In cases reported to date, the pathogen of neonatal. bovis infections is usually treptococcus gallolyticus subsp. pasteurianus (. bovis biotype II/2). treptococcus lutetiensis (. bovis biotype II/1) was identified using and tuf gene sequence analysis of the isolates from blood and cerebrospinal fluid () of a fever-presenting 28-day-old male. culture analysis was performed using automatic equipment (VITEK 2) and identified treptococcus infantarius supsp. infantarius, yet we were unable to get accurate results from the culture. The fever subsided on the second day of hospitalization, and the patient was discharged without neurologic complication after 14 days of antibiotic therapy. In this case, we were able to accurately identify the pathogen using molecular genetic methods. To our knowledge, this is the first case of late onset neonatal bacteremia and meningitis caused by. lutetiensis. 2 Korean J Pediatr Infect Dis Vol.21, No.3, 2014 포함된다 3). 신생아에서. bovis에의한침습감염사례는아종이확인된경우대부분. pasteurianus 에의한것이었다. 저자들은이전에보고된적이없었던. lutentiensis 에의한신생아균혈증과수막염사례를경험하였기에보고하는바이다. 증례생후 28일남아가하루전부터 38 이상의열이나서응급실로내원하였다. 환자는내원당일새벽부터평소보다보채었으며끙끙앓았고잘먹지않았다. 환자는재태연령 38주 4일, 3440 g, 제왕절개로출생하였으며, 산전진찰에서문제가없었고, 분만당시에도특이소견은없었다. 환자는출생이후집에서건강하게지냈으며가족중아픈사람은없었고아픈사람과의접촉도없었다. 예방접종은 B형간염 1차와 BCG를시행한상태였다. 응급실방문당시환자의활력징후는혈압 83/63 mmhg, 맥박수 130 회 / 분, 호흡수 50 회 / 분, 체온 38.3 였고체중 4.6 kg (12-25 백분위수 ), 키 58 cm (50 백분위수 ), 두위 38.1 cm (50-75 백분위수 ) 이었다. 의식은명료해보였으나끙끙거리는소리를내고아파보였다. 두부기형및대천문팽창의소견은없었고, 결막은창백하였으며공막은정상이었다. 양폐야의호흡음은정상이었으며심음은빨랐으나심잡음은들리지않았다. 복부팽만소견은없었고장음은정상이었으며간과비장은촉지되지않았다. 신경학적검사에서근긴장도는정상이었고, 동공반사, 모로반사및빨기반사등원시반사도정상이었다. 내원당시말초혈액검사결과, 혈색소 11.6 g/dl, 적혈구용적 35.6%, 백혈구수 11,820/mm 3 ( 호중구 21%, 림프구 34%, 단핵구 14%, 띠중성구 25%), 혈소판 366,000/mm 3 이었다. 혈청총단백질 5.2 g/dl, 알부민 3.7 g/dl, aspartate aminotransferase (AT) 27 IU/L, alanine aminotransferase (ALT) 19 IU/L, 혈액요소질소 8 mg/dl, 크레아티닌 0.26 mg/dl, Na 129 meq/l, K 4.4 meq/l 이었고, C-반응단백은 0.79 mg/dl ( 정상 치, mg/dl) 이었다. 