These stressful work conditions of being constantly under pressure and exposed to potentially traumatic events put them at higher risk of developing posttraumatic stress disorder (PTSD), compared to the general population. PTSD is a potentially debilitating mental disorder, due to persistent intrusive thoughts, negative alterations of mood and cognition, hypervigilance, avoidance of similar situations and reminders, and re-experiences of the traumatic event. Previous studies have shown a relatively high prevalence of PTSD among firefighters, indicating the need for a systematic approach of early detection and prevention. Therefore, a critical review of the current literature on PTSD in firefighters would provide valuable insights into developing effective prevention and intervention programs. Literature indicated that there are risk factors of PTSD in firefighters, such as pre-existing depression, anxiety, sleep disorders, occupational stress, physical symptoms, and binge drinking, whereas social support and adequate rewards are protective factors. Although there are differences in the prevalence of PTSD across studies, partly due to various assessment tools utilized, different sample sizes, and sample characteristics, over one tenth of the firefighters were estimated to have PTSD. The current review warrants further investigations to precisely assess PTSD and co-morbid mental disorders, functional outcomes, and associated factors, and to develop evidence-based preventive and interventional programs to help firefighters with PTSD. 2 Firefighters and Posttraumatic Stress Disorder Ryu J, et al 외상사건으로부터 이어지는 외상 후 스트레스 장애는 더 이상 군인 등 특수한 집단에만 한정되는 장애가 아니다 외상 후 스트레스 장애는 우울증 알코올 남용 수면 장애 사회 부적응 만성적 불안증상 결근과 퇴직 등을 유발하여 삶의 질을 떨어뜨릴 수 있으므로 적절한 관리가 필요하다 소방공무원은 빈번한 외상사건에의 노출 불규칙한 교대근무 높은 수준의 직무스트레스 로 인해 외상 후 스트레스 장애 발생의 고위험군으로 분류된다 이전에 시행된 대부분의 연구에서도 소방공무원의 외상 후 스트레스 장애는 개별 연구마다 수치의 차이가 있긴 하나 대개 일반 인구집단에 비해 높은 수준으로 보고하고 있다 지난 년 동안 국내에서 이루어진 연구 문헌을 검토한 결과 소방공무원의 직업적 노출로 인해 발생 가능한 신체 및 정신적 건강영향과 관련된 종설은 세 편 확인되었다 하나는 소방공무원이 