판배지에접종하였다 [2]. 각배지에서자란균주의동정은 MicroScan (Siemens Healthcare, Sacramento, CA, USA) 또는 Vitek 2 (biomerieux, Inc., Hazelwood, MO, USA) 를이용하였으며필요에따라서전통적인생화학적방법이나분자유전학적방법을이용하였다 [5]. 3. 통계수집된자료는 SPSS for window 11.0 (SPSS version 11.0, SPSS Inc. Chicago, IL, USA) 를사용하여각검체별, 배지별균주동정결과를비교하였다. 각배양법에따른각검체들의배양양성율비교는 McNemar s 검정을시행하였고, P 값이 0.05 미만인경우를유의한것으로판단하였다. 배지추가에따른균동정결과의비교는 one-way ANOVA 검정을시행하였으며 P 값이 0.05 미만인경우를유의한것으로판단하였다. RESULTS AND DISCUSSION 전체 1,508 검체중기본배지만을사용하면 588 (39.0%) 검체에서 885균주가분리되었으며, 을추가하면 24검체에서 27균주가, 를추가하면 112검체에서 128균주가더분리되었다. 결론적으로 와 를모두추가하면 633검체 (42.0%) 에서총 1,040균주가분리되었다. 먼저, 을추가한효과를검체별로살펴보면, 기본배지에비해복막액에서는 6검체에서, 체액에서는 4검체에서, 창상 / 농양에서는 14검체에서추가로균이동정되었으며그중 45.8% (11/24) 에서복합균이관찰되었다 (Table 1). 검체별로 에서만동정된그람음성균을보면기본배지만을사용했을때보다복막액에서는 233.3% (7/3) 의그람음성균이, 체액에서는 38.5% (5/13) 의그람음성균이더분리된반면창상 / 농양검체는 4.5% (15/336) 의그람음성균만이더분리되어복막액과체액에서는 의추가동정이의미있어보였다. 그러나 에의해복막및체액에서추가분리된경우주로단일균에의한감염 (80.0%, 8/10) 이었으며이는혼합감염이많다는기존보고 [2,5] 와는차이가있었다. 실제대부분의그람음성균은그람양성균에비해증식속도가빨라 BAP에서는그람양성균과같이있더라도그람음성균만을

Abbreviations: BM, basic media;, MacConkey;, Colistin-nalidixic acid blood agar. 더동정될수있는점을고려하면 에서추가동정된그람음성균이단일균분포라면 을사용한것에의한효과보다는추가적으로접종한검체의양이많거나, 오염의가능성을배제할수없다. 한편, 창상 / 농양에서는 에서만그람음성균이배양되었던 14검체중 9검체 (64.3%) 가그람양성균혹은다른그람음성균과의복합균분포를보여 추가가의미있어보였다. 그러나실제 에서추가동정된그람음성균은 4.5% (15/336) 에불과할정도로매우적으며, 창상 / 농양검체특성상병변에서분리된세균은모두병원균은아니고정착균이나오염균일가능성도있다. 본연구에서는검체채취부위가심부였는지, 표재성이었는지확인할수는없었지만, 에서만분리된그람음성균이매우적었기때문에통상적으로모든창상 / 농양검체에단지 만을추가하는것에대한업무적및경제적효율성을고려해볼필요가있다. Table 2에서보면, 에서만추가동정된그람음성균 27균주 (7.7%, 27/352) 는주로장내세균 (77%, 19/27) 이었다. 균종별로는 Escherichia coli 가 25.9% (7/27) 로가장많았고, 다음은 P. aeruginosa (22.2%, 6/27), Klebsiella pneumoniae (18.5%, 5/27) 순이었다. Colistin-nalidixic acid blood agar () 는 colistin이그람음성간균을, nalidixic acid가 Proteus spp. 를억제하고 Columbia agar가호기성균의증식을촉진시킴으로써호기성그람양성균을분리하기위한선택배지로사용된다. 에서는 Staphylococcus spp., Streptococcus spp. 및 Enterococcus spp. 등의용혈성반응이뚜렷이관찰되는데, Phenylethyl alcohol agar (PEA), potassium tellurite blood agar와같은다른호기성그람양성균의선택배지와비교하면, PEA에서는불분명하거나비정형적으로보일수도있는 Streptococcus spp. 의 α 및 β 용혈성성향이잘관찰때문에특히 Streptococcus spp. 동정에유용하다 [7-9]. Table 1을보면 추가시기본배지만사용했을때보다복막액 14검체에서 106.7% (16/15) 의균주가, 체액 8검체에 서 45.0% (9/20) 의균주가, 창상 / 농양 90검체에서 20.7% (103/498) 의균주가더분리되어 추가가의미있었다. 검체에따라단일균및복합균의분포차이가있었는데, 복막액과체액은대부분 (95.5%, 21/22) 단일균인반면, 창상 / 농양검체는대부분 (84.4%, 76/90) 복합균분포였다. 복막액과체액과같이상대적으로세균의농도가낮은검체들에서단일균으로분리되었다면오염의가능성도있지만, 임상적으로의의가있다고해석되는균종 (S. aureus, Enterococcus spp., β-hemolytic streptococci 및진균류등 ) 이라면 가 BAP보다호기성그람양성균의증식속도가빠르기때문 [7] 에 에서만더동정되었을가능성도있다. 따라서복막액과체액검체에서는현재통상적으로시행하고있는 배양과같이 를추가하여검사하는것이실제원인균을분리하고배양양성률을높이는데도움이될거라생각하였다. 