소변검사에서농뇨는없었다. 뇌척수액검사와입원을권유하였으나, 발열증상이호전되자보호자거부하여귀가하였다. 그러나오전내내끙끙앓으며쳐지는증상있고 38 이상의발열증상이지속되어 8시간후응급실로다시내원하였다. 재내원시시행한말초혈액검사결과상혈색소 11.5 g/dl, 적혈구용적 35.1%, 백혈구수 14,060/mm 3 ( 호중구 11 %, 림프구 37%, 단핵구 11%, 띠중성구 39%), 혈소판 384,000/mm 3 이었다. 혈액화학검사결과는앞서검사한결과와다르지않았고, C-반응단백은 6.31 mg/dl로이전검사에비해증가하였다. 뇌척수액검체의색깔은투명하였고, 백혈구수 3,924/mm 3 ( 중성구 94%, 림프구 6 %), 단백 300 mg/dl 이상, 당 25 mg/dl ( 혈청당 149 mg/dl) 로확인되었다. 그람염색결과그람양성구균이보였고, Haemophilus influenzae type b, treptococcus pneumoniae, Neisseria meningitidis, Listeria monocytogenes, Escherichia coli에대한항원검사 ( CO-agglutination assay, PATOREX T ENINGITI 61607, Bio-Rad, California, UA) 는음성이었다. 입원 2일째환자의혈액과뇌척수액에서그람양성구균이자란다는보고를받았다. 혈액배양에서자란균은 VITEK 2 (bioérieux, arcy-l Etoile, rance) 장비에서. infantarius 로동정되었고, 항균제감수성결과 benzylpenicillin, ampicillin, ceftriaxone, cefotaxime, levofloxacin, linezolid, tetracycline, 그리고 vancomycin 에감수성을보였으며, clindamycin, erythromycin 에는내성을보였다. 뇌척수액에서자란균은 VITEK 2 장비에서다섯차례나반복하여검사했지만정확하게동정할수없었다. 뇌척수액에서배양된균은균주동정을위해 CL 서울의과학연구소에 bacterial ribosomal DNA identification 을의뢰하였고, partial (768 bp) 와 tuf (elongation factor Tu) 유전자 (729 bp) 의염기서열분석을통해. lutetiensis 로확인되었다. 그런데이결과는혈액검체에서 VITEK 2 장비로동정된결과와일치하지않았으므로, -70 냉동고에보관중이던혈액에서자란균주로 bacterial ribosomal DNA identifica- 220

3 김지숙외 3 인 : treptococcus lutetiensis 에의한지발형신생아균혈증과수막염 1 례 tion을다시의뢰하였으며, 뇌척수액에서배양된것과동일한균주 (. lutetiensis) 임을확인하였다 (Table 1). 입원당일부터경험적항균제로 vancomycin, cefotaxime, 그리고 ampicillin/sulbactam 을투여하였고, 환자는입원 2일째부터는열이나지않았고수유량도증가하였다. 입원 4일째요추천자를다시시행하였고척수액검사결과백혈구수 280/mm 3, 단백 229 mg/dl, 당 30 mg/ dl로확인되었으며균은자라지않았다. 혈액배양검사역시입원 4일째에다시시행하였고균은자라지않았으며소변배양검사에서도자란균은없었다. 혈액배양검사에서배양된균의항균제감수성결과는입원 4일째보고받았으나, 뇌척수액에서배양된균의경우균동정이지연되어디스크확산법으로시행한항균제감수성검사결과를입원 8일째에보고받았다. 항균제감수성검사결과를확인한이후에는 ampicillin/sulbactam 만투약하여, 총 14일간항균제사용후퇴원하였다. 입원기간동안신경학적이상소견은없었고치료후외래에서시행한뇌자기공명영상검사와청력검사 ( 이음향방사검사, 자동화청성뇌간반응검사 ) 결과도정상이었다. 고찰신생아에서. bovis에의한침습성감염은 1978년에 Heading 등 2) 이처음으로보고하였고, 국내에서는 2013 년에 Park 등 4) 이. pasteurianus 에의한신생아침습성 감염을처음으로보고하였다. 