노출되는 물리 화학 생물학적 위험 요인 및 그로 인한 건강 영향에 대한 검토 였으며 나머지 두 편은 경찰공무원 등이 포함된 고위험 직업군의 외상 후 스트레스 장애에 대해 검토한 문헌이었다 앞서 시행된 연구들은 소방공무원의 신체 정신적 건강영향을 모두 포괄하여 다루고 있거나 일반적으로 외상 후 스트레스 장애의 고위험군으로 알려져 있는 여러 직업군을 한꺼번에 다룸으로써 소방공무원만의 특수한 직업적 노출 상황에 의해 발생할 수 있는 외상 후 스트레스 장애에 초점을 두지 못했다 따라서 소방공무원의 외상 후 스트레스 장애에 대해 좀 더 체계적인 접근이 필요하다 생각하였다 즉 연구마다 편차가 크게 보고되는 현재 소방공무원의 외상 후 스트레스 장애 수준을 국내외 연구를 통해 파악해 보아야 하며 소방공무원의 외상 후 스트레스 장애 발생에 관여하는 위험 요인을 확인할 필요가 있겠다 추가적으로 소방공무원을 대상으로 실시되고 있는 국내외 중재 프로그램 등도 함께 확인함으로써 국내 소방공무원의 외상 후 스트레스 장애 관련 연구에서 미처 이루어지지 않았던 부분을 확인하고 향후 관련 연구 진행에 도움이 되고자 본 연구를 진행하였다 문헌검색 소방공무원과 정신건강영향에 대한 체계적 문헌 고찰 과정을 진행하기 위해 검색 엔진으로 학술연구정보서비스 와 미국국립보건원 산하 미국국립의학도서관에서 운영 중인 를 이용하였다 영어 검색어로는 - 를 사용하였으며 한글 검색어로는 소방공무원 외상 후 스트레스 장애 소방공무원 외상 후 스트레스 장애 악화 소방공무원 정신건강 을 사용하였다 검색된 논문은 최근 년 내에 발간된 것 년 월 일 년 월 일 한글 및 영어로 출판된 것으로 제한을 두었다 검색 결과 상위 검색어에 대해 영어의 경우 총 개의 논문이 검색되었으며 한글의 경우 총 개의 논문이 검색되었다 차적으로 검색된 논문을 대상으로 제목 및 초록의 간략 검토를 통해 본 연구 주제와 관련이 있을 것으로 예상되는 영어 논문 개 한글 논문 개를 재선정하였다 이 과정을 통해 추려진 논문에 대해 면밀하게 초록을 재검토하였다 우선적으로 사람을 대상으로 한 논문을 선택하였고 소방공무원이 포함되지 않은 직업군만을 대상으로 한 논문은 제외하였다 외상 후 스트레스 장애를 포함하는 정신건강 문제가 아닌 신체적 문제만을 연구 대상으로 삼은 논문은 제외하였다 추가적으로 선택된 논문 중 동일한 논문인 경우 단순한 증례 보고 형태의 논문 본문 내용 없이 초록만 제시된 논문의 경우는 제외하는 것으로 하였다 최종적으로 체계적 문헌 고찰을 진행할 논문으로 영어 논문 개 한글 논문 개를 선택하였다 외상후스트레스장애의정의 미국정신의학회 는 년 외상 후 스트레스 장애를 불안장애에 속하는 장애의 하나로 인정하였다 이에 의하면 외상 후 스트레스 장애는 극심한 외상 스트레스를 경험한 이후에 나타나는 사건과 관련된 침습 과각성 회피 등을 주 증상으로 한다 따라서 외상 후 스트레스 장애의 진단에서는 외상사건의 경험이 반드시 선행되어야 한다 년 미국정신의학회에서 설정한 외상사건에 대한 정의 는 다음과 같다 실제 죽음 또는 죽음의 위협이 가해진 사건 혹은 심한 부상 자신이나 다른 사람의 신체 보존에 대한 위협을 경험 목격하거나 직면한 경우가 있어야 하며 에 해당하는 사건을 겪은 후 나타나는 개인의 반응은 강한 두려움 공포 무력감을 포함한다 초기 외상 후 스트레스 장애가 처음 정신과적 진단명으로 채택되었을 당시 외상사건은 전쟁 고문 자연 재해와 같은 거의 모든 사람에게 상당한 스트레스 요인으로 작용될만한 사건만으로 한정 되었으나 여러 연구들에 의해 다소 심각성이 덜하나 일상에서 반복적으로 경험할 수 있는 사건 교통사고 폭력 등 에 의해서도 외상 후 스트레스 장애가 유 journal.biolpsychiatry.or.kr 11