창상 / 농양검체의경우는 에서만분리된균들은다른그람음성균과의복합균을가진검체가대부분 (84.4%, 76/90) 이며실제 만추가 ( 각각 20.7%, 103/498) 해도 와 를모두접종하여추가분리된균주수 (14.1%, 118/834) 와도별차이가없으므로 (McNemar test, P>0.05) 검사의경제적효용성을고려하면 보다는 추가를먼저고려해야할것이다. Table 2에서보면 에서만동정된균은총 128균주로 S. aureus가가장많았으며 (21.1%, 27/128), 다음은 coagulase-negative staphylococci (20.3%, 26/128), Enterococcus spp. (16.4%, 21/128), Streptococcus spp. (10.2%, 13/128) 순이었고, Candida spp. 를포함한진균이 16.4% (21/128) 였다. 는 Proteus spp. 등의그람음성간균이섞여있는검체에서용혈성을보이는 Staphylococcus spp. 와 β-hemolytic streptococci 집락의분리에유용한배지이다 [7,9]. 본연구에서도 에서만분리된 S. aureus 중그람음성균과의복합균인경우가 51.4% (14/21) 로특히복막액에서분리된두검체중하나는 E.coli와혼합감염

5 Jayoung Kim, et al. : for Body Fluids, Peritoneal Fluid, and Wound/Abscess 5 라야하나, 검사실에서얻을수있는채취부위에대한정보가부족하여채취깊이에따라구분할수없었으며둘째, 균의증식정도는따로고려하지않아서복합균이있을때어느균이우세균인지알수없었다. 마지막으로, 체액이나심부농양의경우무산소성세균감염이흔하지만, 본연구에서는무산소성배양결과는포함하지않았다. 왜냐하면본연구의대상검체중일부는각지역병원에서검체채취후임상검사센터로이송된검체들이포함되어있어실제검체채취과정및운송조건을확인하기어려울뿐만아니라다른연구기관에비해장시간의운송시간이소요될가능성이있기때문이었다. 더욱이최근 10년간혐기성세균의분리빈도가감소하는추세이며 [15,16] 국내보고에서도전체체액검체중혐기성세균이분리된검체는 0.1% (1/743) 에불과하였고 [17,18], Clostridium spp., Propionibacterium acne 등은배양이되더라도오염균일경우가많다 [5,15,16]. 이에복수및수술후감염등의무산소성균의감염이의심되는경우를제외하고는호기성배양에중점을두고검체를사용하는것이바람직하다고생각하였다. 결론적으로체액및복막액은실제배양율이낮은검체로이검체의배양검사의민감도를높이기위해 과 를모두추가하는것이필요하다고생각되었다. 그러나 혹은 에서만추가로동정된균들중단일균인경우가많았는데, 이들의증식정도, 액체배지에서의증식여부가확인되지않아오염의가능성도배제할수없었다. 그러나창상 / 농양검체는 에서만분리된균들이실제그람음성균과의복합감염인경우가많을뿐더러 만추가하여동정 (20.4%, 103/498) 해도 와 를추가하여더동정될수있는결과 (14.2%, 118/834) 와별차이가없으므로 (McNemar test, P>0.05) 검사의경제적효용성을고려하면 Clinical Microbiology Procedure Handbook의권장대로 보다는 를우선적으로추가하는것이효과적일것으로생각된다. 6 6 Ann Clin Microbiol 2015;18(1):1-6 = 국문초록 = 체액, 복막액및창상 / 농양검체에서 MacConkey 배지및 Colistin-Nalidixic Acid Blood Agar 의유용성평가 1 가톨릭관동대학교국제성모병원진단검사의학과, 2 가톨릭대학교대전성모병원진단검사의학과, 3 삼광의료재단, 4 가톨릭대학교의과대학서울성모병원진단검사의학과김자영 1, 조성일 2, 김용균 3, 박연준 4 배경 : 체액 ( 혈액, 뇌척수액, 요, 및복막액제외 ), 복막액, 및창상 / 농양검체의배양에대한표준배양법의권장과는달리국내에서는 Columbia colistin-nalidixic acid blood agar () 배지가거의사용되지않고있으며, 대부분그람염색과배양이동시에시행되므로권장사항을따르기어려운면이있다. 본연구에서는이들검체에서 MacConkey 배지 () 및 Columbia colistin-nalidixic acid blood agar () 의유용성을평가하였다. 방법 : 총 1,508 검체 [ 복막액 144 검체, 체액 241검체 ( 담즙액 19 검체, 관절강액 70 검체, 심장낭액 6 검체, 흉수액 104 검체, 및기타체액 42 검체 ) 및창상 / 농양부위 1,123 검체 ] 를대상으로기본배지 ( 혈액한천배지, 초콜릿배지혹은 Staphylococcus aureus가획선된혈액한천배지 ), 및 를각각추가하였을때의균동정결과를비교하였다. 결과 : 전체 1,508 검체중기본배지만을사용하면 588검체에서 885균주가분리되었으며 을추가하면 24검체에서 27균주가, 를추가하면 112검체에서 128균주가더분리되었다. 기본배지사용과비교하면, 을추가함으로써복막액에서는 233.3%, 체액에서는 38.5%, 창상에서는 4.5% 의그람음성균이추가동정되었다. 를추가하면기본배지에비해복막액, 체액, 창상 / 농양검체에서그람양성균및진균이각각 106.7%, 45.0%, 및 20.7% 추가동정되었다. 에서만자란그람음성균주는주로장내세균 (77.0%) 이었으며, 에서만분리되는균주로는 S. aureus가가장많았고 (21.1%), coagulase-negative staphylococcus (20.3%), Enterococcus spp. (16.4%), Streptococcus spp. (10.2%) 순이었고진균도 16.4% 였다. 결론 : 복막액및체액검체에대해서는 와 를추가로사용하는것이, 창상 / 농양검체는 를우선적으로추가하는것이효과적일것으로생각된다.