1978년부터 1982년까지국외보고에서신생아에서발생한. bovis 혹은장구균이아닌 D군사슬알균 (nonenterococcal group D streptococcus) 에의한침습성감염이 7개의증례보고에서총 15례보고되었다. 그중 10례 (66.7%) 는생후 3일이내에발생한조발형감염이었으며, 패혈증혹은균혈증과수막염이동반되었고, 생후 7일이후에발생했던다섯증례중세명 (60%) 의환자는수막염을앓았다 5). 2000년도이후부터 2014년 4월까지보고된 15개의논문에서총 19개의신생아감염사례가보고되었으며그중에서 11개 (58%) 는지발형감염이었고, 증례중 68% 는혈액과뇌척수액에서균이동시에검출되었다 (Table 2). 지금까지보고되었던. bovis에의한침습성신생아감염의원인균주는대부분. pasteurianus 였고 19, 20), 국내에서보고된증례도마찬가지였다.. bovis의분류및명칭은분자유전학적인기법의발달과함께변경이나수정이이루어져왔으므로, 이전에. bovis 혹은장구균이아닌 D군사슬알균으로보고되었던신생아감염의원인균주들이어떠한아종 (subspecies) 에속하는것이었는지알수없으나, 현재까지보고된신생아침습감염의증례중에서는. lutentiensis 에의한것은없었다.. infantarius는간담도계질환이나대장암을포함한악성종양, 심내막염이있는성인에서균혈증의원인균으로확인되었으며 21-23), 소변과영아의대변검체에서분리된바있다 22). 5 김지숙외 3 인 : treptococcus lutetiensis 에의한지발형신생아균혈증과수막염 1 례 생후 28일에열이나서온환자의혈액과뇌척수액에서분리된균주를 와 tuf 유전자염기서열분석을통해 treptococcus lutetiensis 로동정할수있었다. 환자의혈액에서배양된균주는자동화장비 (VITEK 2) 에서 treptococcus infantarius 로동정되었고뇌척수액에서자란균주는동정되지않았다. 환자는항균제투여 2 일째부터열이떨어지고전신상태가호전되었으며, 14일간항균제사용후신경학적합병증없이퇴원하였다. 저자들은. bovis 군에의한침습감염환자에서정확한균주동정을위해분자유전학적검사기법이도움이될수있음을확인하였고본증례의원인균주가신생아침습감염의원인으로알려진사례가없어보고하는바이다. 으나, Takahashi 등 18) 은균혈증과수막염이있었던생후 5주된환자의대변검체에서혈액및뇌척수액과동일한. pasteurianus 를확인하였고, 아기의장에집락을형성한. bovis가수막염과연관이있는것으로추정하였다. 본증례에서는정확한균동정이늦어지면서엄마의질배양검사및직장도말검사, 아기의대변배양검사를시행하지못하였지만, 앞으로는유사한환자가확인되는경우추가검사가필요할것으로생각된다. 현재까지신생아에서침습성감염을일으킨. bovis는대부분 penicillin 에감수성을보였고 4, 16), 본증례에서분리된균주도 penicillin, ampicillin, cefotaxime, 그리고 vancomycin 에감수성이었다. 하지만 penicillin 과 ceftriaxone 에중간내성을보인증례 7) 도있었으므로수막염 과패혈증과같은침습성감염의치료에있어서항균제감수성결과를확인하는것은중요하다.. bovis에의한감염의예후는비교적좋은것으로알려져있으며 7), 2000 년도이후에보고된신생아침습감염환자들중사망한사례는없었다. 수막염이있었던경우환자들은경련혹은경련유사증상을보이기도했고 14), 국내보고 4) 에서도지발형경막하삼출액이있었으나치료이후신경학적이상이나발달장애는없었다. 앞서살펴본바와같이. bovis 는신생아침습감염의원인균중하나이며, 일반적으로임상검사실에서사용하는방법으로균을동정하는데어려움이있을수있으므로분자유전학적검사기법을활용하는것이진단에도움을줄수있을것이다. 또한이증례는국내에서두번째로보고되는사례이므로앞으로신생아와어린영아에서침습성감염을일으키는원인균주로써. bovis 감염사례를지속적으로감시하고적극적으로보고하는것이필요하다고생각된다. 요약 treptococcus bovis에의한신생아침습감염을보고한사례는많지않으며, 지금까지보고된증례는대부분 가원인이었다. 저자들은 References 1) Chapter 11.