3 Korean J Biol Psychiatry 2017;24(1):10-18 발될 수 있음이 확인되었다 미국정신의학회의 최신판 정신장애 진단 통계편람 에 의하면 가족이나 친구 등 가까운 이가 경험한 외상사건의 인지나 외상사건과 관련된 세부사항들에 대한 반복적인 경험 역시 외상 후 스트레스 장애를 유발할 수 있는 사건으로 보고 있다 년 미국에서 발생한 테러 이후 맨하튼 거주자를 대상으로 시행한 연구에서 사건의 직접 경험자가 아님에도 불구하고 외상 후 스트레스 장애의 유병률을 정도의 수준으로 보고하였다 또한 외상사건을 경험한 이들을 자주 돌보게 되며 간접적으로 외상사건에 노출되는 심리상담사 사회복지사 아동복지사 등을 대상으로 한 연구에서 응답자의 가 그들이 돌보는 외상사건 유경험자들의 사건으로부터 영향을 받는다고 얘기하였으며 그 중 정도는 외상 후 스트레스 장애의 핵심적 진단기준을 충족시켰다 평가도구외상 후 스트레스 장애를 평가하기 위해 사용되는 도구는 국내외 포함하여 세 가지 정도로 나뉜다 첫째 사건 충격 척도 이하 이다 이것은 등 이 외상사건을 겪은 후 사람들이 보이는 충격 적응 과정을 연구하기 위해 개발한 척도로 관련 증상에 관한 각 문항들에 대해 자기보고식으로 작성한다 초안에서는 과각성 증상에 관한 설문이 빠져있어서 와 가 수정판 이하 을 고안하였다 최종적으로 설문은 총 문항으로 구성되어 있으며 지난 일주일간의 증상을 점 척도 로 평가한다 현재 우리나라에서는 을 등 이 번안하고 타당도 및 신뢰도를 확인한 척도를 사용하고 있으며 총 점수의 범위는 점에서 점까지이다 임상적 진단을 위한 절단점은 점으로 많이 사용된다 둘째 외상 후 스트레스 진단척도 - 이하 가 있다 등 이 외상 후 스트레스 장애의 진단과 심각도 측정을 위해 개발한 것으로 총 문항의 자가보고식 검사이다 설문에 대해서 지난 한 달 동안의 증상을 점 척도 로 평가하며 총점은 점에서 점 범위 절단점은 점을 기준으로 많이 사용한다 셋째 외상 후 스트레스 장애 체크리스트 이하 가 있다 외상 후 스트레스 장애의 진단 평가 및 선별을 목적으로 개발된 자가보고식 설문지이며 총 문항으로 구성되어 있다 응답자는 점 척도 로 평가하며 총점 범위는 점에서 점까지이며 소방공무원이나 군인과 같은 특수 군에서는 절단점 점을 많이 이용한다 세 가지 평가도구 모두 총 점수가 높을수록 증상이 심각함을 나타낸다 국내 연구에서는 대부분 를 사용하고 있었으며 외국의 경우에는 을 사용하는 경우가 많았다 은 미국에서의 테러 현장 구조 활동을 수행했던 소방공무원을 대상으로 타당화 연구 수행 후 높은 신뢰도와 타당도가 검증되었으나 이러한 사건의 경우 소방공무원들이 흔히 겪는 외상사건이 아니라는 데서 제한이 있으나 국내 소방공무원을 대상으로 을 사용하는 경우에도 과 와 유사한 수준으로 그 신뢰도와 타당도가 검증된 바 있다 소방공무원의외상후스트레스장애유병률국내외 연구 문헌 고찰 결과 측정 도구를 이용한 소방공무원의 외상 후 스트레스 장애 고위험군은 에서부터 까지 범위가 매우 넓었다 공무원연금공단에서 공무상 요양 승인을 받은 경북지역 내의 공상 소방공무원만을 대상으로 한 연구에서는 그 비율이 최대 까지로도 나타났다 이러한 차이는 개별 연구에서 사용하는 외상 후 스트레스 평가도구의 차이 및 절단점의 차이에서 발생할 수 있을 뿐만 아니라 연구 대상 소방공무원의 근무 지역 근무 내용에 따라서도 차이가 날 수 있다 그럼에도 불구하고 외상 후 스트레스 장애 현황에 대한 보고가 있는 국내외 개 논문 중 개의 논문에서 외상 후 스트레스 장애 평가 척도를 이용한 소방공무원의 외상 후 스트레스 장애 고위험군은 이상으로 나타났다 추가로 우울증상을 함께 본 연구도 많았는데 이러한 경우 우울 정도는 를 이용하여 평가하였으며 연구 결과 외상 후 스트레스 장애와 우울을 동시 경험하는 소방공무원의 비율도 적게는 에서 많게는 정도까지도 나타났다 대부분의 연구가 단면연구였으나 테러 시 응급구조활동을 수행한 소방청 직원 명 대상으로 시행된 코호트 연구에서 년간의 외상 후 스트레스 장애 누적발생률은 로 나타났다 세부적인 개별 연구 결과는 과 에 정리하였다 외상후스트레스장애의악화및완화요인외상사건의 심각도와 외상사건에 노출된 기간 및 근접도는 외상 후 스트레스 장애의 가장 중요한 위험인자로 알려져 있다 그러나 사별과 같이 대부분의 사람들이 일생을 살며 경험 가능한 수준의 사건을 겪은 사람들도 사건의 재경험 과각성 등의 증상을 보이는 경우가 있다 반면 충분히 스트레스를 유발할 만한 사건을 경험한 후라고 할지라도 모두가 동일하게 외상 후 스트레스 장애에서 보이는 증상을 호소하 12

PTSD : posttraumatic stress disorder, IES-R-K : Impact of Event Scale-Revised Korean version 진 않는다 이전 연구를 검토한 결과 외상 후 스트레스의 악화 가능 요인은 우울 불안 수면장애 소진 직무스트레스 신체적 증상 음주 등이 대표적이었으며 완화요인으로는 직무 만족 스트레스 대처 등이 대표적이었다 직업적 외상 후 스트레스 장애 관련 편의 논문에 대한 체계적 문헌 고찰을 시행한 연구에서 낮은 사회적 지지가 외상 후 스트레스 장애 증상의 위험을 높이므로 이들에 대한 사회적 지지와 건강한 심리사회적 근무 환경을 만들어주는 것이 중요하다고 언급 하기도 하였다 그 외 국내 연구에서 화재 진압에 비해 구급 업무의 경우 증상 발현의 위험이 높아졌다는 보고가 있었으며 근무년수가 길어질수록 고위험군으로 될 위험이 높다는 연구도 있다 국외 연구에서는 신체적 증상이 동반될수록 사건 현장에 일찍 도착한 사람일수록 해당 외상사건 경험으로 인해 발생한 신체적 정신적 문제로 직장에서 은퇴 한 경우 외상 후 스트레스 장애 증상 발현의 위험이 더 높아진다는 연구 보고가 있었다 그 외에도 직무스트레스 수 journal.biolpsychiatry.or.kr 13

PTSD : posttraumatic stress disorder, CES-D : Center for Epidemiologic Studies Depression Scale, PCL : Post-Traumatic Stress Disorder-Checklist 준이 높을수록 외상 후 스트레스 장애 증상과 양의 상관관계를 보이는 연구도 있었다 최근 외국에서의 외상 후 스트레스 장애 연구는 대부분에서 우울 증상 수준의 파악을 넘어서 주요 우울장애와 외상 후 스트레스 장애의 동시이환율 을 평가하고 두 장애의 상호연관성에 관심을 가지며 각각의 장애가 나머지 장애의 위험도를 얼마나 높일 수 있는 지에 대해 평가하고 있다 소방공무원의 외상 후 스트레스 장애에 대한 주요 악화 및 완화 요인과 관련된 세부 연구 내용은 과 에 정리하였다 소방공무원의외상후스트레스장애중재프로그램국내에서 소방공무원의 외상 후 스트레스 장애에 대한 체계적인 관리는 태동 단계인 실정이다 최근 국내에서 소방공무원의 외상 후 스트레스 장애에 대한 중재 프로그램 효과 평가 연구로는 감정치유기법 사용 후 효과를 평가한 연구와 현재 소방공무원의 정신건강 향상 프로그램으로 실시되고 있는 건강심신캠프 참여의 효과를 살펴 본 연구가 있었다 외국의 경우에는 우리나라에 비해 소방공무원의 외상 후 스트레스 장애 관리가 좀 더 체계적으로 실시되고 있다 미 국의 경우 외상 후 스트레스 장애의 관리는 국가가 운영하는 정보 제공 기관과 지역사회의 치료 기관을 연계한 형태로 진행되고 있다 국가보훈처 내의 국립 외상 후 스트레스 장애 센터 - 에서 운영하는 공식 홈페이지를 통해 원하는 사람이면 누구나 필요한 정보를 얻을 수 있다 즉 외상 후 스트레스 장애에 대한 기본적인 정보에서부터 본인이 거주하는 지역사회에서 접근 가능한 외상 후 스트레스 장애 관련 의료 자원 전문가 전문 병원 등 의 정보 또한 손쉽게 얻을 수 있도록 만들어 이용자의 접근성을 높였다 일본의 경우 지역에 따라 차이가 있긴 하나 동경소방청에는 정신과 전문의와 심리상담사가 소방공무원의 정신건강을 전문적으로 다룬다 또 고베에 위치한 효고현 트라우마 센터는 일본 최초의 재난 전문 센터로 정신과 전문의 임상심리상담사를 포함한 정신 보건 전문인들로 인력이 구성되어 있으며 이들 중에서도 소방공무원을 전담하는 인력이 따로 배치되어 있다 그 외에도 소방공무원을 대상으로 외상 후 스트레스 장애 교육 홍보 검진 등을 주기적으로 시행하며 지역 내 소방서의 요청이 있을 때에는 개별 방문 서비스도 제공한다 14

AUDIT : Alcohol Use Disorders Identification Test, PTSD : posttraumatic stress disorder, OR : odds ratio, CI : confidence interval 본 연구에서는 소방공무원과 외상 후 스트레스 장애라는 주제로 수행된 국내외 연구 문헌들을 검토하였다 평가도구의 차이 및 연구 대상 그들이 경험한 사건의 심각성 노출 기간 차이 등의 이유로 소방공무원의 외상 후 스트레스 장애 현황은 연구마다 차이가 있다 그러나 어떠한 평가도구 및 절단점을 사용했더라도 대부분의 연구에서 외상 후 스트레스 장애 고위험군은 가 넘는 것으로 확인되었다 일반 인구집단의 외상 후 스트레스 장애 평생 유병률이 대개 남성 여성 정도로 알려져 있는 것으로 볼 때 소방공무원의 외상 후 스트레스 장애 위험은 일반 인구집단에 비해 더 높다고 판단된다 최근 외상 후 스트레스 장애 증상 발현에 있어 간접적인 외상사건의 경험도 중요한 요소로 여겨지고 있다 테러 이후 지역 주민을 대상으로 시행한 연구에서 테러 장면이 방송되는 시청 시간이 길수록 외상 후 스트레스 증상을 호소하는 비율이 높았으며 그 외의 연구들에서도 외상사건을 다루는 매체에의 노출이 잦을수록 외상 후 스트레스 장애 증상의 위험이 높아지는 것으로 보고하고 있 다 등의 매체를 통한 간접 외상사건의 경험이 의 진단 구성요소가 될 수 있는지에 대해서는 논란이 있다 매체를 통한 외상사건의 간접 경험은 객관적 노출의 지표가 되기 보다는 개인이 노출을 어떻게 받아들이냐의 문제일 수 있기 때문이다 분명한 것은 이러한 간접적인 외상사건의 경험도 외상 후 스트레스 장애 증상과 관련이 있으므로 중요하게 평가될 필요가 있다 이러한 맥락에서 볼 때 소방공무원은 직접 외상사건을 경험하는 빈도도 잦을 뿐만 아니라 간접적으로 외상사건을 항상 겪게 됨으로써 간접경험 또한 소방공무원의 외상 후 스트레스 장애 증상 발현 및 악화에 영향을 미칠 수 있음을 간과해선 안되겠다 또 외상 후 스트레스 장애의 진단 기준에 있어 외상사건의 범위가 확대되고 있음에 따라 소방공무원의 외상 후 스트레스 장애의 유병률은 현재 수준에 비해 높아질 수 있을 것으로 추정된다 외상 후 스트레스 장애는 공존 장애가 많은 것 또한 특징적이다 일반적으로 주요 우울증 약물이나 알코올 사용 장애 불안장애 공황장애가 함께 존재하는 경우가 많다 외상 후 스트레스 장애가 단독으로 있는 경우보다 주요 우울장애 등의 동반 질환이 있는 경우 우울 증상이 더 심각하며 journal.biolpsychiatry.or.kr 15

AUDIT : Alcohol Use Disorders Identification Test, PTSD : posttraumatic stress disorder, OR : odds ratio, CI : confidence interval 더 빈번한 병원 방문 빈번한 자살 사고와 관련이 있는 것으로 알려져 있다 실제로 검토한 문헌들에서도 외상 후 스트레스 장애와 우울 음주와의 관련성을 살펴 본 연구가 많았다 그 중에서도 특히 우울의 경우에는 음주 불안과 같은 요인들이 외상 후 스트레스 장애와 우울 모두의 공통 위험 요인으로 작용한다 동반 질환이 존재하는 경우 질병 부담은 증가되며 적절한 치료에도 반응이 늦어 각각의 질환이 불량한 예후를 가진다 소방공무원의 외상 후 스트레스 장애에 관한 연구 시행에는 동반 질환에 대한 평가와 공통의 위험요인을 함께 평가하는 것이 필수적이라고 생각된다 외국의 경우 소방공무원의 외상사건에의 노출 빈도 근접도 사건의 심각도를 세분화하여 외상 후 스트레스 장애에 대한 위험 평가를 실시하고 있으며 더 나아가서는 그들이 가 질 수 있는 신체적 문제 가정과 직장에서의 역할 수행의 문제까지 포함하여 연구를 하고 있으나 국내의 경우 아직 이러한 수준으로는 연구가 이루어지지 않고 있다 따라서 국내에서도 소방공무원이 경험하는 외상사건의 빈도 심각도 등에 좀 더 초점을 맞춘 깊이 있는 연구가 필요할 것이다 추가적으로 소방공무원의 개개인의 신체적 사회적 건강에 영향을 미칠 수 있는 개별 요인들도 함께 평가하여 연구가 진행된다면 향후 소방공무원의 외상 후 스트레스 장애에 있어 개입 순서 및 구체적 중재 수준을 결정하는 데 도움이 될 수 있을 것이라 생각한다 외상 후 스트레스 장애는 외상사건의 경험 후 발생 가능한 불안 장애의 형태를 넘어서 전반적인 삶의 질 저하로까지 이어질 수 있는 장애이다 외상 후 스트레스 장애는 소방공무원 16
